  • Recall 2021723 [Canada]: Accessories
Issue: this recall is for certain block heater kits installed on certain vehicles or sold as accessories at toyota or lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt. Note: this recall only affects the following block heater/cord part numbers: C014003034 pk5a408j09 pU14000904 C014003151 pk5a408j13 pU14000905 C014000885 pk5a489j11 pU14000907 C014000888 pk5a489j22 pU14000909 C014000907 pk5a410h00 pU14000911 C014100904 pU14000900 pU14000913 C014100907 pU14000901 pU14003571 C014100911 pU14000902 pk5a408j05 pU14000903 safety risk: engine block heater wiring that overheats or melts could create the risk of a fire. Corrective actions: toyota recommends that you should stop using the engine block heater immediately. Toyota will notify owners by mail and instruct you to take your vehicle to a dealer to disable the engine block heater. For some vehicles, the corrective actions for this recall are under development. For others, a new engine block heater cannot be installed, and the owner will receive a refund of the purchase price of the engine block heater.

  • Recall 2018015 [Canada]: Airbag
On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the passenger frontal airbag. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. These issues could increase the risk of injury. Correction: dealers will replace the passenger frontal airbag assembly. Note: canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected. Matrix model vehicles may have had the original front passenger airbag inflator replaced as a “like-for-like” replacement under a previous recall as a temporary remedy while evaluating a permanent remedy. These vehicles will receive the permanent remedy passenger airbag assembly under this recall.

  • Recall 2016103 [Canada]: Airbag
On certain vehicles, the passenger (frontal) airbag inflator could produce excessive internal pressure during airbag deployment. Increased pressure may cause the inflator to rupture, which could allow fragments to be propelled toward vehicle occupants, increasing the risk of injury. This could also damage the airbag module, which could prevent proper deployment. Failure of the passenger airbag to fully deploy during a crash (where deployment is warranted) could increase the risk of personal injury to the seat occupant. Note: this recall supersedes recalls 2013113, 2014224, 2015197 and 2015269. Correction: all vehicles having not received a replacement inflator as part of the previous recall will now have a replacement inflator installed by dealers. For matrix model vehicles, dealers may replace the front passenger airbag assembly with one equipped with a newly specified inflator.

  • Recall 2015269 [Canada]: Airbag
On certain vehicles, the passenger (frontal) airbag inflator could produce excessive internal pressure during airbag deployment. Increased pressure may cause the inflator to rupture, which could allow fragments to be propelled toward vehicle occupants, increasing the risk of injury. This could also damage the airbag module, which could prevent proper deployment. Failure of the passenger airbag to fully deploy during a crash (where deployment is warranted) could increase the risk of personal injury to the seat occupant. Note: this recall is superseded by recall 2016103. Please see recall 2016103 for more information. Click here for more information

  • Recall 2014560 [Canada]: Airbag
On certain vehicles, a defect in the supplemental restraint system could result in an inadvertent deployment of the front airbag(s) and/or seatbelt pretensioner(s). Unintended seatbelt pretensioner and/or airbag deployment, in a non-warranted (non-impact) situation, could startle the driver, which could result in a vehicle crash causing property damage and/or personal injury. In some instances, inadvertent deployment could cause minor injuries to vehicle occupants. Correction: dealers will replace the airbag control module and may install an in-line jumper wiring harness to correct the issue. Note: this recall supersedes recalls 2013032 and 2014097. Vehicles that were repaired under the previous campaign and did not receive a new airbag control module will also need to be repaired under this campaign.

  • Recall 2014224 [Canada]: Airbag

  • Recall 2013032 [Canada]: Airbag
On certain vehicles, a defect in the supplemental restraint system could result in an inadvertent deployment of the front airbag(s) and/or seatbelt pretensioner(s). Unintended seatbelt pretensioner and/or airbag deployment, in a non-warranted (non-impact) situation, could startle the driver, which could result in a vehicle crash causing property damage and/or personal injury. In some instances, inadvertent deployment could cause minor injuries to vehicle occupants. Correction: dealers will install an in-line jumper wiring harness to correct the issue.

