  • Desiccated air bag inflator rupture investigation # EA21002

    From 2000 through 2017, takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("psan") propellant -- propellant 2004 and propellant 2004l. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 blomquist report at www.Nhtsa.Gov/sites/nhtsa.Gov/files/documents/expert_report-hrblomquist.Pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in us vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004l that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated psan inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All takata inflators produced with propellant 2004l contain desiccant, and none of these desiccated inflators using propellant 2004l are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated psan inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- takata (now known as tk global), the independent testing coalition, and exponent -- have been studying the long-term behavior of takata desiccated psan inflators using propellant 2004l (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004l is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004l.The office of defects investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated psan frontal inflators manufactured by takata. This investigation will require extensive information on takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated psan inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.Gov/recalls?Nhtsaid=ea21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede ea15-001, which remains open.

    Odi received complaints regarding failing tie rod ball studs on model year 2008-2011 dodge ram 2500 and 3500 vehicles resulting in loss of steering control.Chrysler concluded that misalignment of the left (driver side) ball stud on vehicles built with the cross car steering linkage system could allow contact between the bull stud and the ball housing which could weaken the stud and result in fracture.Chrysler submitted a recall notice on august 15, 2011 involving certain my 2003-2011 ram 2500 and ram 3500 pickup trucks built or repaired with the cross car steering linkage.Chrysler will inspect, repair or realign the front end of the subject vehicles as needed.Chryslers announcement of a safety recall on this matter addresses odis concerns.The odi reports cited above can be reviewed at www-odi.Nhtsa.Dot.Gov/owners/searchnhtsaid under the following identification (odi) numbers:10332525, 10357937, 10366591, 10303744, 10377166, 10373240, 10343969, 10375403, 10324831, 10373546, 10370771.

My suspension/steering snapped and the dealer will not releases the vehicle to me and told me to call dodge since there is an open recall on it. I have called 3 times and nobody calls back. My truck has been at the dealer since september 2015.

The vehicle was towed was to the dealer and the contact was advised that the front driver's side tie rod had detached from the vehicle. The manufacturer was contacted and confirmed that a recall applied to the contact's vin under nhtsa campaign id number:10v657000(steering:linkages:tie rod assembly) but was repaired one year prior to the recent failure. The manufacturer offered to make the recall repairs on the vehicle for a second time. The vehicle was not repaired. The failure mileage was 73,000.

The dealer replaced the rear driver side rim, the rear and front driver and passenger side tires, and a replacement related to the rear axle however, the failure recurred. Proceeded to repair shop to have all (4) valve stems and (2) front tires replaced. Repair personnel informed us that this is an epidemic. According to them, in 2010, manufacturers became aware of this problem and began using rubber valve stem/tpms sensors. *tr

1st time electrical problem happened was under 36 thousand miles,, turned the truck off and it would not start or make a sound,, had it towed to bob williams dodge in rome ga.,,was told the problem was in main wiring harness plug. The second time electrical problem happened i was sitting still with truck idiling in a parking lot for 3 or 4 minutes then the engine just shut off and i started smelling smoke and burnt plastic,,, had the truck towed to ed voyles in marietta and they told me that it was the main wiring harness plug that shorted out again and caused the tipm and the fuse box and the circuit board inside the fusebox to burn a hole thru them the size of a baseball,, this happened at 96,000 miles,,,, so they put a band aid on it to get me out the door ,,,i asked them if they found the cause of the problem and they said they found the main harness plug shorted out but thats not the cause of the problem... The electrical problem is starting to occur again for the 3rd time at around 130,000 miles,,cruise control has quit working ,,tail lights and marker lights have quit working on truck,, trailer brakes and trailer lights have quit working,,, fuel guage will quit working and dpf full light will come on but i can move the main wiring harness plug head and the fuel guage will start back to work and dpf full warning will go off for a while,,,i need help with this ongoing issue please

After backing out of my garage, at an approx. Speed of 1 mph, i turned the wheels to the left to run an errand, and the drivers side tie rod end broke. The drag link dropped and the wheel cocked to the full left. At this point i don't know if it caused any other damage. I spoke with the nearest dealer and was told that there are no recalls on this vehicle at this time. Unfortunately i need this vehicle and had to order a new tie rod end at a cost of $116.00. Bearing any other damage, i will also have to get an alignment, at an unknown cost at this time. Thank god that i had not been traveling at a higher rate of speed when this part decided to fail, because i am sure the outcome would not have been good. *tr

Also, the wires burned a hole in the side of the fuse box. The vehicle was taken to a dealer where it was diagnosed that the wiring harness and plug head failed and needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was approximately 30,000.

Seatbelt in truck caught the door handle and locked so you cannot pull out more belt to get it off the handle. This also causes the door handle to be partially lifted which automatically unlocks and opens the door while the vehicle is parked or in motion. Even with the door cracked openas far as the belt will allow the belt will not release the handle and allow the door to open more then a few inches. If this was an accident a person could be trapped in the vehicle and not be able to get out without requiring additional equipment to cut the seatbelt or break a window. So far this has not been corrected on my vehicle we will take the truck to the dodge dealer tomorrow for now no one rides in the passenger seat. I suspect the seatbelt will need to be unbolted from the floor to unhook the belt off the door handle. I do have photos available and the issue can be replicated on the drivers and passengers doors. *tr
