Known Recall Investigations
  • Thomas built cng tank abrasion investigation # PE11029

    On august 29, 2011 odi received a vehicle owners questionnaire (voq) from a los angeles school district indicating a 2003 thomas built saf-t-liner hdx compressed natural gas (cng) school bus in their fleet had insufficient clearance between the cng tank and mounting bracket resulting in damage to the outer tank wrapping.The mathematical analysis division of nhtsa calculated that a sample size of thirty (30) buses would give a high confidence (90%) estimation of whether similar buses were experiencing tank abrasion.In the course of this investigation nearly 70 buses (over 300 tanks) were inspected; no tank other than the subject of the voq was found to have insufficient clearance.Busses manufactured before and after the subject vehicle were inspected.As a result of this investigation, daimler trucks north america (dtna) developed an improved inspection procedure that addresses the concerns identified in the los angeles complaint.In early february 2012 dtna will mail an owner letter along with the new inspection procedure.See the investigative file for copies of dtnas bulletin and owner letter.This preliminary evaluation is closed.The closing of this investigation does not constitute a finding by nhtsa that no safety-related defect exists.The agency reserves the right to take further action if warranted by circumstances.The odi report cited above can be reviewed at www-odi.Nhtsa.Dot.Gov/owners/searchnhtsaid under the following identification (odi) number:10423400

  • Steering column weld cracks investigation # EA07002

    As a result of the odi investigation, freightliner has agreed to recall (number 07v-289) all of the vehicles manufactured with steering columns potentially missing welds.The recall remedy will involve inspecting the recalled vehicles for missing welds and if found replace the steering column.Early warning aggregate data submitted to nhtsa by freightliner suggested that a steering problem (ewr code 01) existed for the 2004 and 2005 century and columbia class vehicles.The count of field reports submitted for the first quarter of 2006 was higher than the average of the previous 6 quarters for the aggregate steering data.Odis further review of the ewr data foundfield reports that described steering column failures as a result of steering column welds cracking or breaking.Information received from freightliner detailed failures of the steering columns on the subject vehicles.These failures were the result of missing and cracked welds on the steering column assembly.During testing of vehicles containing defective steering columns, odi determined that weld failures in the steering column create a situation where the vehicle becomes difficult to steer or may startle the driver upon failing, thus presenting a safety risk.A copy of the recall is attached.Freightliners action resolves the issues raised by this investigation and, therefore, this investigation is closed.

Buses manufactured in such a way that front hub piloted wheels cannot be centered on hub creating excessive vibration at all speeds and causing an unsafe condition. Additionally brakes on the same buses are continually reported as being excessively loud and vibrating severely when applied. Brakes possibly related to uncentered wheels. Problem reported to manufactured/dealer numerous times with no resolution. *tr

The bus quit running, start running without turning the ignition, and they have numerous other electrical malfunctions. The biggest consequence is when the bus quits running while there is a bus load of students on. This leads to students being late to school or home. Furthermore, they lose power steering when the engine dies. This is especially dangerous when the buses are at railroad crossings and intersections. The parts we have to replace to fix it are (thomas part numbers) 6120-2164: main power switch, and 6120-2076, vehicle interface module. Some old parts are available. *tr