  • Recall 2008122 [Canada]: Other
On certain vehicles, the regulator bolts - which attach the front side window glass to the power window regulator - may become loose, causing a loud noise to be generated when the window is raised or lowered. If the window is continuously operated in this condition, the bolts may eventually separate, causing the glass to become misaligned, and in the worst case, allowing the glass to completely shatter. Correction: dealers will replace the flange bolts

Known Recall Investigations
  • Side front power window shatters investigation as of 20080205

For the subject vehicles, there is a possibility that the retention force of the regulator bolts which attach the front side window glass to the power window regulator may not be sufficient due to an interference of the retention nut and the bolt shoulder radius. In this condition the bolt may become loose, causing a loud noise to be generated when the window is raised or lowered. If the vehicle and the power window are continuously operated in this condition, the bolt may completely separate, causing the glass to become misaligned, and in the worse case, the glass may shatter during operation of the power window.General motors corporation (gm) and toyota north america, inc. (toyota) will conduct a safety recall on the pontiac vibe, toyota corolla and toyota matrix vehicles to replace the regulator bolt with a bolt and washer.Gm will recall 122,593 my 2003-2004 pontiac vibe vehicles.Odi is aware of a total of 16,045 warranty and goodwill claims, 274 field and consumer reports, and 23 injuries for these vehicles.Toyota will recall 415,341 my 2003-2004 toyota corolla vehicles. Takata recall - while driving on a highway, there was a slow up, and i braked, but stopped right at the rear bumper of the car in front of me. However, another car was coming behind me and couldn't stop, or didn't notice that traffice had stopped. He pushed me into the car in front of me. At that moment, which was not a severe front end hit, i saw fire, an explosion, and the airbag blew out the entire dash on the passenger side, and flew to the back seat behind the driver's side. Also, the driver's side airbag deployed. Metal was flying out of the passenger side airbag in the front. We could not believe the damage that resulted from such a somewhat minor accident. Additionally, the car was totaled which has been a strain on us! Now, about 30 days after the accident, we just received a card in the mail telling us of the recall. We had no idea there is an ongoing problem with airbag recalls! Today, i checked online for this problem, and found many sites describing the issues. However, my car is not even on the recall list!!! I was in the car alone and was affected by the accident, however, i dread to think what would have occurred if i had a passenger with me. I am including some pictures.

I have been trying to get a recall 15v043 done since jan/feb this year and i keep calling the dealer and getting told they have never been sent the parts ever. I also have the air bag recall 15v285 and this is the second time ive called them regarding this recall as well. Wtf do they never get parts now? Whats really annoying is i keep getting letters in the mail regarding recalls and this last one says "why haven't we heard from you?" even though ive called multiple times.

Takata recall. I was told by a local portland, or toyota dealer that the replacement parts for the takata inflator recall are back-ordered for an indefinite 6-12 month period and warned that it could possibly take even longer. Toyota corporate customer service confirmed that timeline by speaking with a different portland, or toyota dealer. Toyota further informed me that i cannot have a passenger in the front seat until the takata inflator has been replaced. Toyota has been very upfront and helpful in attempting to resolve this issue, which is commendable, but i'm filing this vehicle safety complaint because 6-12 months for a safety recall repair does not qualify for being done in a "timely manner" and not being able to have a passenger in the front seat for up to a year will be very difficult and problematic. To be clear, the passenger airbag in my vehicle has not deployed.

I took my car to the toyota dealer to fix the airbag recall on 8/31/2015 and 1/20/2016. After fixing the air bag, i hear some kind of noise coming from the passenger side dash board next to air bag. I mentioned it to the dealer and have them look at it but they mentioned its nothing. I still have that noise coming out from there. I;m worried. Looks like they didnt replace the airbag inflator properly.

