    Known Recall Investigations
    • Ford Motor Company - High voltage battery contactor failure Defect Investigation # RQ23004

      On august 17, 2023, the office of defects investigations (odi) opened rq23-004 to investigate instances of loss of motor power due to overheating high voltage battery contactors in model year (my) 2021-2022 ford mustang mach-e vehicles (subject vehicles). This recall query (rq) was opened after receiving 12 consumer complaints alleging a high voltage battery main contactor failure in subject vehicles that were included in recall 22v-412 and remedied prior to the reported incidents. The remedy in 22v-412 was a secondary on-board diagnostic control module (sobdmc) software update to monitor contactor temperature and reduce battery power to prevent damage to the contactor, and a battery energy control module (becm) software update to monitor contactor resistance to identify an overheated contactor and reduce vehicle power to prevent further damage. On october 13, 2023, ford filed recall 23v-687 of 34,762 2021-2022 ford mustang mach-e vehicles produced from may 27, 2020 to may 24, 2022 with recalled bussed electrical center parts nk48-10c666-aa, nk48-10c666-ba, or lk98-10c666-ab. Affected vehicles are equipped with the extended range battery. Ford stated that preexisting damage to the contactors caused by heat and accumulated wear may reduce the effectiveness of the software update remedy in 22v-412. The remedy of 23v-687 is to complete a replacement of the bussed electrical center, or high voltage battery junction box. In addition, ford stated that subject vehicles equipped with standard range batteries, in comparison to vehicles equipped with extended range batteries, are less at risk of experiencing this failure due to their smaller battery capacity and lower output electric drive motors. In view of the recall action taken by ford, odi is closing this investigation. Odi will continue to monitor reports of this failure in both extended range and standard range models. The agency reserves the right to take additional action if warranted by future circumstances.       In a part 573 report dated february 24, 2020, mercedes benz usa, llc (mbusa) notified nhtsa of a safety-related defect in one 2019 smart fortwo electric drive vehicle. This filing was assigned recall no. 20v-107. Mbusa noted that this vehicle received an lg chem high voltage battery that may contain a defect allowing for an electrical arc, which can ignite inside the battery cells, increasing the risk of a fire.On oct. 13, 2020, hyundai motor america (hyundai) notified nhtsa of a safety-related defect in certain 2019-2020 kona electric vehicles. This filing was assigned recall no. 20v-630. Hyundai noted that the high-voltage battery system in the subject vehicles, supplied by lg, may have been produced with internal damage to certain cells of the lithium-ion battery increasing the risk of an electrical short circuit, which could result in a fire.On nov. 13, 2020, general motors, llc (gm) notified nhtsa of a safety-defect in all 2017-2018 and certain 2019 chevrolet bolt ev vehicles. The gm filing was assigned recall no. 20v-701. Gm noted that the vehicles were built with high voltage batteries, produced by lg, that may contain latent cell-level manufacturing defects posing a risk of fire when charged to full, or nearly full, capacity. On aug. 20, 2021, gm expanded this recall to include certain 2020-2022 chevrolet bolt ev and 2022 chevrolet bolt euv vehicles. The gm filing was assigned recall no. 21v-650. Gm stated, "the root cause of the failure is the simultaneous presence of two rare manufacturing defects in the same battery cell."on march 1, 2021, hyundai notified nhtsa of a safety-related defect in certain 2019-2020 kona electric and 2020 ioniq electric vehicles. This filing was assigned recall no. 21v-127. Hyundai stated that "the vehicles are equipped with lg produced lithium-ion battery cells where, if the anode (negative) tab is folded, the battery cell could allow the lithium plating on the anode tab to contact the cathode resulting in an electrical short, thereby increasing the risk of a fire while parked, charging and/or driving."on february 11, 2022, chrysler (fca) notified nhtsa of a safety-related defect in certain 2017-2018 pacifica plug-in hybrid electric vehicles. This filing was assigned recall no. 22v-077. Fca noted that pacifica vehicles containing hybrid battery packs produced by lg were involved in twelve reports of vehicle fires. However, fca has not yet determined whether the battery packs were defective or the root cause of the fires.On march 16, 2022, volkswagen (vw) notified nhtsa of a safety-related defect in certain 2021 id4 vehicles. This filing was assigned recall no. 22v-162. Vw noted that the high voltage batteries used in the vehicles may contain insufficient soldering points and thus contain unreliable connections inside the high voltage battery. As a result of the unreliable connections, the vehicles may breakdown or stall while driving leading to a crash. The purpose of this equipment query is to write to lg, and other companies that might have purchased the same or similar equipment from lg, notify them of this defect in any vehicles they manufactured, and to ensure thorough safety recalls are conducted where appropriate.

