P0401

Exhaust Gas Recirculation Flow Insufficient Detected

DTC P0401, sometimes indicated as P040100 is an OBD diagnostic trouble code indicating a generic fault with the vehicle powertrain (i.e. engine and gearbox). This code identifies an error with the auxiliary emissions system causing exhaust gas recirculation flow insufficient detected.

The exhaust gas recirculation (EGR) system is responsible for recirculating a portion of the exhaust gases back into the engine to help reduce emissions. When the EGR system doesn't work properly, it can cause the Check Engine Light to come on and result in decreased fuel economy.
There are a few things that can cause the EGR system to not work properly, including a clogged or dirty EGR valve, a defective EGR valve solenoid, or a defective EGR cooler.
If the EGR system is not working properly, the best solution is to have it inspected and repaired by a qualified technician.

Egr and egr cooler failed at 21,000 miles. Dealer fixed under warranty 3/22/16. Same parts have failed again at 46,000 miles and is out of warranty. Searching the web shows this is a common repeat failure for the 6.7 diesel and costs $2,000 for repair. Engine trouble code is P0401. The parts or design must be defective as they only last for about 20,000 miles.

2010 TOYOTA PRIUS with 100000. miles

Vehicle has lost power on the freeway multiple times forcing me to pull over. In one instance i was almost rear ended by a semi. The vehicle will show a temperature warning and will then shut down and read, "check hybrid system". The Toyota dealership claims it needs a new engine. This is a well known issue and will go in front of a judge in january. I have contacted Toyota about this and they tell me it is out of warranty and not eligible for the warranty enhancement. This should have been an unlimited mileage recall but Toyota refuses to do anything about it. This happened again this morning to my wife who was on a freeway where the speed limit is 70mph. This is an extremely dangerous error and i would hate to see someone die because their car stops on the freeway. This vehicle has also had severay P0401 codes, there is an warranty enhancement for this as well but limited to 150k miles. I was told by the Toyota dealership it was also a culprit for a blown head gasket.

2010 TOYOTA PRIUS with 95000.0 miles

My 2010 Toyota Prius iv has 98,731 miles on it. The engine and power train malfunctioned during a cold start so i had to take it into the Toyota dealer. It had to be towed to the dealership on a flat-bed tow truck because the car transaxle/transmission was locked in the drive position, would not go into neutral, and the car engine would not start rev above an idle. When i attempted to start it the engine acted like it was misfiring very badly and rattled very badly. It sounded like the engine was not getting any oil circulation so i didn't attempt any more cold starts. I could not get the engine into neutral or any other gear so i had it towed to the dealership. When the car was at the dealership i went through my records and found a letter titled 'warranty enhancement program notification - zf3' that i had received several years previous. The document extended my primary coverage until 31 march 2017 due to a potential faulty exhaust gas recirculation (egr) valve. 'a faulty egr valve would cause the engine to rough idle/rattle from the transaxle at a cold start due to a sticking egr valve. If this occurs, the vehicle may illuminate the check engine light and set diagnostic trouble code dtc P0401. If the condition is verified the vehicle will be repaired in accordance with the applicable technical service bulletin under the terms of this warranty enhancement program.' the symptoms my car experience during cold starts begin around 92,000 miles but no 'check engine light' or 'diagnostic trouble code' ever illuminated on the instrument panel. After 92,000 miles it only happened occasionally, and always during cold starts. Normally after a few seconds of idling the rattling would go away and the engine and drivetrain would perform as it should. I took my car in for a service and told the dealership what was happening, but they could not diagnose any problem.

2010 FORD EXPLORER with 90000.0 miles

Check engine light came on and the code was P0106 and P0401. I discovered plastic tubing failed by breaking (disintegrating really), all in the vicinity of a part that i suspect controls those functions. The part is a harness with 4 of these plastic tubes that connect to various places. The fact that they are disintegrating indicates the wrong materials are used. Ford motor company should have this issue addressed as a recall for defective materials. I have samples of the tubing that survived.

2008 MAZDA CX-7 with 39640.0 miles

The vehicle is a 2008 Mazda Cx-7 bought as a pre-owned certified from Mazda dealer in ramsey, nj. It had the engine light on and i took it immediately to the dealer near my work (colmar, pa) and it was diagnosed with code P0401, egr valve failure. With under 40k i was not expecting this to fail and the repair cost was $718. (diagnosis, parts and labor). I looked up online and saw that it is a frequent issue with people having much newer car than mine. Obviously it was not covered under warranty and i had to pay from my pocket. But i am disappointed with the repair as the next day the brake light came on. It comes on in the morning and stays on intermittently. It finally goes away after 15-20 minutes of driving. I plan to go to the dealer tomorrow to see if this is something that resulted during yesterday's repair.

