• Desiccated air bag inflator rupture investigation as of 20210917

  From 2000 through 2017, takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("psan") propellant -- propellant 2004 and propellant 2004l. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 blomquist report at www.Nhtsa.Gov/sites/nhtsa.Gov/files/documents/expert_report-hrblomquist.Pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in us vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004l that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated psan inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All takata inflators produced with propellant 2004l contain desiccant, and none of these desiccated inflators using propellant 2004l are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated psan inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- takata (now known as tk global), the independent testing coalition, and exponent -- have been studying the long-term behavior of takata desiccated psan inflators using propellant 2004l (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004l is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004l.The office of defects investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated psan frontal inflators manufactured by takata. This investigation will require extensive information on takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated psan inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.Gov/recalls?Nhtsaid=ea21002 -- note this information is subject to change/revision as the investigation proceeds). The contact owns a 2016 nissan versa note. The contact stated while driving at 70 mph, the vehicle would inadvertently decrease in speed to 30 mph. The contact stated that the rpms would fluctuate while driving. The contact stated no warning light was illuminated. The vehicle was taken to an independent mechanic but could not be diagnosed by an unauthorized dealer. The vehicle was not repaired but the contact had an oil change. The vehicle was then taken to a local dealer where it was diagnosed with nothing to be replaced. The vehicle was not repaired. The contact then stated two days later, the contact heard a loud scraping noise coming from the engine compartment. The contact then took the vehicle back to the same dealer for a second time where it was diagnosed with the transmission needing to be replaced due to metal shards being found in the transmission pan. The vehicle was not repaired. The manufacturer had not been informed of the failure. The failure mileage was approximately 101,000.

The break peddle will sink on its own. It has done this at a complete stop with the car off, on the highway, on city streets, in corners. It goes off seemingly at random. Based off of other findings and lawsuits, i've began to suspect it is the transmission. I've taken it to the nissan dealership each time it has happened, and they've never been able to give me an answer. I've gotten my warranty extended by them, but that's it.

I got 2 front tires after i noticed the front of car had droped about 10 inches from side front doors to the front of car.I drive the car every day for 2 years i can see the difference. 1) i was driving on the highway us 35 going west bound where my rear passenger tire blew out right before the 75 overpass. 2) passenger rear tire blew out. A 2 inch abrasion on the side wall. 3) i bought a new tire and repaired my rim.

Lost consciousness due to previously undiagnosed medical issue, car was in 2nd gear and hit telephone pole. Car was totaled and i was injured from the impact, airbags did not deploy. I was presumably going as fast as possible in 2nd gear as my foot was on the pedal when i lost consciousness.

When stopping at a traffic light or a stop sign, the car wants to move forward excessively. I have to push the breaks hard to make it stop. Looks like the car wants to continue moving forward on the second gear. Sometimes, i have to push it way harder to the end. Sure it will get worse. On approximated on february 02, 2020 on the north bound 215 fwy existing fwy off ramp on newport blvd in menifee ca, when my 2016 nissan versa stall, it's on drive and not moving. No warning signs of any mechanical issues, no engine light on. I turned off the engine and restarted then i was able to drive but the rpm is moving up and down. I pulled over to the nearest gas station and called my roadside assistant through geico. The next day i drove the vehicle to pedder nissan dealership for a diagnostic of the vehicle. The service dept return my call and notified me that my transmission is out on a less than four years old vehicles that i purchased brand new from this dealership. The dealership & nissan consumer affair have both have given me the runs around. I see there bulletin for my vehicle model and year. I need assistance, please.

Was coming to a stop at a red light when my car suddenly stalled out. Re started it then it was sluggish and my check engine light came on, when i came to a stop again i felt a quick thunk like the transmission was slipping. Finally got it home when put it on code reader it came up with code P0705 transmission range sensor circuit malfunction. I received a notice of recall for the transmission problem but when took it to the dealer they said it wasn't covered. Now i am stuck having to pay around $2000 to get my car fixed.