    • Tesla, Inc. - Battery management software updates Defect Investigation # DP19005

      In a letter dated september 17, 2019, mr. Edward chen petitioned the nhtsa to initiate a defect investigation of certain tesla model s and model x vehicles that received revised battery management software in one or more over-the-air (ota) updates from tesla, beginning in may 2019. The petitioner bases his request on vehicle fires that took place worldwide and ota software updates tesla made to the battery management system (bms) of certain tesla vehicles that resulted in loss of available vehicle mileage range and increased charging durations.On october 1, 2019, the office of defects investigation (odi) opened dp19-005 to evaluate the petitioner’s request. Information provided by tesla in response to odis information request letter for dp19-005 indicated that a firmware update that may limit maximum voltage was installed in certain my 2012 through 2016 model s vehicles (subject vehicles). The voltage limiting firmware is a dynamic algorithm that is enabled in vehicles with high supercharging use histories, which contributes to high usage stress to the high-voltage (hv) battery. Tesla sold approximately 61,781 subject vehicles in the united states and, through august 2021, the voltage limiting firmware had been enabled in approximately 2,062 vehicles. Through december 2020, odi identified 59 complaints from consumers alleging reductions in battery capacity (52) or charging speed (7) in the subject vehicles. Log data from these vehicles showed that the voltage limiting firmware had been enabled in about 58 percent (30 of 52) of the complaints alleging range loss. Subsequent updates have restored some or all of the battery capacity to vehicles affected by the voltage limiting firmware coupled with updates enhancing bms battery brick monitoring algorithms. None of the complaint vehicles have reported any thermal incidents or other safety hazards related to the hv battery.The five non-crash fires referenced in the petition include two fires that occurred in china in early 2019 involving vehicles that: 1) had recently completed supercharging sessions; 2) were at a high state-of-charge (soc) of the hv battery; 3) were parked with the battery cooling system shutoff; and 4) had histories of high-stress usage for the hv batteries. The three fires that occurred outside china did not involve the same fact patterns regarding vehicle state and charging history. The two fires that occurred in the united states include one involving a vehicle with no supercharging history that was driving when the fire occurred and another in which the origin of the fire was external to the hv battery. The fifth fire, which also originated external to the hv battery, involved a vehicle in germany that had been parked at a low soc for an extended period. To date, incidents of fires involving parked vehicles with recent supercharging and histories of high-stress use have only been observed in china, where high-stress use factors appear to be more common. Nhtsa is authorized to issue an order requiring notification and remedy of a defect if the agency’s investigation shows a defect in the design, construction, or performance of a motor vehicle that presents an unreasonable risk to safety. 49 u.S.C. §§ 30102(a)(9), 30118. Given the absence of any incidents in the united states related to fast charging, and the absence of any such incidents globally since may 2019, it is unlikely that an order concerning the notification and remedy of a safety-related defect would be issued due to any investigation opened as a result of granting this petition.       The subject vehicles are the first model year of a new design generation of the prius model line.The vehicles are powered by a hybrid drive system that uses both a regenerative braking system, which converts vehicle momentum into electrical energy to be stored in onboard batteries, and a hydraulic brake system more typical of the brake system used on non-hybrid vehicles.As is the case for other similarly designed hybrid vehicles the interaction and management of the two braking systems during abs braking is complex and requires a sophisticated control system.In a february 9, 2010 defect information report, toyota advised the agency of its plans to conduct a safety recall to address inconsistent brake feel and increased stopping distances that may occur during braking events requiring abs operation, such as would occur when braking on rough or low friction road surfaces.Toyota will reprogram the abs control unit with improved software that addresses this condition.Based on toyotas action the investigation is closed. When the failure occurs the equalizer smokes excessively, filling the bus with smoke.As these buses may be carrying 50 or more people, excessive smoking (with possibility of a fire) is a concern.A fire may also result.A preliminary evaluation is being opened to gauge the scope of the issue, the technical causes of the equalizer failures and to address the resulting public safety concerns.

