Known Recall Investigations
  • Battery cable corrosion investigation # PE11015

    On april 4th 2011, the office of defects investigation (odi) opened preliminary evaluation (pe11-015) to investigate allegations of complete loss of electrical system accessories in certain wheeled coach ambulances. These vehicles are used in emergency response operations, and first responders are dependent on its reliability especially during life-saving situations. Allegations of electrical system failure received by odi include, but are not limited to, lights, sirens, and life-support equipment.The subject vehicles are equipped with a 400 amp fuse buss/relay (commonly known as a remote battery terminal) that is a central conduit for electrical power distribution to the patient cab area as well as other mission critical equipment.It was determined by the manufacturer that the failure mechanism involves the ford o.E.M. Secondary remote battery terminal.According to the manufacturer, the corrosion of the secondary remote battery terminal is due to road salt and other caustic deicing agents used as a road surface treatment in the northeast regions.The calcium chloride in de-icing agents deteriorates the copper compounds found in the 400-amp fuse buss resulting in an electrical short circuit. This short circuit can cause failure of critical systems in the vehicle constituting a safety defect.On september 8, 2011, wheeled coach industries submitted a part 573 defect notice indicating it would conduct a safety recall to remedy approximately 725 affected ambulances (my 2004-2010) operating in the following states: maine, new hampshire, vermont, massachusetts, rhode island, connecticut, new york, new jersey, pennsylvania, delaware, maryland, west virginia, ohio, indiana, michigan, illinois, wisconsin, minnesota, iowa, missouri, and the district of columbia.This investigation is closed with a safety recall 11v-470.Nhtsa will continue to monitor the recalled vehicles to ensure the remedy implemented by wheeled coach is adequate. The agency reserves the right to take further action if warranted by the circumstances.

  • Hvac switch failures investigation # EA10003

    This investigation was opened based on one complaint and reports from four transportation fleets alleging 182 blower motor control switch failures.Odi was concerned that these melting and burning switches could lead to an open flame that could spread throughout the vehicle.After a comprehensive review of all data related to overheating blower motor control switches in the subject vehicles, odi did not identify the existence of a safety defect trend.Odi reviewed 1,036 complaints, claims, and field reports of failed blower motor control switches and determined that only two fires were likely to have been caused by a failing subject blower motor control switch.This represents a failure rate of 0.2 r/100k on a population of 1,076,975 vehicles that have been in service for up to 14 years.Odi has found that when the subject switches do fail and begin to melt, they are constructed in a manner that typically prevents the failing switch from igniting and developing into an open flame.Ford motor company has agreed to take two actions to further reduce the likelihood an overheated blower motor switch in the subject vehicles.Ford informed odi that it has implemented a series of design changes to the subject switch that will help prevent it from developing internal high resistance short circuits.Ford has also informed odi that it would release a technical service bulletin (tsb-11-11-21) instructing subject vehicle owners to replace the electrical connector at the same time a failed switch is replaced.In light of the low rate of failure resulting in open flame and fire spread on the subject vehicles, and fords actions to reduce the likelihood of a switch failure through switch design changes and a tsb, further use of agency resources does not appear to be warranted.The closing of this investigation does not constitute a finding by nhtsa that no safety-related defect exists.The agency reserves the right to take further action if warranted by the circumstances.The odi complaints cited above can be reviewed at www-odi.Nhtsa.Dot.Gov/owners/searchnhtsaid under the following (odi) numbers: 10427875, 10114393, 10257890for additional information, see the attached closing report.

I was driving in to the drive way going into garage when the rv accelerated on it own. I tried to stomp on the brakes but the tires were spinning and i could not stop it, it was accelerating at a high speed and the rear end was fish tailing and the coach just keep going forward and crashed in to the rear wall and through it. I tried to put the coach in park but it was completely out of control. There are skid marks were you could see the tires trying to stop. I managed to put it in reverse and the accelerator was still ramping out and the coach fish tailed back wards and i finally got it slammed into park. I managed to stop just before hitting a natural gas meter. *tr

Yesterday i went to start a 2007 ford e-350 super duty cargo van with a 5.4l triton engine. One of the spark plugs was immediately ejected which resulted in ruining the plug and coil. I was able to re-insert the plug and tighten it up, but it is also the second time it has happened to me. Previously it also happened on a 1999 e-350 with the same engine and also the same cylinder. I traded the 99 in for the 2007 and in less than a month had the same exact problem. *tr

On 2007 ford e350 ten passenger van seatbelts continuously fall down through the seats. This is a serious safety issue and should be recalled for repair. This is an issue on every one of these vehicles. It is very, very difficult to fix these belts after they have fallen through the seats. A better system is needed to keep these seat belts in place. I have reported this issue to ford and have received no help from them. *tr
