The affected trucks are equipped with fontaine ultra lt fifth wheel connectors, which, due to cumulative damage to the fifth wheel and its locking mechanism, have a locking mechanism that may fail to operate or to properly engage.

  • Recall 104022 [USA]: If the active braking feature becomes disabled, the vehicle may not automatically activate the brakes as intended when the enhanced cruise control system is in use, thereby increasing the risk of a crash.

  • Recall 97126 [USA]: Without a signal to disengage the clutch when the brakes are applied, the distance required to stop the truck may lengthen, increasing the risk of a crash.
    Volvo trucks north america (volvo) is recalling certain model year 2007-2014 vah, vhd, vnl, vnm, and vt models. The affected vehicles were built without a redundant clutch disengagement switch. Without a redundant switch, a loss of signal from the brake air pressure switch can prevent the clutch from automatically disengaging when the brake is applied.

  • Recall 96782 [USA]: Overheated brake components could result in a fire.
    Volvo trucks north america (volvo) is recalling certain model year 2011-2013 vhd and vnl vehicles manufactured december 13, 2010, through july 17, 2012. These vehicles are equipped with an mgm type 24 j-series piston parking brake chamber. The brake chambers may leak pressure allowing the parking brake to drag when it is supposed to be released. Brake dragging could lead to overheated brake components.

  • Recall 91019 [USA]: Inadvertent brake application can cause the brakes to overheat and lead to a fire. inadvertent brake application can cause the wheels to lock-up leading the driver to lose control of the vehicle, increasing the risk of a crash.
    Volvo is recalling certain model year 2011-2012 vhd and model year 2011-2013 vnl and vnm vehicles, manufactured from december 2, 2010, through january 18, 2012, and equipped with bendix atr-6 traction relay valves. In extremely cold conditions, these bendix relay valves may potentially develop internal leakage. Internal leakage can lead to air pressure being delivered to affected primary or secondary brake causing continuous brake application.
    volvo will notify owners, and provide a temporary repair until bendix develops a permanent remedy. A permanent remedy has been developed and notification to owners began on april 27, 2012. This service will be performed free of charge. Owners may contact volvo trucks at 1-800-528-6586.

  • Recall 70612 [USA]: If the driver drives away with a trailer that is not properly coupled, the trailer can come loose and potentially result in a vehicle crash.
    Volvo trucks is recalling certain model year 2011-2012 vnl and vnm trucks manufactured from may 1, 2010, through january 31, 2011. During the coupling process, it is possible for the driver to partially close the fifth wheel lock and if solely relying on a tug test (pull test), the driver may think that the fifth wheel lock is properly coupled to the trailer king pin when in fact it is not properly coupled.

  • Recall 76191 [USA]: Slack adjusters with this flow notch could crack under normal braking conditions causing loss of braking which could result in a vehicle crash.
    Volvo trucks is recalling certain model year 2011-2012 vhd, vnl, and model year 2012 vnm trucks manufactured from october 15, 2010, through january 31, 2011, and equipped with gunite automatic slack adjusters. Due to an incomplete "knitting" of the iron at the time of casting, inadequate structural integrity of the slack adjuster housing can occur, which could cause the housing to break at the handle under normal operating loads.

  • Recall 70666 [USA]: This presents a risk of injury with nonoperational conditions such as when the vehicle is being serviced.
    Volvo trucks is recalling certain model year 2011 vnl trucks manufactured from march 23, 2010, through december 22, 2010. There is a potential for the vehicle to start without warning because of an unintended path to ground that exists. This can occur when either the negative battery cable is disconnected at the battery/fusible link or if the fusible link opens as a result of a short circuit.

  • Recall 84887 [USA]: This could allow the glass to come loose and possibly fall out increasing the risk of a crash.

  • Recall 2014528 [Canada]: Tires
    On certain vehicles with a governed road speed setting of 113 kilometers per hour (70mph) or more that are outfitted with 295/60 r22.5 xza2 energy tires and used in an auto transport application (i.E. Truck modified with frame mounted to the truck to carry automobiles over the truck's front axle), a premature tire failure may occur on the front or steer axle as a result of certain operating conditions (high speeds, load and road temperature). A tire failure could cause the driver to lose vehicle control, which may result in a crash causing injury and/or property damage. Correction: dealers will replace the tires and set the road speed governor.

  • Recall 2013275 [Canada]: Visual system
    On certain vehicles, the windshield wipers may fail. Loss of windshield wiping capability, should it occur during a rainy/snowy day, may compromise the driver’s ability to see the road and its users, which could result in a crash causing property damage and/or personal injury. Correction: dealers will replace the wiper motor.