    Tl* the contact owns a 2012 (na) jayco pinnacle equipped with Goodyear Marathon St tires, size: st235/80r16. While the vehicle was being towed at 55 mph, the driver's side tire experienced a blow out. The rv was taken to an independent mechanic to have the tire replaced. The contact was concerned that the trailer was sold with three year old tires. The contact also stated that the batteries in the rv exploded without warning. The trailer was taken to the dealer to have the batteries replaced. The dealer replaced the battery; however, the batteries exploded again after a few days. The manufacturer was notified of the failure. The dot number was unknown. The approximate tire and vehicle failure mileage was 1,400.

    2005 nissan maxima se problem shifting. Delay in engagement from p to d (about 1-2 seconds), followed by a strong jerk or bang. Once in gear, very high revs and engagement from 2nd to 3rd gear, along with 3rd to 4th. Same can be said on deceleration. 4th to 3rd, and all the way down... A definite traffic safety hazard. Just some of the other items i have had to foot the bill for on this 6-year old vehicle: - 3 a/c compressors - both high and low a/c compressor lines - 4 new tires due to cracking and bubbling on the factory tires (at less than 30k miles!) - front rotors due to warping and excessive steering wheel shaking when braking - lumbar support pump (driver's side) is broken - turn signal indicators out of alignment (defective glue to hold them in place?) - 3 batteries in under 5 years - power window motors are dying (not as quick as they used to be) overall, i have come to hate this car and the nissan brand. I will never buy a nissan (or infiniti) again.

    We own a 2008 honda civic hybrid. The ima battery frequently in hot weather goes down to nothing, and car has difficulty. Received the recall notice from honda regarding update to computer software. When i took the car in, i asked specifically if this would decrease the gas mileage, as the ima battery assist would not come on as frequent. I was told, "no, gas mileage should be the same" well, the gas mileage has decreased approximately 3-4 miles per gallon. The ima battery assist does not come on, and makes acceleration for passing and merging onto freeway difficult. If possible, would prefer to have the old software re-installed. I believe honda did this update on the software, as they were probably not wanting to replace anymore batteries at $3,000.00 a pop. The performance is lacking, and needless to say, we are considering trading the vehicle in. Honda needs to do what is right. We do not want this to turn into a toyota debacle.

    There is an electrical/ignition problem. We went through 2 good interstate batteries in a 2 year period and it still hesitates to start. Now the worse thing is the breaking system it is to small from the years(2008-2012 the series 1)breaking system came off of a dodge magnum which is way smaller than the 2009 dodge journey.It goes through rotors every 9000 miles,front pads every3500-4500 miles, and finally the rear pads last only about 5500 miles if your lucky and that is driving at a nice even pace most of the time. In 2012 and so on they made the series 2 dodge journey which they put a big brake kit on it and the brake and rotor wear is doubled. This in my mind for the 2009-2012 should be listed as a brake recall for sure. I've done the research and there is tens of thousands of people who are experiencing these same 2 problems. The electrical problem was taking to a good garage and the problem could not be figured out,it's either a short somewhere in the harness or some kind of sensor or module that the dodge dealership should have to fix. And as far as the brakes i think that the big brake kit that they used on the 2012 model should be put on the 2009 model and it should be called a recall. Nobody should have to spend that much money on brakes and rotors on a single vehicle in a lifetime. That's just unheard of. Please do everything you can do by reading and understanding this and reporting it to dodge so that are recalls on the brakes and the electrical ignition and they get properly fixed just as they should be.     This vehicle design allows for poor rear visibility in general; however, it is amplified in low light situations with the combination of high rear head rests, a glare on the back glass from the third brake light, and if you have a rear spoiler this makes for a really bad combination. Nighttime visibility in a jetta is the equivalence of wearing a blind fold. Oh and the salesman neglected to mention that the all weather package included a trunk that will not shut or stay shut in cold weather. Even the replacement latches need replacing. Vw is making big bucks on the consumer hoping to save a few dollars with fuel economy. Vw not only fails to address or acknowledge the defects with these vehicles, but to exacerbate the situation they attempt to lock you -the consumer into their monopoly, for example:keyless remotes in which the batteries are only replaceable by vw dealers, a synthetic vw oil, a very costly trunk latch, that with my experience you end up replacing annually...The list goes on. Whatever happened to old fashioned customer service? To sum it up my jetta has alot of bugs...Not to worry, i'm sure vw has patented the pesticide, just hope i'm able to afford that-can use the money i'm saving in fuel economy.

