  • Hyundai and kia abs module fires investigation # AQ23002

    The office of defects investigation (odi) has received eight hyundai motor america (hyundai) and eight kia america, inc. (kia) part 573 recall reports between 2016 and 2023. The combined 16 safety recalls all involve the antilock braking system (abs) modules or hydraulic electronic control units (hecu) manufactured by the equipment supplier mando. The part 573 recall reports contain varying defect descriptions and differing remedy descriptions for the same or similar equipment supplied by mando. Safety recalls included in this audit query (aq) review include: hyundai recalls 20v-520, 20v-543, 21v-161, 21v-160, 21v-303, 22v-056, 22v-810, and 23v-651. Kia recalls 16v-815, 20v-518, 20v-519, 21v-137, 21v-331, 22v-051, 22v-800, and 23v-652 the purpose of this aq is to evaluate the timeliness and scoping of hyundai and kia’s defect decision making and adherence with reporting requirements; and understand the varying defect descriptions and remedies between these recalls.

  • Loss of motive power investigation # DP22003

    The office of defects investigation (odi) has received a petition on july 13, 2022, requesting a defect investigation into an alleged defect of the "oil drain pan assemblies" on the following 83 models/model year vehicles stated by the petitioner: 2006-2019 hyundai sonata, 2013-2018 hyundai santa fe sport, 2010-2012 and 2019-2020 hyundai santa fe, 2010-2015 and 2018-2019 hyundai tucson, 2011-2021 kia sportage 2005-2020 kia optima, 2011-2020 kia sorento, 2010-2013 kia forte, 2010-2013,kia forte coup, and 2007-2010 kia rondo vehicles equipped with the 1.6l t-gdi gamma, 2.4l theta ii gdi, 2.4l in-line gdi theta ii, kappa, and 3.8l v-shaped gdi lambda-ii engines. The petition also cites additional hyundai/kia vehicles that use similar drain pan assemblies as the above-mentioned vehicles as having drain pan assembly failures. The petitioner provided subject and peer vehicles information related to alleged engine failures. The petition itself can be reviewed at nhtsa.Gov under odi number 11481745.According to the petition, the hyundai/kia vehicle owners have made allegations of oil leakage failures occurring at highway speeds, without malfunction, indicator lights, or the leakage associated with under-tightening an oil drain plug, or a plug or pan with damaged thread. The alleged defects are causing engines to seize and loss of motive power. A defect petition has been opened to evaluate the issue and determine whether to grant or deny the petition. The petitioner identified a number of complaints received by nhtsa alleging complete engine failure from a variety of models stated above along with other models.   • Engine fires investigation # EA21003

    On march 29, 2019, the office of defects investigation (odi) opened two preliminary evaluations (pe19-003 on hyundai and pe19-004 on kia) to broadly investigate incidents of non-crash fires (regardless of origin or operational status of the vehicle) on various model year hyundai and kia vehicles.  during the pe investigations, hyundai and kia issued several recalls to address vehicle fires, including those due to engine failures involving various engine types.  the remedies for the engine failure recalls typically involve engine inspection (to detect existing damage), and if necessary, engine replacement.  additionally, both manufacturers have been installing an engine control software modification known as knock sensor detection software (ksds) which is intended to detect impending engine failure, alert the driver, and limit engine power (to lessen likelihood of engine failure).Ksds is being installed under recalls and non-safety field actions, and both companies have extended limited engine warranties (see recall files). On december 22, 2021, odi upgraded pe19-003 and pe19-004 to this engineering analysis (ea21-003, covering both hyundai and kia) to evaluate the scope of the prior recalls related to engine failures/fires and the efficacy of the recall remedies for the recalls, as well as the long-term viability of related programs and non-safety field actions being conducted by hyundai and kia.  the former issue involved approximately 550,000 my 2013 hyundai elantra, my 2011 kia sorento, and my 2012-2015 kia rio.  the latter issue involved approximately 2.5 million recalled vehicles, which have been identified as displaying potential remedy concerns: hyundai recalls 15v-568, 17v-226, 20v-746, and 21v-727; and kia recalls 17v-224, 19v-120, 20v-750, and 21v-844. Regarding the recall scope issue, among all of the scope vehicles identified at ea opening, odi found only the my 2011 kia sorento vehicles equipped with both the theta ii 2.4l 4-cylinder multi-point injection (mpi) and the lambda ii 3.5l 6-cylinder mpi engines experienced relatively high rates of reported fires.  in december 2023, kia issued recall 23v-877 that will cover about 80,000 my 2011 kia sorento vehicles equipped with the theta ii 2.4l mpi engines.  the remedy will involve an inspection, and if necessary, a replacement of the engine, as well as the installation of ksds.  it should be noted that in 2021 these vehicles were included in a ksds product improvement campaign with warranty extension.  for the my 2011 sorento vehicles equipped with the lambda ii 3.5l engines, odi believes that future engine failures/fires involving these vehicles will likely be due to a lax in adhering to the proper or scheduled engine maintenance (regular engine oil changes), rather than a design or manufacturing engine issue. Regarding the issue of the efficacy of the recall remedies, odi found the majority of the recalled hyundai and kia vehicles, by model, experienced lower reported fire rates after the remedies were applied.  furthermore, odi found that for most vehicles that did not receive the recall remedies, by model, experienced relatively high fire rates.  to address this problem, hyundai informed odi in december 2023, that it will renotify all of its owners covered by the hyundai recalls of engine failures/fires every 8 months for the next 3 years.  in december 2023, kia stated that it will renotify the majority of its owners every 8 months for the next 3 years.Several previously recalled kia models are excluded from the renotification due to their lower reported rates of engine fires.  overall, the renotification actions will help to increase the recall completion rates, thus reduce the frequency of engine fires in the future. In view of the recall action being taken by kia and renotification actions being taken by both kia and hyundai, odi is closing this engineering analysis.  also, both companies have been released from the 2020-iniated consent orders (kia since november 2022 and hyundai since november 2023).  the agency reserves the right to take additional action if warranted by future circumstances.  to review the reports cited in the closing resume odi report identification number document, go to nhtsa.Gov.

