While traveling downhill, overdrive off, jake brake on, brake applied, when transmission downshifted, heard loud bang and saw black pieces bounce off the road and up past driver's window. Stopped vehicle and found holes in fan shroud, coolant draining on road, hole in fiberglass hood, and black plastic object lodge into wire bundle on right side. Vehicle towed to repair shop where fan was found to have shed plastic outer ring and top sections of blades. Plastic pieces were scattered throughout the engine compartment and embedded in radiator, multiple holes in fan shroud, half dollar size hole and hand size web of fractures in the hood fiberglass. Repairs to date include replacement of radiator, fan and fan shroud, a radiator hose, wires inspected and repaired, the hood is still in dispute. Gm has paid for parts but insists i pay the labor. I suspect the fan was defective and/or the jake brake system/transmission downshifted improperly causing the fan to overspeed and disintegrate. I read the previous posted incident from may 2009 and recognize this is a related event. Gm mechanics have told me this has happened previously but it is rare. While this maybe the case the potential for serious damage/death and/or property damage is high considering the penetration of metals, fiberglass and hoses if one of the projectiles released had hit a brake line. Repair shop kept parts. *tr

Engine shut down after shutdown warning alarm engaged. No running issues up to this point. Vehicle towed to chevy medium duty truck center. Nothing found wrong. *tr

Tl*- the contact owns a 2006 jayco seneca 34ss. The contact stated that the trailer hitch bolts were cracked. The dealer inspected the vehicle and replaced the bolts. The road conditions were clear. There was another occurrence when the contact heard a loud scraping noise coming from the rear of the vehicle. The contact noticed that the driver's side bolt was missing, and another one was bent. The manufacturer provided a new trailer hitch to the contact as part of recall #07v005000. Approximately seven months later the contact noticed another bolt had cracked. The manufacturer advised the contact to go to the rv shop that installed the replacement hitch so that they could replace the bolts with heavy duty bolts. The vehicle currently has 35000 miles. *ak

Tl*the contact owns a 2006 jayco seneca hd 35gs motor home. The contact stated that the main frame welds have been spliced, which caused the vehicle to dog-track. The slides are not closing correctly and diesel exhaust fumes are entering the vehicle. The failure has left black smut on the walls of the motor home. General motors stated that they will not touch any component of these defects. The contact stated that the vehicle is not under warranty. The engine size, # of cylinders, and powertrain were unknown. The current and failure mileages were 4,800.
