• Recall 80456 [USA]: The nuts included in the packaging for these ball joint assemblies are missing the intended prevailing torque (self-locking) feature. if the incorrect nut was to become loose, the joining members may loosen leading to a separation of the ball joint. this may result in loss of vehicle control, possibly resulting in a vehicle crash.
  Federal mogul is recalling 6,903 aftermarket ball joint assemblies shipped between november 9, 2007 and march 26, 2008, which may have been packaged with incorrect nuts. The part numbers packaged with incorrect nuts are: k7147, k7329, k8683, and 104353. These parts were made for use on the various vehicles listed above.
  federal mogul will notify owners and replace the parts free of charge. The recall began on may 22, 2008. Owners can contact federal mogul at 877-489-6659.

• Recall 27926 [USA]: If the liftgate latch opens, occupants can be ejected resulting in significantly increased risk of injury or death.
  Vehicle description: light duty passenger minivans with remote release. This is not a safety recall. There has been no determination that a safety defect exists with the minivan latches. Chrysler is conducting a service campaign to replace the liftgate latch assembly of the affected minivans. In a collision, the rear liftgate latch assembly could open.
  chrysler dealers will replace the liftgate latch with a new stronger latch assembly without charge. Chrysler began notifying owners of these vehicles during july 1996.
  this is not a safety recall.There has been no determination that a safety defect exists with the minivan latches.However, chrysler has decided to provide a stronger latch without charge.Owners can contact chrysler at1-800-minivan (1-800-646-4826).

• Liftgate latches investigation # EA94005

  Liftgate latches in the subject vehicles have failed in low and moderate speed impacts resulting in occupant ejections, injuries, and fatalities.There are two failure modes associated with latch failure: fork bolt-detent lever bypass, and inertial unlatching.Nhtsa analyses of chrysler and peer minivans took the form of static and dynamic crash testing, engineering evaluations, complaint and field analyses, and real world data analyses.A technical report discussing these analyses can be found in the engineering analysis technical report in the public viewing file.Chrysler has advised nhtsa that it will conduct a campaign to replace the latches on all the subject vehicles.The chrysler campaign will consist of notification letters to all owners; free replacement of all latches with stronger, safer latches regardless of the age of the minivans; national advertising to promote customer awareness of the campaign; quarterly reports regarding campaign completion rates; as well as additional measures to ensure a high completion rate, above and beyond the actions that normally would be taken in a formal safety recall.These include follow up written and telephonic notifications to owners who do not bring their vehicles in for corrective action.Nhtsa firmly believes that the crash performance of the latches on these minivans raises serious safety concerns, and that the campaign addresses these concerns.The chrysler corrective action will increase the strength of the liftgate latches and decrease the potential for inertial unlatching, thereby reducing the risk of crash-related openings of the liftgate and possible occupant ejection through the liftgate opening.Owners of chrysler minivans will get a stronger, safer latch free of charge at the earliest possible time, without the need for further technical investigation and complex and time-consuming litigation with an uncertain outcome.Based on the above, odi is closing this investigation.

Driver noticed that the headlamp lenses are to bright. This causes poor visibility to the driver while driving. Vehicle was taken to the dealer for inspection, and mechanic determined that the headlamp assembly needed to be replaced. *ak

1988 plymouth voyager air bags did not deploy when hit on the driver's side.*mr the driver sustained a torn left rotary cuff, shoulder and back injuries. The driver referenced a clocksping recall. *nm

Consumer states that the turn signals would not work. Consumer has replaced multifunction switches put in and the problem is still occuring. Nlm consumer sold vehicle. *jg

The adjustable steering wheel constantly pivots up and down in the drivers hand. The automatic transmission lever is very difficult fto move unless the driver pivots the steering column up holding the steering column in place while moving the gear shift lever. The mechanic suggested that he could force a 1/16th thick, 1 inch wide piece of steel on the underside of the steering column assembly to artifically hold the steering column in a high position, overriding the adjustable capability of the steering column, to stop the problem of not being able to easily move the automatic transmission gear shift lever. The vehicle was taken tot he dealer to be repaired. When the consumer left the repair shop the vehicle began to smoke. The vehicle was returned to the dealer but they were unable to find the cause of the problem. The consumers vehicle no longer operates. *ph *nlm

When driving at night consumer cannot see, high and low beams do not work. There is a substance on light bulbs, preventing driver to see. Dealers is aware of the problem.*ak

Steering gear box gets stuck. *ak *ml

While operating the vehicle with the headlights on, a burning smell developed. This was followed by increasingly visible plumes of smoke which eventually impaired driving visibility. Flame was visible briefly. After disconnecting the battery positive cable, it was determined to be a class c fire as the smoke and fire diminished. The vehicle operated normally as long as the headlamp switch was not turned on. Upon examination by an automotive electrical specialist, it was found that a spring on the dimmer switch used as a variable resistor had separated from the switch housing making contact with the frame of the dash assembly causing a dead short to ground. As the spring was electronically resistive by design, it glowed to a bright orange igniting combustible materials around the switch. The local dealership for the afore mentioned vehicle was contacted. The parts department was not aware of any mandatory or voluntary recalls or service bulletins. However, they knew that the headlamp switch was upgraded with a solid state dimmer component (p/n 56021889-aa).

While driving windshield wiper motor malfunctioned, causing visual difficulty which may result in a crash. Dealer had to replace the windshield motor and pivot arms. Please provide further information. *ak
