• Recall 146142 [USA]: An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.
  Mazda north american operations (mazda) is recalling certain 2003-2008 mazda6, 2004-2005 mpv, 2004 rx-8, and 2006-2007 mazdaspeed6 vehicles. These vehicles had their passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.
  mazda will notify owners, and dealers will replace the front passenger air bag inflator with an alternate one, free of charge. The recall began december 3, 2019. • Recall 34371 [USA]: If the tire is operated in an under-inflated condition, the load carrying capacity of the vehicle may be compromised, increasing the risk of a crash.
  The tire inflation pressure indicated on the vehicle certification label is not correct. The labels show the maximum tire pressure as 240 kpa/32 psi when it should be shown as 240 kpa/35 psi. As a result of this error, the load ratios for the 205/65r15 94h and the 215/60r16 95h tires exceed the levels permitted by fmvss 120.

• Recall 2018578 [Canada]: Airbag
  On certain vehicles, the passenger frontal airbag inflator could produce excessive internal pressure during airbag deployment. Increased pressure may cause the inflator to rupture, which could allow fragments to be propelled toward vehicle occupants, increasing the risk of injury. This could also damage the airbag module, which could prevent proper deployment. Failure of the passenger airbag to fully deploy during a crash (where deployment is warranted) could increase the risk of injury to the seat occupant. Correction: dealers will replace the passenger frontal airbag inflator. • Recall 2016259 [Canada]: Airbag
  Note: the recall for the 2004 rx-8 model has been superseded by recall 2018-038 (mazda recall 2018a), and the recall for the 2004 mpv model has been superseded by recall 2018578 (mazda recall 2618f). On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the passenger frontal airbag. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. These issues could increase the risk of injury. Correction: dealers will replace the airbag inflator. Additional note: canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected.

  On certain vehicles, the liftgate stay dampers and stud bolts may rust and stick together, causing the stud bolds to break off. In the worst case, this condition may allow the liftgate to suddenly drop during opening or closing, injuring the operator. • Recall 2004192 [Canada]: Label
  On certain vehicles, the tire inflation pressure indicated on the vehicle certification label is not correct. Each of the labels includes tire inflation pressure information in kilopascals (kpa) and in pounds per square inch (psi) for one of the three sizes of tires available on mpv's. In every case, the maximum tire inflation pressure specified in psi is incorrect while the information provided in kpa is correct. The labels show the maximum tire pressure as 240 kpa / 32 psi when it should be shown as 240 kpa / 35 psi. Correction: owners will be notified of the non-compliance by mail and will be provided with a corrected label that they can apply to their vehicle.

• Desiccated air bag inflator rupture investigation # EA21002

  From 2000 through 2017, takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("psan") propellant -- propellant 2004 and propellant 2004l. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 blomquist report at www.Nhtsa.Gov/sites/nhtsa.Gov/files/documents/expert_report-hrblomquist.Pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in us vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004l that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated psan inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All takata inflators produced with propellant 2004l contain desiccant, and none of these desiccated inflators using propellant 2004l are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated psan inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- takata (now known as tk global), the independent testing coalition, and exponent -- have been studying the long-term behavior of takata desiccated psan inflators using propellant 2004l (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004l is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004l.The office of defects investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated psan frontal inflators manufactured by takata. This investigation will require extensive information on takata production processes and surveys of inflators in the field. • Air bag inflator rupture investigation # EA15001

