  • Recall 150623 [USA]: In the event of a crash, air bags and/or seat belt pretensioners that do not deploy as intended may increase the risk of injury.
    Toyota motor engineering & manufacturing (toyota) is recalling certain 2011-2019 corolla, 2011-2013 matrix, 2012-2018 avalon, and 2013-2018 avalon hybrid vehicles. During certain crashes, the air bag electronic control unit (ecu) may malfunction, possibly disabling the deployment of the air bags and/or seat belt pretensioners.
    toyota will notify owners, and dealers will inspect the ecu and install a noise filter between the air bag control module and its wire harness, as necessary, free of charge. Owners were notified of the safety risk beginning march 2, 2020. A second letter notifying owners of the remedy repair will be mailed between march 16, 2020 and june 27, 2020.   • Recall 128430 [USA]: If the seat belt is falsely interpreted as being buckled, in the event of a crash, the air bags may not deploy appropriately, increasing the risk of injury.
    Toyota motor engineering & manufacturing (toyota) is recalling certain 2012 toyota avalon vehicles. During a vehicle repair, the seat belt inner buckles may have been replaced with parts that may incorrectly tell the air bag management system that the seat belt is always buckled.
    toyota will notify owners, and dealers will test the seat belt assembly to verify that it is properly detecting being unbuckled, replacing the inner seat belt assembly as necessary, free of charge.   • Recall 112627 [USA]: A constant electrical current flow may cause the sub-woofer to overheat, increasing the risk of fire.
    Toyota motor engineering & manufacturing (toyota) is recalling certain model year 2011-2012 toyota avalon vehicles manufactured february 9, 2010, to october 22, 2012. In the affected vehicles, the sub-woofer speaker located in the trunk may experience an intermittent electrical short which may cause damage to the integrated circuit (ic) in the audio amplifier. In some cases, the damaged ic may allow a constant electrical current flow to the sub-woofer.
    toyota will notify owners, and dealers will replace the audio system amplifier and install a protective cover around the sub-woofer, free of charge. The recall began on july 1,2015. Owners may contact toyota customer service at 1-888-270-9371.

  • Recall 98593 [USA]: A short circuit may cause the air bags to become disabled or inadvertently deploy. an inadvertent airbag deployment can increase the risk of injury or the possibility of a crash. an inoperative airbag can increase the risk of injury in a severe crash. the power steering assist could also become inoperable resulting in increased steering effort and can increase the risk of a crash at low speeds.
    Toyota is recalling certain model year 2012-2013 avalon, avalon hv, venza, camry, and camry hv vehicles. In the affected vehicles, the drain hose for the air conditioning condenser may become clogged causing water to accumulate at the bottom of the air conditioning condenser unit housing. The accumulated water may then leak through a seam in the housing onto the air bag control module potentially resulting in a short circuit of the module.
    toyota will notify owners, and dealers will seal the air conditioning condenser unit housing and install a protective cover on the airbag control module, free of charge. The recall began on february 5, 2014. Owners may contact toyota at 1-800-331-4331.

  • Recall 2021723 [Canada]: Accessories
    Issue: this recall is for certain block heater kits installed on certain vehicles or sold as accessories at toyota or lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt. Corrective actions: toyota recommends that you should stop using the engine block heater immediately. Toyota will notify owners by mail and instruct you to take your vehicle to a dealer to disable the engine block heater. For some vehicles, the corrective actions for this recall are under development. For others, a new engine block heater cannot be installed, and the owner will receive a refund of the purchase price of the engine block heater.

  • Recall 2020013 [Canada]: Airbag
    Issue: on certain vehicles, the circuits within the airbag control module could be damaged in a crash. This could cause the airbags and seat belt pretensioners to not deploy in certain crashes. Safety risk: airbags or seat belts that do not work properly in a crash could create an increased risk of injury. Corrective actions: toyota will notify owners by mail and instruct you to take your vehicle to a dealer to install an electrical noise filter to prevent damage to the airbag control module.

  • Recall 2018457 [Canada]: Seats and restraints
    On certain vehicles that had the driver’s and/or front passenger's seat belt buckle replaced, a switch within the replacement buckle may incorrectly signal that the seat belt is always buckled. In the event of a crash, this may cause the airbag system not work as designed, and could increase the risk of injury to the occupant(s). Correction: dealers will inspect the signal from the seat belt switch and replace the inner seat belt assembly as needed.

