    The office of defects investigation (odi) has received eight hyundai motor america (hyundai) and eight kia america, inc. (kia) part 573 recall reports between 2016 and 2023. The combined 16 safety recalls all involve the antilock braking system (abs) modules or hydraulic electronic control units (hecu) manufactured by the equipment supplier mando. The part 573 recall reports contain varying defect descriptions and differing remedy descriptions for the same or similar equipment supplied by mando. Safety recalls included in this audit query (aq) review include: hyundai recalls 20v-520, 20v-543, 21v-161, 21v-160, 21v-303, 22v-056, 22v-810, and 23v-651. Kia recalls 16v-815, 20v-518, 20v-519, 21v-137, 21v-331, 22v-051, 22v-800, and 23v-652 the purpose of this aq is to evaluate the timeliness and scoping of hyundai and kia’s defect decision making and adherence with reporting requirements; and understand the varying defect descriptions and remedies between these recalls.

    Nhtsa received a petition on or about july 18, 2022, requesting that federal motor vehicle safety standard (fmvss) 141 be applied to all electric and hybrid vehicles operating in the united states. The petition can be reviewed at nhtsa.Gov under odi number 11486072.Fmvss 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 kg or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after march 1, 2021.The petitioner asserts that hybrid and electric vehicles to which the standard does not apply should be found to contain a safety defect. In support of the petition, the petitioner includes findings contained in a bill introduced in the house of representatives in 2009, the pedestrian safety enhancement act of 2009, h.R. 734, 111th cong. (2009). A related bill, the pedestrian safety enhancement act of 2010, became law in january 2011 (pub. L. 111-373).     The office of defects investigation (odi) has received a petition on july 13, 2022, requesting a defect investigation into an alleged defect of the "oil drain pan assemblies" on the following 83 models/model year vehicles stated by the petitioner: 2006-2019 hyundai sonata, 2013-2018 hyundai santa fe sport, 2010-2012 and 2019-2020 hyundai santa fe, 2010-2015 and 2018-2019 hyundai tucson, 2011-2021 kia sportage 2005-2020 kia optima, 2011-2020 kia sorento, 2010-2013 kia forte, 2010-2013,kia forte coup, and 2007-2010 kia rondo vehicles equipped with the 1.6l t-gdi gamma, 2.4l theta ii gdi, 2.4l in-line gdi theta ii, kappa, and 3.8l v-shaped gdi lambda-ii engines. The petition also cites additional hyundai/kia vehicles that use similar drain pan assemblies as the above-mentioned vehicles as having drain pan assembly failures. The petitioner provided subject and peer vehicles information related to alleged engine failures. The petition itself can be reviewed at nhtsa.Gov under odi number 11481745.According to the petition, the hyundai/kia vehicle owners have made allegations of oil leakage failures occurring at highway speeds, without malfunction, indicator lights, or the leakage associated with under-tightening an oil drain plug, or a plug or pan with damaged thread. The alleged defects are causing engines to seize and loss of motive power. A defect petition has been opened to evaluate the issue and determine whether to grant or deny the petition. The petitioner identified a number of complaints received by nhtsa alleging complete engine failure from a variety of models stated above along with other models.   • Engine fires investigation # EA21003

