Known Recall Investigations
  • Loss of motive power investigation # EA23002

    The office of defects investigation (odi) received three letters from consumers in march 2022, requesting the investigation of an alleged defect in the valvetrain of model year (my) 2021 ford bronco vehicles equipped with 2.7l ecoboost engines. The petitioners allege that my 2021 ford bronco vehicles are experiencing catastrophic engine failure resulting in loss of motive power in a variety of driving conditions with no ability to restart. On may 27, 2022, odi opened a defect petition (dp22-001) to evaluate whether to grant or deny the petitioners’ request to investigate. Odi identified 26 vehicle owner questionnaires (voqs) related to the alleged defect, leading to the granting of the petition. A preliminary evaluation (pe22-007) was opened on july 22, 2022, to assess the scope, frequency, and potential safety-related consequences of the alleged defect. Odi sent an information request (ir) letter to ford requesting material for both the subject 2021 ford bronco (25,619 vehicles), as well as additional ford model/model year vehicle populations equipped with 2.7l ecoboost engine including the my 2022 bronco (33,106 vehicles), my 2021-2022 f-150 (155,619 vehicles), my 2021-2022 edge (6,889 vehicles), and my 2021-2022 lincoln nautilus (8,596 vehicles). In their response to odi’s request, ford provided evidence of 328 customer complaints (including field reports), 487 warranty claims, and 809 engine exchanges relating to these vehicle populations. The provided data is comprised of reports of engine failure resulting in several consequences, the most common of which being loss of motive power while the vehicle is in motion. Additionally, analysis of ford’s ir response revealed that the alleged defect is present across the “nano” engine family, which includes both the 2.7l and 3.0l ecoboost engine variants. Vehicles which offer the 3.0l ecoboost engine as either standard or optional equipment include my 2021-2022 explorer and my 2021-2022 lincoln aviator. These model/model year populations will be included within the scope of the engineering analysis and updated population and failure data will be evaluated. During the investigation, multiple contributing factors were identified which can lead to the fracturing of the intake valves in the subject engines. Ford acknowledged that a fractured intake valve can result in catastrophic engine failure and a loss of motive power and noted that following a valve fracture, a vehicle typically requires a full engine replacement. Ford advised odi that the defective valves were manufactured out of a specific alloy known as “silchrome lite”, which can become excessively hard and brittle if an over-temperature condition occurs during machining of the component. A design modification was implemented in october 2021, which changed the intake valve material to a different alloy known as “silchrome 1”, that is less susceptible to over-temperature during machine grinding. Ford has identified that the defective intake valves commonly fail early in a vehicle’s life and has suggested that the majority of failures have already occurred. Odi is opening an engineering analysis (ea) in order to 1) evaluate the scope and frequency of allegations across the expanded scope of model/model year vehicle populations equipped with the 2.7l and 3.0l ecoboost engines, 2) coordinate with nhtsa’s vehicle research and testing center (vrtc) to evaluate field return parts, 3) analyze the effect of time-in-service on the failure rates for the affected engines, and 4) gather and review additional information to evaluate the effectiveness of ford’s manufacturing improvements regarding the alleged defect.   • Rear axle bolt failure investigation # RQ23002

    On july 17, 2023, the office of defect investigation (odi) opened this recall query (rq) after receiving 2 consumer complaints alleging loss of vehicle forward power and/or loss of transmission torque of the rear wheels in model year (my) 2020-2022 ford explorer vehicles (subject vehicles). The rq was opened to focus on the effectiveness of the remedies of recalls 22v-255 and 23v-199 related to the alleged problem. Recall 22v-255 involved replacing the bushing and axle cover in the following subject vehicles: 3.3l fhev police built before october 14, 2020, and 3.3l gas police vehicle built before january 31, 2021. For 2.3l rwd, 3.0l phev, 3.3lfhev, and 3.0lst retail (non-police) vehicles, the recall provided an pcm software update to engage the electronic park brake (epb) when park is commanded. The recall addressed a breakage of the rear axle bolt that can cause the driveshaft/half shafts to disconnect, resulting in a loss of transmission torque to the rear wheels necessary to hold park. If the parking brake is not applied, the loss of the primary park torque will allow the vehicle to roll in park increasing the risk of crash and injury. Recall 23v-199 addressed 674 of the subject vehicles produced from february 2, 2019 to april 4, 2022, which received an incorrect remedy under recall 22v-255. The affected vehicles had received a previous powertrain control module (pcm) update which did not include an automatic epb engagement when vehicle is shifted to park. On october 17, 2023, ford filed a new safety recall (recall 23v-675) covering 238,364 my 2020-2022 explorer vehicles with the following powertrains: 3.3l fhev police, 3.3l gas police, 3.0l gas police, 2.3l rwd, 3.0l phev, 3.3l fhev, 3.0l rwd and 3.0l awd, produced from october 22, 2018 to july 6, 2022, equipped with a 3-point mounted axle design. Ford stated that the cause of the bolt fracture is due to joint design not robust to peak axle input torques and manufacturing variability. The primary contributor is insufficient bearings for maximum joint loads. This can result in bearing deformation, increasing bending stress on the bolt, which may lead to fatigue failure. On january 19, 2023, ford amended recall 23v-675 to add 1,942 my 2020-2022 explorer vehicles with the following powertrain: 3.0l rwd platinum and king ranch variant, produced from october 22, 2018 to july 6, 2022, equipped with a 3-point mounted axle design. In addition to the safety recall, ford will conduct a customer satisfaction program for the following my 2020-2022 explorer vehicles with the following powertrains: 2.3l awd, 3.3l gas retail, 3.0l gas awd platinum and king ranch, produced from october 22, 2018 to july 6, 2022. Ford stated that the explorer powerpack variants not covered under recalled 23v-675 do not present an unreasonable risk to motor vehicle safety due to the combination of lower peak torque levels and torque management strategies to share inputs between front and rear axles, reducing occurrence of higher torque.  according to ford, this is also evidenced by the lower current and projected failure rates. In view of the recall action being taken by ford, odi is closing this rq. Nhtsa will continue to monitor complaints from the customer satisfaction program vehicles and reopen this issue, if necessary.   • Pedestrian alert sounds investigation # DP22005