The engine burns excessive amounts of oil and has now starting making what appears to be either faulty lifter or camshaft noises. The matrix burns at least 1 quart of oil for every 1000 miles driven. I do regular maintenance and monitor the oil level weekly.

Takata recall i received a letter from toyota about a recall concerning my vehicle. In this letter, it states that my local dealership is stocked ( with a sub-wire harness and srs ecu ) and ready to complete my repair. I showed up to my local dealership on 12/12. I was told that the parts needed are not actually in stock and needed to be ordered. Possibly taking up to two weeks. After pointing out the difference in what what they are telling me versus what the notice states, i was dismissed. This recall is a very important recall that received much press due to the fact that the airbags may deploy without warning, causing injury. My main complaint is with the dealership's overall concern with the recall process. When i spoke to the service manager, i was told i wasn't understanding the notice completely. To remedy this situation, i would like someone to contact joe myers toyota dealership in houston to see if they are indeed handling this type of recall properly.

My daughter said the steering totally locked while being driven. Her & her boyfriend could not get the steering wheel to turn at all. They were not speeding because it was on wet roads. They both suffered mild concussions & my daughter has a severe neck sprain. After seeing the car, it went off the road & rolled 2 or 3 times it's totaled, i'm surprised they're not in icu! I did some research & almost 1000 matrix's have had this happen!! I don't know why there isn't a recall. It seems toyota isn't all that safe & doesn't care about it's customers at all.

Received a recall in the mail stating front passenger airbag inflator module is defective. Report to you nearest dealer to have part replaced. When i did they said they have not received the part to replace. Waiting until they receive the part from toyota main branch. Meanwhile in the safety notice for recall it specifically states do not operate vehicle with occupants in the front passenger seat. This vehicle is used for picking people up to be transported to appointments. How long can this be an inconvenience on not allowing anyone in the front seat. When asked how long will this take. I get no response for an answer on how long before parts will be shipped out to local dealerships. This is ridiculous to make a consumer wait for such a long period of time without any conclusion on how long to get a part. The last time i checked next day air was available anywhere in the world so what is the real problem with this recall. This is costing me money for not allowing anyone in this vehicles seat. I would appreciate a response or a deadline on when this can be resolved. Thanks hearing me on this issue. *tr

I received a letter from toyota, 19001 south western avenue, torrance, ca on a vehicle recall for my 2004 toyota matrix regarding a defect, which relates to motor vehicle safety. Toyota stated that the "electronic control module (ecm) for the supplemental restraint system (srs) ... Could have been manufactured with integrated circuits that are susceptible to internal short circuiting... If internal short circuiting occurs, the front airbag(s) and/or seat belt pretensioners could inadvertently deploy. Any airbag that inadvertently deploys can, under some circumstances, increase the risk of injury and the possibility of a crash." on january 4, 2014, i called phillips toyota located at 8629 u.S. Highway 441, leesburg, fl 34788, at tel. 352-728-2030 to schedule the installation of "... A sub-wire harness (filter) to minimize electrical noise from the other vehicle electrical components at no charge to me." i talked to a person by the name of "[xxx]." he told me that he would order the part and call me after it comes into the dealership. It is now february 19, 2014, and i still have not received a call from phillips toyota to schedule an appointment. This is a full 46 days and/or about 1 and 1/2 months since i called the dealership. I am very concerned about the "risk of injury and the possibility of a crash." the letter stated that should the air bag(s) deploy or the seat belt pretensioners, then there is an increase in the "risk of injury and the possibility of a crash." the recall letter from toyota stated that i could file a complaint if i feel the authorized dealership has failed or is unable to remedy the defect within a reasonable time. Under the circumstances, i feel that the dealership has failed and that six (6) weeks is an unreasonable amount of time. Low noise from engine area, then heard a pop or thud. After this came a scratchy/grinding noise when car was in gear and coasting. The noise went away when clutch was depressed or car was taken out of gear to coast. I called a service repair to make an appointment the same day. The very next day the car locked up and was undriveable. Sounded like the engine had fallen out the bottom with horrible metal grinding noise. I had to have it towed ($150) and the service center said i would need a new transmission and clutch and that this is a common problem with toyota matrix and corolla. After reading online, it seems there is a problem with toyota owners with low miles having bearing issues and that this problem has gone unaddressed. During daylight on the above date, as i made a turn from small parking lot into a parking space, car suddenly accelerated and crashed into concrete building. As it was accelerating, that split second, i checked my foot to make sure it was on the brake, and not gas. I pressed hard on it, to no avail. It did not respond. It was totaled by insurance company. I had minor injuries: bruised left chest and shoulder. Following day, a mild stiff neck. All of physical ailments gone, except mild sore neck, in 3 days. Note; i put speed at approximate speed in parking lot before acceleration occurred. *tr

