  • Recall 66714 [USA]: As a result of the corrosion, one or both straps may fail allowing the fuel lines to separate from the tank, or in some cases, causing the tank to contact the ground. either scenario may result in a fuel leak presenting a fire hazard.
    Ford is recalling certain 1997 through 2003 model year ford f-150, 2004 model year ford f-150 heritage, 1997 through 1999 model year f-250 less than 8,500 lbs. Gross vehicle weight rating (gvwr), and 2002 and 2003 lincoln blackwood vehicles manufactured from june 20, 1995, through august 4, 2004, originally sold, or currently registered in, connecticut, delaware, illinois, indiana, iowa, kentucky, maine, maryland, massachusetts, michigan, minnesota, missouri, new hampshire, new jersey, new york, ohio, pennsylvania, rhode island, vermont, west virginia, wisconsin, and the district of columbia. Prolonged exposure to road deicing chemicals may cause severe corrosion of the fuel tank straps which secure the tank to the vehicle.
    ford will notify owners and instruct them to take their vehicles to a ford or lincoln dealer to have the fuel tank straps replaced with straps that have increased corrosion protection. Early in this campaign, if replacement straps are not available, dealers may install a cable support under the strap as an interim repair or a steel reinforcement over the existing strap as a permanent repair.   • Recall 69919 [USA]: In the event of an engine stall, a crash could result without warning.
    On certain heavy duty trucks equipped with 7.3l diesel engines, the camshaft position sensor located on the engine of the vehicle may function intermittently, possibly resulting in an engine stall.

  • Recall 72456 [USA]: If the trailer lights are not working properly, a vehicle crash can occur.
    Certain cequent 90 degree 7-way right angle adaptors sold by distributors in tow packages for aftermarket sales to tow 5th wheel rv travel trailers. Due to incorrect wiring of the harness, the trailer lights may not operate properly.
    all of the defective tow packages are still at the distributor level and have not been sold to any consumers.

  • Recall 47790 [USA]: Decreased lighting visibility may result in a vehicle crash.
    Certain maxzone headlight assemblies, depo brand, p/nos. 331-1129l-asn and 331-1129r-asn, sold for use as aftermarket equipment for the above listed passenger vehicles. These headlamps fail to conform to the photometric requirements of federal motor vehicle safety standard no. 108, lamps, reflective devices, and associated equipment.

    On certain trucks equipped with speed control, gasoline or natural gas engines, the speed control deactivation switch may overheat.

  • Recall 61611 [USA]: A fire at the switch could occur.
    On certain pickup trucks and sport utility vehicles equipped with speed control, the speed control deactivation switch may overheat, smoke, or burn.
    by letter dated september 12, 2005, owners were instructed to return their vehicles to their dealers to have the speed control deactivation switch disconnected. Owners who have had their speed control deactivated are being notified that parts will be available and advised to make an appointment to reconnect the speed control beginning in february 2006. Owners who did not have their speed control deactivated are being notified to have their system remedied beginning in february 2006. Owners are urged to avail themselves of the free disconnect service as soon as possible because of the significant risk of fire. Owners may contact ford at 1-800-392-3673.   • Recall 6519 [USA]: These vehicles do not comply with the requirements.
    Vehicle description: bi-fuel cng pickup trucks and bi-fuel cng vans, modified by cfi control systems, inc., fail to conform to the requirements of fmvss no. 303, "fuel system integrity of compressed natural gas vehicles." the label affixed to the fuel filler door by the vehicle modifier contains the inspection date and expiration date of the cng tank. However, it does not contain the statement "see instructions on the fuel container for inspection and service life."
    owners will be provided a label with the required statement, along with instructions for affixing the label at the fuel filler door. Owner notification began november 2, 2000.   • Recall 12208 [USA]: This can result in the loosening of lug nuts, stud fatigue failure, and the potential for a wheel separation from the vehicle, increasing the risk of a vehicle crash.
    Vehicle description: multi-purpose vehicles and light duty pickup trucks. The lug nuts on these vehicles may not create sufficient clamp load allowing wheel movement in relation to the hub/rotor mounting surface.
    owners of these vehicles will be receiving two notifications. The first notification, which began in may 1998, will provide an interim repair procedure until parts become available. This interim procedure can be completed by owners or their dealers. It specifies that the lug nut torque must be verified to be 100 lbs. Then the lug nuts need to be tightened an additional 1/8 to 1/4 turn more. There will be a second owner notification, which is expected to begin july 20, 1998, under which dealers will inspect the wheel studs and replace the wheel lug nuts.

