  • Windshield trim molding item detachment investigation # PE23001

    On january 27, 2023, the office of defects investigation (odi) opened pe23-001 to investigate allegations of a-pillar trim panel detachment on model year (my) 2011-2019 ford explorer vehicles. At the time of opening, the office had received 164 vehicle owner questionnaires (voqs) reporting this failure. Odi has received 671 voqs regarding this failure, including 1 alleged crash and 2 alleged injuries. Some consumers have reported that the failure occurred while they were driving at highway speeds, with the detached trim panel entering the path of travel of the vehicle behind them, requiring the driver of the following vehicle to take evasive action. It is noteworthy that motorists who experience this type of road hazard are less likely to accurately identify the subject vehicle the item detached from and report the event to the nhtsa. In its april 17, 2023 response to odi’s information request, ford provided 175 consumer complaints (including field reports) and 14,162 warranty claims related to a-pillar trim panel detachment. Ford relayed that the subject vehicles utilize a plastic a-pillar bracket, which is bolted to the vehicle structure during assembly. The a-pillar trim panel is attached to the bracket utilizing 7 plastic clips. Contributory factors which have been identified by ford as leading to the subject defect include incomplete installation at the assembly plant, inadequate part stack up, damage to fasteners during installation, and failure to follow proper repair procedure during windshield replacements or other services. On january 19, 2024, ford filed safety recall 24v-031 for my 2011-2019 ford explorer vehicles to address the subject defect. In the part 573 safety recall report, ford identified that a detached a-pillar trim panel can create a road hazard for other road users, increasing the risk of a crash. The recall remedy includes an inspection of the existing a-pillar trim panel and, if necessary, the replacement of the component. The remedy a-pillar trim panels will be installed with an additional adhesive for robustness and utilize an inspection procedure to ensure full engagement of all the retention clips during installation. In view of the recall action being taken by ford, odi is closing this preliminary evaluation. The agency reserves the right to take additional action if warranted by future circumstances.   • Ford explorer exhaust odor investigation # EA17002

    Odi opened preliminary evaluation (pe) 16-008 in july of 2016 after receiving 154 vehicle owner questionnaire (voq) reports reporting exhaust odors in the vehicle occupant compartment of model year (my) 2011 to 2015 ford explorers, which included one low speed crash event without injury.During the course of the pe, odi identified additional voq reports concerning the exhaust odor issue, totaling 791 as of this writing.The complaints cover my 2011 to 2017 explorers.Eleven of the reports involve police interceptor vehicles, as do two other non-voq incidents odi identified during pe16-008.Overall, odi has identified three crash events and 25 injury incidents citing a total of 41 injuries.The alleged injuries, as affirmatively indicated on the voq reports, range from unspecified to loss of consciousness, with the majority indicating nausea, headaches, or light headedness.One police incident alleged a crash with related injuries, and a second police incident reported a physiological injury allegedly from carbon monoxide (co) exposure.Another reported police incident resulted in a rollover crash event with injuries.The ford reports cited above were provided in the company’s august 24, 2016 response to an odi information request letter sent during pe16-008.Therein, odi identified 2400 reports (485 owner complaints, 1254 warranty claims, 606 dealer field reports, 55 legal claims), involving 2,051 unique vehicles, that appear to relate to the exhaust odor issue, and include 123 reports that are duplicative of the voq reports.A number of the ford reports also discussed health effects similar to the voqs, specifically nausea and headaches.During the pe, working in conjunction with nhtsas vehicle test and research center (vrtc) in east liberty, ohio, odi has tested multiple vehicles, including complaint vehicles.Additionally, odi has conducted field inspections of complaint vehicles and crashes involving police units that occurred while the officers were on-duty.When possible, data has been collected to quantify carbon monoxide (co) levels in the examined vehicles.During this engineering analysis (ea), the vrtc testing efforts and field inspections will continue.To date, no substantive data or actual evidence (such as a carboxyhemoglobin measurement) has been obtained supporting a claim that any of the alleged injury or crash allegations were the result of carbon monoxide poisoning, the alleged hazard.Odi has obtained preliminary testing that suggests, however, that co levels may be elevated in certain driving scenarios, although the significance and effect of those levels remains under evaluation as part of the ea.Ford has issued multiple tsbs related to the exhaust odor issue, and in some cases revised those documents multiple times to provide dealership technicians with procedures to address complaints raised by consumers and police fleets.Concerns over the effectiveness of the procedures have been raised by vehicle owners in some cases.During the ea, odi will continue to evaluate the effectiveness of the tsbs.Through cooperation with police agencies, odi recently learned that the police interceptor version of the ford explorer is experiencing exhaust manifold cracks, which appear to present a low level of detectability, and may explain the exhaust odor.During the ea, the root cause, frequency, and safety consequence of these manifold cracks will be evaluated, as will the extent to which non-police ford explorers are experiencing cracked exhaust manifolds.

A pillar trim broke and flew off. This happened on both sides of the windshield. Passenger side hit windshield and ejected over vehicle. Drivers side slammed to drivers door window then continued on behind vehicle. If window had been open the sharp edges plastic would have hit driver. Motorcycles or convertibles behind vehicle may also be in danger.

Currently left rear knuckle and link failing and needs to be replaced. Dealership service dept will not cover under warranty even though ford had put out a recall for other 2017 ford explorers. Recall# 19v435000. Ford previously replaced under warranty the rear left wheel bearing and then rear axle on two separate service visits, which both services required removal and repressing rear knuckle bushing. Now several months later, i am experiencing severe loud noises from my left rear knuckle.

Initially the cars check engine light came on intermittently for about 3 days, then one day while driving the wrench light came on and the car immediately started decelerating - almost as if i was breaking hard, and would not accelerate over 30. After turning it on and off multiple times the light went away for days until my appointment with the mechanic, that same day it came back on. We went to a ford dealership and they said it was the throttle body and repaired it. Two weeks later the same issue occurred. I know this was a big issue in 2016 and earlier models, i believe the problem was never fixed in 2017 models. Our car is currently in the shop and they are "emailing ford" about what to do. The first incident i was on the interstate when i stopped abruptly and the second was on the highway. I luckily had no one behind me both times otherwise this could've easily caused an accident.

The driver's and passenger side windshield pillar moldings are loose and coming off and making a vibration sound while driving at any speed over 30 mph. I have read multiple complaints of them flying off while driving and potentially causing an accident to other motorists. Plastic molding at the doors is be melting and warping in several areas.

How was your safety or the safety of others put at risk? I am concern with the health issues that this smell could put my family and i in. Specially if theirs a possibility that carbon monoxide could be leaking into the cabin. Has the problem been reproduced or confirmed by a dealer or independent service center?No has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? I had the vehicle inspected by ford but they advise that they couldn’t reproduce the smell. They also advice that their were a two drain plugs missing in the lift gate which would resolve the issue if replaced. I have replaced the two missing plugs and continue to experience the issue. We have approximately 37000 miles on our 2017 explorer. While driving on the interstate yesterday, it began making a near continuous wind vibration sound around the windshield. We stopped several times on our trip trying to determine where the sound was coming from—roof rack, something under hood, trim pieces, windshield support areas?—but we’re never able to find cause or stop the noise. We have an appointment at a ford dealership later this week.
