  Equipment description: aftermarket brake rotors for use on certain ford and mazda trucks. Cracked castings caused cracks to appear on the outer diameter of the rotor hat or around the stud hole.

• Recall 15177 [USA]: If a crack occurs, it could result in one or both studs fracturing. the master cylinder could then separate from the vacuum booster, when the brakes are applied, preventing the service brakes from fully activating increasing the risk of a vehicle accident.
  The studs which attach the brake master cylinder to the power brake vacuum booster assembly can develop stress corrosion cracking after being operated for an extended period of time in the southern california location.

• Recall 16606 [USA]: When the above occurs, it can result in parking brake system ineffectiveness. the parking brake will not hold the vehicle, causing the vehicle to roll freely, if the transmission is not left in gear.
  The parking brake control self-adjust pawl does not line up properly with the self-adjust rachet causing the pawl to slip over one or more teeth in the rachet.

When the parking brake is applied rapidly, the self-adjustment pawl may skip over one or more of the teeth on the ratchet and the parking brake system may not achieve full tension. This could result in the parking brake not applying or diminished parking brake effectiveness. Correction: a lock-in wedge will be installed to lock out the self-adjust mechanism and retain the mesh between the adjustment pawl and the ratchet.

Tl* the contact owns a 1994 mazda b-series truck. While the vehicle was parked, it independently accelerated, crashed into a fence, and drove over the owner of the vehicle. The owner sustained fatal injuries. The vehicle was towed to an impound lot where the failure recurred, almost striking two police officers. A police report was filed. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was not available.

Approximately 4-5 months ago i began to experience engine surging while driving in town. Unable to find any vacuum leaks, i inspected the fuel filler neck to find it badly deteriorated in the area just above the gas tank. I replaced it with a new oem quality neck. At the same time i replaced the throttle position sensor, the coolant temperature sensor, rebuilt the fuel injectors and replaced the intake manifold gasket. Afterward, i did not experience any surging until a few weeks ago when, approaching a stop sign, the engine began racing so badly that i quickly shifted into neutral and steered to the shoulder of the road, narrowly avoiding a rear end collision. Since then i have driven the truck about 100 miles without incident. The truck has been driven 90,000 miles and well maintained. It may not be relevant to this problem, but this engine was built without an egr valve and egr vacuum solenoid. *tr

Tl* the contact owns a 1994 mazda b4000. The contact stated that when refueling, fuel would spill back out from the filler neck. She also noticed a fuel leak when the vehicle was in motion. The vehicle was taken to an independent mechanic and they advised the contact that the fuel filler hose was fractured and needed to be replaced. The vehicle was repaired. The manufacturer was contacted and they offered no assistance. The failure mileage was approximately 202,000. The vin was unavailable.

Tl* the contact owns a 1994 mazda B3000. The contact stated that he noticed fuel leaking from the vehicle. The contact inspected and noticed a fracture in the fuel line hose. The contact called both the dealer and the manufacturer and was told by both parties that the vehicle was not included in nhtsa campaign id number: 94v111002 (fuel system, gasoline:delivery:hoses, lines/piping, and fittings). The vehicle was not repaired. The current and failure mileage was approximately 189,000.

Tl* the contact owns a 1994 mazda b-3000 pick up. The fuel line has developed a crack. Consequently during fueling gasoline leaks from the fuel system. Fuel leakage in the presence of an ignition source can cause a fire. The current mileage was 323,000 miles.

The filler tube going to the gas tank ruptured. When i filled the tank, gas spilled on the ground. Fumes are always around the truck. I paid to have this tube replaced. The truck had 53 k miles. Now it has done the same thing at 74 k miles. I think that this is a very dangerous condition and is costly to keep repairing. *tr

Tl*the contact owns a 1994 mazda b400. The contact stated that fuel leaked from the vehicle and onto the ground while he was pumping fuel into the tank. His vehicle is not included in recall # 94v111002 (fuel system, gasoline:delivery:hoses, lines/piping, and fittings). The current mileage is 98,000 and failure mileage was 92,000.

Tl* - the contact stated that the front fuel line in his 1994 mazda B3000 was cracked and leaking fuel. He call mazda and gave them his vin but they stated that his vin does not fall under the recall. The contact would like the recall extended to include his vehicle. *nm

On long trips, often in cold, wet or snowy weather, the engine will occasionally rev out of control. This is not an issue of the throttle cable icing up because turning the engine off and restarting it (which is the last thing you want to do while still moving on icy roads) temporarily resolves the issue. Therefore, it seems to be most likely an electrical problem. No diagnostic trouble codes (dtcs) are registered in the computer's memory, therefore no information exists by which to diagnose the problem. I have been to dealers and they have said they cannot help me unless they can replicate the problem in the shop. As i said, it has only happened after many hours of driving on long trips, which makes it impossible to replicate in the shop. *nm