  • Recall 13277 [USA]: The condition would not allow a disengagement of the transmission from the park position or affect proper neutral start switch operation. unintended vehicle movement could occur if the parking brake was not set.
    Vehicle description: light duty pickup trucks and multi-purpose passenger vehicles. If the automatic transmission shift cable assembly was not fully attached to the steering column bracket, the shift cable assembly can come out of the bracket. The vehicle operator would then not be able to shift the transmission from one of the drive gear positions into the park position even though the gear shift selector would indicate "park."
    dealers will make the proper attachment of the transmission shift control cable to the steering column bracket and add a tie strap to that connection.

  • Recall 2019420 [Canada]: Other
    Issue: this recall is for aftermarket replacement rear window glass produced by vitro automotriz. Certain rear window glass may not have been properly tempered. If the window breaks, the glass may separate into large pieces instead of very small ones. Note: this is a voluntary recall that is not being conducted under the requirements of the motor vehicle safety act. Safety risk: glass that breaks in to large pieces could create a risk of injury. Corrective actions: the company will notify consumers by mail and instruct them to inspect the designation code marked on the rear window glass. Products affected by this recall have the designation “dot 287” along with the lettering “vmr” (replacement market) and a date code that is before july 2019. This information is printed on the bottom corner of the glass. Consumers should contact vitro's customer service at 1-412-995-6457 or [email protected] to arrange for replacement of the glass.

  • Recall 2011281 [Canada]: Fuel supply
    On certain 1997-2003 f-150, 2004 f-150 heritage, 1997-1999 f-250, and 2002-2003 lincoln blackwood vehicles that were originally sold in or are currently operated in areas of heavy road salt usage during winter months (ontario, quebec, new brunswick, nova scotia, prince edward island, and newfoundland & labrador), the fuel tank straps could corrode and fail. This could allow the fuel tank to contact the ground, which may compromise the tank's integrity and permit fuel leakage. Fuel leakage, in the presence of an ignition source, could result in a fire causing property damage and/or personal injury. Correction: dealers will install fuel tank straps with improved corrosion protection.

  • Recall 2005028 [Canada]: Electrical
    On certain vehicles, the speed control deactivation switch may overheat, smoke, or burn, potentially resulting in an underhood fire. The condition may occur either when the vehicle is parked or when it is in operation. Correction: initially as an interim repair, owners will be instructed to return their vehicles to their dealers to have the speed control deactivation switch disconnected. As soon as replacement parts are available, owners will be instructed to return to the dealers for installation of a new switch or fused wiring harness.

  • Recall 2004164 [Canada]: Steering
    On certain vehicles, ford has issued a scheduled maintenance guide update which specifies that every 20 000 kms, the steering linkage, which includes tie-rod ends, should be inspected. The vehicle owner is asked to place the tie-rod end inspection procedure supplied with this maintenance guide update in the vehicle's glove box for reference and safekeeping. The vehicle owner is to provide the inspection procedure to their repair facility to help properly identify worn tie-rod ends. Performing the vehicle scheduled maintenance, including maintenance inspections, is the responsibility of the vehicle owner. Failure to replace worn tie-rod ends may eventually result in a tie-rod end separation that could adversely affect steering control. If the inspection reveals that a tie-rod end is worn and must be replaced, the repair will be at the owner's expense unless the vehicle is still covered by the basic vehicle warranty, or has an extended service plan coverage that includes these components.