  • Recall 2015038 [Canada]: Electrical
    On certain vehicles equipped with a trunk-mounted sub-woofer, cargo in the trunk could contact the sub-woofer's soft braided wire, which could cause the wire to move out of its normal position. If one of these wires were to come into contact with the metal frame of the sub-woofer, it could result in an intermittent short, causing the integrated circuit (ic) in the audio amplifier to become damaged. In some cases, the damaged ic could create a closed circuit between the power supply and sub-woofer, and electric current could continue to flow to the sub-woofer. If this were to occur, the sub-woofer could overheat, increasing the risk of fire, causing injury and/or damage to property. Correction: dealers will replace the audio system amplifier and install a protective cover around the subwoofer. In the interim, dealers will disconnect the rear sub-woofer.

  • Recall 2013359 [Canada]: Airbag
    On certain vehicles, if the air conditioning condenser drain hose were to become clogged, water could accumulate and leak onto the airbag control module. This could cause the airbag warning light to illuminate, and potentially result in an unintended deployment of the airbags. This could startle the driver and/or cause minor injuries to vehicle occupants. Water damage could also cause a loss of power steering assist, causing the vehicle to revert to a manual steering mode which would require greater driver effort, especially at low vehicle speeds, and increase the risk of a crash causing injury and/or damage to property. Correction: dealers will install a protective shield and sealant.

Known Recall Investigations
  • Air bag acu electrical overstress investigation as of 20190419

    The office of defects investigation (odi) is expanding the investigation to include the equipment supplier and vehicle manufacturers (oems) using this unit.The investigation focuses on acus manufactured by trw, now zf-trw (zf) , the tier-one supplier to hyundai and kia and the other affected oems.The acu senses a vehicle crash to determine whether air bag deployment is required, and if so, deploys the appropriate air bags and other supplemental restraints.Zf supplied subject acus to six oems: fca, honda, hyundai, kia, mitsubishi and toyota.Internal to the acu is an electronic component (an application specific integrated circuit, or asic) that monitors signals from crash sensors.A failure of the asic may prevent deployment of the required air bags and devices, or may otherwise affect the proper operation of the acu.The acu is located in the passenger compartment, and electrical wiring connects the asic to sensors located at the front of the vehicle.Odi’s current understanding is that a crash event may, in and of itself, produce harmful signals on the sensor wiring capable of damaging the asic, although the probability of this occurring appears to be low.While the acu incorporates electrical circuitry intended to protect the asic from harmful signals, the level and effectiveness of the protective circuitry varies by oem customer.During pe18-003, hyundai and kia filed recalls (18v-137 and 18v-363 respectively) to address a defect that could result in acu disablement and non-deployments.According to the filings, the disablement occurs in certain types of frontal crash events.Both filings discussed a condition known as electrical overstress (eos) that affected the subject asic and was likely caused by electrical signals that entered the acu via sensor wiring.The recalled vehicles used acus that had the lowest levels of asic protection while non-recalled hyundai and kia products using subject acus had higher levels of protection.Odi has not identified any eos failures in the non-recalled kia and hyundai populations.In september 2016, fca filed recall 16v-668 for certain model year (my) 2010 to 2014 chrysler, dodge and jeep products also manufactured with the subject acu.In that filing, fca also discussed an eos condition that resulted in a failure of the subject asic, which caused air bag non-deployment. Fca noted that the defect condition had only been observed in vehicles equipped with sensor harnessing routed across the front of the vehicle.Other fca vehicles that also used the subject acu, but not the cross-car harnessing, had not experienced eos failures, despite similar time in service. The recalled fca vehicles used a mid-level form of asic protection.Other fca vehicles that did not use cross car wiring, or used higher levels of asic protection, have not been recalled.Odi has not identified any eos failures in the non-recalled fca population.Recently, odi has identified two substantial frontal crash events (one fatal) involving toyota products where eos is suspected as the likely cause of the non-deployments.The crashes involved a my 2018 and a my 2019 corolla equipped with the subject acu that incorporated higher levels of asic protection.Additionally, both acus were found to be non-communicative (meaning the acu could not be read with an event data recorder) after the crash, a condition found in other cases where eos occurred with other oems.No other eos events have been identified for other toyota products (including corolla models that used the subject acu since my 2011), or for the honda and mitsubishi vehicles that use the subject acu.Odi plans to evaluate the susceptibility of the subject acu designs to electrical signals, as well as other vehicle factors that can either lead to, or reduce the likelihood of, an eos event.Additionally, odi will evaluate whether an unreasonable risk exists that requires further field action.

