  On certain vehicles, a control board for the intelligent power module (ipm), part of the hybrid system inverter assembly, could fail while the vehicle is underway. This could cause various warning lamps, including the malfunction indicator lamp, slip indicator lamp, brake system warning lamp, and master warning lamp, to illuminate on the instrument panel. If this occurred, in most cases, the vehicle would enter fail-safe driving mode, resulting in reduced motive power in which the vehicle could still be driven for short distances. In some instances, the fuse of the power supply circuit could blow, causing the hybrid system to stop functioning. The resultant loss of vehicle propulsion, in conjunction with traffic and road conditions, and the driver's reactions, could increase the risk of a crash causing property damage and/or personal injury. Correction: dealers will inspect and, if necessary, replace the intelligent power module.

• Recall 2011081 [Canada]: Other
  On certain vehicles, if the forward retention clip used to secure the floor carpet cover, which is located in front of the center console, is not installed properly, the cover may lean toward the accelerator pedal and interfere with the operation of the accelerator pedal arm. If this occurs, the accelerator pedal could temporarily become stuck in a partially depressed position, rather than return to the idle position. This could result in a loss of throttle control and a crash causing property damage and/or personal injury. Correction: dealers will replace the floor carpet cover with a newly designed version. Note: this campaign also covers vehicles which may have been previously included in recall 2006168 (toyota 609).

  Nhtsa received a petition on or about july 18, 2022, requesting that federal motor vehicle safety standard (fmvss) 141 be applied to all electric and hybrid vehicles operating in the united states. The petition can be reviewed at nhtsa.Gov under odi number 11486072.Fmvss 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 kg or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after march 1, 2021.The petitioner asserts that hybrid and electric vehicles to which the standard does not apply should be found to contain a safety defect. In support of the petition, the petitioner includes findings contained in a bill introduced in the house of representatives in 2009, the pedestrian safety enhancement act of 2009, h.R. 734, 111th cong. (2009). A related bill, the pedestrian safety enhancement act of 2010, became law in january 2011 (pub. L. 111-373). The petitioner requests that nhtsa require all electric and hybrid vehicles comply with fmvss 141, regardless of production date.The defect petition has been opened to evaluate the issue and determine whether to grant or deny the petition.The associated product list is not exhaustive and may contain vehicles that are equipped with pedestrian alert sounds.

• Unintended and uncontrolled acceleration investigation # RQ10003

  On february 16, 2010, nhtsas office of defects investigation (odi) opened rq10-003 to determine whether the scope of prior toyota recalls relating to potential unintended acceleration were sufficiently broad, including, among others, toyota recalls 07e-082, 09v-388, 10v-017, and 10v-023.Nhtsa also requested information regarding potential electronic causes of unintended acceleration in toyota and lexus vehicles.I. Pedal interference from floor mats, carpet covers and plastic pads in carpets.During the rq10-003 investigation, odi reviewed a large volume of documents to assess whether additional vehicles should be recalled.Following the agencys analysis, nhtsa requested that toyota recall additional vehicles.Toyota complied with the agencys request.The details of these recalls are set forth more fully in toyotas reports to nhtsa pursuant to 49 cfr part 573.These recalls are as follows:recall 11v-112: (1) model year (my) 2004-2006 toyota highlander and highlander hybrid; and, (2) my 2004-2007 lexus rx330,rx350, and rx400h (hybrid model). The total estimated population under this recall is 769,379 vehicles.This recall remedies potential accelerator pedal entrapment caused by a loose floor carpet cover (trim panel). Potential electronic causes of unintended acceleration.After nhtsa opened rq10-003, nhtsa launched a ten-month study of potential electronic causes of unintended acceleration in toyota vehicles.Nhtsa launched the study in the spring of 2010 in light of concerns aired in congressional hearings.Nhtsa enlisted engineers at the national aeronautics and space administration (nasa) with expertise in areas such as computer controlled electronic systems, electromagnetic interference and software integrity to assess whether electronic systems or electromagnetic interference played a role in incidents of unintended acceleration in toyota vehicles.That study has concluded.Two reports are associated with the study and are available on nhtsas website.Nasas report is entitled technical support to the national highway traffic safety administration (nhtsa) on the reported toyota motor corporation (tmc) unintended acceleration (ua) investigation, nesc assessment no. Ti-10-00618 (jan. 18, 2011).Nhtsas report is entitled technical assessment of toyota electronic throttle control (etc) systems (feb. 2011).Both reports should be read in conjunction with each other. As stated in its report, nasa did not find an electronic cause of large throttle openings that can result in unintended acceleration incidents. Nhtsa did not find a vehicle-based cause of unintended acceleration incidents other than the physical pedal interference causes that are being addressed by toyotas recalls. This rq is closed.