  • Recall 1998095 [Canada]: Suspension
    On certain vehicles, the lug nuts on these vehicles may not create sufficient clamp load allowing wheel movement in relation to the hub/rotor mounting surface. This could result in loosening of the lug nuts, stud fatigue failure, and the potential for a wheel to separate from the vehicle. Correction: wheel studs will be inspected and wheel lug nuts will be replaced with revised versions.

  • Recall 1997180 [Canada]: Structure
    On certain vehicles, the automatic shift cable assembly may not be properly seated and could separate from the steering column bracket. Should this occur, the operator would not be able to shift the transmission from one of the drive gear position into the park position even though the gear selector would indicate "park". This could result in unintended vehicle movement if the parking brake is not set. Correction: shift control cable will be properly attached to the steering column bracket and a tie strap will be added to that connection as a redundant measure to secure the attachment.

  • Engine stalling investigation as of 20060622

    In a december 3, 2007 letter, ford advised nhtsa that it will conduct a safety recall to replace the cam position sensor (cps) in approximately 1.2 million subject vehicles to address reports of engine stalling.A new design cps with improved performance and durability will be installed as a remedy.Owner notifications will begin in december 2007. The subject vehicles are durable, full-sized, medium duty trucks commonly used for commercial purposes, rescue/emergency response, and commercial or recreational towing.Cps failure is comparable to unexpectedly turning the key off since the signal it produces is vital to the electronic engine control system.Cps signal loss terminates fuel injection resulting in an engine stall.Once stalled, the engine may restart right away, or may restart after a delay (typically 5 to 10 minutes), or may not restart at all.In addition to exposing the driver and other motorists to crash risk due to loss of motive power or vehicle disablement, engine stalling also effects the power assisted steering and braking. Through consumer interviews, odi determined that cps failures occurred without any form of warning, at any vehicle speed (50% at highway speeds), and under any driving condition, such as accelerating.Consumers reported that about a third of the vehicles failed to restart, with another third reporting delayed restarting.Half the vehicles that did restart experienced another stall on the same or a subsequent drive cycle (before cps replacement) re-exposing those consumers to the risks associated with a stalling event.In their voq reports, half of the odi complainants described difficulty controlling the vehicle due to lose of power assist systems, especially those who were towing at the time of the incident. The one alleged injury incident occurred in an intersection when a subject vehicle stalled while turning across oncoming lanes of traffic.Although unsubstantiated, the complainant alleged an injury to a child occupant during odis interview.The other crash allegations mostly involved low speed, loss of control incidents often caused by lack of power assist; no injuries are reported in these incidents, and property damage, if any, was minimal.Consumers also reported other incidents with significant safety risks, such as disablement in a lane or on a shoulder of a high-speed roadway or interstate, or extended disablement in remote areas during severe weather conditions. The population above is fords estimate of the 1.4 million subject vehicles produced that are currently registered.The ford complaint and warranty counts noted above are current as of fords last submission dated june 21, 2007; they do not include f-450, f-550, or econoline counts as these products were not formally within scope of the investigation when failure information was requested.Warranty data analysis indicates that about half the claims involved a stall while driving event ( fords assessment) and that poor cps durability was a longstanding concern.Ford reported that the new cps design should meet or exceed their 10 year, 150k mile life expectancy design requirement.