2006 SCION XA with 60000.0 miles

However, this is not restricted to just this model and is caused due to a defect in the design of the fuel system, that allows water intrusion into the charcoal canister and the filter, leading to expensive repairs. Many other Scion models as well as other economy sized toyota vehicles share the same defect in the design leading to the service bulletin, which actually should be a recall so that this design defect can be fixed properly at the dealership by toyota. If neglected by consumers or if the fix is postponed due to it being so expensive, it can lead to serious safety issues related to the fuel system of the vehicle.

2006 SUZUKI GRAND VITARA with 180000. miles

I was accelerating onto a super highway when my vehicle engine light came on and the engine started running rough. When i returned home i put my engine scan on the computer and it stated egr valve....I believe it may have been a P0401 code. I ordered a new egr valve and replaced it along with numerous gaskets and cleaned everything according to my service manual......Which by the way, is the $100 shop service manual the dealership uses! It corrected the problem for about 6 months time period and then began all over again. I use premium shell gasoline in the vehicle and do preventative maintenance on a regular basis as described in the service manual. I have done significant research online and have found numerous complaints regarding this issue however, Suzuki has not responded with a solution. I see ford and some others had this similar complaint and redesigned their egr valves to resolve the issue. Also, i have received several Suzuki recalls however, there are no Suzuki service centers in america now to get theses issues resolved. The latest recall was sent to me from Suzuki stating the issue and that they would notify where to get it resolved......Soon?? Never heard another word! This is like 2-3 recalls now.....Air bags, power steering pulley, and several others. I did have the abs brake sensors and the transmission lock issues resolved at the dealership shortly after my purchase of the new vehicle however, the rest have been unresolved! This egr valve is either a defective engineering issue or related to ethanol added to the gas which is the biggest nightmare ever for any engine and should be stopped immediately as any mechanic will tell you!! It was a money maker for the ultra rich and nothing to do with resolving emissions or anything else as a defective egr will spew more pollution than not having it at all! Updated 09/21/15*lj

Tl* the contact owns a 2001 Mitsubishi Eclipse Spyder. The contact stated that the vehicle was taken to the dealer for a tune up and the failure codes for P0301, P0401, and the egr appeared. The codes were cleared, but the P0301 failure code reappeared. The vehicle was repaired; however, the vehicle was later taken in for smog inspection and failed. The vehicle was diagnosed for egr failure. The vehicle was taken to an independent mechanic. The technician repaired the problem, however, the failure persisted. The vehicle was taken back to a dealer but the failure was unable to be duplicated. The manufacturer was not made aware of the failure. The approximate failure mileage was 315,000.

2001 Ford winstar recall 09so09 they did not fix the problem. The dealer said they only needed to change out the cruise control connector!!! I asked about the abs and other parts listed to be inspected in the recall and he denied that any of that was to be inspected under this recall. So they may have fixed the cause of the problem but did nothing about the damage to the vehicle. My symptoms are: loss of brake fluid (problem # 1) burning smell or smoke (problem # 1) brake light on after brake service. (we think this back pressure breaks the deac switch) (problem # 1) brake light (red) (problem # 1 or problem # 2) 15 amp fuse #10 blowing out (problem # 1) speedometer and odometer stop working. (problem # 1) cruise control will not activate or deactivates without warning. (problem # 1) abs brake light (yellow) (problem # 2 or problem # 1) Ford trouble code C1185. Almost been in a couple accidents because the vehicle will not brake efficiently. Brakes lock sometimes when braking, causing vehicle to slide!!! Ford denied to my face that any of these problems were related to the recall, but your report and other investigations prove that it was and that i have been lied to about a very important safety issue that has already put me and my families lives at risk!!! I have a check engine light for P0401 code for clogged ege system and P1739 for the transmission code my transmission was already replaced this is the 2nd transmission from Honda. On august 2010 my check engine light came on i took the car to pep boys. They pulled up the 2 codes P0401 and P1739 for clogged egr system and transmission code. I replaced the egr valve and still the same code came up. My car is now out of warranty and there is a service bulletin that has to be performed to unclogg the egr system that is to Honda bulletin 00-009 to drill a hole in the manifold and install a pipe. This should have been done under warranty it is a known defect. The transmission is also bad a gain because it has the P1739 code. I want both of these fixed under the warranty.