    Tl*the contact owns a 2007 forest river . While driving 55 mph, the rear of the vehicle began making loud banging noises. The dealer stated that the vehicle had not been loaded and it was too light. When the vehicle was fully loaded the noise was worse. The spring arch was not functioning and the shocks were destroyed. The failure mileage was 2,433 and the current mileage was 3,000. The consumer stated that the mh was taken to camping world in san martin, ca for inspection and repair of suspension system. The dealer replaced failed shock absorbers, added front sway bar, and replaced house batteries that had come in at time of sale.     I have a gem car made by dailmer/chrisler. The batteries do not charge fully. They come with 130 amp batteries and the charger only puts out 105 amps causing a constant undercharging of the batteries. I've just about used up my second set of oem batteries and only have 4,000 miles on the car. I take great care of these batteries as they are expensive. They advertised 30 miles per charge. I realize this would be a max, but on both sets after about 6 months of use i'm down to about 5 miles per charge. Just on battery cost i have spent about $0.42 per mile. I was never told the batteries would only last 2,000 miles or less. One problem i have learned is the charging curve on the onboard charger is not strong enough for the batteries. The charger charges the batteries to a max of 105 amps and the batteries are rated at 130 amps causing a constant undercharging of the batteries resulting in stratification and shortened life. Not sure what kind of a safty issue is involved besides the car having dead batteries in lanes of traffic. At the cost of a minimum of $850 for a set of batteries this breaks down to $0.45 per mile just on batteries. I have no idea as to the cost of charging the batteries each time. At the least i was lied to by omission as to the life of the battery pack and the cost per mile to operate. If you aren't the agency to coomplain to can you inform me where to go as i can't afford to keep buying batteries. Besides that, the car is fun to drive. I did try a slightly set of smaller batteries from walmart and they only lasted 6 months, now i'm about ready to buy my fourth set of batteries in four years. This will be reflected in the # of failulres below indicating each set of batteries. Safety issue would be related to unexpected dead batteries at anytime before the 30 mile range as advertised. *nm

