• Recall 103304 [USA]: A failure of the hazard warning lights could reduce the trucks visibility, increasing the risk of a crash.
  Mack trucks inc. (mack) is recalling certain model year 2010-2014 chu, cxu, and mru, 2007 ct and cv, 2008-2014 gu, and 2011-2014 leu, and td trucks manufactured february 28, 2006, through november 8, 2013, and equipped hamsar electronic flasher. The hazard warning lights may fail.
  mack will notify owners, and dealers will replace the electronic flasher unit, free of charge. The recall will begin by mid-june 2014. Interim letters went out to owners on june 23, 2014. • Recall 102075 [USA]: Deposits of contaminants may result in the circuits being bridged, creating a high resistance short, increasing the risk of a fire.
  Mack trucks, inc. (mack) notified the agency on february 20, 2014, that they are recalling certain model year 2010-2012 chu, cxu, gu and td vehicles manufactured from june 16, 2009, through july 8, 2011. While continuing to research the scope of the issue, mack expanded the recall on november 18, 2014 to include certain model year 2008-2009 and 2013-2014 chu, cxu, gu and td vehicles manufactured from june 16, 2009, through july 8, 2011. Having completed its investigation, mack once again expanded the recall during february 2015 to include an additional 53,780 vehicles from model years 2007 and 2015. The recall now covers model year 2007-2015 includes chu, cxu, gu and td vehicles produced october 13, 2006, through october 3, 2014. In the affected vehicles, the electronic power distribution module (epdm) may be contaminated from water leaks in, and around, the windshield. If exposed to water leaks, contaminants such as road salts may deposit on the epdm.
  mack has notified owners, and dealers will inspect the epdm. If contamination has occurred, dealers will replace the epdm and address the leak, free of charge. Owners of model year 2010-2012 chu, cxu, gu and td vehicles were initially notified april 24, 2014. Owners affected by the recall expansions were notified in february 2015. • Recall 99662 [USA]: If the nut comes loose, the hub assembly may separate from the vehicle, increasing the risk of a crash.
  Mack trucks is recalling certain model year 2013-2014 chu, cxu, gu, leu, and mru vehicles manufactured with pro-torque axle spindle nuts from august 28, 2012, through august 16, 2013. The pro-torque nut may not have been installed correctly, which may result in a loosening of the nut.

• Recall 97071 [USA]: A loss of brake function at one wheel will lead to increased stopping distances and pulling to the left or right when braking, which may increase the risk of a crash.
  Mack trucks, inc. Is recalling certain model year 2013-2014 chu, cxu, gu, mru, and leu trucks manufactured july 16, 2012, through march 18, 2013 and equipped with drum brakes. The clevis pin for the brake slack adjuster may be missing a cotter pin. If the cotter pin is not installed, the clevis pin may loosen and fall out causing a loss of brake function on the affected wheel.

• Recall 2016642 [Canada]: Steering
  On certain vehicles, the front hubs do not support the gross axle weight rating of the front axle. Specifically, 5,987 kg (13,200 lbs.) rated front hubs were used on trucks requiring a 6,622 kg (14,600 lbs.) rated front axle. If the axle load exceeds the maximum weight capacity of the hub, there is an increased risk of hub and bearing failure, which could cause a complete separation of the wheel(s) and a loss of vehicle control, increasing the risk of a crash causing injury and/or damage to property. Correction: dealers will replace the front hub assemblies with hub assemblies rated for 6,622 kg (14,600 lbs). Note: if damage occurs due to overloading, the abs malfunction light may illuminate as the distance between the abs tone ring and the sensor increases (i.E. Sensor loses contact with the tone ring).

• Recall 2016495 [Canada]: Powertrain

  On certain vehicles, the fifth wheel locking mechanisms could be susceptible to damage and/or wear. This could cause the locking mechanism to fail properly engage, potentially resulting in a towed trailer unit separating from the truck tractor. As a result, the trailer could strike another vehicle, stationary object, or bystander, causing injury and/or damage to property. Correction: fontaine dealers will replace all ultra lt fifth wheel units with ultra nt model fifth wheel units.

  On certain vehicles, the chassis electrical harness and air brake lines may not have been properly secured at the crossmember. This could cause the electrical cables and air lines to fall down and get twisted in the driveline. This could affect safety systems such as brakes, increasing stopping distances or causing unintended parking brake application. This could increase the risk of a crash causing injury and/or damage to property. Correction: dealers will inspect and repair the vehicle as needed.

• Recall 2015100 [Canada]: Accessories
  On certain vehicles, the fifth wheel remote unlocking switch may not work as designed. As a result, the driver may inadvertently release the 5th wheel latch when the park brake is applied if he/she presses or bumps the switch. This could cause the trailer to decouple from the truck increasing the risk of injury and/or damage to property. Correction: dealers will add a redundant measure such as an additional switch or adding a thumb lock that requires 2 actions to push the switch.

  On certain model vehicles, water around the windshield can migrate into the electronic power distribution module and contaminate it. As a result, circuits may be bridged creating an electrical short. This could potentially result in a fire, increasing the risk of injury and/or damage to property. Correction: dealers will inspect the module, and if found contaminated, will replace it. Additional measures will be taken to address the leak if contamination is found. • Recall 2014146 [Canada]: Lights and instruments
  On certain vehicles, the directional/hazard signal flasher may fail. Actuation of the turn signal or hazard lights in this condition would cause them to remain illuminated and not flash as intended. As a result, drivers may not be able to clearly signal their intentions to turn, change lanes or warn others in an emergency situation. Correction: dealers will replace the turn signal/hazard flasher module.

  On certain vehicles, spindle nuts may have been installed incorrectly and could loosen, possibly resulting in the hub, wheel and tire assembly separating from the vehicle. Also, should a wheel separate from the vehicle, it could pose a danger to nearby motorists or bystanders, which could result in injury and/or property damage. • Recall 2013161 [Canada]: Brakes
  On certain trucks, the cotter pin which retains the brake slack adjuster’s clevis pin may have been omitted during vehicle assembly. As a result, the clevis pin could loosen and fall out, causing a loss of brake function at the affected wheel. This could cause the vehicle to pull to one side under braking, or increase stopping distance, which could result in a crash causing property damage and/or personal injury. Correction: dealers will verify cotter pin installation on each brake slack adjuster. Replacement cotter pins and/or clevis pins will be installed as needed.

  On certain vehicles, the wheel rims may have been manufactured incorrectly and could fail. A wheel separating from the vehicle could cause a loss of control and a crash. In addition, the errant wheel assembly could strike a vehicle, stationary object, or a bystander. These issues could result in property damage and/or personal injury. • Mdrive transmission failure to disengage investigation # PE20005

  In a review of field reports submitted under early warning reporting (ewr) 49cfr 579.22(c), over several years by mack trucks, the office of defects investigation (odi) identified a trend of drivers who complained that their transmissions failed to disengage from gear while slowing the vehicle. This failure to disengage resulted in the truck wanting to pull through the service brakes when coming to a stop. Affected trucks are equipped with an mdrive automated manual transmissions. The repair has involved the replacement and/or adjustment of the service brake switch.Odi previously investigated (pe13-268) a similar concern on 2007-2012 volvo trucks equipped with an i-shift automated manual transmission in which the clutch would not automatically disengage as intended. The investigation resulted in volvo releasing safety recall 13v-268.Both volvo i-shift and mack mdrive transmissions share similar configurations in their design and function.