    In 2017, the office of defects investigation (odi) opened two recall queries (rq17-003 and 004) following multiple recall actions initiated by hyundai and kia for “theta ii” engine-equipped vehicles. The rqs were opened to investigate both the timeliness and scope of the recalls as well as the compliance of each company to meet their reporting requirements established under the motor vehicle safety act. Numerous violations of the requirements were found for both companies, and in on november 20, 2020, both companies entered into consent orders with nhtsa to resolve nhtsa’s assertions of violations of law. Both consent orders remain in effect.In 2018, the center for automotive safety (cas) petitioned the agency under defect petition (dp) 18-003 to investigate non-crash fires on various hyundai and kia models, citing 120 voqs alleging vehicle fires at varying points of origin and occurring under various non-crash related circumstances (including key-off fires). In opening dp18-003, odi noted that many of the 120 fire incidents appeared to originate in the engine compartment area and made reference to the two open rq investigations as likely pertinent. Odi issued information request (ir) letters to hyundai and kia and several peer oems in its efforts to properly evaluate and disposition cas’ petition.After reviewing responses to the dp requests, odi granted cas’ petition and in 2019 opened two preliminary evaluations (pe19-003 and 19-004, one for each company) to broadly investigate incidents of non-crash fires (regardless of origin or operational status of the vehicle) on various model year hyundai (sonata and santa fe) and kia (optima, sorento and soul) models. Odi issued additional, more broadly scoped ir letters to both hyundai and kia, which upon review, identified certain vehicle populations exhibiting higher rates of non-crash fires for various defects and points of origin, including some due to engine failure.To date, hyundai and kia have issued several recallsto address vehicle fires, including those due to engine failure involving various engine types (theta ii gdi, theta ii mpi, theta ii mpi hev, nu gdi, and gamma gdi). The remedies for the engine failure recalls typically involve engine inspection (to detect existing damage), and if necessary, engine replacement. Additionally, both manufacturers are installing an engine control software modification known as knock sensor detection software (ksds) which is intended to detect impending engine failure, alert the driver, and limit engine power (to lessen likelihood of engine failure). Ksds is being installed under recalls and non-safety field actions, and both companies have extended limited engine warranties (see recall files). They have even replaced engines if the ksds software detects an impending engine failure.The 78 voqs cited above, which all involve fires likely due to engine failure, consist of 36 remedy-related and 42 scope-related incidents. The injury allegations involve eye and burn injuries not requiring medical treatment. With the 83 other voqs, odi is aware of 161 fires potentially due to engine failures. First, the investigation will continue to evaluate the scope of recalls initially begun under prior odi actions (see below), and will also include other recalls for fires due to engine failures, which hyundai and kia have subsequently launched (many influenced by odi investigations). Second, the investigation will monitor and evaluate the efficacy of the recall remedies for engine fire related recalls, as well as the long-term viability of related programs and non-safety field actions being conducted by hyundai and kia.

The petitioners allege the subject vehicles exhibit a range of dangerous vehicle speed control problems including sudden uncommanded acceleration, runaway throttle conditions, surging, stalling and loss of motive power, all due to various defects and vulnerabilities in the electronic throttle control system.This defect petition has been opened to evaluate the allegation and determine whether to grant or deny the petitioners request.

  • Air bag acu electrical overstress investigation # EA19001

    The office of defects investigation (odi) is expanding the investigation to include the equipment supplier and vehicle manufacturers (oems) using this unit.The investigation focuses on acus manufactured by trw, now zf-trw (zf) , the tier-one supplier to hyundai and kia and the other affected oems.The acu senses a vehicle crash to determine whether air bag deployment is required, and if so, deploys the appropriate air bags and other supplemental restraints.Zf supplied subject acus to six oems: fca, honda, hyundai, kia, mitsubishi and toyota.Internal to the acu is an electronic component (an application specific integrated circuit, or asic) that monitors signals from crash sensors.A failure of the asic may prevent deployment of the required air bags and devices, or may otherwise affect the proper operation of the acu.The acu is located in the passenger compartment, and electrical wiring connects the asic to sensors located at the front of the vehicle.Odi’s current understanding is that a crash event may, in and of itself, produce harmful signals on the sensor wiring capable of damaging the asic, although the probability of this occurring appears to be low.While the acu incorporates electrical circuitry intended to protect the asic from harmful signals, the level and effectiveness of the protective circuitry varies by oem customer.During pe18-003, hyundai and kia filed recalls (18v-137 and 18v-363 respectively) to address a defect that could result in acu disablement and non-deployments.According to the filings, the disablement occurs in certain types of frontal crash events.Both filings discussed a condition known as electrical overstress (eos) that affected the subject asic and was likely caused by electrical signals that entered the acu via sensor wiring.The recalled vehicles used acus that had the lowest levels of asic protection while non-recalled hyundai and kia products using subject acus had higher levels of protection.Odi has not identified any eos failures in the non-recalled kia and hyundai populations.In september 2016, fca filed recall 16v-668 for certain model year (my) 2010 to 2014 chrysler, dodge and jeep products also manufactured with the subject acu.In that filing, fca also discussed an eos condition that resulted in a failure of the subject asic, which caused air bag non-deployment. Fca noted that the defect condition had only been observed in vehicles equipped with sensor harnessing routed across the front of the vehicle.Other fca vehicles that also used the subject acu, but not the cross-car harnessing, had not experienced eos failures, despite similar time in service. The recalled fca vehicles used a mid-level form of asic protection.Other fca vehicles that did not use cross car wiring, or used higher levels of asic protection, have not been recalled.Odi has not identified any eos failures in the non-recalled fca population.Recently, odi has identified two substantial frontal crash events (one fatal) involving toyota products where eos is suspected as the likely cause of the non-deployments.The crashes involved a my 2018 and a my 2019 corolla equipped with the subject acu that incorporated higher levels of asic protection.Additionally, both acus were found to be non-communicative (meaning the acu could not be read with an event data recorder) after the crash, a condition found in other cases where eos occurred with other oems.No other eos events have been identified for other toyota products (including corolla models that used the subject acu since my 2011), or for the honda and mitsubishi vehicles that use the subject acu.Odi plans to evaluate the susceptibility of the subject acu designs to electrical signals, as well as other vehicle factors that can either lead to, or reduce the likelihood of, an eos event.Additionally, odi will evaluate whether an unreasonable risk exists that requires further field action.