  • Recall 2013221 [Canada]: Other
    On certain vehicles, the secondary clutch disengagement switch may have been omitted during vehicle assembly or, in some instances, removed during servicing. As such, primary switch failure would cause the automated manual transmission’s clutch to remain engaged when the brakes are applied. This could increase stopping distances, possibly resulting in a crash causing property damage and/or personal injury. Correction: dealers will install a micro-switch at the brake pedal and update the software to run a diagnostic on the switch during engine start-up.

  • Recall 2013126 [Canada]: Brakes
    On certain trucks, the brake chambers may develop an air leak. As a result, the parking brake may not fully release. Park brake drag could result in a fire causing property damage and/or personal injury. Correction: dealers will inspect and, if necessary, replace the brake chambers.

  • Recall 2012032 [Canada]: Brakes
    On certain trucks equipped with bendix atr-6 antilock traction relay valves, in extremely cold conditions (at or below -18 degrees celsius), internal atr valve leakage can occur. This could cause unintended brake application, which could overheat the affected brakes and cause a fire. Also, in certain low traction conditions, unexpected brake application can cause a loss of vehicle control and a crash. These issues could result in property damage and/or personal injury. Correction: dealers will update the cover assembly on the atr valves.

  • Recall 2011183 [Canada]: Brakes
    On certain vehicles, the slack adjuster housings may have been manufactured incorrectly, which could allow the housings to fracture and fail. As a result, the wheel brake to which the slack adjuster is attached would no longer function, which could increase vehicle stopping distances. Should a steer axle slack adjuster fail, it could also cause the vehicle to pull to one side during braking. Either problem could result in a crash causing property damage and/or personal injury. Correction: dealers will inspect and, if necessary, replace the slack adjusters.

  • Recall 2011033 [Canada]: Electrical
    On certain vehicles, there is a potential for the engine to start without warning due to an unintended path to ground. This can occur when either the negative battery cable is disconnected at the battery / fusible link or if the fusible link opens as a result of a short circuit. If the engine starts without warning, while being serviced, for example, it could cause personal injuries to anyone in the immediate vicinity. Correction: dealers will install a jumper wiring harness between the engine harness and the chassis harness.

  • Recall 2010083 [Canada]: Other
    On certain vehicles, the adhesive bond between the upper bunk glass and the window frame may be insufficient, allowing the glass to come loose and fall out. If the window were to fall on the road in front of other vehicles, it may create the risk of a crash causing property damage, personal injury or death. Correction: dealers will remove and reseal the window.

  • Recall 2010347 [Canada]: Label
    On certain vehicles, the tire description may be incorrect on the certification label, which is a non-compliance to canada motor vehicle safety standard 120 - tire selection and rims. The description listed on the label may be for a tire with a lower rating that cannot support the gross axle weight rating (gawr) listed, which may result in a potential tire failure. Correction: updated labels will be mailed to owners of affected vehicles, along with instructions for proper installation.

  • Discrepancies in required cfr reporting investigation as of 20181022

    On october 22, 2018, an audit query (aq18-005) was opened to assess the completeness and timeliness of volvo groups early warning reporting, safety recalls, and communications submissions to the agency.Nhtsas investigation determined that volvo group failed to recall vehicles in a timely fashion and to comply with other recall and reporting requirements, including notifying owners of recalls and reporting death and injury incidents.Nhtsa and volvo group entered into a consent order on january 27, 2023 to resolve aq18-005.As part of the consent order, volvo group agreed to a total civil penalty of one-hundred and thirty million dollars ($130,000,000), of which, sixty-five million dollars ($65,000,000) was paid on february 21, 2023.Also, of that total civil penalty, volvo group agreed to spend twenty million dollars ($20,000,000) on the specific performance obligation to create a safety data analytics infrastructure to detect and study emerging safety-related defect trends on its vehicles. The consent order also holds forty-five million dollars ($45,000,000) of the civil penalty in abeyance pending volvo groups satisfactory completion of the requirements of this consent order, and its compliance with the safety act, and regulations thereunder.The consent order is for a three-year term and may be extended by up to two years if nhtsa reasonably finds that an extension is warranted.In the consent order, volvo group agreed to several performance obligations, including enhanced oversight of its compliance with the safety act and the terms of the consent order through a third-party auditor and regular meetings with nhtsa.The third-party auditor will recommend and oversee implementation of process improvements to enhance volvo groups safety compliance processes and oversee the companys review of past recalls and submissions to nhtsa for accuracy and completeness.Also, via the consent order, volvo group agreed to create updated written policies, procedures, and training to ensure that its employees understand the requirements of the safety act, the regulations thereunder, and this consent order.Volvo group further agreed to launch a user interface on its websites to allow users to search vehicle identification numbers (vins) for open recalls and implement a system to support nhtsas vin lookup tool.Based on the consent order, this audit query investigation, aq18-005, is closed.Closure of this aq has no bearing on the terms of the consent order, including volvo groups ongoing performance obligations or legal obligations, and does not represent a determination by nhtsa as to volvo groups performance under the consent order to date.

