    • Diono - Recall administration concerns Defect Investigation # AQ18003

      On september 14, 2017, diono llc (diono) submitted a part 573 recall report to the national highway traffic safety administration (nhtsa) describing various noncompliances with federal motor vehicle safety standard (fmvss) no. 213 in 519,052 child restraints manufactured by diono (nhtsa recall no. 17c-003). On july 19, 2018, nhtsa opened audit query (aq) 18-003 to better understand and evaluate dionos compliance with its safety act obligations. On july 20, 2018, nhtsa sent an information request (ir) letter to diono requesting information as part of its aq18-003 investigation.Nhtsas inquiry led it to determine that diono had violated several provisions of the safety act, and regulations thereunder. Diono did not dispute that certain violations did occur. Rather, diono asserted that the principal violations were committed by past employees and prior ownership. Diono represented that a new chief executive officer (ceo) formally joined diono in november 2017, and that since that time, the organization has been overhauled and new policies and processes have been put in place. Nhtsa and diono entered into a consent order on january 19, 2021, to resolve aq18-003. As part of the consent order, diono agreed to pay a civil penalty of one hundred and twenty thousand dollars ($120,000) and agreed that an additional civil penalty of three hundred eighty thousand dollars ($380,000) (abeyance amount) would be deferred and held in abeyance pending dionos satisfactory completion of the requirements of this consent order. Failure to comply with the safety act or terms of the consent order may result in the abeyance amount or portions thereof coming due.In the consent order, diono also agreed to several performance obligations. Diono is required to have a consultant advise and assist diono to develop a training plan and to train diono employees on topics such as the safety act and regulations. The consultant is also required to conduct an audit of dionos compliance with the safety act and regulations thereunder. Diono is required to submit to nhtsa a written report from its consultant detailing the factual findings and recommendations of the audit.Diono is also required to provide a detailed statement to nhtsa regarding any new or revised procedures that address the result of the audit and the recommendations of the consultant. Diono has agreed to repurchase or replace seats recalled under nhtsa recall no. 17c-003, for those owners who need to use the seat in a harness mode for children over sixty-five (65) pounds with a lap belt only and who do not have a tether anchorage available, under certain terms and conditions. Diono also agreed to provide supplemental reports to nhtsa relating to its early warning reporting, by providing a statement to nhtsas trends analysis division (tad) if it does not have any information to report for that quarter. The consent order also requires diono and its consultant to have quarterly meetings to discuss the actions diono has taken to satisfy the terms of the consent order.Based on the consent order, this audit query investigation, aq18-003, is closed. Closure of this aq has no bearing on the terms of the consent order, including dionos ongoing performance obligations or legal obligations, and does not represent a determination by nhtsa as to dionos performance under the consent order to date.

    P255 GOODYEAR FORTERA HL with 28266.0 miles

    During a scheduled maintenance at my dealership, located in carrollton, ohio the repair technician discovered that all four Goodyear fortura hl size 255/65r18 tires that were original on my vehicle, were cracking on the sidewall near the tires bead. I learned that this particular tire, the same make, model, size, and defect, was on a recall from Goodyear. My vehicle, a 2016 chevy traverse lt awd was taken to the local Goodyear dealer. The dot number on my tires is 4bxmaror or dr. The dealer determined that my tires were manufactured after the Goodyear recall had expired. So the issue here is that the Goodyear tires on my car are having the same defects as the expired recall tires had. To me that means there are many Goodyear tires that are defective driving the roads. This presents a serious safety issue due to these defective tires blowing out at or near the cracking sidewalls. The miles on my car when the defect was discovered was 28,266. I ended up purchasing a set of cooper tires and was charged a pro-rated cost of $362.95. If i purchased the same Goodyear Fortera Hl tire, i would have paid a pro-rated cost of $608.00.I felt and still feel that Goodyear owes me a new set of tires at no cost due to having the same defective tires as the expired recall tires. I wrote a letter to the Goodyear tire & rubber company ceo, who passed it off to their customer service office who offered no remedy other than the pro-rated cost. In closing, the main issue here is that there are currently defective Goodyear Fortera Hl tires size 255/65r18, that are being driven on our roadways by unsuspecting consumers. There needs to be an immediate total recall on these tires to prevent potential loss of life.

    The front tire on my volvo v70 developed a crack on the side wall by the wheel rim under normal usage conditions less than a month after i bought the car. I took the car back to smythe volvo of san jose where the purchase was made. The sales manager, michael, refused to address the problem or even take a look at my car, and initiated a challenge for me to sue volvo. The service manager initially promised to look into resolving the problem for me with volvo corporate, but ultimately fell thru. I escalated the issue all the way up to the ceo of volvo north america, and they said that they don't cover the tire under the warranty. The tire manufacturer, Pirelli, has also refused service so far. The problem is with the design of the tire. I did a brief research on the internet, and pulled up 18 pages of mostly complaints from other volvo and audi owners who have had similar problems with their tires. The side wall of this Pirelli P6 tire develops bubbles and cracks easily if it runs into any road irregularity. I did a search on other tires of similar configuration and found no such problem. Pirelli designed and manufactured the tire. Volvo engineers and management choose the tire to put on its v70 wagon as an oem. Now both of them are disclaiming responsibility via finger pointing and technicalities on their warranties. They have given me the usual run-arounds, and in one case, outright rude treatment.*ak

    My 2002 nissan altima has complete loss of power while driving on three separate occasions, twice i lost control of car and miraculously avoided a crash. I have been told that the problem was repaired the second time having it towed to the dealership. I no longer feel safe placing my children in this car. This car is a death trap, i'm surprised the government (lemon law) requires a total of 5 times same occurrence,report after the third time, then the dealership has another shot of repair #4, only then, they will consider replacement or buy back , after 5th time of engine failure. This to me is russian roulette, the worse part is , i'm paying for it (car pymt.)who is it that actually chose the criteriia for the lemon laws? And whose favor are they in ? The consumer and their families at risk, or the multi-billion dollar a year car manufacturers? I want a safe car!!!! I can not find the discount tire corporation ceo listed in the letter with the state of texas comptroller's office or the dill air controls company either. Is the letter i received legitimate or just a sales gimmick?

