    Odi opened this pe on november 13, 1995 based on an owner complaint concerning a september 1995 accident involving a 1995 1/2 ton gm pickup truck.The vehicle owner alleged the bolt used to join the 2-piece steering shaft was missing.The shafts then separated and steering control was lost. During the course of this investigation, odi has gathered information from staff at the following: auto collision insurance companies, third-party products liability industry, independent repair shops, involved vehicle operators, general motors corporation. According to gm, the steering column and shafts were changed for the 1995 c/k models at the start of the 1995 model year.The change was made to incorporate driver air bag systems for the c/k10/20 with gvwr of less than 8500 lbs.However the change was made for all c/ks. For the 1995 model year, the steering column shaft was connected to the upper intermediate shaft with a bolt through the column shaft and the universal joint coupling.No nut was required.This connection was made at the steering column assembly facility and the steering column is shipped to the vehicle assembly plant.This joint is located inside the vehicle cab.The upper intermediate shaft was connected to the lower intermediate shaft assembly is also connected to the steering gear at the vehicle assembly plant. The configuration of the upper intermediate shaft was changed at the start of the 1996 model year, with a small number of 1996 steering columns pulled ahead in the 1995 models assembled at the oshawa and ft. Wayne assembly plants.The universal joint of the upper intermediate shaft was replaced with a pot joint.This change was made to utilize a joint manufactured internally instead of externally supplied universal joint.The pot joint style connection to the steering column shaft was made with a through bolt and nut.This change was effective for 1995 vehicles assembled at ft. Wayne and oshawa in october 1995 and for all 1996 vehicles. Odis analysis found that, of the 24 field reports, 14 involve vehicles produced at the ft. Wayne assembly plant.All 7 of the accidents involve trucks built there, as well.However, because the ft. Wayne assembly plant produces approximately the same number of vehicles as the other subject vehicle assembly plants, this finding can not be explained on the basis of vehicle population and exposure.Odi, with the information currently available, is unable to explain this phenomenon.Odi does have information indicating that the problem occurs infrequently and very early in the life of the vehicle.For the 24 field reports, including those reporting accidents, a failure occurred, on average, 129 days and 3900 miles after the truck was built.Based on information from all sources, current through november 1995, odi has found that one incident has occurred involving a subject vehicle built since july 8, 1995, - a period of 257 days.The data reviewed indicates accidents, allegedly due to the subject failure, occur even earlier in accidents, allegedly due to the subject failure, occur even earlier in the life of a subject truck, at an average of approximately 103 days. Finally, based on the 115 warranty and filed reports combined, the incident rate is 12/100,000 vehicles.Of the 115 reports, 69 involve vehicles built at the janesville and ft. Wayne assembly plants (35 and 34 respectively).The janesville failure rate is 15/100,000 and the ft. Wayne rate is slightly lower at 14/100,000. The contact pulled over, turned off the engine, opened the hood, and noticed flames near the transmission area. The fire department was called; however, the vehicle was completely burned. I just recently began having trouble with my 2000 chevy malibu and it prompted me to being researching common issues with chevrolet vehicles. In my research i came upon an interesting fact that several makes and models seem to have intermittant turn signal functionality and hazard light issues. In my case my 1995 chevrolet suburban (which i love mind you) was being checked for a trailer to be hooked up. When the person performing the inspection attempted to turn on the hazard lights it caused a malfunction with all of my turn signals, breaks, hazards(no equipment was attached to the truck so no it wasn't a faulty connection to the trailer, that had not been attached yet.) after several minutes of messing with signals and pushing buttons it finally reset to normal. This funny wiring problem occurs every time i try to turn on my hazard lights. Up to this point i have not had a need to use them but i am aware that they will not work if i attempt to. My advise to anyone reading this is if you are having trouble with your turn signals mess with the hazard light button before you go replacing flashers and bulbs. It took several times of pushing the hazard button and turning the signals on and off before they started to work again. I am very disappointed with chevy that this issue has not be address now that i am finding it to seem so common a problem. I was even more disapointed to learn that my malibu has issues that are similar to others with that make and model and those issues are ignored as well. A side note if you read my other complaint. The 1996 chevy s10 my spouse and i were going to trade the malibu for also has fuel issues. What a bummer i used to really like chevy. Channel 9 news anyone. "nhtsa it shouldn't take someone dying before you make them fix it. If it is a known problem that can be proven is not normal ware they should have to fix it." *tr

There has been a recall for this issue. I bought the car used and have had problems identified by the prior recalls with gradual increasing difficulties (action numbers ea97022 and ea02006). I contacted the dealer with the recall information and was told that the part had already been replaced by the recall but did not have a replacement date. They noted that because the part had already been replaced and it had been over 1 year they are no longer under any further obligation. My problem now is that the replacement part has the same defect problems and creates the same hazards as the original part that was covered in the recall. I am going to have to get it fixed but wanted to file a complaint that replacement is just as defective and dangerous as the original part that was recalled. If the replacement part is recalled, i would like a reimbursement. *tr

When the brake pedal was engaged the vehicle would not slow down. The vehicle came to a stop within a half a mile after repeatedly engaging the brakes. The contact drove onto the emergency lane to unhitch the boat trailer from the vehicle, and resumed with caution. The vehicle was taken to an independent mechanic who stated that both front rotors had broken in half. The vehicle was repaired. Also, on a separate occasion the brake line exhibited excessive rust with loss of pressure. The contact was in the process of repairing the vehicle. The failure mileage was 78,000.

Tl*the contact owns a 1995 chevrolet suburban 1500. While driving approximately 55 mph in the rain, the windshield wipers failed to operate. Approximately five minutes later, the windshield wipers began operating while the switch was in the off position. Shortly after, the wipers stopped working and remained in the up position on the windshield. They remained in that position the rest of the day. The following day, when the vehicle was started, the windshield wipers began and stopped operating on their own while in the off position. She called a local mechanic and was informed that the electronic circuit board was defective and that there was a recall relating to her vehicle. The dealer stated that the vehicle was not included in any recall. The manufacturer has not been notified. The current mileage was 302,000 and failure mileage was 301,000.

Driver's side seat frame on my 1995 chevrolet suburban has broken and i'm hearing a trend of this happening to others with the same year / make vehicle. This could be a hazard because this happens when then vehicle is being driven and the seat becomes unstable.