  • Throttle control system vulnerabilities investigation # DP21003

    The office of defects investigation of the national highway traffic safety administration received a petition letter dated october 10, 2021 requesting an investigation of an alleged defect(s) in the electronic throttle control system in certain model year (my) 2005-2016 kia optima/k5, my 2006-2015 kia sorento, my 2007-2016 hyundai elantra, my 2007-2016 hyundai santa fe, my 2006-2015 hyundai sonata vehicles. The petition was filed as a vehicle owner questionnaire that can be reviewed at nhtsa.Gov under reference (odi) number 11439675. The petitioners allege the subject vehicles exhibit a range of dangerous vehicle speed control problems including sudden uncommanded acceleration, runaway throttle conditions, surging, stalling and loss of motive power, all due to various defects and vulnerabilities in the electronic throttle control system.This defect petition has been opened to evaluate the allegation and determine whether to grant or deny the petitioners request.

  • Non-crash vehicle fires investigation # PE19003

    In a letter dated jun 11, 2018, the center for auto safety (cas) petitioned the national highway traffic safety administration (nhtsa) to initiate a safety defect investigation into non-collision related fires on model year (my) 2011-2014 kia optima and sorento, and hyundai sonata and santa fe vehicles.In a letter dated july 24, 2018, cas requested that my 2010-2015 kia soul vehicles be added to the petition.Cas stated that there is a higher number of complaints to nhtsa regarding non-crash fires on these vehicles compared to other similar vehicles.The office of defects investigation (odi) had already opened a recall query (rq17-004) on all hyundai vehicles equipped with theta ii engines for engine failures that may cause stalling and fires.The query covers most of the petition subject sonata and santa fe vehicles.On august 21, 2018, odi opened a defect petition (dp18-003) to evaluate whether to grant or deny the cas petition.Odi sent an information request letter to hyundai (september 2018) requesting information on all engine and non-engine related fires in the petition hyundai vehicles.Odi performed extensive analysis on the information obtained from hyundai.During the dp18-003 analysis, hyundai initiated three additional safety recalls related to engine compartment fires on certain petitioned and non-petitioned vehicles, (nearly 240,000 total vehicles: recalls 18v-934, 19v-063, and 19v-204). In addition to engine compartment fires, odi has received allegations of vehicle fires affecting other components, including taillight housings, taillight wire harnesses, and/or light bulbs. Based on odis review of the applicable materials, including responses to information requests sent to multiple manufacturers, vehicle owner questionnaires (voq), and early warning data, nhtsa has decided to grant the petition.The petition is hereby granted. A preliminary evaluation has been opened to assess the scope, frequency, and potential safety-related consequences of alleged defects relating to non-collision vehicle fires in the 2011-2014 sonata and 2011-2014 santa fe.While prior recalls of vehicles covered by this pe have primarily related to engine fires, nhtsas preliminary evaluation is not limited to engine components and may cover additional vehicle systems or components as well.Attached is a list of the reference numbers for the 160 voqs cited above.

  • Scope and timeliness investigation # RQ17004

    On september 10, 2015, hyundai motor america (hyundai) filed a defect information report (dir) recalling 470,000 my 2011 and 2012 sonata vehicles with “"theta ii" engines (recall no. 15v-568). Hyundais dir described the defect as an issue involving manufacturing debris. Hyundai limited the population of recalled vehicles to those produced on or prior to april 12, 2012, stating that a process change in april 2012 resolved the issue of manufacturing debris. On march 31, 2017, hyundai expanded its original recall to include 572,000 my 2013-2014 sonata and santa fe sport vehicles with "theta ii" engines (recall no. 17v-226). The dir continues to describe the defect as an issue involving manufacturing debris.This rq is being opened to investigate both the timeliness and scope of hyundais "theta ii" engine recalls, and hyundais compliance with reporting requirements.

