    Nhtsa received a petition on or about july 18, 2022, requesting that federal motor vehicle safety standard (fmvss) 141 be applied to all electric and hybrid vehicles operating in the united states. The petition can be reviewed at nhtsa.Gov under odi number 11486072. Fmvss 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 kg or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after march 1, 2021.On january 27, 2023, nhtsa opened defect petition (dp) 22-005 to evaluate the subject matter described in the petition. On june 24, 2023 and as supplemented on june 25, 2023, the petitioner notified nhtsa he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioners withdrawal, dp22-005 is closed. Closure of this dp does not represent a determination by nhtsa regarding the subject matter of the petition.

While receiving an oil change by an independent mechanic, the contact was informed that there was a large crack to the engine cradle located on the undercarriage. The contact was informed by the mechanic that the vehicle was unsafe to drive. The contact notified the manufacturer of the failure and was informed that the vin was not under recall. The vehicle was not repaired. The failure mileage was approximately 159,000.

The contact owns a 2006 toyota prius. The contact stated that the vehicle intermittently failed to shut-off as needed with several unknown warning lights illuminated. The contact had taken the vehicle to the dealer on three separate occasions and the mechanic was able to either disconnect the battery or shut-off the vehicle as designed. The manufacturer had been notified of the failure and informed him that the vehicle was out of warranty and that there were no recalls on the vehicle. The contact filed a case with the manufacturer. The vehicle was not diagnosed nor repaired. The approximate failure mileage was 160,000. Consumer stated toyota is refusing to responsibility even with extended warranty if problem occurred after 2012.

The toyota prius 2006 was hit from behind (rear ended) on 05/13/2021 and driver's seat collapsed backwards, causing a head injury. This was not expected and is not acceptable by any safety standard. Fortunately there were no other occupants in the vehicle. This was a chain collision and the prius was stationary when it was hit in a secondary collision and still this happened, what if our car was hit in the primary collision or what if there was a child on the back seat?! The vehicle immediately behind was also stationary before it experienced the first order collision. These seats deserve a safety recall. One can find many other seat collapsing events, even with children killed or brain damaged on back seats. Place of the accident: the chesapeake expressway (168). Accident report attached.

While i am not getting any visual indicators of failure at this moment i believe (due to my own research) that when i do there is a high chance of me getting into a car accident. I have read on priuschat.Com that several people had brake failure due to the faulty pump which caused a car wreck. This is a dangerous predicament to be in and toyota should extend their warranty to cover the faulty equipment they have installed on these vehicles before someone gets seriously hurt or dies.

This is an unsafe condition and could happen while braking for a red light or braking to avoid hitting a deer. The cost to fix this brake actuator issue will be appx $3000. Upon further research, toyota knew of the issue and offered an extended warranty that expired dec 2017. There are several other complaints of this same issue with other '06 prius owners of this happening before and after the warranty expired in dec of 2017. This is a safety issue which could cause injury or death when the brakes do stop working and needs to be a recall, not an extended warranty through toyota that expires.

Known problem with combination meter - found it searching online once i realized this was the problem. I bought my car used and since it is no longer covered by toyota, it didn't come up in search. Having issues with there being no display on the dash and vehicle not shutting off for the entire time i have had the car. It only happens intermittently. This is a safety issue for sure! Can't see speed, gas, gears...Anything about car whether car stationary or moving.
