Subject
ISX15 / QSX15 CM2250 and ISX12 / QSX12 CM2250 Engines have Fault Codes 1139, 1141,
1142, 1143, 1144, or 1145 caused by Fuel Pump Inlet Banjo Sealing Washer Debris
Issue
Engines have returned after previous repair with injector fault codes 1139, 1141, 1142, 1143,
1144, or 1145. The root cause of these fault codes is debris from the fuel pump inlet banjo
sealing washer, Part Number 3963988 or 3963991, blocking the fuel returned from the fuel
pump head. The most common occurrences have been noted with the following campaigns.




C1555 ISX15 CM2250 Fuel Pump Barrel/Plunger and Tappet Campaign
C1556 ISX12 CM2250 Fuel Pump Barrel/Plunger and Tappet Campaign
Verification
It is critical that fuel system clean care is followed during removal and installation of the fuel
supply line. The presence of debris can not be verified after it is introduced. The location
where the debris accumulates is not visible during service.
Resolution
When opening the fuel system, clean and inspect all components thoroughly. When removing
the fuel supply line from the fuel filter outlet and the high-pressure fuel pump, discard the
sealing washers as indicated in the service procedure. Before reinstalling the fuel supply line to
the fuel filter outlet and the high-pressure fuel pump, clean the fuel supply line banjo fitting with
contact cleaner to flush any debris that may have accumulated. Replace the fuel pump inlet
banjo sealing washers and install the fuel supply line. See Procedure 006-024 in Section 6 of
the appropriate service manual.