    5 days after paying harr toyota of worcester 2000.00 for a 60,000 mile check up, brakes and my third set of run flat tires. I experienced a blow out on the highway which was caused by improper installation of the tire. This was noted by toyota of nashua who then replaced the tire which again blew out 60 miles down the highway. This is the third set of tires i have had with 56,000 miles on the car. Not including the two i had to replace due to inferior work. This is insane and very dangerous. My family's lives were endangered twice in one day! Every toyota mechanic i spoke to complained about the tires. After getting no satisfaction from either dealership i wrote a lettter to yuki funo the ceo of toyota who kindly sent the letter back to harr who has still done nothing! Unbelievable that they are allowed to sell such an inferior product at such a high price that is not only expensive but dangerous!!! Also unbelievable that a ceo of a company is aware he is endagering people's lives and does nothing about it.

    I am the owner of a 2001 hyundai elantra. I purchased the vehicle new. Less than one year ago, my air bag light came on. I took it back to the dealer and they had to replace it because it had "dried up"! They replaced the bag. Less than a year later, but now my warranty is just up, the light is back on. I have notified the dealer, the consumer division of hyundai, the ceo of hyundai of my concerns and all they have offered to do is split the part, only, with me at 50%. I do not feel that this is a responsible solution for a company to take on a part from the manufacturer that is less than a year old. It has to be a safety issue to have faulty Airbags! I have never had an accident with the vehicle, it has never been inflated...Why would it "dry out"? I have a couple of other issues, but i don't believe they could be safety related.     I am ceo of palmetto state wholesale located at 1403 success way, florence,s.C.2950. I purchased five "e" series 450s and one "e" series 550 ford trucks with 6.0 diesel, 2003 and 2004s and from the start these trucks have spent more time in the shop than on the road servicing our customers. These six trucks have been repaired over and over for the same problems such as brake problems, electrical issues, oil pressure problems and injector issues to name a few. The issue that concerns me the most is that these six trucks will lose power while in operation without warning. These trucks as they are operated can drop in speed of 65 to 70 mph down to 35 to 40 mph in a matter of seconds. These six trucks manufactured by Ford Motor Company not only will lose power at times but will stall making them almost impossible to handle as they move off the roads to safety. The ford "e" series trucks aren't only a problem for palmetto state wholesale and the customers that depend on our services but these trucks are a danger to my drivers and to the public as these trucks are driven on major public roads everyday. The issue that angers me even more is that these trucks have been repaired over and over again for the loss of power as well as many other non life threatening issues and psw was informed at the end of last month(may) that ford motor has known of latent defects manufactured into these trucks and it wouldn't matter if psw would bring these trucks in on a daily basis the root cause to the issues relates to electrical and mechanical problems built into the trucks. I was also informed that the reason for no re-call by Ford Motor Company is due to the magnitude of the cost associated with such a re-call. On june 14th i wrote a letter to mr. William clay ford,jr. And shared this information. I am forwarding a copy of my letter to mr. Ford to the nhtsa address in washington, dc.

    1.The clock on the dashboard: the time is running almost 1 minute faster per day. If you dont fix it for 10 days, it would run 10 minutes ahead. 2.The drivers seat: when i purchased this car, i asked for power seats but the dealer didnt have it. I was a little uncomfortable with the manual seat, but the sales person convinced me, so i decided to take the one with manual. Since then, ive felt that the drivers seat is always sinking down so i consistently have to pull up the stick to push up the seat quite often while im driving. 3.Rear seat: i had to fold the rear seat once to load something in the back. After, i tried to unfold the seat but it didnt go back. I brought this car to the dealers service on nov. 15th, 2004. While one of the service persons was trying to unfold the rear seat, he had to cut the seat belt and has scratched the seat and broken some piece on the seat. He said hes going to order some parts and it would take a few days. Ever since then, nobody knows the status of this order. Both the service dept and the customer care dept, in headquarters have always been telling me its on backorder with no further information, and they dont know when for last six months. I asked them if you dont have the part, even in germany, how long will it take for the manufacturer to make the part? And they keep saying is dont know and we are still checking the status of the part everyday. In a common sense, the period of four months should be enough time to make the part and ship it to the dealer. I also sent a complaint letter to the ceo of the dealer/mckenna & vw north america headquater a couple of months ago. But still they don't know about the part.

    9999 U HAUL AUTO TRANSPORT

    In the fall of 2008 i bought a uhaul 17157 3 to 2 wire taillight converter--an adapter to hook trailer lights up to when towing a trailer. It was installed by a uhaul technician. In the fall of 2009, i started having very strange electrical problems: my brake light and tail lights would fail intermittently-sometimes they would work, other times they wouldn't. Directional signal lights on my dashboard would also light up for no reason. I thought i had an electrical short and spent weeks looking for it. Eventually i took it to a mechanic who told me the problem was the uhaul box. He removed it and the problem was fixed. I contacted the ceo of uhaul, joe shoen in phoenix, asking for reimbursement for the cost of the box, plus the money paid to the mechanic, and the amount of a traffic ticket i got for having no brake lights. I was told they would not even reimburse me for the cost of the box because it only came with a 90 day warranty. Clearly the box was junk to have only a 90 day warranty, but, for safety's sake, it should obviously be designed that if it should fail, the lights and electrical connections of the car it was installed on should still work properly. It was not designed that way and is a safety hazard.