I own a 2004 toyota matrix. It has less than 100,000 miles and have been well taken care of. On august 20, 2010, driving 30-35 miles per hour, the front axel broke! I did not run over a pot-hole, there was no accident---nothing to remotely cause the front axel to break. My 2004 toyota matrix xr with manual transmission has suddenly displayed severe mechanical noise from the engine/transmission area. Smog technicians wouldn't take liability to run engine for smog tests to register for the coming year, so is now registered as planned non-operation until i can get the car fixed. Googling "toyota matrix transmission noise" or "toyota matrix engine noise" leads to several sites in which too many owners are either experiencing complete engine loss due to "oil sludge problem" or total transmission loss due to "admitted transmission bearing design flaw". I will not drive this car again on the roadway due to the possibility of sudden and complete loss of transmission function at any time. This car is a great design for functionality, but i am totally dismayed and frustrated that at 120,000 miles, the car is off the road for serious engine and transmission flaws that appear to be very common, considering i bought toyota for the reliability that we all know so much about. I have kept up on the oil changes since they were pre-purchased for 5 years when i bought the car, and have kept fresh after the 5 years expired. Very disappointed with the fate that this car has presented me, and the costs that i will have to incur to get it back on the road. *tr

I have never had mechanical issues with my 2004 toyota matrix. Last weekend while trying to merge onto interstate highway 10 in az for a weekend camping trip without warning my engine seized while i was trying to merge onto the highway. The engine stopped and thankfully i was able to drive off onto the shoulder before the semi behind me rear ended me. I am thankful to be alive as well as the infant i had with me in the car. When we towed the car to a mechanic he accused me of driving the car without oil because the engine was full of sludge. Thankfully my father was with me and he explained to the mechanic that i regularly have my car serviced and we had oil in the car at the time of the incident. After some searching on the internet i have found that this is a common problem with toyotas. I can not understand if this is a known issue why toyota has not informed their customers so that we can protect ourselves and the passengers in our cars. *tr