Told by Honda that this was a recognized problem and a warranty extension had been issued, but since my car has 113k miles, it won't fall under the extension (80k) and it will cost me $370 to fix. 1997 FORD F-150

Vehicle was bought in 2/01 and had approx one week when it stalled everytime it decelerated. Dealership (e-z way auto sales) took vehicle back and replaced pcm at no charge to us. In 01/02 it started hesitating as if it were slipping out of gear. Does not do all the time-just unexpectedly.Had transmission flushed (80$-01/31/01). Still sputtering--replaced fuel filter and air filter over the course of 2 months--problem was not major and did not happen all the time-just when you least expected it. Then in march it just stalled while my girlfriend was driving it and daughter was in the backseat at 60mph! Took to local mechanic and they could not receive a diagnostic code-finally after two weeks of driving it to see if they could make it sputter or stall it happened and they receive a code for a egr valve which was fuel related--did not fix, cost 105$--took back to them and they kept an additional 2 weeks and said they gave up--could not diagnose. Took to local Ford dealer(tri-county Ford 05/02) and they had 3 days before it happened again. Replaced pfe sensor (192$-P0401) which i am told is the one which controls the valve that was replaced. Did not fix-returned to Ford dealer(05/02) and they had a few more days and received another code(P0605) and was told my pcm needed replacing. When i told the tech it was just done a little over a year ago he said it was probably a junked part. E-z way will not fix. They said that the warranty was expired and it was up to Ford. Ford will not replace part, have already been that route. Said if it had been repaired at a certified Ford dealer the whole time maybe they could help and all parts have 12mth warranty. E-z way offered 200$ toward the part but only if we take it to the place that originally replaced the part(which happens to be operated by the owner of e-z ways brother). 1996 HONDA ACCORD with 230000 miles

One day just like that i was driving home from work and i came to a red light so after coming to a complete stop the light turned green my car was on in drive and i accelerated and the car just wouldn't move. Got it towed and i got check engine light codes and it was P0401 and P0108 so i cleaned the egr valve and bought a new map sensor and installed it. The car had a new battery, the transmission oil was full and with no visible black color or smell, the car started but would not get into gear and just wouldn't move. Called a specialist mechanic and he brought his tool to check the whole computer part of the car wich is the next best thing to towing it to a transmission shop, the result came back inconclusive with almost certain transmission failure. After corona a lost of vehicle and job i am judt about to sell it as is cut my losses and maybe buy a bicycle for now. I just need to leave about one hour before work and buy a good axe body shower spray and attach life ones again.

1981 BUDD BUDD

Bought new in '01 ford focus. Noticed right off brakes would make a grinding noise and squeak. Tires were worn real bad on inside on one front size since we bought it. Repeated alignments with no improvement. Still experience this problem. Started experiencing problems with it missing during acceleration. Check engine light came on in february 2005 and took it to the dealership. They replaced sensors that had recall,was recall code P0401. Engine light stayed on. They said that we were using gas that was dirty and waiting too late to put gas into the car and that was why it was stalling. We paid $272.98 for them to replace this whole section that had to do with the fuel being filtered. Car loses power and stutters going around curves with even over a half a tank of gas. Stalling to the point we must pull over into the emergency lane afraid of accident! I own car i bought brand new and it jerks and sputters like some piece of crap from jd byrider! Everytime we start the car you smell oil burning for15 minutes afterwards. Had it checked no leaks or anything could be found. April of 2005 noticed when the car idled at a traffic light the thermostat would show shoot up showing it was running hot! Took it in to the dealership and they found anti-freeze leaking. Was told the thermostat housing and thermostat had to be replaced. Retested it and was told problem solved. Our bill was $375.00. This was first brand new car and was taken well care of, it only has around 78,000 miles on the odometer. I was told there was a silent recall on these for the engine problems and they were being fixed for no cost to the owner. We were not notified of a recall and paid for all of this out of pocket .Car is paid off now and cannot enjoy having a paid off car with low mileage because we are scared to drive it and bought a new car that was safe to drive around with our kids! Time goes on and it is getting worse. Sometimes not wanting to start at all! Engine still stalls despite all these repairs.
