• Recall 137254 [USA]: If the vehicle can be moved while the wheelchair lift is being used, there is an increased the risk of injury.
  Daimler trucks north america llc (dtna) is recalling certain 2002-2018 thomas built buses minotour school buses built on a ford chassis and equipped with an optional wheelchair lift. The wheelchair lift may be allowed to operate when the vehicle is in neutral or drive, due to missing relay in the interlock wiring harness.
  dtna will notify owners, and dealers will inspect the wheelchair lift wiring, correcting it as necessary, free of charge. Daimler issued an interim notification to owners on june 19, 2018. • Recall 126707 [USA]: In the event of a fire, if the fire extinguisher does not function properly, it can increase the risk of injury.
  Daimler trucks north america llc (dtna) is recalling certain 2002-2017 thomas built buses saf-t-liner c2, saf-t-liner hdx, fs65, and minotour school buses, equipped with kidde plastic-handle or push button 'pindicator' fire extinguishers. These extinguishers may become clogged, preventing the extinguisher from discharging as expected or requiring excessive force to activate the extinguisher. Additionally, in certain models, the nozzle may detach from the valve assembly with enough force that it could cause injury and also render the product inoperable. • Recall 66698 [USA]: Incorrect tire information may lead to vehicle overloading that can result in a tire failure which may lead to a vehicle crash.
  Certain model year 2000 through 2009 thomas built school buses manufactured from april 17, 2000, through december 21, 2009, were equipped with an incorrect tire load certification label. The certification labels list "d" load range tires with a recommended cold tire inflation pressure greater than 65 psi. The buses are equipped with load range "e" tires that require a higher minimum tire pressure. Accordingly, these buses fail to conform to the requirements of federal motor vehicle safety standard no. 120, "tire selection and rims for motor vehicles other than passenger cars."
  dtna will notify owners and dealers will replace the incorrect label with a correct label free of charge or, if the owner wishes, they may replace the label themselves. • Recall 70372 [USA]: In the event of an emergency, a person on the outside of the bus may break the door handle trying to open the emergency exit potentially delaying evacuation and increasing the risk of personal injury to the vehicle's occupants.
  Daimler trucks is recalling 243,435 my 1995-2008 thomas built school buses manufactured between january 1994, and march 10, 2008. (additional buses have been added to this recall campaign. Please see supplemental report no. 4) the emergency exit door handle begins corroding around the securing roll pin and square drive mechanism causing the protective finish to chip and flake. Continued corrosion and pitting weakens the handle. Handles weakened by corrosion may break while trying to open the door from the exterior.

  Certain 2002-2005 my thomas built minotour, conventional, mvp-er, er, hdx, and mvp-ef model school buses manufactured between january 8, 2002, and march 8, 2005, equipped with specialty mfg "5" series stop arms originally sold in or currently registered in states other than the following states: alaska, colorado, connecticut, delaware, idaho, illinois, indiana, iowa, kansas, kentucky, maine, maryland, massachusetts, michigan, minnesota, missouri, montana, nebraska, nevada, new hampshire, new jersey, new mexico, new york, north dakota, ohio, oregon, pennsylvania, rhode island, south dakota, tennessee, utah, vermont, virginia, washington, west virginia, wisconsin, wyoming, and the district of columbia. In extremely cold weather, the microswitches used internally to position the stop arm sign in the open and closed positions may malfunction, causing the sign to open or close in an improper position, or to not open at all.
  for those buses operated in or near any of the states listed above, thomas built is encouraging the bus owners to obtain the replacement switch. For those buses operated in the warmer states, dealers will replace the switch upon owner request. For those customers who request the remedy, thomas built will replace the original switch with a switch pack that is not sensitive to extreme cold weather and will inspect to ensure the microswitch heater wiring is properly connected.   On certain trucks equipped with caterpillar 3126e hydraulically controlled electronically actuated unit injected diesel engines with high efficiency pumps (hep8), the pump components may experience a high pressure seal failure which can result in a short period of rough running and subsequent engine stall due to a loss of injection actuation pressure.

  Certain vehicles fail to conform to the requirements of canada motor vehicle safety standard 120 - tire selection and rims. The vehicle certification label incorrectly lists tire load range "d", but instead should read load range "e". As a result, the vehicle may inadvertently be fitted with incorrect replacement tires.   Certain school buses equipped, at customer request, with passenger seatbelts, may suffer seatbelt failure. The edge of the metal seatbelt retainer strips may cause chafe damage to the belts. Correction: dealers will inspect and, if chafe damage is found, replace seatbelt assemblies. Seatbelt retainer strips will either be replaced or modified to prevent future chafing. • Emergency door handle failure investigation # PE07054

  The office of defects investigation opened this preliminary evaluationafterbecoming aware that fleets were having handle failures on the rear emergency door exit.The failure resulted in the door handle separating from thedoor latch mechanism.When the handle fails, the rear emergency exit door can not be opened from outside the bus.The failure results from corrosion of the exterior door handle around the securing roll pin and square drive latch mechanism, causing the protective finish to chip and flake.Continued corrosion weakens the handle and eventually the handle may break while trying to open the exit door.Odi was concerned that during an emergency, a person outside of the bus would break the door handle while trying to open the emergency exit door,potentially delaying evacuation of the bus.Under the safety recall, the aluminum handles will be replaced with a more robust handle.The recall action resolves the issues raised by this investigation. • Seat belts webbing abrading on frame investigation # PE06057

  This investigation was opened after odi received notification from two fleets alleging thediscovery ofnumerous seat belts with cut webbing as a result of the webbing abrading against anunderseat metal bracket.The subject vehicles are class "a" school buses weighing under 10,000 pounds.They are required by federal standard fmvss 208 to have seat belts.As a result of this investigation, freightliner will conduct a safety recall to inspect all affected buses for seat belt ¿chafing¿ and will replacedamaged belts as needed. Also, the seat belt retainer strip will be modified or replaced to prevent future chafing.Freightliner expanded the scope of the recall (beyond the original investigation population) to include approximately 10,251 additional buses having gross vehicle weight ratings greater than 10,000 lbs.They determined that these buses are also susceptible to the chafing action.There is no federalrequirement for seat belts in school buses weighing over 10,000 pounds.However, some customers have ordered seat belts and some states have requirements for seat belts.Therefore, freightliner has agreed to address those buses even though there is no applicable federal standard.The recall action taken by freightliner resolves the issues raised by this investigation.Accordingly, this investigation is closed.