  • Spark plug ejection from cylinder head investigation as of 20050922

    On september 6, 2005, odi received a petition requesting that the agency investigate allegations of engine spark plug ejection in certain model year 1997 through 2004 ford vehicles with triton v-8 and v-10 engines.Odi received a total of 474 non-duplicative complaints on the subject vehicles where the complainant, or the dealer repairing the vehicle, reported that a spark plug detached from the cylinder and/or ejected from the engine.As of december 8, 2005, odi is not aware of any allegations where the alleged defect resulted in a loss of vehicle control, a crash, an injury, or a fatality in any of the 10,319,810 subject vehicles.In addition, odi is aware of only two incidents where the vehicle stalled without restart.Information contained in the odi consumer complaints and obtained from 72 telephone interviews with complainants showed the following:(1) 99% of the complaints were on my 1997 to 2002 subject vehicles.(2) most the complainants reported hearing a loud pop while driving or upon starting up the vehicle followed by a loud, repetitive clicking or popping sound.(3) many of the complainants reported that the popping sound was accompanied by some loss of vehicle power; however, in 99% of the incidents reported, the vehicle did not stall.In the very few incidents where the vehicle did stall, most vehicles could be restarted.(4) only a small percentage of the complainants cited that they smelled gas or a slight burning smell when the incident occurred.(5) in all but a very few incidents, vehicle damage was limited to the engine.In one incident, the complaint reported that the fuel rail was damaged and replaced after one of the spark plugs ejected from the engine; however, the complainant reported that the damage did not result in any type of fuel leak or fire.In another incident, the only incident where a fire was alleged, the complainant reported that no fluid leak was observed, but that a fire resulted after the spark plug had ejected from the engine and he had restarted the vehicle and driven to another location.None of the complainants reported any damage to the vehicle hood.(6) only two complainants reported that they observed what appeared to be some drops of fuel coming from the cylinder where the spark plug had failed or on the spark plug itself; however, each of these complainants reported that there was no smoke or flames as a result of his incident.As the petitioner noted, and odi¿s analysis showed, it is possible for a spark plug to detach from the engine cylinder threads in the subject vehicles.However, odi¿s analysis of 474 complaints describing such incidents found only a very few alleged any safety-related consequences.None of these showed any evidence of a serious safety consequence.Given the large population and relatively long exposure time of the subject vehicles, the complaint analysis indicates that the risk to motor vehicle safety from the alleged defect is very low.In view of the foregoing, it is unlikely that nhtsa would issue an order for the notification and remedy of the alleged defect at the conclusion of the investigation requested in the petition.Therefore, in view of the need to allocate and prioritize nhtsa¿s limited resources to best accomplish the agency¿s safety mission, the petition is denied.

  • Engine compartment fires investigation as of 20050322

    Ea05-005 is closed with ford¿s actions in recalls 05v-017, 05v-388, and 06v-286, recalling approximately 6.7 million vehicles equipped with texas instruments speed control deactivation switches (scds).The brake systems in these recalled vehicles generate a vacuum that can potentially cause the scds to fail and, in certain switch installation orientations, catch fire.Ford is also including the entire population of 1998 explorers.Ford has informed odi that testing to determine the cause of failures in the 1998 explorers will continue after this investigation is closed.Odi believes that the vehicles exhibiting the factors causing scds failure described in this report correlate well with the observed failure rates on these vehicles by model and model year.The closing of this investigation does not constitute a finding by nhtsa that a safety-related defect does not exist in the non-recalled vehicles manufactured with scds that are not included in ford¿s recalls.Odi will continue to monitor the non-recalled population for incidence of engine compartment fires.The agency reserves the right to take further action if warranted by the circumstances.See attached closing report for details.

After the old straps were removed the tank started to leak fuel and ford nor the dealer would replace the tank. If the straps are covered for corrosion then the tank should be also due to the corrosion transferring to the tank surface. The dealer placed new straps over a rusted tank. The truck will need over $1200.00 in repairs for safe operation and to remedy the now leaking fuel. *tr

The evening of july 26th, 2011 approximately 10:00pm est, an engine fire occurred in our 1998 ford f-250 truck (ext cab, triton v-8 engine, lariat ed). The f-250 had been parked in a remote parking area on the interior area of the mason county fair grounds. Due to county fair activities during that day, the truck had not been started for over 10 hours when this fire occurred. The local fire department was called and extinguished the fire. It totaled the engine, buckled the hood, blistered the paint, etc. Luckily there was no property damage, nor was there any bodily injury. We had purchased the truck in april of 2010. This truck was in pristine (eat off the engine) condition. Since we have owned it, it had regular oil changes (mobil-1 synthetic only). Aside from that, and other normal maitenance, there had been no modifications to the vehicle, which had stock eqpt when we purchased it. No-off roading, my son babied this truck. He did not even like going to his girlfriend's home with it, because she lived on a dirt road. He is more than just left without a vehicle, he is heartbroken, to say the least. I have checked that there was a police report. 911 was called, but ultimately it is the fire department that made the report. I notified ford on july 27th, 2011. On july 30th, 2011 i received a letter from maria aguilera, product claims legal analyst, ford's office of general council in dearborn, mi. We are working to gather information required by her at this time. *kb