  • Non-crash vehicle fires investigation # PE19003

    In a letter dated jun 11, 2018, the center for auto safety (cas) petitioned the national highway traffic safety administration (nhtsa) to initiate a safety defect investigation into non-collision related fires on model year (my) 2011-2014 kia optima and sorento, and hyundai sonata and santa fe vehicles.In a letter dated july 24, 2018, cas requested that my 2010-2015 kia soul vehicles be added to the petition.Cas stated that there is a higher number of complaints to nhtsa regarding non-crash fires on these vehicles compared to other similar vehicles.The office of defects investigation (odi) had already opened a recall query (rq17-004) on all hyundai vehicles equipped with theta ii engines for engine failures that may cause stalling and fires.The query covers most of the petition subject sonata and santa fe vehicles.On august 21, 2018, odi opened a defect petition (dp18-003) to evaluate whether to grant or deny the cas petition.Odi sent an information request letter to hyundai (september 2018) requesting information on all engine and non-engine related fires in the petition hyundai vehicles.Odi performed extensive analysis on the information obtained from hyundai.During the dp18-003 analysis, hyundai initiated three additional safety recalls related to engine compartment fires on certain petitioned and non-petitioned vehicles, (nearly 240,000 total vehicles: recalls 18v-934, 19v-063, and 19v-204). In addition to engine compartment fires, odi has received allegations of vehicle fires affecting other components, including taillight housings, taillight wire harnesses, and/or light bulbs. Based on odis review of the applicable materials, including responses to information requests sent to multiple manufacturers, vehicle owner questionnaires (voq), and early warning data, nhtsa has decided to grant the petition.The petition is hereby granted. A preliminary evaluation has been opened to assess the scope, frequency, and potential safety-related consequences of alleged defects relating to non-collision vehicle fires in the 2011-2014 sonata and 2011-2014 santa fe.While prior recalls of vehicles covered by this pe have primarily related to engine fires, nhtsas preliminary evaluation is not limited to engine components and may cover additional vehicle systems or components as well.Attached is a list of the reference numbers for the 160 voqs cited above.

  • Air bags may be disabled during crash investigation # PE18003

    The office of defects investigation (odi) opened pe18-003 on 3/16/18 to investigate incidents involving crashes, resulting in significant collision related damage, where the frontal airbags failed to deploy. Odi reviewed complaint data, available design, test, and field data on the model year (my) 2011 hyundai sonata, my 2012-2013 kia forte and forte koup vehicles. Odi also evaluated data from the automotive air bag system supplier, zf-trw (zf), to review vehicles that may contain the same air bag control unit (acu) used in the subject vehicles. Hyundai motor america (hma) issued recall no. 18v-137 for my 2011-2013 sonata and my 2012 sonata hybrid vehicles. Kia motors america (kma) issued recall no. 18v-363 for my 2010-2013 kia forte and forte koup, my 2011-2013 optima, my 2011-2012 optima hybrid, and my2011-2012 sedona vehicles. The two recalls address vehicles containing acus manufactured without adequate circuit protection by installing a wire harness filter kit to prevent internal damage to the acu. In the event of a crash, acu damage may result in the disablement of the vehicles airbags. Each respective manufacturer has mailed letters to affected vehicle owners informing them of the remedy and its availability. Odi plans to evaluate the susceptibility of the subject acu to electrical signals, as well as other vehicle factors that can either lead to, or reduce the likelihood of, an electrical overstress event in other vehicles manufactured with the subject zf acu.Accordingly, this preliminary evaluation has been upgraded to an engineering analysis (ea19-001), to expand the scope of the investigation to include the tier-one supplier and any manufacturers who installed the subject acu in production vehicles.   • Scope and timeliness investigation # RQ17004