    Nhtsa received a petition on or about july 18, 2022, requesting that federal motor vehicle safety standard (fmvss) 141 be applied to all electric and hybrid vehicles operating in the united states. The petition can be reviewed at nhtsa.Gov under odi number 11486072.Fmvss 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 kg or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after march 1, 2021.The petitioner asserts that hybrid and electric vehicles to which the standard does not apply should be found to contain a safety defect. In support of the petition, the petitioner includes findings contained in a bill introduced in the house of representatives in 2009, the pedestrian safety enhancement act of 2009, h.R. 734, 111th cong. (2009). A related bill, the pedestrian safety enhancement act of 2010, became law in january 2011 (pub. L. 111-373). The petitioner requests that nhtsa require all electric and hybrid vehicles comply with fmvss 141, regardless of production date.The defect petition has been opened to evaluate the issue and determine whether to grant or deny the petition.The associated product list is not exhaustive and may contain vehicles that are equipped with pedestrian alert sounds.

The transmission in the ford explorer failed on june 28th. This was noticed by observing the vehicle struggle to shift gears, and sometimes failing to shift at all resulting in extremely high rpms. It also resulted in rough transitions from park to drive, combined with lurching forward suddenly. The vehicle is currently awaiting service at the ford dealership, which was originally scheduled for june 29th. Upon notification to the dealership (where vehicle was purchased) of a transmission issue, the dealership moved the appointment to july 6th, thus encouraging me to drive a dangerous vehicle until then. During the next couple of days, the car intermittently exhibited both check engine and service lights, but neither consistently. It also produced another alert which automatically put the car into park from drive. Once the car turned off and back on, the alert did not repeat. The car was then taken to a separate dealership where it was diagnosed with a defective transmission due to an engine coolant leak. The dealership did not provide any means of transportation in response despite the vehicle being under warranty.

The front tires chewed to pieces, one blew, and the dealer initially felt it was an alignment issue from the factory. Up one further inspection, they found an aluminum toe rod with a 45 degree bend. I believe this was a factory issue as we hit nothing on the highway. It seems from an fmea point of view, it is likely the blown tire and momentum of the car bent the tie rod and that the tire damage was caused by a part failure or some other issue with the car. It is a 3 month old car with 9,400 miles. The car struck nothing on the highway.

I purchased a ford explorer st 2021 in new york from city world ford on boston rd in the bronx on april 22, 2021 at approximately 9:37pm. I took my car home the same same day. The following day (second day) i started experiencing a pre-collision assist not available and powertrain malfunction/reduced power light on my dashboard at 12:49 pm. The car began to jerk and felt like it was on awd mode. The gas pedal was heavy and difficult to drive while i was on the highway with my children. I started to smell fire coming from the inside of my car. Once i started experiencing those problems from my car i immediately parked the car. I immediately called the dealership didn't get an answer so i drove to them personally after i let it cool down for a couple of hours. I explained the problem i experienced with the car to them. The dealership stated to me to take it to the repair shop the monday and i did. The next day (tuesday) i picked up the car from repair service center. The next day i started experiencing the same problems with the ford. I went back to the dealership on wednesday (next day) and stated i wanted a new car or exchange for a different car. Manager stated i had to take the car for second time for repair. I then took the car for the second time for repair on wednesday. The car is currently still in the repair service center and still displaying malfunctions after being repaired in the repair service center. I have not yet received my car back and repair center stated they are not sure when will the problem will be fixed and working properly.

I noticed transmission fluid on the garage floor. March 27,2021. Dropped the car of at the dealership where i bought the car. 2021 ford explorer st with 2120 miles i purchased on january 30, 2021. Problem was diagnosed as leaks at the transfer case seals on the transmission output drive train. The boots were leaking transmission fluid. The transfer case needed to be removed and the seals replaced. This resulted in waiting 6 weeks for the parts to be delivered and 1 week more to repair and install. Today i still do not have my car back. Week 7 will be next week when completion is estimated to be .

The wheel wells of the vehicle collect large amounts of snow and ice. This build up constricts the wheels of the vehicle contacting the tires restricting rotation and steering. This is an extremely dangerous potentially deadly situation. We have contacted ford motor company and the dealership. No remedy to this situation has been offered. Please investigate and any assistance is appreciated. I believe this is a wide spread problem ford is unwilling to deal with. This condition occurs in cold weather and snowy icy conditions. The build up hazard happens even driving short highway distances.
