Known Recall Investigations
  • Hyundai and kia abs module fires investigation # AQ23002

    The office of defects investigation (odi) has received eight hyundai motor america (hyundai) and eight kia america, inc. (kia) part 573 recall reports between 2016 and 2023. The combined 16 safety recalls all involve the antilock braking system (abs) modules or hydraulic electronic control units (hecu) manufactured by the equipment supplier mando. The part 573 recall reports contain varying defect descriptions and differing remedy descriptions for the same or similar equipment supplied by mando. Safety recalls included in this audit query (aq) review include: hyundai recalls 20v-520, 20v-543, 21v-161, 21v-160, 21v-303, 22v-056, 22v-810, and 23v-651. Kia recalls 16v-815, 20v-518, 20v-519, 21v-137, 21v-331, 22v-051, 22v-800, and 23v-652 the purpose of this aq is to evaluate the timeliness and scoping of hyundai and kia’s defect decision making and adherence with reporting requirements; and understand the varying defect descriptions and remedies between these recalls.

  • Engine fires investigation # EA21003

    In 2017, the office of defects investigation (odi) opened two recall queries (rq17-003 and 004) following multiple recall actions initiated by hyundai and kia for “theta ii” engine-equipped vehicles. The rqs were opened to investigate both the timeliness and scope of the recalls as well as the compliance of each company to meet their reporting requirements established under the motor vehicle safety act. Numerous violations of the requirements were found for both companies, and in on november 20, 2020, both companies entered into consent orders with nhtsa to resolve nhtsa’s assertions of violations of law. Both consent orders remain in effect.In 2018, the center for automotive safety (cas) petitioned the agency under defect petition (dp) 18-003 to investigate non-crash fires on various hyundai and kia models, citing 120 voqs alleging vehicle fires at varying points of origin and occurring under various non-crash related circumstances (including key-off fires). In opening dp18-003, odi noted that many of the 120 fire incidents appeared to originate in the engine compartment area and made reference to the two open rq investigations as likely pertinent. Odi issued information request (ir) letters to hyundai and kia and several peer oems in its efforts to properly evaluate and disposition cas’ petition.After reviewing responses to the dp requests, odi granted cas’ petition and in 2019 opened two preliminary evaluations (pe19-003 and 19-004, one for each company) to broadly investigate incidents of non-crash fires (regardless of origin or operational status of the vehicle) on various model year hyundai (sonata and santa fe) and kia (optima, sorento and soul) models. Odi issued additional, more broadly scoped ir letters to both hyundai and kia, which upon review, identified certain vehicle populations exhibiting higher rates of non-crash fires for various defects and points of origin, including some due to engine failure.To date, hyundai and kia have issued several recallsto address vehicle fires, including those due to engine failure involving various engine types (theta ii gdi, theta ii mpi, theta ii mpi hev, nu gdi, and gamma gdi). The remedies for the engine failure recalls typically involve engine inspection (to detect existing damage), and if necessary, engine replacement. Additionally, both manufacturers are installing an engine control software modification known as knock sensor detection software (ksds) which is intended to detect impending engine failure, alert the driver, and limit engine power (to lessen likelihood of engine failure). Ksds is being installed under recalls and non-safety field actions, and both companies have extended limited engine warranties (see recall files). They have even replaced engines if the ksds software detects an impending engine failure.The 78 voqs cited above, which all involve fires likely due to engine failure, consist of 36 remedy-related and 42 scope-related incidents. The injury allegations involve eye and burn injuries not requiring medical treatment. With the 83 other voqs, odi is aware of 161 fires potentially due to engine failures. The downloadable version of the resume available at nhtsa.Gov contains additional information on the recall actions and id numbers for the voqs.The office of defects investigation (odi) is opening this engineering analysis for two primary purposes. First, the investigation will continue to evaluate the scope of recalls initially begun under prior odi actions (see below), and will also include other recalls for fires due to engine failures, which hyundai and kia have subsequently launched (many influenced by odi investigations). Second, the investigation will monitor and evaluate the efficacy of the recall remedies for engine fire related recalls, as well as the long-term viability of related programs and non-safety field actions being conducted by hyundai and kia.