  • Windshield wiper failures investigation as of 20130318

    Through regular communications with heavy truck fleets, odi learned of a potential issue involving failures of both the windshield wiper motor or linkage on model year 2011-2012 volvo vnl trucks.Odi searched its early warning reporting (ewr) database and identified a significant number of claims of wiper system problems reported by volvo.In some cases the windshield wiper linkage separated and in others the wiper motor failed.In both failure modes, the visibility of the driver is significantly diminished.In response to an odi information request (ir) dated 25 march 2013, volvo provided warranty data that identified three distinct windshield wiper failure modes on the subject vehicles: wiper motor, wiper linkage, and wiper arms.Analyzing the warranty data revealed elevated rates of wiper motor and wiper arm failures.The warranty data further showed that the wiper arm failures occurred early in the vehicle service life suggesting that those that were likely to fail had already doneso.However the wiper motor failures were found to be an ongoing concern.Owners continued to file warranty claims at an elevated rate at the time of this investigation.Following a meeting on 25 july 2013, volvo notified odi on 9 august 2013 they had determined the wiper motor drive gear may prematurely fail on subject vehicles manufactured from 1 february 2010 through 30 september 2010 and 1 june 2011 through 30 november 2011.Volvo states that the failure presents a risk of decreased visibility through the windshield, which may result in a vehicle crash.Volvo initiated a safety recall (nhtsa recall no. 13v-349) to address the safety defect in approximately 17,824 model year 2011 and 2012 vah, vhd, vnl, and vnm trucks.Volvo?S action to remedy the subject vehicles for the defective wiper motor resolves the issue raised by this investigation and therefore this investigation is closed.

  • Stop lamp switch signal investigation as of 20130207

    The office of defects investigation (odi) received early warning reporting (ewr) data from the manufacturer that indicated some number of subject vehicles equipped with the subject automated manual transmission had allegedly experienced a condition where the clutch failed to automatically disengage the transmission from the engine as intended. This may result in extended stopping distances and/or unintended forward motion of a vehicle previously at rest.To date, odi is aware of 134 subject vehicles with one or more complaints or field reports related to this condition.Also, odi is aware of 164 subject vehicles with one or more warranty claims related to this condition.By letter dated june 25, 2013, volvo trucks north america (volvo) notified odi that it has initiated a safety recall (nhtsa recall no. 13v268 and volvo recall no. Rvxx1303) to address a safety defect in approximately 23,003 model year (my) 2007 through 2014 volvo vah, vhd, vnl, vnm and vt model trucks. The affected vehicles were manufactured from december 20, 2006 through may 22, 2013 and were originally equipped with the subject automated manual transmissions.According to volvo, reports of the subject transmission clutch mechanism failing to automatically disengage as intended are associated with the removal of a brake pedal micro switch that provided a redundant signal to the transmission control module indicating the brakes had been applied.Previously, volvo had removed this brake pedal micro switch from new vehicle production and from certain vehicles already in-service via two field service campaigns (sp 432-024 and sp 432-026) because it decided the switch was unnecessary and/or a maintenance issue.In the absence of this redundant brake pedal micro switch, the clutch mechanism may fail to disengage the transmission from the engine at low vehicle speed should the stop lamp switch fail to send and/or the transmission control module fail to receive the primary signal.At the time of this closing, volvo is evaluating available remedies and will provide nhtsa with details when the recall remedy procedure has been finalized.Volvo will notify affected owners by mail by august 25, 2013.Please see nhtsa recall no. 13v268 for details.Accordingly, this investigation is closed.

  • Volvo sleeper glass de-bonding investigation as of 20110523

    Odi opened recall query rq11-001 after a review of ewr field reports identified lower bunk window separations on the subject vehicles.Volvo had issued three recalls for sleeper window separations since 2003.Additionally, volvo had issued two bulletins for sleeper window issues during the same time period.The frequency of related actions led odi to open this formal investigation.An information request (ir) letter was sent to volvo on 23 june 2011.Volvo responded on 17 august 2011 and then provided updated warranty information on 9 january 2012.Volvo provided production change data and warranty information which was correlated to vehicle build month and failure date.Odi analyzed the data to determine if additional production time frames should have been included in previous recalls.Odi also requested information from its network of fleet owners.The response from this network reaffirmed the accuracy of the updated warranty information.Based on the information and data received, odi has concluded that all production periods have been appropriately addressed by volvos previous recalls and no additional recall action is required at this time.

While driving, vehicle engine suddenly started running intermittent when hitting a bump or accellerating, then abruptly shut completely down while driving. Luckely the speed was low and we were able to stop, but ended up in the middle of the road. Upon inspection on the road, sparks came off from under the cab with electrical burning smell. Shut down battery system. Found positive battery cable behind the engine rubbed against chassis support bracket and welded itself to the bracket. 40a fuse in engine fuse compartment burned through.
