• Recall 109741 [USA]: If engine oil comes into contact with a hot surface, there could be an increased risk of a fire.
  Mack trucks, inc. (mack) is recalling certain model year 2011-2016 mack mru and leu trucks manufactured june 25, 2010, to april 30, 2015, and 2015-2016 lr6 trucks manufactured from december 18, 2014 through march 6, 2015. Excessive engine crankcase pressures may cause the 90-degree elbow of the vent tube assembly to detach from the crankcase ventilation breather, possibly allowing engine oil to come into contact with hot surfaces.
  cummins will notify owners on behalf of mack, and cummins service centers will administer the repairs which include securing the ends of the 90-degree elbow and reprogramming the electronic control module (ecm) with software that can adjust the engine output under misfire conditions to limit crankcase pressures. On these vehicles the alternator ground cable runs directly to the starter, which defeats the circuit protection.

• Recall 77388 [USA]: A complete loss of steering may occur increasing the risk of a vehicle crash.
  Mack trucks is recalling certain model year 2009-2012 mru heavy trucks manufactured from november 26, 2008, through april 26, 2011. The bolts used to mount the steering gear were not tightened to specification. The bolts can loosen which over time may cause the steering gear to disconnect from its mounting bracket.

• Recall 76881 [USA]: Slack adjusters with this flow notch could crack under normal braking conditions causing loss of braking which could result in a vehicle crash.
  Mack trucks is recalling certain model year 2011-2012 chu, cxu, gu, leu and mru trucks manufactured from october 15, 2010, through january 31, 2011, and equipped with gunite automatic slack adjusters. Due to an incomplete "knitting" of the iron at the time of casting, inadequate structural integrity of the slack adjuster housing can occur, which could cause the housing to break at the handle under normal operating loads.

• Recall 66569 [USA]: In the event of an electrical overage, the fuse that should prevent the over current from reaching the mirror switch circuit may fail, such that the mirror switch can overheat. an overheated switch could cause a vehicle fire.
  Mack trucks is recalling certain model year 2008-2012 mru and leu heavy trucks manufactured from january 3, 2007, through february 3, 2011. The electrical circuitry in these vehicles may not provide adequate overcurrent protection.
  dealers will install a relay harness to prevent the mirror switch from overheating. They will also inspect the mirror switch and if signs of overheating are shown, replace that switch. Correction: dealers will install clamps at both ends of the elbow of the breather tube assembly and reprogram the electronic control module (ecm) to improve diagnostic capabilities.

  On certain vehicles, the directional/hazard signal flasher may fail. Actuation of the turn signal or hazard lights in this condition would cause them to remain illuminated and not flash as intended.   On certain trucks, the steering gear mounting bolts may not have received adequate tightening torque during vehicle assembly. Should the bolts loosen, the steering gear could separate from its mounting bracket, which may affect steering performance. A loss of steering control could result in a crash causing property damage and/or personal injury. Correction: dealers will replace the steering gear mounting bolts.

  On certain vehicles, the slack adjuster housings may have been manufactured incorrectly, which could allow the housings to fracture and fail. As a result, the wheel brake to which the slack adjuster is attached would no longer function, which could increase vehicle stopping distances. Should a steer axle slack adjuster fail, it could also cause the vehicle to pull to one side during braking. Either problem could result in a crash causing property damage and/or personal injury. Correction: dealers will inspect and, if necessary, replace the slack adjusters.

• Recall 2011148 [Canada]: Electrical
  On certain vehicles, the rearview mirror switch (mounted on the dashboard) may overheat, causing the plastic to melt. An overheated switch may not allow rearview mirror remote adjustability and, as such, the driver may not have full use of the mirrors. In addition, excessive heat can cause smoke within the instrument panel, which could distract the vehicle operator and result in a crash causing property damage and/or personal injury. Correction: dealers will inspect the rearview mirror switch and install a relay wiring harness. Switches that show signs of overheating will be replaced.

• Vehicle roll away investigation # PE20003

  The office of defects investigation (odi) received a tip from a large fleet operator of refuse trucks about a potential defect condition that may result in a vehicle roll away. The subject vehicles are single-unit trucks equipped with certain bendix pp-1 push-pull pneumatic park brake control valves. These subject components are mounted to the dashboard and allow the driver to apply and release the parking brakes. They have a 1/8? Npt threaded fitting in the exhaust port and an exhaust pipe to vent exhaust air outside the truck cab. The subject components may experience an accelerated wear out condition in certain severe-duty applications with high-frequency use of the parking brakes. When the knob is pulled out to apply the parking brakes, supply side air may continuously leak past the worn valve and escape through the exhaust port. This wear out condition may not be readily detected by the driver because the sound is muffled by the exhaust pipe configuration. An exhaust flow restriction, such as an insufficient diameter exhaust port, threaded fitting or exhaust pipe, may allow back-pressure to develop and pressurize the delivery port. This may inadvertently send a signal to the park control port of the sr-7 spring brake modulating control valve to release the parking brakes, possibly resulting in a vehicle roll away.The reporting fleet operator is aware of at least 4 roll away incidents related to the alleged defect on subject vehicles in the united states. In one case, surveillance video recorded the driver applying the parking brakes moments before the vehicle rolled away. In some cases, the root cause was determined by post-incident examination of subject components by the component supplier and/or by a third-party forensic engineering company hired by the fleet operator. In one incident, a subject vehicle rolled more than 100 meters away and crashed into a parked car and a residential structure, causing in excess of $150,000 in property damage. In another incident, a subject vehicle rolled away and crashed into several other refuse trucks parked at the depot. The reported injury incident occurred when a driver dumped a load of refuse at a landfill, pulled into the clean out area, set the parking brakes, locked out the main battery disconnect and proceeded into the rear body to perform normal clean out duties. While inside the rear body, the driver reportedly heard the sound of rushing air, the parking brakes reportedly released and the truck began to roll away. In a panic, the driver jumped out of the moving vehicle and broke his ankle, which later required surgery. The truck came to rest approximately 50 meters away against a dirt berm. A preliminary evaluation has been opened to assess the alleged defect.
