• Recall 102061 [USA]: Deposits of contaminants may result in the circuits being bridged, creating a high resistance short, increasing the risk of a fire.
  Mack trucks, inc. (mack) notified the agency on february 20, 2014, that they are recalling certain model year 2010-2012 chu, cxu, gu and td vehicles manufactured from june 16, 2009, through july 8, 2011. While continuing to research the scope of the issue, mack expanded the recall on november 18, 2014 to include certain model year 2008-2009 and 2013-2014 chu, cxu, gu and td vehicles manufactured from june 16, 2009, through july 8, 2011. Having completed its investigation, mack once again expanded the recall during february 2015 to include an additional 53,780 vehicles from model years 2007 and 2015. The recall now covers model year 2007-2015 includes chu, cxu, gu and td vehicles produced october 13, 2006, through october 3, 2014. In the affected vehicles, the electronic power distribution module (epdm) may be contaminated from water leaks in, and around, the windshield. If exposed to water leaks, contaminants such as road salts may deposit on the epdm.
  mack has notified owners, and dealers will inspect the epdm. If contamination has occurred, dealers will replace the epdm and address the leak, free of charge. Owners of model year 2010-2012 chu, cxu, gu and td vehicles were initially notified april 24, 2014. Owners affected by the recall expansions were notified in february 2015. • Recall 95439 [USA]: If the low beam headlights go out, the driver may lose visibility, increasing the risk of a crash.
  Mack is recalling certain model year 2007-2010 chu and cxu vehicles, manufactured from october 13, 2006, through december 23, 2009 and equipped with daytime running lights and fog lights. A short in the fog light circuit may cause the low beam headlight circuit to cut out.

• Recall 31988 [USA]: This condition can cause a delay in the application of the spring brakes to park the vehicle after the operator pulls the dash valve button. these conditions can occur without warning, leading to unintended vehicle rollaway, brake drag, or in the case of loss of primary circuit, inability to modulate the spring brakes.
  On certain trucks equipped with parker single check valves (scv) provided by bendix. The defect exists in the parker scv that connects with the supply port of the sr-7 spring brake modulating valve. After prolonged use of the parker scv, the valve body retainer may become excessively worn and eventually break apart. Pieces of the parker scv can become lodged inside the sr-7 spring brake modulating valve, potentially causing either leakage out of the sr-7 valve or preventing air from properly exhausting from the sr-7 valve.

• Recall 88384 [USA]: This could delay or failure in applying the parking brakes which could result in a vehicle roll away, increasing the risk of a crash.
  On certain trucks equipped with bendix sr-7 spring brake modulating valves, the internal rubber check valve may leak causing a delay in the application of the spring brakes to park the vehicle after the operator pulls the dash valve button.

• Recall 32497 [USA]: If the condition exists and the key is left on without fuel in the filter, the heating element may heat to a point where a flame becomes present at the tip of the element which could result in a vehicle fire.
  On certain trucks, an unprotected lead in the thermoswitch of the heater may make contact with another post possibly creating a short circuit, which can result in continuous operation of the heating element.

• Recall 88985 [USA]: If the nut were to come loose and be left untreated, the steering shaft could potentially separate increasing the risk of a crash.
  On certain trucks, the nut used to retain the steering shaft may or may not be a lock nut that has the locking feature. A lock nut is required to provide additional resistance to ensure that the nut does not come loose.

• Recall 2014527 [Canada]: Electrical
  On certain model vehicles, water around the windshield can migrate into the electronic power distribution module and contaminate it. As a result, circuits may be bridged creating an electrical short. This could potentially result in a fire, increasing the risk of injury and/or damage to property. Correction: dealers will inspect the module, and if found contaminated, will replace it. Additional measures will be taken to address the leak if contamination is found. • Recall 2012300 [Canada]: Lights and instruments
  On certain trucks, an electrical short circuit in the fog light circuitry could result in a loss of low beam headlamp function. Loss of headlamp illumination, should it occur during a rainy/snowy day, or during hours of darkness, could compromise the driver’s ability to see the road and its users, which could result in a crash causing property damage and/or personal injury. Correction: dealers will add an extra wiring harness between the cab harness and the daytime running lamp (drl) module, in order to isolate the fog light circuit.

