• Recall 134188 [USA]: In the event of a fire, if the fire extinguisher does not function properly, it can increase the risk of injury.
  Isuzu technical center of america, inc. (isuzu) is recalling certain 2011-2017 isuzu npr, 2008 npr hd and npr-hd gas, 2011-2018 npr hd and nrr, 2015-2018 npr xd, 2012-2018 nqr, 2012-2017 npr gas and npr-hd gas, 2016-2017 chevrolet low cab forward 3500 hd, 3500 and 4500 and 2017-2018 chevrolet low cab forward 4500 hd, 4500 xd, 5500 hd and 5500 xd vehicles, equipped with certain walter kidde plastic-handle fire extinguishers, models fc10 and fc110. These extinguishers may become clogged, preventing the extinguisher from discharging as expected or excessive force may be needed to activate the extinguisher. Additionally, in certain models, the nozzle may detach from the valve assembly with enough force that it could cause injury and also render the product inoperable.
  isuzu will notify owners, instructing them to contact kidde to get a replacement fire extinguisher, free of charge. The recall is expected to begin in december 2017. • Recall 124551 [USA]: A stuck open accelerator pedal may result in very high vehicle speeds and make it difficult to stop the vehicle, which could cause a crash, serious injury or death.
  Isuzu technical center of america, inc. (isuzu) is recalling certain model year 2008-2009 and 2012-2013 isuzu npr and npr hd vehicles and 2008-2009 gmc w3500 and gmc w4500 vehicles. The affected vehicles, equipped with a gasoline-powered engine, have a throttle pedal that may get stuck under the steel stopper bolt, causing the pedal to remain in the full throttle position.

• Recall 75065 [USA]: This condition could result in a vehicle crash.
  Isuzu is recalling certain model year 2012 n-series trucks equipped with gasoline engines, manufactured from april 27, 2011, through july 26, 2011. Due to a calibration error in the ecm software, in the event of a failure or malfunction of the brake pedal switch, application of the brake pedal will not cause the cruise control to be canceled.

Correction: dealers will inspect and tighten the rear left wheel bearing hub locking bolts as necessary.

• Recall 2011321 [Canada]: Electrical
  On certain vehicles, the electronic control module (ecm) software may not detect a brake pedal switch failure. If the brake pedal switch malfunctions, application of the brake pedal would fail to deactivate the cruise control. The driver would, however, be able to disengage the cruise control by other means such as; pressing the cruise control main switch located on the instrument panel, shifting the transmission into neutral or another gear, lowering the speed of the vehicle by more than 48 km/h (30 mph) through braking and/or shifting gears, or by turning the engine off. Failure or delay in disengaging the cruise control can increase the risk of a crash causing property damage and/or personal injury. Correction: dealers will update the ecm software.

  On certain vehicles with gasoline powered engines, the accelerator pedal could move laterally, allowing the accelerator pedal bracket to be hung up on the stopper bolt on the floor panel bracket. This would cause the accelerator pedal to remain in the open position, increasing the risk of a crash causing injury and/or damage to property. Correction: dealers will remove and replace the stopper bolt with a flanged plate that will be bolted to the floor bracket to prevent lateral movement of the accelerator pedal. Note: this recall is an expansion of recall 2016-545.

• Recall 2013212 [Canada]: Electrical
  On certain vehicles equipped with an oil pan heater, over time, dust, dirt and moisture may collect at the contact point between the oil pan heater cord and the extension cord. As a result, the extension cord could overheat, which may melt the oil pan heater plug and/or block heater plug (if equipped). This could result in a fire causing property damage and/or personal injury. Correction: dealers will inspect and clean the oil pan heater plug and the engine block heater plug (if equipped). Heat damaged plugs will be replaced. In addition, dealers will replace the extension cord and advise owners to store the cord indoors when not in use.

• Unintended movement investigation # PE19005

  The office of defects investigation (odi) opened this investigation on april 19, 2020. Odi was made aware of seven consumer complaints related to model year (my) 2012-2015 isuzu n-series gasoline-powered trucks for unintended movement. Odi worked with isuzu and peer manufacturers to understand the severity, failure mechanism and frequency of the issue. Attached is a report outlining these details, in addition to the investigations chronology. This investigation is being closed with isuzu releasing a technical service bulletin (tsb), with owner notification, to resolve the unintended movement issue in my 2012-2017 vehicles. The closing of this investigation does not constitute a finding by nhtsa that a safety-related defect does not exist. The agency will continue to monitor the issue and reserves the right to take further action if warranted by new circumstances.Please review the attached closing report for full details.
