• Recall 109746 [USA]: If engine oil comes into contact with a hot surface, there could be an increased risk of a fire.
  Mack trucks, inc. (mack) is recalling certain model year 2011-2016 mack mru and leu trucks manufactured june 25, 2010, to april 30, 2015, and 2015-2016 lr6 trucks manufactured from december 18, 2014 through march 6, 2015. Excessive engine crankcase pressures may cause the 90-degree elbow of the vent tube assembly to detach from the crankcase ventilation breather, possibly allowing engine oil to come into contact with hot surfaces.
  cummins will notify owners on behalf of mack, and cummins service centers will administer the repairs which include securing the ends of the 90-degree elbow and reprogramming the electronic control module (ecm) with software that can adjust the engine output under misfire conditions to limit crankcase pressures. • Recall 102275 [USA]: The improper operation of the sensor could induce elevated exhaust temperatures or flames from the exhaust pipe, increasing the risk of a vehicle fire or burn injury to a person near the exhaust pipe.

  Mack trucks is recalling certain model year 2013-2014 chu, cxu, gu, leu, and mru vehicles manufactured with pro-torque axle spindle nuts from august 28, 2012, through august 16, 2013. The pro-torque nut may not have been installed correctly, which may result in a loosening of the nut.

• Recall 97070 [USA]: A loss of brake function at one wheel will lead to increased stopping distances and pulling to the left or right when braking, which may increase the risk of a crash.
  Mack trucks, inc. Is recalling certain model year 2013-2014 chu, cxu, gu, mru, and leu trucks manufactured july 16, 2012, through march 18, 2013 and equipped with drum brakes. The clevis pin for the brake slack adjuster may be missing a cotter pin. If the cotter pin is not installed, the clevis pin may loosen and fall out causing a loss of brake function on the affected wheel.
  mack trucks, inc. Will notify owners, and dealers will inspect the brake slack adjusters and install the brake hardware, as necessary, free of charge.   On certain vehicles, the cap nut that retains the interaxle driveshaft yoke to the rear axle input shaft may prematurely loosen. If the nut were to loosen and come off, the yoke could separate from the axle input shaft and cause the drive shaft to disconnect. A disconnected drive shaft would result in a loss of motive power, and create a road hazard, both of which could increase the risk of a crash causing injury and/or damage to property. Correction: dealers will inspect the torque on the cap nut, and the nut will be replaced if the torque is found under specification. Note: this is an expansion of recall 2016-079.

  On certain vehicles equipped with natural gas powered engines, the turbocharger oil supply line may make contact with or interfere with one or more of the following: the turbocharger inlet elbow, the oem clean air intake clamp, or the air fuel control tube. The resulting contact or interference may damage the turbocharger oil supply line, possibly resulting in an oil leak. Oil leaking onto hot engine surfaces could increase the risk of a fire, and could increase the risk of injury and/or property damage. Correction: dealers will replace the oil line.

Correction: dealers will install clamps at both ends of the elbow of the breather tube assembly and reprogram the electronic control module (ecm) to improve diagnostic capabilities.

  On certain vehicles, the directional/hazard signal flasher may fail. Actuation of the turn signal or hazard lights in this condition would cause them to remain illuminated and not flash as intended. As a result, drivers may not be able to clearly signal their intentions to turn, change lanes or warn others in an emergency situation. Correction: dealers will replace the turn signal/hazard flasher module.

  On certain trucks, the cotter pin which retains the brake slack adjuster’s clevis pin may have been omitted during vehicle assembly. As a result, the clevis pin could loosen and fall out, causing a loss of brake function at the affected wheel. This could cause the vehicle to pull to one side under braking, or increase stopping distance, which could result in a crash causing property damage and/or personal injury. Correction: dealers will verify cotter pin installation on each brake slack adjuster. Replacement cotter pins and/or clevis pins will be installed as needed.

  On certain vehicles, the wheel rims may have been manufactured incorrectly and could fail. A wheel separating from the vehicle could cause a loss of control and a crash. In addition, the errant wheel assembly could strike a vehicle, stationary object, or a bystander. These issues could result in property damage and/or personal injury. • Vehicle roll away investigation # PE20003

  The office of defects investigation (odi) received a tip from a large fleet operator of refuse trucks about a potential defect condition that may result in a vehicle roll away. The subject vehicles are single-unit trucks equipped with certain bendix pp-1 push-pull pneumatic park brake control valves. These subject components are mounted to the dashboard and allow the driver to apply and release the parking brakes. They have a 1/8? Npt threaded fitting in the exhaust port and an exhaust pipe to vent exhaust air outside the truck cab. The subject components may experience an accelerated wear out condition in certain severe-duty applications with high-frequency use of the parking brakes. When the knob is pulled out to apply the parking brakes, supply side air may continuously leak past the worn valve and escape through the exhaust port. This wear out condition may not be readily detected by the driver because the sound is muffled by the exhaust pipe configuration. An exhaust flow restriction, such as an insufficient diameter exhaust port, threaded fitting or exhaust pipe, may allow back-pressure to develop and pressurize the delivery port. This may inadvertently send a signal to the park control port of the sr-7 spring brake modulating control valve to release the parking brakes, possibly resulting in a vehicle roll away.The reporting fleet operator is aware of at least 4 roll away incidents related to the alleged defect on subject vehicles in the united states. In one case, surveillance video recorded the driver applying the parking brakes moments before the vehicle rolled away. In some cases, the root cause was determined by post-incident examination of subject components by the component supplier and/or by a third-party forensic engineering company hired by the fleet operator. In one incident, a subject vehicle rolled more than 100 meters away and crashed into a parked car and a residential structure, causing in excess of $150,000 in property damage. In another incident, a subject vehicle rolled away and crashed into several other refuse trucks parked at the depot. The reported injury incident occurred when a driver dumped a load of refuse at a landfill, pulled into the clean out area, set the parking brakes, locked out the main battery disconnect and proceeded into the rear body to perform normal clean out duties. While inside the rear body, the driver reportedly heard the sound of rushing air, the parking brakes reportedly released and the truck began to roll away. In a panic, the driver jumped out of the moving vehicle and broke his ankle, which later required surgery. The truck came to rest approximately 50 meters away against a dirt berm. A preliminary evaluation has been opened to assess the alleged defect.