Brake actuator failed at 117000 miles. Verified by dealer and suggest replace brake actuator and pump assembly. No notification ever

Car has been making a clicking noise during stops for a year. I chalked it up to something unimportant. On 2/20/21, the 'check avs,' 'check abs,' and 'check brake system' lights all came on, and car lurched while backing out of my driveway. I was driving my wife and daughter at the time. Brought lexus to my local mechanic on 2/22/21. After several days, mechanic returned the car to me, saying he could hear the clicking and, according to his research, it could be the brake actuator given lexus had done a warranty extension on this part in 2016. He told me this repair would cost in the thousands of dollars, and advised me to contact lexus. Spoke with lexus brand manager on 2/25 who told me about warranty extension zlg for actuators in 2016, and told me that in order for lexus to ascertain whether or not the car was experiencing an actuator failure, it must be inspected by a lexus service center. Because i was told about the specific extension and asked to take it to lexus to diagnose the issue, i assumed lexus would cover the repair cost. I took the car to lexus santa monica on 3/2/21 and was notified that the problem was related to the brake actuator, and that the cost to repair would be $3000 plus tax. I relayed to lexus sm what i was told by lexus corporate, and lexus sm told me they would call lexus corporate and see what they had to say about warranty extension 'zlg.' ultimately, lexus would not cover cost because the part was outside the extended warranty. I spoke with another lexus case manager about why a warranty extension was offered as opposed to a recall, and he said i should lodge a complaint with nhtsa. I also spoke with his supervisor, who said lexus would never pay for a part outside a warranty, even a faulty part given an extended warranty. Nobody in the lexus chain of command can tell me if the car is safe to drive. I will support class action suit.

Brake actuator failure has been added to a bulletin for extended warranty and not a recall. This is a costly malfunction that toyota is aware of and they should take care of all vehicles as not to compromise braking and accident avoidance.

This started a few weeks ago, and would happen intermittently, and progressively longer in the dysfunctional brakes phase. I had an employee at auto zone clear the code 2 days ago, but the problem started again, intermittently upon vehicle start, but would go away shortly, with normally responsive brakes. This intermittent issue occurs when i am driving, therefore this is a serious road hazard as my brakes become significantly less responsive when the problem occurs. In other words, this occurs when stationary and when driving, intermittently.

I purchased a used 2007 lexus rx350 for my 16 year old daughter. It had a little over 100k miles. Everything worked perfectly, no complaints whatsoever. Until, my son borrowed it, and while he was driving it, he said it quit. He lost all power steering, so he pulled over. We called a tow truck to tow it to a mechanic. When the mechanic called me, he said the oil cooling line busted, which made the oil pump empty the engine's oil. He said it would need a whole new engine. He said the car is worth nothing. I paid $9k for this car a year ago, and now it is not worth $100. The mechanic said this is a known problem for toyota/lexus, but there was no recall. To me - this is unacceptable. In my opinion, since toyota knew of this problem, but didn't recall it, they either need to cover the cost of a new engine, or buy the car back from me. I am very upset over this.