When i drive in the winter time and apply the brakes on a slippery road the car doesn't want to stop no matter how many times i try to use the brakes. Instead, the brakes pop and click until my car slows down on its own. I ended up sliding literally in the middle of the street at a busy intersection. Other drivers were aware my car was having trouble stopping and allowed me to proceed. That happened in the 1st time in december 2017. The following year i had my brakes check and the service station claimed the couldn't find any thing wrong with them. I had my brakes serviced before the winter of 2018. My brakes still do the exact same thing all winter long on the slippery streets. Also i'm having trouble trying to turn the key in the ignition because it's locked. When i finally managed to get the car up and running a little while later it wants to stall. This just happened the last 2 days. I'm going to get that serviced asap. That day was november 23, 2019. This vehicle has several issues and it's only 3 yrs old. That's not a good sign.

Purchased a 2016 nissan versa (brand new) car ran well until 11/18/19. When i left work, started car as usual and headed home. Passed a roundabout along with some freeway driving. While on roberts rd. At a traffic light (waiting for right away) car seemed fine, but once i applied the gas to cross the intersection on a green light. Nothing happened. All the dummy lights came on, radio and heat was still working but car shut down at rush hour. I could not move. I called for the police to help then had it towed to the nissan dealership. I later found out that there was a technical service bulletin sent to dealers about this issue of cars stalling. Why was i not made aware and sold a lemon. I thank god that it did not happen on the freeway or roundabout. The dealer claimed they fixed the problem, but my hunch told me it's a lemon and not to keep it. I now own a 2020 toyota corolla. I don't want anyone to go through what i went through, very scary. Fyi if you goggle nissan cars stalling there is a news story from ktnv channel 13 out of nevada.

Sudden loss of power steering control on 2016 nissan versa note while vehicle in motion on city street (queens, ny) thankfully, we were able to avoid collision with a steering wheel that was extremely difficult to turn. No previous issues. P/s light appeared on dashboard system indicating issue with power steering. The vehicle has less than 33k miles. Dealer states that steering motor requires replacement. However, they also state that nissan corporate also requires replacement of the entire steering system as the motor is not sold separately. Repair estimated at $4200 plus tax.

Additionally, on several occasions when the brake pedal was depressed, the stopping distance increased. The vehicle was included in nhtsa campaign number: 15v507000 (vehicle speed control, service brakes, hydraulic). The contact filed a complaint with the better business bureau to have the vehicle replaced due to the failures. The vehicle was replaced; however, the replacement vehicle also experienced similar failures. The vehicle was not repaired. The vehicle was taken to nissan of queens (located at 93-25 rockaway blvd, ozone park, ny 11417, (718) 835-8300) to be inspected, but no fault codes were found. The manufacturer was contacted. Trying to leave for work, turn on my car and then there was a squeal (like a belt) and then the engine shut off, not the car (battery is working). I do this about three times to see if maybe the gas needed to be pressed or if the car needed to be in gear, same thing happened. I call my father for advise, and i wanted him to hear the noise before the shut off, so i turn off the radio, ac, everything. Then the car starts and stays on. Turns out that if i had the fan going it was shutting off the engine. I take it to sisk nissan and they inspect, say first that its the condenser and after the part is ordered and the car taken apart they say that the whole ac system is contaminated. The compressor had stopped working. One week without ac and one week in the shop to replace the whole ac system.

Soon after that, my car began to hesitate while driving and then began to smell like something burnt. The car was well maintained, i did regular maintenance as instructed in the manual, but it was around 60,000 mileage when issues began to show up and around 90,000 when it began hesitating and smelling. It is unsafe to drive on city streets because it will not accelerate properly and so i cannot drive it on the highway. I contacted nissan while i had mileage in the range of 80k-84k to see if they will examine my cvt, given the current lawsuit, free of charge. But they stated that the lawsuit hasn't been settled yet and that by the time it is, i will likely be over the extended warranty.

During colder weather, as the car is moving, air comes in through the bottom seam of the backseat and through the edges of both rear doors. The weather stripping around these parts is inadequate. This inflow of air causes all windows to fog up and condensate, which the defroster is unable to contend with. Water literally drips down the windows and makes it hazardous for the driver because they can't use their mirrors to navigate and are wiping the winshield so they can see the road in front of them.

I was hit from behind in a crash and was knocked into a car in front of me. Suffered mild head injury. None of the air bags deployed. The injury could have been worst. *jb

On or about november 5th of 2018 while traveling on the highway home the vehicle quit accelerating. We limped the vehicle home where a local transmission shop inspected the transmission. It was apparent the transmission was slipping. The quote we received was that the transmission was "shot". The vehicle had been purchased used and was only owned about 16 months.