    I own a 2001 ford focus ztech. My car was stalling at random in the middle of driving and finally wouldnt start at all. I took it to a dealership. They said my battery cables were melted, so they replaced them, rerouted them, along with my clips, etc. They couldn't figure out why my cables would be like that and said that is probably why my car doesn't start all the time, but they couldn't be positive so they recommended that i replace my starter as well. They charged me $998. This also happened to me in previous years. I went to the vehicle recall site and see that ford started a recall in 2003 on this very same problem. Why didn't i receive recall information on my car? I've stalled before in the middle of traffic last year for the same problem and had my battery replaced. This happens too much. I have small children's safety to worry about. What bothers me in addition to that is that the ford dealership that i used should have noticed this recall and related it to my vehicle's problem.     Auto xs car jumper cables purchased at aldi this week: model #91555, product code: 92645 10/15. Rubber coating on all 4 clamps started smoking profusely when i tried to jump my car. One of the clamps produced a flame. The rubber coating on the clamps covers the clamps up to the tip. The heat of the battery current melted the rubber on all clamps, producing smoke and flame.     Both fobs (car keys) went bad on my 2009 Nissan murano, unable to lock and open doors using the fob. I had to remove the key from the fob to unlock the doors. At night this was a safety issue as i had to struggle to take the key out of the case in the dark. Replaced batteries in the fob and did not resolve the issue. On january 22, 2014 i mailed a letter to Nissan of america asking for reimbursement of the new fob i had to purchase. Nissan was unable to offer financial assistance with the replacement of the fob as the vehicle being outside of the new vehicle limited warranty.     May 02 my vehicle was taken in for brakes and front end noise, the brakes were replaced and was told that the dealership did not have time to correct the front end problem (which they noted was covered by my warranty) and to reschedule for that problem. I did so in july 02 and at that time, they charged me for the work and tried to blame me for the problem so i paid $281.75 for this work which was previously noted that would be covered by my warranty. My battery has been replaced 3 times. The vehicle was overheating several times: in october 3, 2002, october 29, 2002, and in december 2002. I was forced to pay for at a cost of $280.52 for the dec. 2002 problem. In march 2003 the vehicle was taken in to the dealership again for front suspension noise, and it was determined that a right upper strut mount had callapsed. In may 2003 the vehicle has again overheated and they could not explain why the problem continues. The young man that i spoke with when i arrived indicated that they (the dealership) was unsure as to why the vehicle was overheating and to just drive it because his truck does it all the time on the highway and they had no explanation as to why the batteries that they were replacing kept cracking and leaking. When i expressed my concern regarding the problem and my safety concerns, he just blew me off as if i was just complaining. Once again, it is now july 2003 and for the 4th time, my vehicle is overheating and overheating. In addition on serveral occasions i also indicated to the dealership, that there was a vibrating and shimbling when my brakes are applied and it makes it hard to controll the vehicle, not once would they address the issue with me nor have i been notified or any defects or recalls with the brake system for the 2000 chevy malibu.     When i visited my sister in the midwest, she gave me an e-320 model mercedes to drive, and later offered the car for my keeping, which served as an extra car, and had not been driven much, as evident from the year and mileage comparison with other same year models. My sister expected me to pay maintenance costs, and i didnt mind the extra costs associated with such a high end car. I didnt even mind paying a few thousand a few years ago, to change both batteries, the alternator and the overhead dome, when the mechanic could not detect the origin of a battery drain. However, what transpired recently is an entirely different story. A few months ago, i noticed a smell of gasoline as i stepped into the garage, adjacent to the kitchen. After backing the car out of the garage, i repeatedly looked for any possible fluid leaks on the garage floor, but never found any. Therefore, i dismissed the matter as a non-emergency. The pcm causes stalling at a red light or at low speeds--got rear ended twice, vehicle not turning over/won't start, drains batteries (replaced it 3 times),(car was towed to chrysler because it wouldn't start) power steering mechanism leaks (replaced twice) causes electrical problems on dashboard, radio & cd player wont work (cd is stuck in player), causes car lock/alarm system to malfunction (replaced twice) car lock/system has locked me out of my car and fog lights/head lights won't turn off which then drains battery. Chrysler dealership "supposedly" replaced this pcm chip 1.5 yrs ago and the repair took a month and a half while i waited without a loaner car even though my chrysler extended warranty allowed me to have one! When i got the car back i noticed that the fog light/headlight indicator light was on and went back to dealer to tell him it's happening again and he indicated that i not ever return to his dealership. Now 1.5 years later, i have the same problems with my vehicle and it's the pcm chip causing them. I have lost my job from not being able to drive to work while they 'fixed' my car for a month, lost thousand of $$$ repairing this vehicle, my warranty is now expired, am still financing vehicle, but car won't start and is dangerous to drive because it stalls while driving, etc. The car isn't worth 3k and the repairs are in the thousands to fix all the issues. consumer wants an update on complaint. 1/27/10 *nj updated 02/01/10

    First, the battery dies if the radio is turned on for more than 10 minutes with the engine off. The car needed a jump-start. This happened three different times. Second, there is a delay when trying to start the car. The dealer replaced the battery stating "the battery was defective." less than a year later, same thing starts again. So i take it to the dealer for an inspection again. Ironically, this time the battery actually died at the dealership. After a few hours, the dealer states that the battery is "defective" again!! And now they're trying to make me pay for the old defective battery that they replaced. My 2007 yukon denali xl was purchased brand new and went through three new batteries. The vehicle only has 30,000 miles on it. What will happen if my wife and three kids are stranded again??? This is not a battery problem. It is an electrical problem gm is trying desperately to cover up. Please help. Getting very aggravated and frustrated with this issue. It may not seem like a life-threatening issue but this problem does have the potential to become one.

    The rotary engine rx8 (my 2005) continues to have starting problems. I have just over 100,000 miles on my vehicle. I've replaced the starter three times now, the spark plugs at least twice, three batteries. It is an unreliable vehicle and has had to be towed six times now. Obviously a dangerous situation in that you don't know if the car stalls if or when you can restart the vehicle. *kb