Another driver rand a red light and i (t-boned his passenger side) was in a head on collision. I was breaking at the time, but still imagine i hit him at between 30-40 mph. The front end crumpled as expected but the airbags did not deploy. Firefighters cut power to prevent the airbags from deploying unexpectedly afterwards. A check passenger airbag light had come on in recent days that i did not get fixed before the accident, but it should not have affected the driver side airbag i expect. Liberty mutual is currently in possession of the car (they deemed it a total loss).

When the contact would turn off the engine, it would sometimes restart on its own. The contact called bowling green toyota (2398 scottsville rd, bowling green, ky 42104, (270) 843-4321) and was informed that a new rear shade motor was needed. The vehicle was not repaired. The manufacturer was made aware of the failure and did not assist. The failure mileage was 37,000.

Tl* the contact owned a 2012 toyota avalon. The frontal airbags failed to deploy when an 18 wheeler collided into the vehicle while travelling at 65 mph. The cause of the failure was unknown. The vehicle was destroyed and the driver and passenger were hospitalized with spinal injuries and bruises. The contact stated that while traveling approximately 65 mph, an 18 wheeler stroke the vehicle in the middle rear when attempting to switch lanes. None of the frontal air bags deployed. The police was present and a report was issued. The vehicle was not taken to the dealer for diagnostics. The contact also received a recall notification for nhtsa campaign number: 18v569000 (air bags, seat belts) the vehicle was towed to an impound lot. The manufacturer was not notified of the failure. The vehicle was destroyed and totaled. The approximate failure mileage was 71,000.

My vehicles airbags did not deploy i was hit by an 18 wheeler at a high rate of speed. The vehicle had at least 2 rear end hits by this 18 wheeler estimated at a speed in excess of 25 miles per hour faster than mine (i was going at 70mph northbound on us 75, nine miles south of tulsa oklahoma on a sunny monday at 3pm). The street was dry. The truck initially hit me on my rear and on my left rear quarter. I was spun in front of the truck and he hit me again on the rear quarter and the bumper and spun me in the opposite direction and then pushed into the guardrail at an estimated 40+ mph. The airbags did not deploy for any of the three collisions (two in the rear and one in the front).

The contact also stated that there was a mold odor in the vehicle. Rear deck sunscreen activates and comes up without operator activating it. And it will not go back down without manual assistance. With assistance when it does go down there is a loud grinding noise and then it will come back up. To correct this situation requires replacement of complete assembly. (dealer quoted $1674.86) or pull the fuse and deactivate the rear sunscreen. Safety issue is that sunscreen up at night or during inclement weather restricts rear vision and driver can't do anything about it unless he happens to know which fuse to pull and where it is located. When asked why toyota has not corrected this issue, dealer speculated that since this is an avalon only problem that the numbers of failures are likely small. My response was, that may be true but by researching this issue on the internet, it has been a problem since at least the mid 1990's until at least 2013 and probably beyond and toyota has not recalled or fixed the problem so far as i can tell. Regards our specific car a 2012 avalon, it is a feature almost never used and as of this date the car has less than 18000 miles on it. Mileage and age seems not to be a factor in the failures. It seems more likely a poor design of the feature itself. As far as when it happens and where, it can happen anytime the car is running, in the driveway or while driving..

The rear window sun shade screen does not work properly. Sticks in up position blocking site out rear window. With screen down while cranking car, putting car in reverse causes a loud noise of gears stripping. There are hundreds of reports on the internet complaining of this problem on all toyota products that have this screen. This is obviously a design flaw and should be subject to recall. The subwoofer recall is bogus. What they don't tell you until it's too late is that they put a plastic cover around the subwoofer to prevent wires from touching the chassis; they instead replace the jbl amplifier. The work drastically alters the sound. I no longer have a premium sound system; the new amp clearly doesn't have the same wattage as the original one. Lustine toyota and toyota corporate are copping out and won't fix it. The thing has even cut out twice. I'm beyond pissed. That might have been the best part of the car.