I pulled in to a parking spot i was in park. Next to a 4x4 diesel pickup, he started to back up and turned his wheels to were he was going to run me over i held the horn down he did not hear it and ran me over from the front drivers side to the back door. He stopped when he realized he was on top of my car.

The front driver side continental contiprocontact p185/65 r15 tire exploded while parked. The vehicle had been parked for an hour and a half, and had only been driven less than ten miles prior to the explosion. The outside temperature was 65 degrees fahrenheit and it was raining, so heat was not a factor. The tire dot # is p5fp3yP2715. The tires themselves are three years old and have 54,710 miles on them.

Vehicle hesitates when accelerating jerks car when getting speed when going at highway speeds car wanders back and forth hard to keep it on the road , clunking noise comes from cvt transmission when changing gears also produces severe vibrations when car is in motion getting worse and worse have been having these problems since i purchased car 1 year ago at 27,000 miles now have 70,000 miles and car is not very safe to be on the road i have replaced struts hub bearings control arms and tie rod ends along with motor and transmission mounts but the problem keeps getting worse and worse every day. Took it to local dealer and was told they couldn't do anything about it because it was out of warranty. The hertz car rental place that sold it to me was made aware of the problem took it twice to pep boys but i was told they couldn't find anything wrong i've spent about a 1,000 dollars in parts to try and correct the problem but still no luck

My vehicle was in motion when it loose speed then was unable to get it out of geer. Was told need a new transmission. I just bought the car use as is, no gurantee. I have no money for a new transmission will cost me over $2000??????????????

My vechicle starte to cut out on me after 37000 miles. I was driving from jury duty in the afternoon in traffic and it just stalled out on me. The engine cut off. I was able to crank it again and moved over to the side of the highway. I waited 45 minutes. I did not turn the air condition on. It cranked and i went home. A week later it did the same thing but it would not crank again. I had to get it towed to a mechanic shop. He said it was the compressor. He replaced the compressor and i picked it up again and in a few days later it cut off on me and my family again. We took it back to the same mechanic and he said it was something wrong with the electrical box. He replaced it and it was running again.However in 2 days it turned off while driving again.This is a relatively new vehicle. It only has 38k miles on it. Now it will not drive. I am stuck with a car note and no car to drive.

Takata recall - my airbag light has come on and i haven't had any accidents. I haven't had the car for 6 months yet and really need to get this looked at. I woke up one morning to take my daughter to school and soon as i crunk up the car the light was flaking and won't go off.

When i step on the brake pedal for a street light or anywhere i need to use the break, my car makes a noise similar to a loose fan, and once i release the brake the noise stops.

The nissan versa 2016 has a faulty brake system. I was in motion driving with ongoing traffic i saw the traffic stopped i pressed the brakes and kept pressing brakes and the car did not stop. I could not go left or right because cars were in both lanes. I hit car in front. The officer verified that the car was in speed motion with no screeches or mark in street. The officer stated that my car was in normal speed motion. The air bags deployed and my sister and i were injured. The car i hit passengers was ok. This accident was caused by brake failure. I reported this to the nissan corporation.

The vehicle was shifted into drive and the abs light came on and the brakes failed to stop the vehicle. The vehicle had to be steered into a curb to stop it before it entered oncoming traffic. This vehicle also hesitates upon turning which is a hazard while driving.

Have been to classic nissan in statesville nc 4 times for air bags, have changed 1 on left side. Has now come back on and the deals says they have to see light on. Nissan says they are not responsible and they did not know the car airbags was under recall when they sold car, even though i ask the sales women. Moving in parking it does not matter the light blinks on and off, have video of it. Have paper from getting fix.

I have taken my brand new nissan to the dealer 6 times now. Breaks keep making a noise like a tapping noise while driving. The last time i took the car to the dealer and picked it up after a week the breaks are screeching to the point where i cannot drive. Nissan dealer and nissan factory are claiming this is not their responsibility.