    Took my car in 10/23/15 to have the tires rotated and was told by the shop i needed new tires that they were almost bald and the thread indicators were showing on all 4. My tires had only 20k miles on them and the warranty was for 60k miles. Shop said they would contact Falken tires and call me back. Falken jerked the shop around for over a week passing him from dept. To dept. Red flag!!!! In the meantime i checked w/ several other shops and in online forums and reviews. All said they had they're tires wear out in 6-8 months or more. The shops said because of the wear problems they wouldn't sell them and Falken had shortened the warranty to 40k and 5 years. Many submitted claims and were denied 2 or 3 times. The shop was told by Falken for me to buy new tires and they'd reimburse me if they found a problem w/ the tires. Am disabled on a fixed income and can't do this. What good is a warranty if the company is going to ignore you? I don't trust Falken the company or the brand based on their deception and not recalling a defective product based on many many problems. These are Falken ziex 912's size 205/65/r15. Things remain right at a stalemate, but i have sent a letter to the Falken ceo.

    The computer malfunctions. It is available for inspection. The computer lights up and causes failure in the trailer brakes. Glenn polk the dealership in sanger texas has been working on a fix but has yet to come up with one. I have spoken with general motors representatives several time trying to resolve the problem without result. I also emailed the ceo last friday and have heard nothing. There were no warning lights until seconds before failure.

    2020 CHEVROLET SILVERADO 1500 with 4660.0 miles

    2020 silverado lt with only 4660 miles was parked in a neighborhood drive way where our 17 year old son had spent the night with a friend. He started the truck to leave and immediately had several alarms; brake system failure, drive no faster than 62 mph, service trailer brake, service esc. He tried to ease out of the drive to test brakes, about 5-10 mph; he had no brakes, zero brakes! He rolled to the edge of the driveway and turned the truck off. After about 5 mins he started the truck again and the brakes were working and alarms were cleared but check engine light remained on. This was a very dangerous situation and could have led to much worse ending. After researching online, we found that this is very common and gm has done 2 recalls on this problem yet if you aren't aware of it, you, your family or other innocent drivers could be killed because gm and the nhtsa have not moved strongly enough to warn owners, there should be a nationwide 'do not drive' alert issued on these vehicles. I have contacted my attorney to see how to proceed and our son will not risk his life in this truck again. No brakes and no emergency brake!! Would gm's ceo let their 17 year old drive these dangerous trucks? The nhtsa needs to take immediate dire action on this, look at your own website at how many complaints there are on this exact problem! My attorney read your site and he says you all can't deny knowing the problem exist, take action before drivers die. Gm and nhtsa are being negligent on this and i will gladly testify in any class action lawsuit and unfortunately there will be some. Gm should do the right thing and immediately advise all drivers of 2019-2020 chevy silverados to stop driving their new $40,000 trucks and call a local dealer to come get it. Drivers who read this please be advised, this is very dangerous and some gm driver will die because of this brake failure.

    2019 BMW X4 with 11827.0 miles

    On may 03, 2019, i bought a 2019 Bmw X4 m40i, at braman motorcars. This is not my first Bmw, we have 3 more Bmws at home. Unfortunately, this Bmw X4 vehicle has not performed well as expected! On may 27, 2020 i was driving while it was raining when the car suddenly stopped and never turned on again, it was completely useless and very dissatisfying. I am very frustrated and disappointed because i sent the car to braman Bmw service center to check it out and request the warranty and they said that the warranty does not cover it the solution is just to pay more than $30,000.00 in repair plus parking fees. Also, my insurance carrier said my policy does not cover mechanical issues. Alse sent a letter asking for help to Bmw ceo and they responded that the warranty do not cover " outside influences". So this grabages car cant be in a rain. So now i have more than 1 and half months without the car, also called Bmw financial services for help and the solution was that i can return the car signing a voluntary repo damaging my credit history.     2019 CHEVROLET SILVERADO 1500 with 5000.0 miles

    Repeated attempts by jones chevy, sumter sc, they have not identified the vibration problems with my new vehicle. 22+ days in repair, multiple visits, and gm classifies harsh vibration as "normal characteristic" and closed the case, saying its final. I have contacted gm ceo with certified letter, discussed with exec. Customer support team, and posted on social media to find any solution. Gm customer service is the worse i have ever experienced. So far dealer has conducted transmission drain and flush per a tsb, changed torque converter, evaluated vibration with pico scope, and problem still exists and persist. Now today it's back at dealer for a prop shaft swap, from a new truck. My vehicle now has 6k miles, and problem started at 5k after the drain and flush. Gm says the vibration is normal because of the dfm, dynamic fuel mgmt. System. I say bs. They are wrong, and no vehicle should vibrate at low or high speed to the point you feel it through the seat, steering column, steering wheel and harmonic distortion in the cab. Gm knows what the problem is, they just don't want to admit. Next step, arbritration? Any feedback is much appreciated to solve this vibration issue. Gm - do the right thing and issue a recall before someone gets hurt. Remember the ignition problem? This problem isn't going away.

