• Recall 79422 [USA]: A short circuited grid heater could result in a vehicle fire and injury to vehicle occupants.
  Daimler trucks north america is recalling certain model year 2005 through 2012 freightliner m2, sterling acterra, 7500 & 8500, and thomas built hdx vehicles manufactured from june 23, 2004, through march 15, 2011, equipped with specific mbe900 epa04 and epa07 model engines. The intake air grid heater may short circuit and fail due to excessive internal temperatures.
  daimler trucks will notify owners, and dealers will repair the vehicles free of charge. Repairs will be performed by detroit diesel corporation authorized repair facilities.   Daimler trucks is recalling 18,198 my 2003-2009 thomas built er, mvp-er, hdx, and mvp-ef school and transit buses equipped with a dual power switch and manufactured from february 1, 2002 through october 17, 2008. Solid state circuit breakers may trip unnecessarily potentially resulting in a loss of power to the bus chassis and body electrical circuits causing an unexpected loss of engine power and exterior lighting.

• Recall 81779 [USA]: The user of the lift could be injured should the lift move unintentionally.
  Daimler trucks is recalling 1,419 my 2006 through 2008 thomas built school buses and recreational vehicle chassis manufactured between march 7, 2005 and may 27, 2008, equipped with ricon platform style wheelchair lifts. These buses fail to comply with the requirements of federal motor vehicle safety standard no. 404, "platform lift installations in motor vehicles." the threshold warning system may not detect the presence of a wheelchair or mobility aid user in a certain spot within the defined threshold area.

• Recall 77646 [USA]: If the weld fails, vehicle stability while cornering could be affected resulting in loss of vehicle control, possibly resulting in a crash. in addition, the transverse beam could drop to the ground causing sparks that could cause a fire hazard.
  Daimler trucks is recalling 2,506 my 2008 xc motor home chassis and thomas built hdx school and transit buses equipped with saf-holland adl series rear suspensions. In some instances the transverse beam was not properly welded which could result in a failure of the weld.
  daimler trucks is working with saf-holland to have a visual inspection of the adl transverse beam performed. If a weld defect is identified, saf-holland will provide a service replacement kit that contains a new transverse beam, all necessary installation hardware, beam replacement, and instructions free of charge (please see 08e031). • Recall 70366 [USA]: In the event of an emergency, a person on the outside of the bus may break the door handle trying to open the emergency exit potentially delaying evacuation and increasing the risk of personal injury to the vehicle's occupants.
  Daimler trucks is recalling 243,435 my 1995-2008 thomas built school buses manufactured between january 1994, and march 10, 2008. (additional buses have been added to this recall campaign. Please see supplemental report no. 4) the emergency exit door handle begins corroding around the securing roll pin and square drive mechanism causing the protective finish to chip and flake. Continued corrosion and pitting weakens the handle. Handles weakened by corrosion may break while trying to open the door from the exterior.
  daimler trucks will notify owners and will replace the handles with a more robust handle free of charge. Repairs will be performed by thomas built buses dealerships. • Recall 75642 [USA]: A vehicle fire can occur possibly resulting in a crash.
  Certain 2001 through 2008 thomas built fs-56, er, mvp er, hdx, and mvp ef school buses manufactured from october 23, 2000, through january 24, 2006, equipped with tyco pc board and type ii auto reset circuit breakers. Type ii auto reset circuit breaker temperatures may exceed the rating of the tyco pc board causing the boards to melt potentially resulting in a vehicle fire.

  Freightliner is recalling certain school bus chassis, heavy trucks, van chassis, rv motorhome chassis manufactured from january 5 to may 17, 2007, equipped with axle alliance company (aac) front axles. During the front axle assembly, the fasteners that attach the steering arm or tie rod arms to the spindle may be over or under tightened.

  On certain school buses, the stem which joins the outside emergency door handle to the emergency door opening mechanism may potentially corrode. Excessive material loss of the stem due to corrosion could result in the outside handle separating from the door mechanism while trying to open the door from the exterior of the bus. In the event of an emergency, this could delay evacuation of the vehicle and increase the risk of injury to occupants. Correction: dealers will inspect the stems and either replace or protect them against further corrosion.

• Rear leaf spring fractures investigation # PE19007

  Nhtsas office of defects investigation (odi) has identified field reports in daimler truck north americas (dtna) 2018-4th quarter early warning reporting (ewr) data describing cracked rear leaf-style suspension springs in thomas built school buses. Odi reached out to 3 municipal school bus fleets operating subject vehicles who described similar failures where a fractured piece of leaf spring separated from the vehicle while driving. Odi also contacted a major nationwide school bus transportation service provider who also identified numerous potential failures.A fractured piece of leaf spring that detaches from a moving school bus could potentially strike a pedestrian or a following vehicle.Odi is opening this investigation to evaluate the cause, consequence, scope and frequency of the alleged defect (item detachment) in the subject thomas built school buses.

• Emergency door handle failure investigation # PE07054

  The office of defects investigation opened this preliminary evaluationafterbecoming aware that fleets were having handle failures on the rear emergency door exit.The failure resulted in the door handle separating from thedoor latch mechanism.When the handle fails, the rear emergency exit door can not be opened from outside the bus.The failure results from corrosion of the exterior door handle around the securing roll pin and square drive latch mechanism, causing the protective finish to chip and flake.Continued corrosion weakens the handle and eventually the handle may break while trying to open the exit door.Odi was concerned that during an emergency, a person outside of the bus would break the door handle while trying to open the emergency exit door,potentially delaying evacuation of the bus.Under the safety recall, the aluminum handles will be replaced with a more robust handle.The recall action resolves the issues raised by this investigation. • Side electrical fuse panel fires investigation # PE07029

  The office of defects investigation (odi) opened this investigation after becoming aware of two school bus fires that started in the side mounted electrical fuse panel on a pair of thomas built school buses.Thefleet owner had reported the bus interiors filled with smoke, certain electrical devices became inoperative, and the buses stalled. The two buses involved in the fire incidents have consecutive vin numbers and the fires were similar, but not identical to a previous recall (06v-136) conducted by thomas built.These buses were not included in the prior recall, which involved a defective power distribution module (pdm).After further technical evaluation, freightliner decided they would conduct a safety recall to correct deficiencies in theprevious recall and also expand the population to capture all affected buses.The corrective action freightliner plans to take will be forwarded to odi when available. Accordingly, this investigation is closed.