  On certain trucks, the locknut used to retain the steering shaft may not have had the lock feature. As a result, the nut could loosen, allowing the steering shaft to separate, causing a loss of steering control which could result in a vehicle crash with injury or death. • Recall 2007256 [Canada]: Fuel supply
  On certain trucks, when the ignition key is in the 'on' position, the pre-heater, located in the fuel filter base, is energized. If this pre-heater is energized while maintenance is being performed on the fuel filter, diesel fuel could ignite within the filter housing. A fire within the fuel filter housing may spread possibly resulting in property damage or personal injury. Correction: dealers will inspect and, if required, replace the fuel heater assembly.

  On certain school and transit buses, the parker single check valve (scv) that connects with the supply port of the bendix sr-7 spring brake modulating valve may become excessively worn and eventually break apart. Pieces of the parker scv can become lodge inside the sr-7 valve, possibly causing either leakage out of the sr-7 valve or preventing air from properly exhausting from the sr-7 valve. This condition can cause a delay in the application of the parking brakes, the spring brakes not fully releasing or loss of isolation between the primary and secondary circuits. These conditions can occur without warning, leading to unintended vehicle rollaway, brake drag, or in case of loss of primary circuit, inability to modulate the spring brakes. • Air compressor discharge line investigation # PE08069

  Odi opened this investigation based on a review of early warning field reports which indicated instances of the air compressor discharge line coming loose or separating on certain model year 2008 mack trucks.Further review indicated that mack had issued a service bulletin (#sb 531014) in may 2008.A revised air line was incorporated into production in may 2008 and the ferrule type fitting was changed to a 37 degree flare compression type fitting.The failure resulted in a diminished air supply to the trucks braking system.During the investigation, odi collected information from mack, examined warranty claims, discussed the problem with fleet managers and confirmed that the subject vehicles meet the requirements of fmvss 121, (air brake systems).This standard includes a requirement for an air pressure gauge, and an audible and visual warning signal. The air system normally operates at 100 -120 psi.If the air discharge line becomes loose, the system will continue to hold air but replenishment may be diminished depending on the degree of leakage.If the air line separates, air system replenishment will cease and air pressure will drop as system air is used during brake applications.As the air pressure drops, the dashboard mounted gauge provides a continuous pressure status. Drivers are required to have a cdl (commercial drivers license) with air brake endorsement and part of the training to obtain such a license includes learning how to monitor and respond to the air pressure system gauges.As the pressure drops below 60 psi, the audible/ visual warning device activates alerting the driver to take appropriate action to stop the truck in a safe location.If the air pressure is allowed to drop below 45 psi, the parking brakes will began to apply.Odi confirmed that approximately 94% percent of the trucks have "engine brakes" that provide supplemental braking and allow a driver with a leaking discharge line to use the compression of his engine to slow the truck, thus further preserving available air pressure.Using warranty claim data, odi contacted 47 owners/drivers to gain an understanding of the problem and learned that 16 of the 47 discovered the problem while the truck was parked.Of the 31 drivers that discovered the problem while driving only 13 found it necessary to stop on the roadways shoulder.Of those 13, four made temporary repairs in place and continued driving the truck. The remaining 18 were able to safely drive to a location away from the roadway to wait for assistance.Thus, 38 of the 47 vehicles surveyed were repaired in a garage or facility. Nine of the 47 were apparently towed.Odi has carefully reviewed the warranty claims and concluded that 639 reports are applicable in that warranty was performed on those trucks to replace or repair the defective air compressor fitting.Based on fleet interviews and warranty claim review, odi believes the vast majority of the warranty claim repairs were performed in safe locations.The data indicates that the majority of failures were resolved off the highway in a safe location due to the warning system required by fmvss 121 or during the drivers daily truck inspection required by the federal motor carrier safety administration.During the investigation, odi established that no crashes, injuries or fatalities have occurred as a result of this problemon may 21, 2009, mack announced that they would conduct an owner notification program to address this matter and as of june 24, 2009, 824 trucks have been repaired. After examining all the relevant facts (no crashes, deaths or injuries, mandated audible and visual warning, 94% of the subject vehicles have an engine brake, the onp has a 24% completion rate after 31 days, and the majority of repairs are made off the highway), odi believes that the onp is sufficient and further action is unwarranted at this time.Therefore, this investigation is closed.