    2019 VOLKSWAGEN ATLAS with 5900.0 miles

    Upon accelerating my 2019 vw Atlas down an empty street between 10 to 20 miles per hour the autonamous emergency brake sensor malfunctioned and the car suddenly came to a complete stop jolting us forward. It was so unexpected that my hands slipped off the steering wheel and landed on the dash board. The halt came with a very loud noise. As i explained to the dealership technician and vw corporate office regional case manager, there was no vehicle in front of me. There were no pedestrians, animals, obstacles, etc. On the roadway. The day was sunny and the grill emblem that houses the system sensor was unobstructed and clean. If there would have been a vehicle behind me i would have been rear ended. The dealership told me that "they could not duplicate the issue" and per corporate engineers the sensor is working per specifications and no fault codes were found with the sensor. Imagine receiving this explanation after that scary incident happens to you out of no where. Now i can't get back into the vehicle without thinking when it will happen next. Too have written a letter to the ceo of Volkswagen seeking a resolution to the problem. My thought is that it's a problem with the sensor's software code and Volkswagen doesn't have a solution. Please i ask that this issue be investigated and a recall be issued.     2019 PACE JOURNEY with 4000.0 miles

    I bought trailer in february 2018 new, i lost side screws early on and continue, lost passenger side fender and hit another vehicle on interstate, lights kept blowing fuses, trailer failed vehicle inspection in september, entire suspension rebuilt but problem was never corrected. Issue actually is the way trailer axle hangers were welded to frame after talking to axle supplier and discovered after three private company inspections. Trailer was taken back to factory on december 27th 2018, however manufacture doesn't care to repair states they don't see issues i'm speaking about when police even saw some of what my concerns were. Police report as well as state inspection and sales invoice is in writing and can also be obtained if needed. Other original fender is showing signs of stress cracking and welds on trailer are beginning to show extreme stress related to improper axle install. I contact ceo of look trailers about my concerns, waiting for a response.

    2018 TOYOTA CAMRY with 30000.0 miles

    In deep white sands desert car keys locked inside the trunk. After opting for the passenger side door using the hanger anchor method whole car computerized system went down and there is no way to open then trunk from inside the car. 2018 Camry has no lever any pull-down seats to open the trunk. Park ranger took me to the nearest town Toyota dealership(desert sun Toyota,3333 n white sands blvd, alamogordo, nm 88310), the dealership was not having the ability to cut the new laser key they were not even aware 2018 you can't open the trunk by a lever or pulling down seats. It was cold and dark and family with kids sitting in the dunes alone while i was working with Toyota dealership for a new keys. After doing all brainstorming the last solution came to me was make a hole behind number plate to open the trunk latch. A good man agreed to come to the park and open the trunk by drilling the hole. I was able to make my family safe. After reaching dallas (my home) i sent an email to Toyota north america ceo james about the whole incident and his office called me after the email but not accepting this as a defect and denied to help me to fix my holey car. The 6 hrs i spent with family to just open the trunk is a serious defect in the 2018 Camry. This is very common to lose keys while taking stuff out of the trunk. If the happens on nowhere lands and no cell phone signals then it is dangerous.

    2018 HONDA ACCORD with 26600 miles

    The cmbs (collision mitigation braking system) had another failure. This is due to the radar sensor being blocked and then other safety systems in the vehicle having cascading failures. Last winter, the radar sensor had failed catastrophically and i had to take the vehicle in to the dealer where they, supposedly, performed some sort of an adjustment. During a phone call with a Honda representative, the representative claimed that "this would never happen again." however, what happens now is i get an error message claiming that there is a failure in the cmbs system, every time the radar sensor is blocked, directing me to "take the vehicle to the dealer." hence, she lied. Furthermore, the failures indicate Honda's failure in producing a safety system that is reliable in a wide range of weather conditions. Honda ceo recently claimed that Honda will be producing level 3 autonomous vehicles. That is horrifying, considering Honda can't even make a cmbs and a radar sensor that does not fail constantly. Please, please, please make Honda recall all the models which use the radar sensor and the cmbs that is similar to the one in the 2018 Accord before someone gets really hurt. Please.

    2018 RAM 2500 with 6010.0 miles

    The fca has issued a v06 recall for the drag link on my vehicle. As stated in the recall, the outboard steering linkage jam nut may loosen, allowing one end of the drag link to separate. I checked my truck and one jam nut was loose. According to the recall, if the torque values meet requirement, the nuts will be welded to the adjuster sleeve. If the torque values do not meet requirement, the drag link assembly will be replaced. I then called my dealer and he confirmed that they could not weld it. Unfortunately, he also stated that no replacement parts were currently available. So where does that leave me? I emailed the fca ceo and eventually was told that i needed to drive the truck to the dealer for inspection. I was appalled that i was suppose to drive the truck with a loose drag link and that they couldn't do anything else to help me until i did so. I even sent them pictures of the loose drag link, yet they will take no action. I do not want to put my life, or the lives of family members at risk to drive the truck in to be told the same thing i was over the phone. Some people have posted online that they have been reimbursed for an aftermarket drag link or the dealer has replaced the drag link with a newer version. I'm hoping to have one or the other done.

