  • Inadvertent slide-out extension movement investigation # PE06005

    This investigation was opened based on odis receipt of a congressional letter on behalf of a constituent.The constituent reported incidents where the "slide out" room of his motor home would inadvertently move (extend) during highway use.Odi was concerned that a fully extended slide out would degrade the vehicles dynamic handling characteristics and/or possibly impact an object or another vehicle, resulting in a crash.Odi contacted fleetwood and discovered that slide out rooms were installed in motor homes, travel trailers and folding trailers. As part of the investigation odi visited a rv show and the fleetwood paxinos plant to examine "slide outs" in various new make and model vehicles and gain an understanding of how the systems work.During the investigation odi learned that there are two methods used to extend slide out rooms.Some manufacturers use hydraulic systems and others, such as fleetwood, use an electric motor. We were travelling on the i44 east of st.Louis, on 04/14/16 when the first tire on the drivers side rear inside tire blew out, the tread had peeled off the tire and tore out the wiring in the wheel well for the 120 vac and the 12 vdc and damaged 2 compartments and about 5 feet of the tailpipe was crushed. The second tire on the passenger side inside back,(04/16/16), also on the rear inside, blew out while we were travelling on the i70 7 miles from the i270 turnoff west of cincinatti ohio. The tread had also peeled off the tire and also damaged the 2 compartments nearest the tire were heavily damaged and the wiring was also torn out. The appraised value of the damage was $11,000. The tire pressures were checked in both instances and were at an acceptable range ,well below the 100 psi range.

Tl* the contact owns a 2005 fleetwood bounder. While driving long distances at lower speeds, the abs warning indicator illuminated. There was an abnormal burning smell as the front driver side brake seized. The vehicle was taken to a dealer. The technician diagnosed that the front driver side brake calipers, seal, brake pads, and hoses failed and needed to be replaced. The vehicle was previously repaired under nhtsa campaign number: 09v198000 (service brake, air). The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 5,000.

Driver side thermal pane window and passenger side thermal pane window seals have failed resulting in fog between the window panes. With no way to clear these windows, critical rear/side vision is diminished since side view mirrors must be viewed through these fogged panes. This ongoing problem of diminished rear and side vision due to these failed thermal seals is a serious safety issue and may result in a crash. *tr

Cooling unit in the norcold 1200lrim failed spewing ammonia. I had the unit replaced by an rv service shop. I was not advised it was on the recall list and was charged for the replacement.. Unit was parked.

Vehicle was parked and slides extended running on generator. Lighting seemed to dim and would flicker. Have new house batteries, inverter was rebuilt. We were in the front of coach our dog indicated something was wrong and was barking in the bedroom area. Upon inspection found smoke coming from the 12v fuse center. Opened door and center bolt was glowing and fuses were too hot to touch. Shut down 12v and the fuses cooled down. Turning power back on did not duplicate issue until the 12v accessories were turned on (lighting, fans etc.). Switched coach to shore power (shut down generator) and fuses did not get as hot. Dealer replaced the fuse center and problem was resolved. Had we been away from the coach or the dog did not alert us, this may have not ended well. The dealer says he has seen this before claims it is the way the power wire feeds the fuse bus causing the wire to loosen. Suggest that all owners have this checked. *tr

1. Radiator tank crimp failure. Excessive corrosion due to undetermined origin caused tank crimps to rust to the point of failure. Vehicle was kept excessively clean and dry. Crimps cannot be seen via normal walk around inspection. Entire radiator had to be replaced. 2. Transmission control module. Corrosion on "j" connectors between transmission and engine caused short and failure of non protected tcm. Transmission would not shift. Display indicated "tcm failure". Tcm and j connectors had to be replaced. Connectors cannot be seen via normal walk around inspection. Tcm is not protected from shorts. 3. Engine blow- bye. Oil blow-bye is ejected from the engine directly onto the engine side of the radiator thereby causing the radiator fins to clog and prevent engine cooling. Freightliner kit to fix issue has not been publicized, but was made standard on subsequent bounder models.Freightliner kit had to be installed and engine compartment cleaned. Engine side of radiator cannot be seen via normal walk around inspection. *tr

Tl*the contact owns a 2005 fleetwood bounder rv. While driving approximately 55 mph pressure was applied to the spongy brake pedal, followed by no power response. The antilock brake system warning light indicator illuminated on the instrument panel. The contact was able to pull over to the side of the road. The engine was turned off and after waiting a few moments the vehicle was restarted. The vehicle was able to resume in operation, although the brakes remained spongy. The vehicle was taken to an authorized dealer where the technician stated that the components which required replacement were the calipers, rotors, brake shoes and the antilock brake system sensor which had melted. The vehicle was repaired at the expense of the manufacturer. The failure mileage was 27,000.

The contact stated that while driving 35 mph, the windows on the driver side and passenger side became foggy and he could not see. Neither the dealer nor the manufacturer was contacted.