2004 matrix transmission failed while wife was driving on highway on a rainy night. Prior to this the transmission began to have noises which one might attribute to an auto at 110,000 miles. After this failure transmission would not shift except with great difficulty. I had the matrix towed home and am in the process of determining solution. The rest of the car is flawless but this early failure of a manual transmission in a toyota is discouraging. The situation was a dangerous one as failure of the transmission caused those vehicles around her to become aggressive towards the slowly moving matrix. My wife was extremely fortunate to coax the car to safety. These unexpected repairs will cost me around $2500 to $3000 to resolve. While on the highway, our 2004 toyota matrix, approx. 98k miles started to buck and make a terrible grinding noise. We then lost 4th and 5th gears. We were fortunate to be able to take control of the vehicle and get to the side however we were stuck at night on the highway. The transmission bearings shredded causing the clutch to fail. Replacement cost $3500.00. Since, we had read about hundreds of other similar complaints about this vehicle and were shocked that toyota has not accepted responsibility for inferior parts and recalled the transmission. Toyota's reputation and thought of dependability and safety has faltered. It is sad because we have owned several other toyota's in the past but will not consider purchasing another. I have a 2004 matrix (5 speed manual) with 85k and the throwout bearing has failed. I was driving home from a concert with a few youth kids on saturday night. I was entering the 1-10e trying to merge in with traffic when the throwout bearing failed. I tried to shift into 3rd gear, but it was stuck in 2nd. I had to slam the gear into place to prevent getting run over by a suv. This was my second near-death experience this year in my car due to the transmission. Earlier, when the car was at 74k, the front axle on the transmission broke. This broke in the middle of a busy intersection where i had to keep swaying my body forward until the car had enough momentum to roll across. After replacing the front axle, i really do not have the money to fix another transmission issue. I have been doing research on this model year matrix to see if it was just my car doing this, but it looks like there have been other people with 2004 matrix that have the same problem. What happens is parts of the fly wheel or clutch dysfunction. It makes the car stop and it makes a grinding noise. My car has close to 70,000 miles on it now, but as i stated previously, it happened at around 20,000 miles the first time, which is unusual for a toyota, or any car for that matter. I owned a 2004 toyota matrix that was totalled in a car accident in 2006. I purchased the car new and was the only owner. At the time of the accident my sister was driving, and i was the passenger. Basically, we were driving on cruise control in the left lane of the highway, a semi cut us off, and when my sister pressed the brake pedal, the cruise control would not disengage and the car would not slow down. I witnessed her pressing the brake, and nothing was happening. My sister swerved into the right lane to avoid rear-ending the semi, lost control of the car, spun into the side of the semi, and then bounced off the semi and ran off the highway. The engagement of the engine, the speed, and acceleration of the car did not change throughout the entire course of the accident. When the police came out, both my sister and i described these events to the police officers. I also explained this to my insurance company. At the time, i believed so strongly that the car malfunctioned in some way that i tried to investigate further, but the insurance company was not interested. At the time i did some research to see if other drivers were having the same problems, but i could find nothing. Since the car was totalled, i could not keep the car to have it examined and i had little choice but to let the matter go. Now it is four years later and there is a massive toyota recall, but the 2004 matrix is not on the recall list. I feel obligated to file a complaint, not for gain on my part, but because i strongly feel that the mechanical malfunctions in toyota cars have been going on far longer than toyota will admit, and the malfunctions are much more widespread than the years and models currently listed. I also own a 2006 matrix that is not on the recall list. I am begging that there be a widespread investigation to make sure that all the affected years and models are added to the recalls. This must be done to save lives. *tr

Car did 360 degree turns across freeway (from left lane to right lane) and over the edge, down a hill, over a fence, and into a ditch. Minor injuries. We were lucky not to have been killed. Car was totaled. Accident is still being investigated to determine if there was a mechanical defect.*jb

I have a 2004 toyota matrix. When in drive, foot on the brakes, the car on occasions will lurch forward. This happens every day. I have taken it to toyota numerous times and they keep telling me there is nothing wrong. I have this documented back to 2005. *tr

My 2004 toyota matrix has expereinced sudden acceleration while my foot was on the brake several times since i have purchased it; i have always been able to stop the car from moving forward by further depressing the brake. But on saturday march 26 while at a stop sign, the sudden acceleration happened again and i was unable to immediately stop my car from lurching forward. It moved several inces into oncoming (perpendicular) traffic before i finally threw it into park and pulled the emergency brake to stop it. No accident occurred but it came close, and i had my toddler in the back seat, so i consider this a serious matter. I took it to my dealership (ourisman toyota in chantilly, va) that monday, and they kept it for two days. They ran all diagnostic tests possible and found no problem. Nothing was fixed because no test showed a problem but i am scared to drive my car -- the sudden acceleration happens only randomly and cannot be predicted. I drive with only one foot so this is not a case of two-footed driving and accidentaally stepping on the accelerator. In each case my foot was off the accelerator and on the brake. *ak

