  Toyota motor engineering & manufacturing (toyota) is recalling certain 2010-2016 4runner, 2003-2006 tundra, 2003-2013 corolla, 2009-2010 corolla matrix, 2004-2005 rav4, 2002-2007 sequoia, 2011-2013 sienna, 2008-2012 scion xb, 2008-2009 lexus is-f, 2007-2012 yaris and lexus es350, 2010-2017 lexus gx460, 2002-2010 lexus sc430, 2006-2012 lexus is250 and is350 and 2010-2015 lexus is250c and is350c vehicles. These vehicles had their driver or passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.

• Recall 86548 [USA]: Fuel leakage, in the presence of an ignition source, could result in a fire.
  Toyota is recalling certain model year 2006-2009 lexus is and model year 2006-2007 lexus gs vehicles. Due to insufficient tightening of the fuel pressure sensor connected to an engine fuel delivery pipe with nickel phosphorus plating, there is a possibility that, over time, the pressure sensor could loosen. If loosening occurs, fuel could leak past a gasket used in the connection between the sensor and the pipe and through the threaded portion of the sensor.
  dealers will inspect for fuel leakage from the threaded portion of the fuel pressure sensor and if a fuel leak is not confirmed, dealers will further tighten the fuel pressure sensor with the proper torque. If a fuel leak is confirmed, the gasket between the sensor and the fuel deliver pipe will be replaced with a new one, and the sensor will be tightened with the proper torque. • Recall 68079 [USA]: A stuck open accelerator pedal may result in very high vehicle speeds and make it difficult to stop the vehicle, which could cause a crash, serious injury or death.
  Toyota is recalling certain model year 2004-2010 passenger vehicles. The accelerator pedal can get stuck in the wide open position due to its being trapped by an unsecured or incompatible driver's floor mat.
  toyota filed an amended defect report on november 25, 2009, stating that dealers will modify the accelerator pedal and, on certain vehicles, alter the shape of the floor surface under the pedal. These changes address the risk of pedal entrapment due to interference with the floor mat. Redesigned accelerator pedals will become available beginning in april 2010 and dealers will replace any modified pedal with the new pedal if desired. Also, dealers will replace any genuine toyota or lexus all-weather floor mats with redesigned all-weather mats, or repurchase the previous mats from owners who do not want the new ones. Additionally, software modifications will be installed on camry, avalon and lexus es 350, is 350 and is 250 models that will ensure that the brake overrides the accelerator in the event both brake and accelerator pedals are applied.   Issue: this recall is for certain block heater kits installed on certain vehicles or sold as accessories at toyota or lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt. • Recall 2018014 [Canada]: Airbag
  On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the passenger frontal airbag. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. These issues could increase the risk of injury. Correction: depending on the vehicle model, dealers will either replace the passenger frontal airbag inflator, or replace the airbag assembly. Note: canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected.

  On certain vehicles, the fuel delivery pipes in the engine compartment may have been produced with particles from the anti-corrosion plating on the gasket seating surface where the fuel pressure sensor is installed. This could affect the sealing properties of the gasket seated between the pressure sensor and the pipe, potentially resulting in a fuel leak. A fuel leak in the presence of an ignition source could result in a fire causing injury and/or property damage. Correction: dealers will replace the gasket and repair the gasket seating surface.

  On certain vehicles, the wiper arm securing nuts could have been insufficiently tightened during assembly. If the movement of the wipers is restricted by an external load, such as heavy snow on the windshield, the wiper arms could slip and one or both wipers could become inoperative. If this occurs, driver visibility could be reduced, which could increase the risk of a crash causing injury and/or property damage. Correction: dealers will clean the mating surface of the wiper arm and motor shaft and install improved wiper arm nuts.

• Desiccated air bag inflator rupture investigation # EA21002

  From 2000 through 2017, takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("psan") propellant -- propellant 2004 and propellant 2004l. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 blomquist report at www.Nhtsa.Gov/sites/nhtsa.Gov/files/documents/expert_report-hrblomquist.Pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in us vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004l that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated psan inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All takata inflators produced with propellant 2004l contain desiccant, and none of these desiccated inflators using propellant 2004l are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated psan inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- takata (now known as tk global), the independent testing coalition, and exponent -- have been studying the long-term behavior of takata desiccated psan inflators using propellant 2004l (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004l is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004l.The office of defects investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated psan frontal inflators manufactured by takata. This investigation will require extensive information on takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated psan inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.Gov/recalls?Nhtsaid=ea21002 -- note this information is subject to change/revision as the investigation proceeds). At approximately 85k total miles, is250 automatic began dropping from 5-6k rpm to 2-3k rpm whenever changing road grade to an incline or decline, usually above 50 mph, dropping to 35 mph on the highway. To maintain proper speed, i would have to downshift from d to s and floor the accelerator. Traction control, parking brake, and abs lights would sometimes coincide with this problem. This continued until i sold the is250 at 88k miles.

Defective passenger occupancy sensor: my srs / airbag warning light illuminated and would not turn off, indicating that the srs system was deactivated. I had my car diagnosed by a lexus dealership service department on 12/4/2020. Johnson lexus advised me that the passenger side front seat pressure sensor failed and that the repair was to replace the seat for $2769.31. The seat has not been subjected to any unusual or harsh use. Research indicates that this is a common defect among lexus is models and some other lexus and toyota models of this generation. Many lexus drivers report great difficulty regarding this defect and the thousands of dollars in repairs that dealerships are demanding to fix the defect. There are hundreds of complaints about this very issue on the nhtsa database. I am very surprised there has not been a mandatory recall or a lexus program to address this common sensor failure affecting a critical, nonoptional safety feature. I contacted toyota of north america and received a polite call from the executive office stating that the company was sorry but would not do anything to help. Lexus is usually excellent with its customer service, so i am very surprised at the lack of concern here. I hope it does not require deaths from air bags failing to deploy for lexus to address this defect.