Takata recall. I was traveling on us 77 in virgina when the traffic can to a stop due to a merging arrow. I merged and stop. I was rear ended by the car behind me as it did not stop. My car hit the car in front of me and then rolled over. The air bags did not deploy anywhere in the car. I believe the driver's air bag should have been enabled after hitting the car in front of me. The seatbelt was able to easily become disengated by hardly touching the release. There was an auto accident that resulted in the paralysis and eventual death of the driver. The driver was t-boned while making a left turn on a green light at the intersection. The other driver was cited for excessive speed and failure to keep a proper lookout. It is our belief that his forearm brushed the release on the seatbelt and caused the seatbelt to discharge at the time of impact. The contact stated that the windshield was not struck by any debris. The vehicle was taken to the dealer to be inspected. The manufacturer was not made aware of the issue. The approximate failure mileage was 7,000.

"takata recall" back in march 17 i was in an accident on the highway by dia the steering wheel locked up and i was force to run into the back of a lady in another lane. My air bag did not deploy

On december 12, 2016, buy the nissan versa note, on january 7 2017 2.30 pm, crash at 30 or 35 mph, brake, but only when i hit the other car, on february 28 i crash again in full stop 8 am to 20mph, haboa lot of traffic and i had a car of separation, they all advance and brake suddenly, no one crashed, only me that for more brake that i put no pair until finding the other, the service nissan puerto rico says the brakes are well, but also this shock at 20mph broke mask, condenser, i double radiator focus, as if it were xe pastio. I feel my english because i had to use a translator, thank you

Everything time i stop and park the car to do a short errand and then after getting back in the car, i put the key in the ignition and it won't turn. I have to wiggled the key in the steering wheel for at least five (5) minutes or more. It is happening more frequent and often. I am very concerned.

Air bag failed to deploy during head on collision driving about 70mph on the 1604 hwy animal ran across i swerved out of the way to avoid hitting it but lost control on the grass and went head on into a pole as i tried to steer the car from hitting i spinned out of control after the impact then landed on the access road. Front of vehicle got pushed in but not one air bag deployed during the whole incident nissans excuse is i didn't hit the car correctly which i'm not quite sure what would be the correct way to crash a vehicle for future reference.

I have a 2016 nissan versa with approx 12,000 miles and have constant issues with the steering of this vehicle. The car veers off and has a mind of its own frequently and feels as if it is being pulled by wind. This is unsafe and causes controlling the car to be a problem. This began at first in early 2017 and is now a regular occurance

I have had a recurring problem with the car when filling it with gasoline. The gas nozzle has been ejected from the car during filling. Typically the gasoline pump will stop dispensing gasoline when the nozzle is ejected. However, there was one incident when the nozzle continued dispensing gasoline while the nozzle was laying on the ground. I had to quickly pick up the nozzle and turn it off. This problem does not occur every time i fill the car with gasoline, but i feel that there is a recurring issues here and it should be resolved because of the risk and hazards associated with spilled gasoline.

Bought the 2016 vehicle october 15, 2015. Right away noticed a problem with the automatic gear shift not moving smoothly from gear to gear. The brakes groaning/whining and the emergency brake loosening. Car drifting/pulling right on highways and left on streets. Rattling from under car carriage. Popping and squeaking on right front passenger door. The a/c straining and not cooling car cabin as it should and the a/c interior switch not engaging or staying engaged when pushed. Thumping/clicking sound heard when braking. After numerous complaints made the dealership verified all of my complaints but not in a timely manner. First complaint was acknowledged on october 28, 2015 but claimed there wasn't any abnormalities found. On december 8, 2015, march 16, 2016, april 22, 2016, august 17, 2016 and august 23, 2016 repairs were repeatedly made consisting of replacing gear shift knob, entire gear shift assembly, sanding rotors, replacing brake pads, replacing entire brake assembly and tightening emergency brake. But as to date problems still exist. On february 9, 2016 repair to the suspension and steering. On april 5, 2016 repair to lower rear exhaust shield remounted. On may 31, 2016 door check straps re-secured. On june 23, 2016 faulty bmc switch replaced but problem with a/c still not resolved and a second problem has arisen. A/c turns on when defroster is engaged. On september 23, 2016 problem with both front inner cv joints and axles acknowledged but part on back order. Repairs made on october 27, 2016 in another state. Problem not resolved. Sound still heard when driving over speed bumps, cracks or slight bumps on streets or highways.