The forward (front) gas tank strap failed and the gas tank fell to the pavement causing the release of approximately 22 gallons of fuel. While starting my ford truck with the 5.4 l engine i heard a very loud pop and the engine ran as if the head gasket blew. A non-ford mechanic verified the no. 3 spark plug on the passenger side blew out and stripped the threads from the cylinder head insert that this ford dealer inserted to repair an earlier blown spark plug. The old insert for the previous failure was removed and replaced with a new type according to the ford mechanic. This is the second time for this to happen. On july 10, 2003, approximately 1 yr and 2 days before this failure the no. 3 spark plug blew out stripping the treads in the cylinder head. Numerous non-ford mechanics i've talked to have verified this been a fairly common occurrence in the 5.4 l ford engine in trucks and suv's. One side of truck sat low, shortly after the other side sat low. Rear end airbag suspension gave way as well as the springs. Air bags were replaced twice with in 3 month period. After recieving truck from repair shop it was noticed by body shop that frame on left side was cracked. This crack was in front view of service people and not detected. I do not recall the crack from the previous time the air bags were replaced. I do not recall the crack when i dropped the truck for second replacement of airbags and springs. The truck frame on left side is held together by two inches of connecting steel and i have been advised not to drive the vehicle. This truck is my livelyhood and only means of transportation. Ford body shop will not weld or repair frame do to liability. They recommend new frame. After discussion with service people and body shop manager it was revealed that ford made changes to the frame in the 1999 year. This is why in the 1998 f-250 a disiel engine was not available. Other words the 1998 f-250 was found to be a weaker frame than any other year. Also, the design of the airbag suspension system of this truck is weaker than even the f-150 models. The springs are of lessor materials and lessor strength. This was explained to me that this was done do to the airbags capability to make up the difference is support. My view is that all these components work in unisonand when one system failed the others followed. The repairs i have done are not remedying the problem and i have paid $ 2000.00 and much lost time. There is a defect in the frame and ford needs to correct this problem. .*ak

Have had 4 failures in the rear suspension system. First time, the traction bars were making a horrible metal-metal creaking/grinding noise; traction bars were replaced. Second time the entire bracket bolted to the frame came loose, sliding and banging around on the frame; tractions bars, traction bar brackets, and traction bar bolts all replaced. Third time air spring (air ride suspension) leaked down and caused the truck to pull to one side (truck was not level; driver's side rear rode very low, passenger side front rode very high); air spring was replaced. Fourth time the traction bar (passenger side) broke; service center not open, plan on taking it in tomorrow. Rear window seal has been 'fixed' twice, yet continues to leak water into the cab, last time shorting out some aftermarket electronics behind the rear seat. Braking at highway speed (50-75) results in a front-end vibration, rotors were turned by the dealer recently due to a front disc brake problem that resulted in the inside pad wearing down to bare metal and much grinding (wear indicators are on the outside pad and were not engaged (squeaking); so they were not indicating pads needed replacement). The other side pads were worn non-parallel to the pad backing (pad was thicker on one edge than the other).

Both doors of vehicle exhibit a fatigue crack on the outside of the vehicle near the window area, rear edge, about 3 inches up from the bottom of window frame, crack begins at the rubber weather seal, then progresses rearward horizontally for about ,5 inches. We then checked my buddies 1997 f150 and his had it too. He then check 3 of his friends, and they also had the cracks. Seems that auto body shops know of this problem, check it out seems to be a high percentage from a very small sample. *ak
