  Ford motor company (ford) is recalling certain model year 2011-2012 ford f-150 trucks, 2012 ford expedition, lincoln navigator, and ford mustang vehicles. The affected vehicles are equipped with an automatic transmission that may unexpectedly downshift to first gear, regardless of vehicle speed.

• Recall 90490 [USA]: If this condition exists, the transmission may not go into reverse or when the driver pushes the shift lever in the reverse ?r? position and the vehicle?s transmission does go in reverse, the ?r? may not illuminate on the dashboard of the f-150, expedition, or navigator models and/or the backup lamp on the rear of the f-150, expedition, navigator, or mustang vehicles may not illuminate. these conditions increase the risk of a crash or a pedestrian being struck due to the vehicle?s not signaling it is in reverse.
  Ford is recalling certain model year 2011-2012 ford f-150, 2012 expedition and lincoln navigator, and 2012-2013 mustang vehicles, for failing to comply with the requirements of federal motor vehicle safety standard nos. 102, "transmission shift lever sequence, starter interlock, and transmission braking effect" and 108 ?Lamps, reflective devices and associated equipment.? These vehicles may have a transmission range sensor (trs) that was calibrated out of specification for reverse gear.

  Ford is recalling certain model year 2012 expedition and lincoln navigator vehicles, manufactured from november 19, 2011, through december 2, 2011, for failing to comply with the requirements of federal motor vehicle safety standard no. 202, "head restraints." these vehicles may have inadequate welds on the head restraint support brackets for the second row, right-hand seat.

  Certain vehicles equipped with a 6-speed automatic transmission may experience an intermittent loss of the transmission output-shaft speed sensor (oss) signal to the power control module. This could potentially cause an unintended downshift into first gear, which depending on vehicle speed at the time of the downshift, may cause the rear tires to slide or lock up until the vehicle slows. This may result in a loss of vehicle control without warning, increasing the risk of a crash causing injury and/or damage to property. Correction: dealers will reprogram the powertrain control module with updated software.

  Certain vehicles may have been equipped with an incorrectly manufactured transmission range sensor. In some instances, when the shift lever is placed in the "r" position, signal may not be provided to the electronic transmission range indicator "prndl" for "r" illumination and/or for backup lamp activation. Should the backup lamps fail to illuminate, the driver's intentions to reverse may not be conveyed clearly to other motorists and/or pedestrians, which could result in property damage and/or personal injury. Correction: dealers will inspect and, if necessary, replace the transmission range sensor.

• Recall 2012028 [Canada]: Seats and restraints
  On certain vehicles, the second row right-hand seat may have been manufactured incorrectly. Improper weld penetration between the head restraint guide sleeve tube and the seat back frame could allow the headrest to fail during a rear-end vehicle crash, increasing the risk of injury to the seat occupant. Correction: dealers will inspect and, if necessary, replace the seat back frame for the second row right-hand seat.

• Unexpected transmission downshift investigation # RQ17010

  The office of defects investigation (odi) opened an investigation based on consumer allegations of transmissions unexpectedly downshifting to a lower gear without input from the driver on model year (my) 2011-2013 ford f-150, expedition and navigator vehicles. Odi has received 455 unique reports related to the alleged problem that were not covered under a previous recall for this issue (16v-248).Expedition and navigator complaint rates are lower than f-150 complaints. Of those 455 reports, 34.7% claimed that the rear wheels locked up when the transmission downshifted. There were 4 accident claims with no injuries. There are many instances where drivers report leaving their lane or the road because of this issue.In response to odis information request (ir) letter, ford motor company (ford) responded with 783 unique reports related to the alleged problem. There were 6 accident claims with 3 injuries. Ford also submitted 1206 warranty claims related to transmission downshifting failures. The failure can occur at any time, but typically at higher speed, and include symptoms such as sudden transmission downshifting, loss of speedometer, erratic shifting of transmission, and transmission stuck in one gear. The failures typically occur without warning and may result in the rear wheels momentarily locking up. The driver often had to fully stop the vehicle and restart it to get the vehicle to shift properly.On february 2nd, 2019 ford notified odi that it would conduct a safety recall (19v-075) to remedy transmission downshifting failures on all 2011-2013 f-150 vehicles. Ford will also offer extended warranties to 2011-2013 expedition and navigator vehicles with the same issue. Per ford, some vehicles may experience an intermittent loss of transmission output speed sensor (oss) signal to the powertrain control module (pcm), potentially resulting in a temporary unintended downshift into first gear. Ford will reprogram the pcm with software that will prevent an unintended downshift into first gear and illuminate the malfunction indicator light in the event of an intermittent oss signal loss.The recall action initiated by the manufacturer appears to address the safety risks of concern to odi.Accordingly, the investigation is closed.The odi reports cited above can be reviewed at www-odi.Nhtsa.Dot.Gov/owners/searchnhtsaid under the identification (odi) numbers found in the attached list.

The contact owns a 2012 ford expedition. The contact stated while driving 60 mph, the vehicle independently down shifted to first gear causing both rear tires to lock up. The contact stated that there was no warning light illuminated. The vehicle was not taken to the local dealer nor an independent mechanic to be diagnosed. The vehicle was not repaired. The contact related the failure to nhtsa campaign number: 16v248000 (power train). The manufacturer had been informed of the failure and informed the contact that the vin was not under recall. The failure mileage was approximately 164,000.

The contact owns a 2012 ford expedition. The contact stated that while driving at 30 mph, there was an unexpected transmission downshift and the vehicle speed decelerated inadvertently. The vehicle was not diagnosed nor repaired. The dealer and manufacturer were not made aware of the failure. The failure mileage was 123,000.

While driving, the vehicle suddenly downshifted into 1st gear, rpms revved up, speedometer stopped working, pushing the gas pedal temporarily had no effect, "fuel level low" light came on and "tire pressure sensor fault" light came on. I was still able to drive the vehicle, though the speedometer was not working. I stopped, turned off the vehicle and restarted it and the vehicle worked fine. This event happened three times over the past month. Sheehy ford in ashland, virginia said that i needed a new "lead frame" and that the charge would be $900. Upon further research, i learned that there had been a recall for this issue for the 2012 ford expedition, but my 2012 expedition vin number is not part of the recall. I believe my vehicle should have also been included in the recall and fixed by ford. I could have had a serious accident if this had happened while driving at a high rate of speed. Randomly drops into first gear while driving and will not go out of first gear after that unless disconnecting battery and resetting everything. No check engine light or anything. This cannot happen with three kids in the car. What if this happened at 70mph on the highway? Occurred 2/3/21 and 2/8/21.

Vehicle is downshifting gear while driving. It caused jerks in the suv as it tried to shift gears and then became stuck on first gear. This happened while driving in the highway and again on a city street. Ford dealership reporting it is not covered under their current recall although other 2012 ford expeditions with the same issue are.
