Known Recall Investigations
  • Unexpected transmission downshift investigation # RQ17010

The office of defects investigation (odi) opened an investigation based on consumer allegations of transmissions unexpectedly downshifting to a lower gear without input from the driver on model year (my) 2011-2013 ford f-150, expedition and navigator vehicles. Odi has received 455 unique reports related to the alleged problem that were not covered under a previous recall for this issue (16v-248).Expedition and navigator complaint rates are lower than f-150 complaints. Of those 455 reports, 34.7% claimed that the rear wheels locked up when the transmission downshifted. There were 4 accident claims with no injuries. There are many instances where drivers report leaving their lane or the road because of this issue. In response to odis information request (ir) letter, ford motor company (ford) responded with 783 unique reports related to the alleged problem. There were 6 accident claims with 3 injuries. Ford also submitted 1206 warranty claims related to transmission downshifting failures. The failure can occur at any time, but typically at higher speed, and include symptoms such as sudden transmission downshifting, loss of speedometer, erratic shifting of transmission, and transmission stuck in one gear. The failures typically occur without warning and may result in the rear wheels momentarily locking up. The driver often had to fully stop the vehicle and restart it to get the vehicle to shift properly. On february 2nd, 2019 ford notified odi that it would conduct a safety recall (19v-075) to remedy transmission downshifting failures on all 2011-2013 f-150 vehicles. Ford will also offer extended warranties to 2011-2013 expedition and navigator vehicles with the same issue. Per ford, some vehicles may experience an intermittent loss of transmission output speed sensor (oss) signal to the powertrain control module (pcm), potentially resulting in a temporary unintended downshift into first gear. Ford will reprogram the pcm with software that will prevent an unintended downshift into first gear and illuminate the malfunction indicator light in the event of an intermittent oss signal loss. The recall action initiated by the manufacturer appears to address the safety risks of concern to odi.Accordingly, the investigation is closed. The odi reports cited above can be reviewed at www-odi.Nhtsa.Dot.Gov/owners/searchnhtsaid under the identification (odi) numbers found in the attached list.

The contact owns a 2012 ford expedition. The contact stated while driving 60 mph, the vehicle independently down shifted to first gear causing both rear tires to lock up. The contact stated that there was no warning light illuminated. The vehicle was not taken to the local dealer nor an independent mechanic to be diagnosed. The vehicle was not repaired. The contact related the failure to nhtsa campaign number: 16v248000 (power train). The manufacturer had been informed of the failure and informed the contact that the vin was not under recall. The failure mileage was approximately 164,000.

The contact owns a 2012 ford expedition. The contact stated that while driving at 30 mph, there was an unexpected transmission downshift and the vehicle speed decelerated inadvertently. The vehicle was not diagnosed nor repaired. The dealer and manufacturer were not made aware of the failure. The failure mileage was 123,000.

While driving, the vehicle suddenly downshifted into 1st gear, rpms revved up, speedometer stopped working, pushing the gas pedal temporarily had no effect, "fuel level low" light came on and "tire pressure sensor fault" light came on. I was still able to drive the vehicle, though the speedometer was not working. I stopped, turned off the vehicle and restarted it and the vehicle worked fine. This event happened three times over the past month. Sheehy ford in ashland, virginia said that i needed a new "lead frame" and that the charge would be $900. Upon further research, i learned that there had been a recall for this issue for the 2012 ford expedition, but my 2012 expedition vin number is not part of the recall. I believe my vehicle should have also been included in the recall and fixed by ford. I could have had a serious accident if this had happened while driving at a high rate of speed. Recall information found online: "ford has issued a safety recall for the 2011-2012 ford f-150, and 2012 ford expedition for a potential problem with the output speed sensor in the transmission's lead frame; in certain conditions the part can lead to a temporary downshift into first gear. Depending on how fast the vehicle is going, that could lead to an abrupt reduction in speed that could cause the rear tires to slide or lock up. Ford said it's aware of three accidents reported and no injuries related to the issue. The affected vehicles have a 6r80 transmission in certain ford f-150s built at dearborn assembly plant and kansas city assembly plant, aug. 19, 2011 through march 9, 2012; 2012 ford expedition vehicles built at kentucky truck plant, aug. 19, 2011 through dec. 19, 2011; ford mustang vehicles built at flat rock assembly plant, aug. 19, 2011 through feb. 21, 2012; and lincoln navigator vehicles built at kentucky truck plant, aug. 19, 2011 through dec. 17, 2011.

Randomly drops into first gear while driving and will not go out of first gear after that unless disconnecting battery and resetting everything. No check engine light or anything. This cannot happen with three kids in the car. What if this happened at 70mph on the highway? Occurred 2/3/21 and 2/8/21.

Vehicle is downshifting gear while driving. It caused jerks in the suv as it tried to shift gears and then became stuck on first gear. This happened while driving in the highway and again on a city street. Ford dealership reporting it is not covered under their current recall although other 2012 ford expeditions with the same issue are.