2012 toyota avalon. Consumer writes in regards to vehicle radio sub-woofer recall notice issues. *smd the consumer stated on two occasions, he took the vehicle to the dealer regarding the issue. On both occasions, the dealer stated the had not received the means or the information as to how to correct the sub-woofer problem but he would be contacted when the remedy was available. On may 13th, there was a loud crackling noise coming from the rear of the vehicle and the radio stopped working. The vehicle was taken to the dealer, where the consumer told the service advisor, he believed it had something to do with the recall on the sub-woofer. The dealer called the consumer the next day, and informed him the issue was indeed related to the sub-woofer, because when the sub-woofer was disconnected the radio immediately began to work. A few days later, the vehicle would not start, because the battery was dead. The consumer believed the dead battery was related to the sub-woofer. The dealer disagreed with the consumers theory and stated the issue was a bad battery and alternator. The consumer went to get a second opinion and was informed the battery and alternator were working properly and the problem was due to either the previous dealer had disconnected the sub-woofer improperly or waited too long to disconnect it while they were trying to find the problem. *jb

The passive restraint headrest has become a liability the passive restraint headrests are set at such an angle that if i sit correctly in the seat (back/shoulders against the seat back) my head is positioned so that my eyes are aimed at the floor and not at the road. I've adjusted the height of the headrest to try to accommodate this, but no correction can be made. The result is pain in my neck and back while driving. In order to correct the angle and alleviate the pain felt while driving and after, i have removed the headrest, turned it backward, and reinstalled it. Doing this is the only way that i can drive for any distance and not have pain. The passive restraint headrest is a liability.

I purchased the avalon at the end of the 2012 year. It now has 16,000 miles on it. The car is garaged outside in massachusetts. On november 10, 2012 (month after purchase) and yesterday, january 28, 2014, the car was brought back to the dealership for a rear suspension clunking noise that happens during cold weather (32 to -10 degrees fahrenheit). Both times the mechanics say they cannot find anything wrong, nothing is lose with the suspension, and there are no recalls about it. The best way to describe the noise is when going over pot holes in cold weather, i hear a clunk from rear suspension (both left and right side) like something is loose in the trunk (trunk was completely empty yesterday). The sound is not too loud, but audible enough to notice. The rear struts perform well, and the car does not bottom out. As soon as the weather warms up, the noise goes away. It is very really annoying to hear this clunk and the dealership not be able to identify the problem. *tr

2nd complaint (first complaint - #10522765) - on july 2, 2013, around 5:20 p.M., my fourth incident of brake failure occurred. I was going about 5 miles and applied brakes. Brakes failed and engine sounded like it was racing. I crashed into a the rear of a sedan which was stopped. No injuries were sustained. We were able to drive the cars to a development to exchange insurance information. I drove the car to the dealership for trade-in since it was unreliable for driving at slow speeds. An earlier potential incident occurred in the morning when i was braking and slowing down to make a right hand turn, the individuals were crossing the street on which i was turning into and the brakes seemed to not engaged and the engine began to race. I applied more pressure on the brake pedal and the brakes engaged and stopped the car, barely. I am extremely disappointed in the lack of quality in the toyota avalon. I am also disappointed in the toyota service center's focus of the braking system without examining computer systems' engine controls and speed controls. *tr

Purchased used certified-used avalon with 28.3k miles on june 8. On june 15, the first incident occurred from parking lot to stop sign. Applied brakes and no response. Avalon sped up and had to turn the car to avoid accident. Applied brakes again and was able to stop the car. Continued home and no problems. On june 16, approaching stop sign from downward hill and applied brakes and no response. Avalon sped up and was able to make turn with being hit by on coming traffic. Took car to dealership for inspection. Found nothing wrong and attributed to user error. On june 30, approaching a 4 way stop, was able to stop and wait for car in front of me to go. When i apply gas to move up to the stop area, i removed my foot from gas pedal and applied the brakes. Brakes did not respond and i continued to go forward. I steered out of the way of a person making a right hand turn and cleared the stop sign. After 60 feet, applied the brakes and they worked. Was very upset that this had happened for the third time and knew that first 2 incidents were not user error. On july 1, returned to servicing dealer who performed brake system health check and attempted to duplicate what i experienced. The service department said that all of their tests showed normal operations. Avalon is under manufacturer's warranty. Remain concern that this may happen again and something worst may occurred.