  The office of defects investigation (odi) opened pe14-016 in june 2014 based on six inflator rupture incidents involving consumer owned vehicles produced by five vehicle manufacturers.All six vehicles were operated in florida or puerto rico at the time of the rupture and for the majority of their service life, and were equipped with inflators produced by takata, a tier-one supplier of automotive air bag systems.During the course of pe14-016, odi determined that five additional vehicle manufacturers used inflators of a similar design and vintage also supplied by takata. No evidence of field failures was found in vehicles produced by these five additional manufacturers.Nonetheless, at odis insistence, all 10 vehicle manufacturers initiated a regional recall within approximately two weeks of the opening of the investigation.The regions recalled initially included florida, puerto rico, hawaii, and the u.S. Virgin islands, areas with high absolute humidity and climatic conditions believed to be a significant factor in the inflator ruptures.As part of the recall actions, inflators removed from remedied vehicles are to be returned to takata for testing.Takatas initial test results on passenger inflators from remedied vehicles indicated a much higher than anticipated rupture frequency for inflators returned from florida.Accordingly odi requested all 10 manufacturers expand the regional recalls for passenger inflators to include other geographic areas where high absolute humidity conditions exist, including the gulf states and other coastal areas.Takatas testing of the passenger inflators to date continues to indicate this geographic area as having the highest risk, with no ruptures occurring from inflators returned from outside the expanded recall regions.During pe14-016 four additional passenger inflator field events occurred, all in vehicles from the same expanded geographic region.Also during pe14-016 four additional driver inflator field events occurred including two in vehicles from regions not known for high absolute humidity, specifically california and north carolina.Accordingly, odi requested all five of the affected vehicle manufacturers currently using the subject takata driver inflators expand to nationwide recalls.Significantly, neither of the affected vehicle manufacturers or takata provided any explanation to account for these two driver air bag inflator ruptures outside the area of high absolute humidity.Takata testing of returned driver inflators indicates a lower rupture frequency as compared to passenger inflator testing.All test ruptures reported by takata to date have occurred on inflators returned from high absolute humidity areas.The investigation now includes all manufacturers and vehicles known to be affected at this time.Odis investigation will focus on, among other things, root cause analysis, other potential defect consequences, identification of affected vehicles scope, and adequacy of the remedy.The five odi reports cited above can be reviewed online at http://www-odi.Nhtsa.Dot.Gov/owners/searchnhtsaid under the following identification numbers: 10537899, 10568848, 10585224, 10605877, 10651492

Suspension subframe rusted and cracked.

Tl* the contact owns a 2004 mazda mpv. While the contact's wife was driving 15 mph, she struck another vehicle on the driver's side door. The air bags did not deploy. There were no injuries and a police report was filed. The vehicle was towed to an unknown location. The vehicle was not taken to a dealer to determine the cause of the failure. The manufacturer was made aware of the failure and informed the contact to send documentation from the dealer. The failure mileage was approximately 166,000.

Front cross member corrosion; frame damaged; car dangerously unstable. Wheel alignment cost me $72.64 and frame repair cost $693.36. The vehicle in the shop for 4 days. Mental anguish. Mazda dealer declined repairs. Mazda customer service declined to pay. Now they are admitting that problem on mazda 6 vehicles. It's an old problem probably related to metallurgy or improper paint protection.

I went into the dealership to get my safety recalls done at the mazda dealership on torrance but they said i needed to fill out a form and to wait for them to receive the auto parts. It has been more than 6 months since i went, i have not heard from the at all.

Tl* takata recall. The contact owns a 2004 mazda mpv. The contact stated that the recall notice for nhtsa campaign number: 16v354000 (air bags) was received in may of 2016. After contacting the manufacturer on multiple occasions, the contact was informed that the parts needed for the recall remedy were still not available and no estimated time for receiving the parts could be provided. The local dealer was not notified. The contact had not experienced a failure. Parts distribution disconnect.

Air bag inflator replacement first notice rec.8/1/16,called bomarito(local dealer)-no parts-would call called again1/2/17-same response second notice rec.4/17/17-called again same response.

Takata recall: i received mazda offer to replace defective part and accommodate for transportation while the car is being repair. The only requirement to have this accommodations is to make an appointment for them to deliver such offer to fix the inconveniences that this brings. I called to make the arrangements but now mazda refuses to give another transportation while fixing the car, even though this is included in their written offer.

There is no way to adjustment the alignment of the rear wheels on this year mazda mpv, thus the tires ware out early because they are not in alignment. Tire failure occurred while driving on highway mazda dealer says no adjustment can be done as mazda did not provide a way to correct this alignment problem so early tire failure can result. Mazda dealer work sheet for alignment states that the rear wheels are out of factory specification and if not corrected can lead to excessive tire wear, handling and safety issues. Mazda by there own admission did not provided any way to correct this safety issue. I'm driving a vehicle with tire safety issues due to lack of ability to align the rear wheels to mazdas own specifications. *tr
