• Recall 195517 [USA]: The misalignment may cause the propeller shaft to separate, causing a loss of power to the rear wheels and a stall, increasing the risk of a crash.
  Isuzu technical center of america, inc. (isuzu) is recalling certain 2015-2017 isuzu npr, reach, 2016-2020 isuzu npr xd, npr hd, nqr, 2016-2021 isuzu nrr, 2017-2020 chevrolet 4500xd, 4500hd, 5500hd, and 2020 chevrolet 5500xd vehicles. The u-bolt nuts for the rear axle may not have been properly tightened and may loosen, causing the axle to misalign.
  isuzu will notify owners, and dealers will inspect and, if needed, replace and tighten the rear axle u-bolt nuts, free of charge. The recall began on april 23, 2021. Owners may contact isuzu customer service at 1-866-441-9638. Isuzu's number for this recall is v2103.

• Recall 134192 [USA]: In the event of a fire, if the fire extinguisher does not function properly, it can increase the risk of injury.
  Isuzu technical center of america, inc. (isuzu) is recalling certain 2011-2017 isuzu npr, 2008 npr hd and npr-hd gas, 2011-2018 npr hd and nrr, 2015-2018 npr xd, 2012-2018 nqr, 2012-2017 npr gas and npr-hd gas, 2016-2017 chevrolet low cab forward 3500 hd, 3500 and 4500 and 2017-2018 chevrolet low cab forward 4500 hd, 4500 xd, 5500 hd and 5500 xd vehicles, equipped with certain walter kidde plastic-handle fire extinguishers, models fc10 and fc110. These extinguishers may become clogged, preventing the extinguisher from discharging as expected or excessive force may be needed to activate the extinguisher. Additionally, in certain models, the nozzle may detach from the valve assembly with enough force that it could cause injury and also render the product inoperable.
  isuzu will notify owners, instructing them to contact kidde to get a replacement fire extinguisher, free of charge. The recall is expected to begin in december 2017. During manufacturing, the stamping equipment may have created burrs on the frame crossmember that the transmission is mounted to. These burrs may cause the transmission mounting studs to break.

• Recall 124071 [USA]: A stuck open accelerator pedal may result in very high vehicle speeds and make it difficult to stop the vehicle, which could cause a crash, serious injury or death.

(isuzu) is recalling certain model year 2014-2016 isuzu npr and npr hd vehicles manufactured august 26, 2013, to november 11, 2016, and 2016 chevrolet low cab forward 3500 and 4500 vehicles manufactured july 27, 2016, to november 11, 2016. The affected vehicles have a throttle pedal that may get stuck under the steel stopper bolt, causing the pedal to remain in the full throttle position. Note: in gasoline models only.

(isuzu) is recalling certain model year 2016 npr-hd, npr-xd, nqr, and nrr trucks manufactured july 3, 2015, to september 2, 2015 and equipped with 5.2l diesel engines. The certification label on the affected vehicles may have typographical errors in the rim size, tire pressure and weight information that may prevent the owner from accurately loading and maintaining the vehicle. As such, these vehicles fail to comply with the requirements of 49 cfr part 567, "certification."

  On certain vehicles with gasoline powered engines, the accelerator pedal could move laterally, allowing the accelerator pedal bracket to be hung up on the stopper bolt on the floor panel bracket. This would cause the accelerator pedal to remain in the open position, increasing the risk of a crash causing injury and/or damage to property. Correction: dealers will remove and replace the stopper bolt with a flanged plate that will be bolted to the floor bracket to prevent lateral movement of the accelerator pedal.

  Issue: on certain trucks, the u-bolts that attach the rear axle to the suspension may not be properly tightened. As a result, the axle could slide back and separate from the propeller shaft. If this happens, there could be a sudden loss of power to the wheels. Safety risk: a loss of power to the wheels could increase the risk of a crash. Corrective actions: isuzu will notify owners by mail and instruct you to take your truck to a dealer to inspect and, if necessary, replace and tighten the rear axle u-bolt nuts.

• Unintended movement investigation # PE19005

  The office of defects investigation (odi) opened this investigation on april 19, 2020. Odi was made aware of seven consumer complaints related to model year (my) 2012-2015 isuzu n-series gasoline-powered trucks for unintended movement. Odi worked with isuzu and peer manufacturers to understand the severity, failure mechanism and frequency of the issue. Attached is a report outlining these details, in addition to the investigations chronology. This investigation is being closed with isuzu releasing a technical service bulletin (tsb), with owner notification, to resolve the unintended movement issue in my 2012-2017 vehicles. The closing of this investigation does not constitute a finding by nhtsa that a safety-related defect does not exist. The agency will continue to monitor the issue and reserves the right to take further action if warranted by new circumstances.Please review the attached closing report for full details.

On 2/7/2018 the driver of the truck reported that when he came to a complete stop the truck lurched forward moving the truck with the brakes applied. Driver felt unsafe as the vehicle could not be controlled for a few seconds. Truck was driven for 22 minutes when incident occurred.

On 1/23/2018 the driver of the truck reported that when he came to a complete stop the truck lurched forward moving the truck with the brakes applied. Driver felt unsafe as the vehicle could not be controlled for a few seconds. Truck was driven for 18 minutes when incident occurred.

On 1/3/2018 the driver of the truck reported that when he came to a complete stop at the bottom of the exit ramp of a highway the truck lurched forward moving the truck with the brakes applied. Driver felt unsafe as the vehicle could not be controlled for a few seconds. Truck was driven for 20 minutes when incident occurred. Vehicle was taken the same day to local dealer and they could not reproduce the same condition.