  • Throttle control system vulnerabilities investigation # DP21003

    The office of defects investigation of the national highway traffic safety administration received a petition letter dated october 10, 2021 requesting an investigation of an alleged defect(s) in the electronic throttle control system in certain model year (my) 2005-2016 kia optima/k5, my 2006-2015 kia sorento, my 2007-2016 hyundai elantra, my 2007-2016 hyundai santa fe, my 2006-2015 hyundai sonata vehicles. The petition was filed as a vehicle owner questionnaire that can be reviewed at nhtsa.Gov under reference (odi) number 11439675. The petitioners allege the subject vehicles exhibit a range of dangerous vehicle speed control problems including sudden uncommanded acceleration, runaway throttle conditions, surging, stalling and loss of motive power, all due to various defects and vulnerabilities in the electronic throttle control system.This defect petition has been opened to evaluate the allegation and determine whether to grant or deny the petitioners request.

  • Headliner bracket detachment investigation # EA12008

    In march 2013, hyundai issued a safety recall to reinforce the attachment of the metal support brackets to the headliner assembly in approximately 186,254 model year 2011-2013 hyundai elantra vehicles built in korea and sold in the u.S. The airbag light on my hyundai elantra has been turning on for a couple of minutes when i start the vehicle and then will turn off. After a few more minutes it will happen again as if there is a malfunction with the airbags and the computer is trying to trouble shoot the problem but still finding an error. I would like a follow up to insure that if someone were to hit my vehicle that the airbags would deploy properly.

After the completion of my work shift, i return to my vehicle and discovered a fire has taken place. Lucky, the fire didn't get out of control and wasn't active at the time. The passenger seat was completely burnt including the seat covers and the items in the seat. The items in the seat included: steak & shake leftover trash, and a perfume bottle. This fire also damaged a door dash bag that i had in the back passenger seat. Not to mention, the impact of the fire that created black duct all over the vehicle windshields, radio, seats, dashboard etc... No police report, nor fire report has been reported at this given time.

After starting car i tried to put the driver left side seatbelt on. It seemed to be stuck and would not budge after trying to pull over me. I noticed other people who owned hyundai cars same year also had the same trouble. When i called the dealership they gave me a quote of $500. Not only does is this a safety hazard but fraudulent as well. Hyundai corporation sent me a letter stating it was not a lemon. I never said it was. But it is defective and should be recalled!

The contact owns a 2013 hyundai elantra. The contact stated while driving 55 mph, the traction control warning light illuminated and the vehicle decelerated. The vehicle was making an abnormal sound. The contact switched to econ mode and the vehicle operated as designed. The vehicle was not taken to a dealer nor an independent mechanic to be diagnosed or repaired. The local dealer was made aware of the failure and the contact was awaiting a callback from the dealer. The contact researched the failure online and related the failure to nhtsa campaign number: 20v061000 (service brakes, hydraulic) however, the vin was not included in the recall. The manufacturer was not notified of the failure. The failure mileage was approximately 44,000.

I have an 8 year old 2013 hyundai elantra gt. For the past month i have had several incidences where the accelerator to the car does not work. This has occurred getting on the freeway, going up hills, attempting to "go" when stopped at a red light, etc. The accelerator virtually does not work even though my foot is pressed on the gas pedal to the floor board. There are not safety warnings on the dashboard to warn me of this malfunction. The check engine light has come on and off but not consistent with these episodes. I have taken the car to the hyundai dealer for service and they have been unable to find the problem telling me the codes are all ok and that the technician drove the car several miles and could not replicate the problem. I have found several blogs and posts with the exact same problem i have been having. I cannot drive my car for fear of it not accelerating as i get onto the freeway with the cars honking at me as i have had to manually move my car out of oncoming traffic. I told the dealer when i took it in today not to give it back until they tell me what the problem is. I can't even trade it in knowing it is unsafe to drive due to the "mysterious" accelerator problem. The dealer states the problem could be due to many reasons but it will be cost prohibitive to attempt to just go through a list in an attempt to find the problem. I know there is no recall on this issue but it is a serious safety hazard. I don't know what to do.

When i start up and drive my car, the airbag light is on and it reamins on. I have nothing on or against the seat. Not sure if this means airbags are malfunctioning?

My vehicle had been parked in my driveway and when i went to start the car it wouldn't start, but it began making this knocking noise. I had no idea what caused this, but it was extremely embarrassing and the only thing i could think of was to disconnect the battery. And that's when i heard the noise coming from the engine.

My airbag light came on. I have had no accident. I called the dealer and they want $185 diagnostic fee. Shouldn't airbags and safety be covered by the dealer if no accident has occurred? I am driving without an airbag right now due to no fault of my own. The airbag light just appeared about a week ago.