    On september 10, 2015, hyundai motor america (hyundai) filed a defect information report (dir) recalling 470,000 my 2011 and 2012 sonata vehicles with “"theta ii" engines (recall no. 15v-568). Hyundais dir described the defect as an issue involving manufacturing debris. Hyundai limited the population of recalled vehicles to those produced on or prior to april 12, 2012, stating that a process change in april 2012 resolved the issue of manufacturing debris. On march 31, 2017, hyundai expanded its original recall to include 572,000 my 2013-2014 sonata and santa fe sport vehicles with "theta ii" engines (recall no. 17v-226). The dir continues to describe the defect as an issue involving manufacturing debris.This rq is being opened to investigate both the timeliness and scope of hyundais "theta ii" engine recalls, and hyundais compliance with reporting requirements.

  • Front passenger seat belt anchor failure investigation # PE16018

    The office of defects investigation (odi) opened this investigation on december 27, 2016 based on two odi consumer complaints reporting a failure of the front passenger seat belt to remain connected to the seat belt anchor and pre-tensioner during a crash.On march 8, 2017 hyundai motor america (hyundai) filed a defect report with the agency stating that it would conduct a safety recall on 977,778 model year 2010-2014 sonata vehicles produced from december 11, 2009 through may 29, 2014 and model year 2011-2015 sonata hybrid vehicles produced from december 2, 2010 through january 9, 2015.The remedy for the recall was to inspect the front seat belt anchor connector for proper attachment to the seat belt pre-tensioner.Odi continued the investigation in order to pursue a remedy that would be more lasting, rather than a one-time inspection.The connector is designed so that it can be removed from the pre-tensioner, when required, during vehicle maintenance.If it is not properly connected when originally installed or when re-installed after maintenance, the connector could potentially release in a crash. On september 4, 2017 hyundai filed an amended defect report and advised the agency that the remedy would be modified to provide a permanent label, to be attached to the connector, at the time of inspection.The label provides graphic instructions and illustration as to the proper orientation of the connector lock when it is properly installed.On october 4, 2017 hyundai filed an additional defect report (see 17v-671) to have the vehicles that have had only theinspection procedure performed return to dealers to have the permanent label applied to the connector.Odi is closing this investigation based on hyundais recall action to add a permanent label to the connector. Check engine light began flashing and my 2013 hyundai sonata went into "limp" mode. I checked the code with a diagnostic tool and a P1326 knock sensor detection system code was returned. The car burns oil at an alarming rate, i changed my oil 4,000 miles ago and it burned nearly 4 quarts of oil. There's 133k miles on the car. The dealership in escondido, california keeps giving me the run around and is trying to charge me full price despite there being a well known engine failure/rod knock recall/campaign with this model/year car.

You go to them they just want to charge us $400 for diagnostic test. Last week, hyundai dealer told my car problem is with engine belt and tensioner assembly and i charged $900. But it did not fix the issue. I am not sure if above part was really needed to replaced, now the message on charging system and engine safety ( picture attached ) is too frequent. Ref. Engine recall on this 2013 sonata hybrid, hyundai dealer say car life is finish with 114k miles. And car engine is not issue as decide by hyundai head office.

There are multiple issue with this car since last a year or more. ( picture attached) * at times computer flashes below message .. " hyundai system warning safety stop do not drive" * steering lock up * while driving a screeching sound and then it goes away. We received the recall notice and my daughter jeranica took it to the hyundai dealer for repair. They charged us $900 mentioning the issue is with belt & assembly and engine is ok. We informed then of all the above issue. Even after repairing the above we still have the same issue with the car. It look like hyundai dealer are using this opportunity to rip-off the customer instead of actually fixing the issue. We still have above issue with, car computer asking us to stop on highway and steering lockup, dealer does not seem to be interested in fixing the issue. At one time he mentioned we have 114k mileage on the car and the life of engine is finish.
