Recall 193414 [USA]: Uncontrolled platform leveling increases the risk of injury and death.
Altec industries, inc. (altec) is recalling certain 2020 aa5x/an5x, aa5x(e)/an5x(e), aa6x/an6x, aa6x(e)/an6x(e), an4x-oc/an4x(e)-oc, an5x-oc/an5x(e)-oc, aa/an67-e100, and aa/an67(e)-e100 vehicles. The leveling sprocket may have a failed weld between the sprocket and hub assembly. altec will inspect and replace the damaged sprocket assembly, free of charge. If no damage is observed, a secondary retention kit will be installed to limit rotation of the platform, free of charge. Owner notification letters were mailed on september 15, 2021. Owners may contact altec's customer service at 1-877-462-5832. Recall 2021659 [Canada]: Other
Issue: on certain aerial devices, the platform leveling sprocket may not have been properly welded and could fail. If this happens, the platform may not stay level and could tip suddenly. Safety risk: if the platform tips suddenly, a person could slip from the platform and fall, creating the risk of an injury. Corrective actions: altec industries will notify owners by mail and provide them with instructions for how to inspect the leveling sprocket. If the sprocket assembly is cracked, it will need to be replaced. If it is not damaged, a secondary retention kit will be installed to limit rotation of the platform if the sprocket weld fails.
Recall 2021022 [Canada]: Structure
Issue: on certain aerial devices and digger derricks, a snap ring in the hydraulic check valve assembly could fail. If this happens, the snap ring can get stuck in the hydraulic system. This could result in continued (uncontrolled) movement of the boom. Note: this recall only involves units equipped with a dc pump. Safety risk: a boom that drifts or continues to move on its own could create a risk of injury to the operator and nearby people. Corrective actions: altec industries will notify owners by mail and provide you with instructions for how to inspect the check valve assembly. If the check valve assembly needs to be replaced, altec will supply a replacement part or set up a service appointment with an altec service center or mobile service technician.