I'm very happy with my matrix except for one thing. The hub caps are so lose. I'm constantly pushing them back on tighter. I haven't had the car more than a few months and 3 hubcaps have already fallen off, withins days of me making sure they were tight enough. When they fly off my car, all the pieces that hold them onto the rim are broken off and they are useless. I have lost 3 hubcaps in the last 3 weeks!*ak

The front spoiler on the matrix is constantly getting scraped up on driveways. It is too low. The only way to fix the problem is to put a whole new bumper on the car, at my cost. The spoilers are listed as a option, but where i am from it is impossible to find a xr without them installed. The car is less then a year old, and already looks bad. Who would think they would design a car that would constantly get all scraped up. It would be nice if this "option" was truly a option.*ak

Front wheels vibrate at highway speed, if snow slush freezes in low profile steel wheel rims. This happened twice this winter to me, i first thought a wheel weight came off the shake was so bad, the dealership checked it out an said it was ice inside the back of the wheel rim, he melted it out an it was find. It happened again latter on in the winter you only feel the shaking if you bring the car up to highway speed,i have never had this problem with wheel rims before an i live in upstate n.Y. Where we get enough snow. When the ice melts or breaks out of the back of these deep dish rims the wheels stop shaking, i consider this a dangerous problem that should be addressed be fore some one gets into an accident because of it. The vehicle is a 2004 toyota 4wd matrix. *jb

Clutch failure at 5100 miles; --- complete-- replaced by dealer at 8600 miles---- clutch failure. Dealer refuses to deal with warranty. Tells me that it is my fault, " i don't know how to drive a manual transmission". Indicates that my wife and i are "riding" the clutch and perhaps "racing" mentioned vehicle. After a combined driving experience of nearly 3/4 of century on driving manual transmissions with zero problems, i seriously doubt that the cause of my clutch failures are my driving abilities or habits. Dealer service mentions that there is no evidence of history of complaints against this particular vehicle. Instead wishes to pass the "blame" and cause onto the customer. *jb

Takata recall. I've been receiving recall notices for this vehicle for years. I have been attempting to have three issues resolved, only one of which is the takata airbag problem. I have attempted to make an appointment for the repairs at the nearest toyota dealer to me (about a 2.5 hour-rt drive from my home) for the past year+, but i am repeatedly told that they are awaiting the part(s) for the takata airbag repair. To make things worse, i constantly receive flyers from toyota urging me to have the repairs done asap. I would like to. But apparently, toyota is having problems getting the parts to their dealerships. In the meanwhile, i'm driving a potentially dangerous car.

2004 toyota matrix purchased 12/8/03 is supposed to get at least 26 mpg in city traffic. Mine consistently gets no more than 21 mpg. Dealer can find nothing wrong but something is, either false advertising for the entire matrix line or something with this car. I would not have traded a paid up 99 corolla that got 26 mpg for this car, had i known it would cost me far more to run (as well as to finance). *la

I purchased a 2004 toyota matrix on november 10, 2003. The first day i pulled into my driveway which has a slight slope, the bumper dragged on the driveway. Even going very slowly, i realized that the front end of this vehicle was entirely too low. On november 17, pulling out from a store, i noticed it was still dragging and the drive was not as much of a slope as my drive. I notified classic toyota where i purchased the vehicle on november 17 about the problem. I went in to speak with them on november 22 since i lived an hour away. I was told that once i purchase the vehicle it was my responsibility. When they looked at the front bumper, they noticed that it was coming a part. I was told that i would have to pay for it. I took it to their parts department. The gentleman there told me that there was a major problem with the bumper and he had replaced several. He said they even had to replace the ones on their own lease cars and because of the constant complaints. This front bumper is barely 2 inches from the ground. Toyota told me to just park on the road whenever to avoid any sloops. I am 20 years old and was very proud of paying for my first vehicle. Toyota was aware of this problem and did nothing. I have taken all necessary care to use my vehicle in the manner it was intended, just drive it and my ability to do this is impaired by their design defect. The parts department will change the bumper out to their other make for this vehicle, but they are charging me for it. *la
