  • Pedestrian alert sounds investigation # DP22005

    Nhtsa received a petition on or about july 18, 2022, requesting that federal motor vehicle safety standard (fmvss) 141 be applied to all electric and hybrid vehicles operating in the united states. The petition can be reviewed at nhtsa.Gov under odi number 11486072.Fmvss 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 kg or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after march 1, 2021.The petitioner asserts that hybrid and electric vehicles to which the standard does not apply should be found to contain a safety defect. In support of the petition, the petitioner includes findings contained in a bill introduced in the house of representatives in 2009, the pedestrian safety enhancement act of 2009, h.R. 734, 111th cong. (2009). A related bill, the pedestrian safety enhancement act of 2010, became law in january 2011 (pub. L. 111-373). The petitioner requests that nhtsa require all electric and hybrid vehicles comply with fmvss 141, regardless of production date.The defect petition has been opened to evaluate the issue and determine whether to grant or deny the petition.The associated product list is not exhaustive and may contain vehicles that are equipped with pedestrian alert sounds.

  • Desiccated air bag inflator rupture investigation # EA21002

    From 2000 through 2017, takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("psan") propellant -- propellant 2004 and propellant 2004l. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 blomquist report at www.Nhtsa.Gov/sites/nhtsa.Gov/files/documents/expert_report-hrblomquist.Pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in us vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004l that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated psan inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All takata inflators produced with propellant 2004l contain desiccant, and none of these desiccated inflators using propellant 2004l are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated psan inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- takata (now known as tk global), the independent testing coalition, and exponent -- have been studying the long-term behavior of takata desiccated psan inflators using propellant 2004l (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004l is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004l.The office of defects investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated psan frontal inflators manufactured by takata. This investigation will require extensive information on takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated psan inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.Gov/recalls?Nhtsaid=ea21002 -- note this information is subject to change/revision as the investigation proceeds). In normal operation, when the car is slowing, and almost at a stop, it comes out of regenerative braking mode, and enters thrust mode, so that if the brake is released when stopped, the car will begin to roll. In this vehicle, sometimes the car exits regenerative braking mode and enters thrust mode when slowing at high speed, such as when exiting a highway ramp. This is extremely dangerous, as braking is momentarily and unexpectedly lost when it is needed most. I took the car into the dealership, and they were unable to find any issues or fault codes. When attempting to reverse out of parking stall vehicle would not move, almost as if emergency brake was being applied even though ebrake was off. Turning on and off ignition did nothing. Replace 12 v battery did nothing. Warranty work on control unit performed multiple times in 2017/2018/2019 even though the software 'fixed' the issue related to the recall nhtsa campaign id: 15v655000 in 2015. Dealer states must replace entire power train at a cost of $11,500. Vehicle has 72k miles and loan balance is over $12k. The power train provided by tesla was replaced in tesla's own vehicles as well as in toyota rav4 models from the same year.

Vehicle battery warning light came on instructing to stop driving vehicle immediately. Like so many other owners who have filed complaints, mercedes dealer-mechanic said that coolant had gotten into battery and drive unit needs to be replaced. Estimated cost $12,000 - rendering car totalled. Independent ev specialist has pressure tested cooling system and finds no leaks. Fortunately, i was in a safe place at the time i needed to stop the car. If i had been out on the freeway during heavy traffic or at night, this would have been unsafe. If there are problems with the coolant and drive unit, it leads me to believe that problem could lead to fire, if not potential crash if driver loses power in congested traffic at high speed. As an all electric car, brakes and transmission rely on power to safely bring car to a stop.

The "malfunction" light appeared and vehicle lost power immediately. After coming to a complete stop, then switching the ignition off and back on again, the car operated normally and the "malfunction" light disappears. This occurred three times: during highway driving (55 mph), on a country road (45 mph), and at almost a stop (5 mph). One occurrence was in the early evening on a hot day and two were early in the morning when temperatures were cool. Two of these failures were very dangerous circumstances and it is very lucky that no one was nearby on the road at that time. Since it is a fully electric car it loses momentum very quickly and i had to desperately look for a turnout at once. One time my two young children were in the back seat of the car. Mercedes benz dealership diagnosed the cause to be the drive unit at a cost of $9,000. They claim this is a different problem than the previous recall (which the car had completed). I have read through the previous recall information and it sounds very much like what this car is doing. I would like to request the nhtsa investigate whether the previous recall actually fixed the problem. The car has 30,000 miles but mercedes benz refuses to fix the car because the warranty expired in december 2018 (seven months ago). They only offer a 4 year/50,000 mile warranty (whichever occurs first).

This is an electric vehicle. When slowing down the paddle shifters can be used to put the car in 'd-' mode, for more aggressive engine braking. This increases the rate of decl without using the mechanical brakes. This has worked well for four years and 65,000 miles. Today there was a 'one time' malfunction. I did the usual 'd-' 'down shift' to slowdown from 30 mph, in traffic, for red light. The car did not aggressively slow down and i applied the brakes. I noticed the car need excessive break pedal pressure to make what should be a normal stop. I glanced down at the power meter and it is indicating power is still being applied to the motor. The indicator was half way to the first tic and dropping as i slow down. This is about 25 amps powering the motor, not recharging the battery. This aggressive braking incident is not related to cold the weather battery protection mode where engine breaking is disabled until the battery reaches minimum operating temperature. The outside air temperature was 40f. To recap, while applying the mechanical brakes, the electric motor was still producing power as indicated by the ammeter, stopping the car required more than the usual amount of brake pressure.

On september 20th, 2018, the car suffered a sudden and abrupt catastrophic shut down while i was driving my son to school. We were on city streets driving straight at normal speed and it stalled right in an intersection but i was lucky to be able to still have some momentum to get into the left turn lane. There was a flashing red text warning on the display for a few seconds- something about turn off immediately and do not restart. Then the car shut down. I was terrified that we would get into an accident all throughout the time we had to wait for the tow truck. If this had happened on the freeway, it would have been disastrous. Note that the electric drive unit on this car was replaced on the following dates: 3/5/18 - 3/15/18(10 days to repair) 9/20/18- 10/3/18 (14 days) on 10/10/18 the car suffered another "malfunction visit workshop" and has been in the repair shop since 10/11/18. The car was towed. It appears to be the electric drive unit again and will be the third time it is replaced. I believe this car is defective and dangerous and mercedes benz does not appear to be able to repair the problem. I have read about similar drive unit replacements from other b class owners.