    Tesla's ceo, mr. Elon musk, has made so many fraudulent statements in the past 10 years. Yet, no government agency including the sec, has done anything about these deliberate, fraudulent and obvious statements. Please consider the following fact: consumer report today announced that it is now recommending Model 3 of tsla because the braking problem has been solved. According to an article that i read today, a senior engineer who has been working on this and similar issues for the past 28 years, indicated that the retest that consumer report conducted was on dry asphalt. According to him, now that the caliber has changed, it is very likely that the braking distance on wet and snowy asphalt has been impacted negatively. According to him, this is a fundamental issue which endangers public safety. Let's get Tesla, consumer report and an independent third party to test the braking distance on wet and also snowy asphalt, and see if those distances have been affected negatively. Again, according to an expert with 28 years of experience, it is very likely that the new calibration has affected the braking distance negatively on wet and snowy asphalt. Also according to this expert, Tesla keeps rushing and doesn't conduct enough safety tests that is standard in the industry. Tesla and consumer report have fiduciary responsibilities to the world to conduct these tests as well. It's a matter of public safety. The car manufacturing industry has been around for over 100 years. Also, according to this expert,this is the first time that a car manufacturer has been able to reduce the braking distance of its car on dry asphalt over the air by using a software recalibration. The expert is warning that we are definitely in unchartered territory, and these tests absolutely must be conducted to ensure public safety for the world.     In 2018 i had a cracked windshield, they told me a small rock hit my windshield, had it repaired at the m benz dealership, then in 2019, august i had the same thing happen to my windshield, they said the same thing, had them repair it again, september 2019 & october 2019,had a cracked windshield with an explosive sound, had it repaired, that's 3 in a row at the same dealership, they brushed me off after i complained to them, saying it was " pure bad luck" 3 in a row in aug,sept, & oct 2019.The vehicle was in motion on surface roads & a highway too at the time of occurrence, it was a small sound followed by a explosive sound & cracks about 10" long appeared on my windshield, the dealership was totally unhelpful, spent $10,000/ repairing it, & when asked "did anybody have a similar experience , 3 months in a row" they said no.1 complained to the service manager, saying the glass was defective, but to no avail, so i talked to the m benz distributor in georgia , but he was evasive too.     2017 CHEVROLET SILVERADO 2500 with 13758 miles

    Truck had 13758 miles and a/c would not work in temperatures above 100 degf when sitting at idle. While moving the system would cool a little. Initially took to all american chevy of midland and they could not replicate the problem. Took the truck to a second dealership, sewell gmc of midland and they were also unable to replicate the problem. I took the truck back to sewell gmc of midland and they were able to find the problem. The fan clutch was replaced. They were unable to repair it under warranty since it was 4 months out of warranty with 29599 miles. I had to pay for 40% of the repair cost despite the issue first occuring while in warranty. But because the two dealerships could not diagnose a bad fan clutch i was stuck with 40% of the bill. Gm customer service involved, gave me a $100 service voucher. Letters sent to ceo and vp of global manufacturing. No resolution because the "factory trained technicians" were unable to find the problem.

    2017 JEEP COMPASS with 28000.0 miles

    I bought my car in 2018 brand new with less than 10 miles. In the first few months, it required a brand new radio system and rear camera replacement. In some months following that, i have had my car stop on me while driving, approximately 4-5 times with only excuses as to why the car would unsafely do this. First time was an update that needed to be done. Second time was the keyfob battery but accused it of being lack of oil changes (i don't need to go to the dealership for oil changes). Next, was no explanation at all. Last was yesterday when it died twice in one day in the middle of the road. They called to let me know that it probably is because my oil is 3/4 way depleted even though i had an oil change done less than 3k miles ago. Their suggestion is that i return every 500 miles so they can top me off if the oil seems to be an issue. These are thankfully in less busy streets mostly driving straight but once while turning. Once included an error code that i was told was an electrical system error but it didnt register. I have pictures and videos on my phone from the incidents.     My vehichle had been making a noise and started letting out a gray smoke from te exhaust. I took it into the dealership and they said it was leaking "oil" into the motor and i needed a new engine. That they did not know where the problem was coming from therefore Ford approved a new engine replacement sice i am still under warranty ( 31,000 miles). Even if a new engine is replaced. The fact that they do not know where the vehicle is leaking oil from still does not rule out that this problem can happen again and when. Also every time i come to a stop my car turns off and does not turn back on when i release the brake. When is Ford going to finally say recall these vehicles???? I have young children i drive every single day.. My children's safety in on the line here. Do Ford ceo's realize that? Do Ford ceo's have children? Grandchildren?     I am trying to have gm reinstall the safety feature, lane departure warning. Gm recently literally took away the haptic warning function recently by "updating" the software in the 2017 Suburban. On page 298 of the owner's manual it states that when the car drifts to the left or right, the lane departure feature will either beep or there will be a rumble in the seat on the left or right side depending on the drift. This is a safety feature i paid for and like very much. It is very useful to be subtlety reminded that the car is drifting to the right or left. I have contacted customer service twice to no avail.     Recall campaign # 18v-755 exhaust gas recirculation (egr). I had waited 7 months to get the fix completed. I wrote a letter to the ceo of Bmw usa. They had a representative assigned to my case. I told them that this defect is a safety issue for my family and gave then 30 days to fix it which expired on july 15th. They inspected it which doesn't show failure but it is still a concern as it says - "it would increase the risk of a fire". They still don't have a fix that has been broadly rolled out. I am in fear of driving this vehicle. I have asked them to buy back my vehicle as i am the original lessee. They have refused to correct the problem as they are prioritizing the fixes to their resale vehicles and not vehicles already on the road.     2017 BUICK ENCORE with 95.0 miles