Takata recall. Low tire sensor is not functioning properly. This dysfunction occurs when the mornings are cold, and will at times not reset itself. When the "low tire"error is displaying, other display will not function. This may occur while the vehicle is stationary or in motion.

Unknown at a stop, when the a/c kicks on, i hear a very audible rattling noise coming from somewhere between the engine and the transmission. Whenever i am starting off, for example, going down my street, i can hear a very audible, yet different, rattling noise coming from somewhere behind the glove box. I thought that noise was something i had in the glove box, but i already tried emptying all of the compartments and driving and it still made that noise, which tell me that it must be something just behind the glove box inside the dash. The electroluminescent lights for the instrument panel are already beginning to fade out in some places. These problems began at 12k, 15k, and 17k miles respectively. I have mentioned these problems at 2 different nissan dealership service departments 4 individual times and the only response i get is that it will be around $250 to check and see what the problem is and that the warranty probably will not cover the repairs. The vehicle has 33k miles currently and i will be trying one more time to have the issues resolved at the 35k mile service before the warranty expires at 36k. I am afraid that the instrument panel lights may go out at night and i won't be able to read the speedometer. As far as the rattling noises go that i and others are hearing is questionable. Whatever is rattling could eventually break and cause a very serious accident and injure/ kill one of me or my passengers. I cannot remember the exact day these problems began but i'm sure it was sometime between oct 1, 2016 and december 31, 2016

Nissan versa, always had issues with gas gauge fluctuating. Igauge read 79 miles in driveway, i pulled out of driveway into main road and went about 25 feet, has gauge started flashing. Told grandson i had to go to gas station before taking him to school. We drove about 1/4 mile and heard pop under hood, car jerked really hard to right. I had no control and hit mail box, car kept moving forward along ditch. Finally come to stop with windshield busted and front bumper and undercarriage badley damaged. Air bags didn't deploy, police were called and car was towed.

I recently purchased a 2016 nissan versa and when i drive it, it veers to the left which is a matter of safety for my family.. Took it in to service dept and they did an alignment but it is still happening...I am re-thinking my decision about this car because i refused to drive a unsafe vehicle.

On 7/13/2016----- my passenger and i were traveling on clemson road where the posted speed limit is 45 mph, its a four lane highway with no median. A truck did a u turn in front of a lot of traffic my vehicle struck his at about 40-43 mph though me and my passenger had seat belts on my passenger still managed to hit his head on the windshield through the airbag. My passenger was 5'2 135 pounds so the seat belt did fit properly and was also being worn properly, with the lap band snug against hip one and shoulder strap fully across chest and latch buckled. My passenger was took to the hospital and was examined for injuries that may have occurred from the head trauma of hitting the wind shield.. Why didn't the passenger seat belt keep my passenger from hitting the windshield??? I believe that the seat belt didn't lock in place like it should have once the breaks were pressed- my passenger kept traveling forward as the vehicle slowed down. Note that my passenger was holding his head once we exited the vehicle!

I rented a nissan versa and was driving at dusk. I didn't realize that i had not turned on the driving lights until it was quite dark. The problem is that when the lights are off, the interior lights look like you would expect with the lights on... I.E., the interior is fully lit up. The driving instruments are lit up. Everything looks normal for nighttime driving.... Except that when i pulled over, i see that the exterior has no lights on whatsoever. To be clear, the lights were working fine. If you turn them on, they come on. But it's very dangerous that the car somehow always has the interior lighted, or perhaps it happens with a light sensor, leading you to not realize you have not turned on the exterior lights.

Acceleration delay with my recently purchased 2016 nissan versa note. Particularly dangerous when making a left turn in front of oncoming traffic as the initial pressing of the accelerator pedal has a delayed response, sometimes for 3 seconds or more. To compensate for the delay i press heavy on the pedal resulting in a sudden surge when it finally kicks in. I took it in for repair and was told by the nissan dealership there was nothing wrong. The problem continues to present a danger when driving.

Driving on a two lane road heard a pop, car sudden pull right front end hitting a ditch on the side of road front end sliding down in the ditch coming to a stop in a vertical position. The air bag didn't deploy.

The gas pedal flipped up upon trying to adjust the foot causing the vehicle to spin striking a another vehicle. 10 minutes later the air bag deployed, before striking the vehicle.
