• Recall 110728 [USA]: A fuel leak in the presence of an ignition source increases the risk of a fire.
  Isuzu manufacturing services of america (isuzu) is recalling certain model year 2006-2008 isuzu npr and npr-hd and chevrolet w3500 and w4500 trucks manufactured march 16, 2005, to august 13, 2008 and 2006-2008 gmc w3500 and w4500 trucks manufactured july 18, 2005, to august 13, 2008. The affected vehicles have a fuel tank that may corrode and leak fuel.
  isuzu will notify owners, and dealers will replace the fuel tank, free of charge. Isuzu issued an interim notification to owners on december 18, 2015. The recall began on april 8, 2016. Owners may contact isuzu customer service at 1-866-441-9638.

• Recall 79455 [USA]: If the engine seizes while the vehicle is being driven, the vehicle may experience sudden deceleration leading to an increased risk of a crash.
  Isuzu is recalling certain model year 2006-2007 isuzu npr, nqr, nrr, gmc w3500, w4500, w5500, and chevrolet w3500, w4500, and w5500 vehicles. The oil pipe bolts were not tightened properly during the vehicle production process. If not adequately tightened, the bolt may loosen after prolonged vehicle operation, which can result in oil seepage or leakage from the packing seals. In general, as oil is lost, the oil warning lamp will illuminate, and there may be abnormal engine noise and loss of oil can cause engine seizure.
  dealers will replace the packing seals with new ones and the bolt will be tightened with a specified torque. This service will be performed free of charge. The safety recall began on january 28, 2011. Owners may contact isuzu at 1-800-255-6727.

• Recall 75486 [USA]: Fuel leakage, in the presence of an ignition source, could result in a fire.
  Isuzu is recalling 16,859 my 2006-2007 isuzu npr, nqr, nrr, gmc w3500, w4500, w5500 and chevrolet w3500, w4500, w5500 trucks equipped with diesel engines. Diesel fuel may seep or leak from the fuel pipe at the upper or lower fuel port on the diesel fuel supply pump.
  dealers will inspect the vehicles and if no seepage or leakage is detected, the fuel pipe fittings will be tightened. If seepage or leakage is detected, the fuel supply pump will be replaced with a new one. The recall will begin on june 30, 2008. While driving at highway speed, if you hit a pothole or seam in a certain way it sets in motion what we call the "death wobble" -- an uncontrollable, violent shaking of the front end that can only be stopped by slowing down dramatically (usually below 30 mph). It feels like some sort of mechanical harmonic feedback loop is suddenly triggered in the front suspension that increases in violence and intensity as long as the vehicle continues forward. It feels like the whole truck is going to fly apart. I have had the truck into an isuzu dealership multiple times to have this addressed with no permanent fix. The problem can be masked with new tires but it returns, well before the tires have reached what should be the end of their life. If you google "isuzu death wobble" you will find that this is a commonly observed problem with no known solution. It's clear that this is a design issue and is endemic to this vehicle. Because of the violence of the shaking, the sudden, unpredictable onset, and the need to brake suddenly in highway traffic this is a serious safety issue which needs to be addressed by your organization before somebody gets killed. Driving vehicle over 50-55 mph vehicle randomly starts to violently shake (front end) until it is slowed to approximately 25-30 mph. Usually triggered by potholes or road joints.

The vehicle was taken an independent mechanic, where the vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The vin was not available. While driving at an unknown speed, the brake line went over the frame rail on the front passenger side tire and caused the brake pedal to travel to the floor. The failure occurred without warning and made a loud squeaky noise. The contact could not find a dealer in the united states that had a replacement brake line. The manufacturer was made aware of the failure and stated that some parts were ordered from japan. The manufacturer informed the contact that the orders were all taken, which meant that the contact had to wait for a future order. The vehicle was not repaired. The failure mileage was approximately 150,000.

The front cab of the truck starts shaking like a tire fell off. I had the truck to the dealership a few times and have changed the tires and other front end parts, i've changed every part that the dealer told me to change and i'm still having the problem. I started looking the problem up online and found that this is a common problem that people are having and they don't know what to do to fix it. *tr

The truck has what is called a "death wobble". At speeds over 55 mph the truck will shake violently until it is slowed to 15 to 20 mph. The dealer acts as if he has never heard of it but the internet is full of incidences and reports. We have two of the trucks and it is the same on both trucks. *tr