    I had just leased this car, drove less than 2 miles from home when my brakes failed. I applied brakes to stop, but the car would not stop, it slowed to 14 mph - kept moving until i slammed into truck approx 3-400' ahead of me, had plenty of room to stop. Pretensioner failed - driver seatbelt collapsed around me while locked in place the second i slammed on brake. Driver seatbelt was on passenger side - visa-versa - gm rep saw the seatbelts at my home - i showed them to him. He told me which belt belongs on which side and i told him they were on the opposite sides. He picked each up and showed me the 'r-l' on the end of the belt and the "r" was the collapsed belt. The gm rep said the 'intact' belt would have bruised the person, it did, my passenger. This can be verified by my passenger. Airbag sensor dispensed gas during same accident-airbag did not deploy. The car had over $10k damage, fixed - gm rep checked car because i continually saying the brakes and seatbelt failed to anyone at west point that would listen. After checking car he stated nothing was wrong. Different dealership diagnostic equip showed airbag was fixed but seatbelt never recalled, then the story changed that it was. The diagnostic info printout showed where i 'braked' but then no info recorded anywhere. My passengers seatbelt worked, i was thrown into steering wheel and was injured. I have been in contact with gm ceo with letters and the claims dept who tell me they can't help me. If my lease wasn't recalled and i apparently had the above issues, they should have fixed them before my receiving it. They would not even speak to my passenger, officer at accident, tow truck driver who saw the seatbelt hanging out my door at the scene. I am still being treated and only want fairness and a different car. This one has had so many little problems and oddities besides what i am discussing.

    We have 3 or more of these Polaris vehicles that are stalling. The dealer can not figure it out and Polaris refuses to help us from the ceo down. This is a huge safety hazard since here in arizona they are on the highway and when stalled we almost had someone killed.     2017 VOLKSWAGEN PASSAT

    I own a 2017 Volkswagen Passat sel premium. One of my primary concerns is that the autonomous emergency braking system does not function according to page 271 of the owner's manual, or as promoted by their dealers, and in tv commercials. I took my Passat to two dealers, both supposedly checked it and said it tested 100% a week later, the car did not slow at all when i was close to a vehicle in front of me going 65 miles and hour and it slammed on its brakes. If i had not been paying attention, a serious accident would have occurred. I wrote a letter regarding my concerns to mr. Hinrich woebcken, ceo of vw of america. The response from vw was that my system works fine according to their dealers, and they will do nothing further for me! After that, i did some research and discovered the results of front collision prevention testing that was done by the insurance institute for highway safety. Their tests on 2017 cars included those of over 20 manufacturers. There are 3 different parts to the test: forward collision warning (on the Passat this is a two car symbol in the instrument cluster) low speed auto brake (this is for speed under 12 mph) high-speed auto brake (this is for speeds over 25 mph) the results of these tests shows that all comparable manufacturer's sedans either scored 2 or 3 points on the high-speed auto brake test. However the vw Passat scored "0" points, and was unable brake the car or reduce the car speed at all! The 2017 vw touareg also failed the high- speed auto brake portion of the test. I feel that vw needs to inform owners, and stop advertising that their cars have a functioning high-speed auto braking, as they do not! They should quickly come up with a fix, and recall all 2016 to 2018 Passats, and other models that have this faulty system! Drivers have a false sense of security for a system that does not work!

    2016 CHEVROLET MALIBU

    The first incident happen on 2/27/2019. I was existing the highway on highway 75 in kennesaw, georgia going north bound and coming off on barrett parkway. As i exited the highway my brakes was stuck and my brakes wouldn't press. I panicked because i was going at least 60 mph trying to slow down to get on the exit with semi trucks in the front and in the back, because i was a little skilled with cars i down shifted my car and stepped on my emergency brakes and ran the car off the road into the grass. I towed the car to my mechanic in hiram, ga. (donnie's automotive) to have him work on the issue as i wasn't sure what was going on. I got my car back on 3/1/2019. Now the next incident happen on 8/16/21 my engine decreased and my car wouldn't accelerate more than 40 mph. Took it to my mechanic again and it was the throttle sensor. Now the most recent incident happen 6/12/21 where my brakes failed to pressed again and bc i was aware of this incident happening in 2019 i did the same thing and ran my car off the road. My kids were scared. Now all these issues have been a issue with chevy and they even had a class action lawsuit against them for the throttle sensor issue. My car wasn't part of the recall but my car has suffered the same issues they've know about for years. I called thornton road chevy and they told me it's nothing they can do. I've also emailed the ceo and all others for chevy. I want chevy to repair my car before this car kills me or my kids.

    My 2016 chevy Malibu has 93,000 miles on it. No issues until this day. Last week i was driving it in a neighborhood about 40 miles from my home. When i got to the neighborhood i had to apply my brakes. The brakes were locked at top of the pedal. I was unable to stop. Scared the [xxx] out of me! Dropped the people off. Drove 3 blocks to a 7 11, scared to drive any further. Parked and the car died and would not start. Waiting 3 hours for the tow truck, finally got the chevy Malibu and myself home a 5am. Had a mobile mechanic run a diagnostic on it. Multiple issues came up. Everything from multiple brake issues and even more engine issues. Had to towed to a chevy dealership near by. Brake tech ran a diagnostic, said not brakes it's an engine issue. Engine tech says it 'maybe' timing or cam shaft. $2500 on a maybe, best they can tell from the diagnostic. From all the reviews i have read about this very same issue and the guessing game at owners expense. None of these have fixed the issues. While reading the reviews, i read 3 people have died from this very same issue. In addition gm has remove any and all warranty on this vehicle above the 36,000 miles. Gm will not take any responsibility for the chevy Malibu death trap which they are fully aware of. No recall, no repair support for the dealership that now is involved with this worthless product. My question for gm is how many people have to die before they will do anything about it? I say to the ceo of gm. I want one of your family members to have my car as there daily drive. But wait the engine will not even turn over. Nothing but a paperweight now. To all head honchos at gm i have large car parts the need to be placed were the sun don't shine.     The vehicle currently has 15,593 miles and 2 of the continental tires have developed bubbles and have been replaced. Vehicle leased new in june, 2016. Both continental and gm claim they do not have a problem and have disregarded my concern. The tires were replaced by the chev dealer at my expense with goodyear tires. I still have 2 continental tires on the vehicle and have 22 months remaining on my lease and i am very concerned. Gm and continental have a big problem they are ignoring.     2016 FORD EXPLORER with 8200.0 miles