  • Front axle shaft failure investigation # PE13025

    In a letter dated august 9, 2013, hyundai-kia america technical center, inc. (hyundai-kia) submitted two defect information reports (dir) to nhtsa identifying a safety defect related to fracture of the right front axle shaft in my 2013 front wheel drive hyundai santa fe sport vehicles built from july 13, 2012 through march 12, 2013 and my 2014 front wheel drive kia sorento vehicles built from january 7 through march 12, 2013 (recall nos. 13v-355 and 13v-356).According to hyundai-kia, the right front axle driveshaft may develop a crack in the area of the stub at the wheel end of the shaft due to problems with the stub suppliers forging process for axles installed in vehicles built prior to march 12, 2013.When moving from a stop, the crack can lead to a separation of the shaft, causing a loud noise and loss of motive power.In addition, should the driver fail to engage the parking brake on an inclined surface, the vehicle may roll.In either case, a crash could result.Odis review of consumer complaints and data submitted by hyundai-kia, identified 21 complaints alleging right-front axle shaft failure and 60 warranty claims related to right front axle replacements.All of the vehicles identified in the complaints and warranty claims were manufactured during the vehicle builds periods identified in the dir.The preliminary evaluation is closed.The odi report cited above can be reviewed at www-odi.Nhtsa.Dot.Gov/owners/searchnhtsaid under the following identification (odi) numbers: 10524563, 10524274, 10508727, and 10505768.

My vehicle had been stalling on i assign for a couple of months until one day in june 2021 it wouldn’t start at all. I got a new battery installed and assumed that all was well. Weeks later, i was in route to visit family when my abs, traction, and down hill break lights all came on. I pulled over and tried cutting my car off and on a few times to see if that would resolve the issue and it did not. My car also seemed to slow down a bit and struggled slightly accelerating up hills. In 2019 my turbo engine was replaced after my vehicle stopped accelerating while i was on the interstate. There is currently a recall on 2013 hyundai santa fe’s that reads: “ hyundai motor america (hyundai) is recalling certain 2013-2015 santa fe sport vehicles. Brake fluid may leak inside the anti-lock brake system (abs) module, possibly resulting in an electrical short… if the abs module short circuits, there is an increased risk of an engine compartment fire.“ unfortunately my vin is not included in the recall and i fear my car may catch fire any day now as a result of this failure.

The problem related to the class action for the theta ii gsd engine happened to my engine (rod bearing failure). This is causing the engine to knock. Will have to replace the entire engine with less than 100k on the vehicle. Ran the vehicle normally and then parked it. Once i got back in the vehicle, i tried to start it and it would not start. I brought it to the dealer, and they came back and said there is a rod bearing failure causing the engine to knock.

Tl* the contact owns a 2013 hyundai santa fe. The contact stated while driving 50 mph, the vehicle stalled with several unknown warning lights illuminated. The vehicle was taken to hyundai of north charleston (8485 rivers ave, north charleston, sc 29406) where it was diagnosed with the engine needing to be replaced. The contact also stated that metal shavings were found in the oil pan. The vehicle was not yet repaired. The manufacturer was informed of the failure and agreed to pay a partial amount for the engine replacement. The failure mileage was approximately 74,374.

We pulled off the highway to get gas and noticed smoke coming from the hood. We immediately parked at the gas station and my husband, our dog and i got out of the car and within minutes the entire front of the car was in flames. Several passersby stopped and sprayed the fire with their fire extinguishers. We called 911 and the fire department quickly arrived and put out the fire, but the entire front of the car was destroyed. The front of the car melted along with the windshield from the intense heat. The police inspector came and inspected the car and took photos. The car was then towed and our insurance company took possession of the car. There were no warning lights or messages prior to the failure. The only thing we noticed was a slight hesitation in power as we climbed a hill several times in the half hour preceding the failure. My husband, my dog and i would have been in mortal danger from the fire if we had not stopped immediately and gotten out of the car. The safety of other people nearby had also been at risk if the fire department had not arrived so quickly to put out the fire as the smoke began as we were entering the gas station and parked about 50 feet from the gas pumps to escape the vehicle. We have no idea what caused the fire. The engine had been replaced by hyundai due to a defect that had caused it to fail about 2 years prior to the current failure.

On a common day two weeks ago, i was driving my 2013 santa fe and i accelerated to pass a vehicle when it instantly turned off and began to emit smoke from the engine. I parked and waited for the smoke to finish coming out with the smell of burning plastic. I proceeded to call a tow truck and then a mechanic. The mechanic checked it and told me that the engine was stuck and had to be replaced.

First we heard what we thought was making a belt loose driving down the road in our local area of denver, then suddenly acceleration was not happening. Giving it gas and nothing. Check engine light came on. Made it home, thank goodness as acceleration was intermittent( and nothing over 25mph) but immediately had it towed to mechanic. Mechanic fixed one sensor and more problems arose. He felt it should be a warranty item but surprisingly its not. Now he thinks the resolve will be in the form of replacing the engine as he's run every other diagnostic test he could.
