Known Recall Investigations
  • Steering column separation investigation # DP03002

Mr. Larry a. Sackey submitted a petition to the national highway traffic safety administration (nhtsa) by letter dated april 18, 2003, requesting that nhtsafurther investigate the defective collapsible steering shaft system on 1987-1995 vehicles manufactured by the daimlerchrysler corporation. In part because of 49 cfr ? 30120(g), nhtsa has decided to limit the scope of the petition vehicles to 1993-1995 models, excluding the 1993 jeep grand cherokee and 1994-1995 dodge ram series trucks that were recalled (93v210 & 96v230) for steering column failure. If the analysis of these vehicles had identified a potential problem, the scope could have been expanded in an investigation.A review of odis data base shows no recent complaints and a lack of a complaint trend concerning the subject issue on the subject vehicles as a whole or by individual models. Therefore, the petition is denied. For details, see federal register notice volume 68, no. 130, july 8, 2003.

  • Steering wheel separation investigation # RQ00011

There is no summary currently available

  • Inadvertent air bag deployments investigation # EA98006

Complaints allege that the front air bag (s) deployed inadvertently--in most cases while starting the vehicle.

  • Air bag circuit/windshield wiper motor investigation # PE97049

There is no summary currently available

  • Inadvertent air bag deployments investigation # PE97046

Complaints allege that the subject vehicles air bags deployed inadvertently (in most cases when starting the vehicle).

  • Defective air bag crash sensor investigation # DP96014

Cfas sent a follow-up letter on december 12, 1996, to change the intent of the petition to "airbag aggressivity and deployment variability."nhtsa received the letter on 12/12/96; accordingly, the petition due day was changed to april 12, 1997.Cfas petitioned the agency to conduct an investigation to determine whether the "airbag crash sensor and/or deployment systems" for the 1991-92 chevrolet corsica/beretta, 1990-92 ford taurus, and 1994-96 chrysler minivans are defective.Cfas allege that "information on low speed crashes here occupants have suffered fatal injuries suggests [that the above vehicles] have defective airbag crash sensor and/or deployment systems."

  • Safety blt latch operation investigation # PE96008

There is no summary currently available

  • Liftgate latches investigation # EA94005

Liftgate latches in the subject vehicles have failed in low and moderate speed impacts resulting in occupant ejections, injuries, and fatalities. There are two failure modes associated with latch failure: fork bolt-detent lever bypass, and inertial unlatching.Nhtsa analyses of chrysler and peer minivans took the form of static and dynamic crash testing, engineering evaluations, complaint and field analyses, and real world data analyses.A technical report discussing these analyses can be found in the engineering analysis technical report in the public viewing file.Chrysler has advised nhtsa that it will conduct a campaign to replace the latches on all the subject vehicles. The chrysler campaign will consist of notification letters to all owners; free replacement of all latches with stronger, safer latches regardless of the age of the minivans; national advertising to promote customer awareness of the campaign; quarterly reports regarding campaign completion rates; as well as additional measures to ensure a high completion rate, above and beyond the actions that normally would be taken in a formal safety recall. These include follow up written and telephonic notifications to owners who do not bring their vehicles in for corrective action.Nhtsa firmly believes that the crash performance of the latches on these minivans raises serious safety concerns, and that the campaign addresses these concerns. The chrysler corrective action will increase the strength of the liftgate latches and decrease the potential for inertial unlatching, thereby reducing the risk of crash-related openings of the liftgate and possible occupant ejection through the liftgate opening.Owners of chrysler minivans will get a stronger, safer latch free of charge at the earliest possible time, without the need for further technical investigation and complex and time-consuming litigation with an uncertain outcome.Based on the above, odi is closing this investigation.

I bought my van on 03/02/2012. It was inspected on 03/03/2012. On 03/08/2012 i was driving and it was all over the road and very hard to control. I stopped at monroe muffler and had the tires checked, they were fine. I made an appointment to have the van totally checked on 03/12/2012. I was told i had a bad tie rod and ball joint and that they should not have passed inspection. I have tried several times to contact the dealer and he now seems to be avoiding me. I have four kids and need the van fixed and safe asap. *tr

Tl* the contact owns a 1994 plymouth voyager. While parked, the contact noticed that the rear passenger side sliding door would not close. The vehicle was not taken to the dealer for diagnostic testing and was not repaired. The approximate failure mileage was 145,000.

Tl*the contact owns a 1994 plymouth voyager. While closing the lift gate, the lift bar failed and shattered the rear window. The contact sustained cuts and bruises on his hands. The insurance inspector stated that the cause was due to a mechanical failure. Recall # 99v213000 (structure:body:hatchback/liftgate:hinge and attachments) was found. The current and failure mileages were 114,000.

No known events other than use and rust lead to failure.. The problem was discovered when passenger side rear tire began rubbing wheel well. Please help, both leaf springs snapped/broke on our 1994 voyager! Please be advised, be on watch for any other dodge/plymouth vans doing this, could cause dangerous loss of control and or accident/injury! This should not have happened! Warning! This part # 80556pn/1504* main leaf springs is defective and amazingly broke completely into at the front mounts. I have many pictures to send. Please recall asap. Vin: rr809177 mileage: 184000 i am very concerned that this could be just the "tip of the iceberg" and many vans could have the same problem with disastrous results if leaf springs break at highway speed! You folks need to get after this asap! I got a good used 95 whole axle with leafs and had the old axle replaced with the used one. Old parts/leaf springs with old axle have been saved and are available. *jb

Dt*: the contact stated the vehicle was purchased two weeks ago and the transmission failed. The transmission began to make a banging noise while driving at 50 mph. When the vehicle was moved off the road and placed into park, the gear position indicator shifted out of park . However, the vehicle would not move in either direction. The dealership was contacted, and they determined the transmission needed to be replaced. According to the contact this transmission was notorious for its high rate of failure and poor quality. The contact felt the seller knew about these problems and failed to solve them. The consumer checked the internet and discovered there were over 1,000 irate people who had similar transmission problems. The manufacturer should acknowledge the problem and issue a recall. Dear sir/madam: safety issue: my plymouth van caught fire somewhere in the lower dashboard on driver side. We had just driven the car and parked it, when minutes later the horn sounded and we say smoke and flames. The fire department was called. The car doors locked themselves, one front light went on, rear tail light was on during fire. Fortunately no one was in the van. Why did the fire start and why did the van doors lock. This is a real safety issue. Please send a reply as to what you plan to do about this safety issue. Thank you.

Dt: the contact stated the parking brake will inadvertently release after locking it. This has been happening for over 4-5 years. It has gotten progressively worse over the years. It will click as if it was locked, then it will jerk. It will make a very loud popping noise before it jerked. The dealer told him the car was too old. The manufacturer said it was out of warranty. *ak *jb

The head lights become cloudy because of ultraviolet damage to the plastic. The plastic needs ultraviolet protection, like our eyeglasses have ultraviolet protection. The headlight develop cataracts, literally, becoming cloudy and very difficult to see clearly at night. I have to drive very slowly. I have found maguire's pastx does help for a few months and then the headlamps or lenses become cloudy again. Of course each time i use this plastic cleaner and polisher, i am diminishing what little uv might be left in the plastic. This cloudiness of the plastic also is bad enough to prevent the passing of state inspections in pennsylvania - it's regarded as a possible hazard obviously according to the state inspection. *jb