    Carbon monoxide leak coming through back vents. I have literally felt fuzziness, nauseous, intermittent severe headaches, difficulty changing lanes from confusion, and loopy while driving. Called my freeway Ford, and they verified that my car did in fact have exhaust coming through back vent. They fixed under tsb - technical service bulletin, not an official recall. I assumed that i had something severely wrong with my health until the news stories came out about the police officers passing out, and having some of the same symptoms. Bingo, it was my car! Ford should be ashamed of themselves for not putting out an official recall. There motto is to go further, but they are simply sitting back while they compromise the safety of drivers.     My mom purchased a 2016 chevy Trax for me at Chevrolet of homewood, 18033 s. Halsted, homewood, il on 6/13/16 under her name. Within the 1st week of purchase the brakes began squeaking while in motion. After the 2nd week the brakes began squeaking loudly as onlookers responded to the noise they say were coming from the back brakes. I took the car to Chevrolet of homewood on july 7, 2016 for brake inspection. Service test drove the vehicle, heard the noise from brakes but said it was normal. On 7/10/16, the brakes were squeaking so badly i took my car to firestone on elston & addison in chicago, for a vehicle inspection. Firestone noted the brakes were squeaking & told me to take the Trax back to the dealer as the brakes were squeaking all the time. I emailed ceo mary barra on 7/22/16 to complain & she set me up with antoinette baker under case# 8-2249264993, who worked with me until 7/27/16, & without notice turned me on to gm's legal nicole rodella. On 10/27/16 @ 11:03am nicole rodella, called to inform me that gm was gathering information from dealer to determine my claim & she would call me in a few days. On 11/15/16, nicole called telling me she needed an affidavit from my mom to further discuss my complaint. I asked to speak to her supervisor. Gm's ms. Barber (supv.) told me my mom needed to sign an affidavit for them to deal with me re: the complaint, although both names are on gm's paperwork. I sent barber dealer paperwork with both names of me and mom, & barbara said that was not enough on 11/15/16. Barber told me she would have nichole get paperwork from dealer & call me back on 11/16/16 & never did. I called barber back on 11/16/16, left message & she has not responded.     I bought that suv on 12/9/16. On 17/1/17 i delivered (dealer) it for repair, i mean, to function for first time the interior (soffit) lights. On 21/2/17 they delivered (to me) the unit, after installed a new fuse box (sent from the manufacturer). On 4/ix/17 (night) from the left side of the soffit began to outburst smoke/burning smell. At the end, the left side of the soffit and the base of the sun protector (with mirrior) were burned. Next day, i took the vehicle to the dealer, and they have it yet. I wrote a letter, with copies to the Ford ceo, vp customer affairs, vp quality control and dealer ceo. Later i file a claim at the local consumer affairs off.     2016 DODGE DURANGO with 3000.0 miles

    Autonomous acceleration. The vehicle without driver assist accelerated twice when pulling into parking spots. In both cases i was making a left turn into a parking spot when the acceleration commenced. Only by standing on the brake while at same time placing the vehicle in park and pressing engine off button was i able to stop vehicles forward motion. The Durango came close to ramming parked car in the space in front of the one i was pulling into. Durango had approx. 3000 miles on odometer when the 2 incidents of acceleration occurred. The incidents occurred on consecutive days, oct 9 & 10, after driving the vehicle approx. 6-8 hours straight each day. Purchasing dealer was contacted immediately to inform it's service department of the 2 incidents. Vehicle was taken to an out of area dealer on the morning following 1st incident. They read the fault codes and informed me that nothing registered other than the ecm had broken contact. Upon the occurrence of 2nd incident we cut short our trip and returned to our home so that we could take vehicle to purchasing dealer, suburban chrysler in farmington hills michigan. After two days in service the report that came back was no problem found. My concern is that this acceleration could occur during a more problematic situation where ability to control Durango will be limited which could result in bodily injury or property damage. Letters of record have been sent to [xxx], ceo Dodge, and [xxx], service mgr at suburban chrysler in farmington hills mi. I have taken photos of brake & gas peddles to show there relation to a weather tech which the dealer has suggested as a possible cause for acceleration. Consumer states there were 2 more occurrences. Dash board went completely blank during last occurrence.     2016 BUICK VERANO with 45.0 miles

    I purchased my 2016 Buick Verano on july 23, 2016. That evening while visiting friends, my wife and i left after it became dark. As we started to drive we noticed we could only see out of the lower quarter of the windshield. If i put the high beams on viewing becomes better. But you cannot drive around with the high beams on, all the time! I cannot drive this vehicle at night for fear of having an accident and hurting my family or another family in another vehicle. Tried to trade but they wanted to deduct $10,000.00 of my vehicle. Gm has no concerns for their customer's complaints as i even sent a letter directly to ceo of gm. In one communications with gm they told me, there was no fix for this and case was closed.

    2016 FORD EXPLORER with 21000.0 miles

    2016 Ford Explorer sport - fully loaded 3.5l eco boost. Within weeks of buying we noticed an exhaust/sulfur smell in cabin when climate control on and front + rear air going with recirculate on as well. It started while accelerating/passing gear on hills/grades but now does it all the time...Even when stopped at a red light! We have taken it to dealer four times while they have had my Explorer for over 50 days total. Dealer will not acknowledge smell even though we have taken multiple test drives with portable carbon monoxide reader showing extremely high levels with smell present & recording the incidents at the same time (Ford tech in vehicle with us witnessing the co reader levels). Corporate Ford engineers are denying and downplaying odor & co readings. We have been dealing with this for over four months - Ford has yet to provide a loaner but they want me to pick up my car when it's not repaired but they deem "safe". Quite a liability don't you think? We are "extreme commuters" so we are in our Explorer at least 15 hours a week and now experience migraines, drowsiness, being nauseous, and my wife is currently being tested for cancer. Please include the 2016 Explorer models with your investigation so my family and i can be healthy + safe while driving. We implore the nhtsa to help us and others with families that Ford is willfully exposing co poison to and they are not acting to remedy this situation on their own with your intervention. I invite Ford ceo's along with their families to drive my car that they claim is 'safe' for a month to see how they feel being unprotected and laid bare to their own contaminants.

    2016 TESLA MODEL S with 200.0 miles

    While test driving a Model S at a Tesla store, sales rep told me to remove my hands from the steering wheel while on auto pilot. Car suddenly swerved to the right at highway speeds causing the car to leave the regular lane i was traveling in. The Tesla rep instructed me that i did not have to keep my hands on the wheel which left me unprepared for the sudden swerve. Name/address are left this complaint off due to concern about the retaliatory nature of the Tesla ceo. Tesla ceo's overly defensivebehavior has a chilling affect on reporting issues.

    2015 DODGE JOURNEY with 10.0 miles

    I have recieved 3 recalls and all of them the manufacturer just says to contact dealer for vehicle inspection and part repair. Its been 3 mos and poway Dodge dealer said to wait till i got another recall notice that parts have been distributed. I am wondering if i need to have the accident before the company accepts responsability for repairs or worst deaths in my family. I have a two year old and an 8 year old to protect mr. Ceo/vp. First it was the breaks, then seat belt, now navigation system. I also have not been able to use one of the air condition vents in front seat cause its never circulated since i bought it. Was my car made on a friday before happy hour?

    2015 GMC YUKON with 15.0 miles

    The heated seats do not heat up. The car was in the shop for this problem within the 1st 1000k miles. They told me to "be patient" and they'll get warm. The car has been in the shop a total of 7 days for the seats not working and i have yet to get it resolved. Emailed the ceo of gm and got a phone call from a person who stated gm considers the seats are working although they don't work most of the time and when they do get some sort of heat (the shop told me they got one seat to heat up to 87 degrees! Wow!) it is intermittent and uneven. The seats haven't worked at all in the last 3 weeks and the average temp hasn't been above 32 degrees. I spent a fortune for this car and i can't get anyone at the dealership nor in detroit to acknowledge this car is a lemon or they have a design flaw since they switched to blowers from heated coils. I am supposing the a/c in the seats won't work when it's 100 outside and i'll be told to be patient again. I've asked for my $$ back and i'll return the car. It's got 4,200k mi and i am so done with this lemon bucket.

    2015 BMW I3 with 41000.0 miles

    Only 5 years old 41k miles, mint condition, while driving the light came on saying "driver restraint system malfunction. After checking all the I3 blogs this seems to be an ongoing issue. I took my car over to Bmw today, they looked at it and said the airbag has malfunctioned same thing as the blogs are outlining. I was advised but the service person that it would be 1800.00 i told him this is an issue with the I3 and its all over the blogs, he acknowledged his awareness, in addition airbags should be good for 10 years / guaranteed? Anyway i emailed the following Bmw north american management asking why this is not a recall. Bernhard kuhnt - president and ceo of north america/Bmw group region americas richard hart - warranty department head north america shaun bugbee - executive vice president Bmw the have just been allowing people to fix their defective airbag, as they installed substandard equipment on a $50k car. Anyway this needs to be looked into i have pictures of the warning but it will not allow me to update as they are too big and i don't know how to compress. Thank you for your time and service..

    2015 FORD FUSION with 107000. miles

    I have 107,000 miles on 2015 Ford Fusion. On my way home my car lost power and was not shifting. I was able to make it home. I had car towed to tuttle click Ford in san juan capistrano, ca. I was notified that i was going to need a new transmission. Totally unacceptable, and i voiced my concerns to the service manager. I have done research online and found many complaints with the exact same symptoms with Ford Fusions. I have sent emails to mr. Ford and the customer service manager and ceo of Ford and have not received any response. There are no recalls for this issue for my model year, yet it seems to be an ongoing problem. Also Ford focus and festival have had a number of complaints as well. My car was on a city street in motion when the sudden loss of power and jerking motion started.

    2015 JEEP CHEROKEE with 51833.0 miles

    The Jeep turned off while my wife was driving multiple times. The engine would shut off, she would have to turn the Jeep off and shift to park. She would then have to start the Jeep and put it into gear to drive. All these steps had to be done while in traffic. One time she was turning left at a busy intersection. We took it to the dealer to fix, the mechanic was able to replicate the issue. The said the oil needed to be changed (charged us over $100). They changed the oil and gave us the Jeep back. Two months later the Jeep did the same thing. This time in our driveway (not as scary). I took it back to the dealer the next day. They had the Jeep 1 week and could not replicate the issue again. They told me to come get the Jeep. I was not happy with this answer (my wife and two boys drive this Jeep). I emailed the ceo of chrysler fca us llc my concerns. I was called back by regina knox. She had the dealer keep the auto for another week. They still did not find any issues. I was told to get the car and that they would not do anything for me. I told her i felt the Jeep was unsafe. I told her i didn't want to drive it. She said "i cannot buy it from you", she did offer me employee pricing if i wanted another new auto from chrysler. The error message reads "shift to p then desired gear"
