• Recall 139006 [USA]: The driver may operate the vehicle with the snow plow headlights on but the marker and tail lamps off, reducing the vehicle's visibility and increasing the risk of a crash.
  Volvo trucks north america (volvo) is recalling certain 2004-2019 volvo vhd trucks equipped with snow plow lamp preparation kits. The snow plow headlights can be turned on without other exterior lights, and high beam indicator will not work when the snow plow high beams are on. As such, these vehicles fail to comply with the requirements of federal motor vehicle safety standard (fmvss) number 108, "lamps, reflective devices, and associated equipment."

• Recall 91023 [USA]: Inadvertent brake application can cause the brakes to overheat and lead to a fire. inadvertent brake application can cause the wheels to lock-up leading the driver to lose control of the vehicle, increasing the risk of a crash.
  Volvo is recalling certain model year 2011-2012 vhd and model year 2011-2013 vnl and vnm vehicles, manufactured from december 2, 2010, through january 18, 2012, and equipped with bendix atr-6 traction relay valves. In extremely cold conditions, these bendix relay valves may potentially develop internal leakage. Internal leakage can lead to air pressure being delivered to affected primary or secondary brake causing continuous brake application.

  Certain vehicles equipped with a snowplow preparation kit may fail to comply with the requirements of canada motor vehicle safety standard (cmvss) 108 - lighting system and retroreflective devices.   On certain vehicles, the secondary clutch disengagement switch may have been omitted during vehicle assembly or, in some instances, removed during servicing.   Certain vehicles may fail to conform to canada motor vehicle safety standard 210 – seatbelt assembly anchorages. The weld used to secure the nut to the center-passenger seatbelt d-ring mounting bracket may not be sufficient to meet the strength requirements of the standard. Failure of the seatbelt d-ring anchorage to restrain the occupant would increase the risk of personal injury during a crash. Correction: dealers will replace the d-ring for the middle seating position.

  On certain trucks equipped with bendix atr-6 antilock traction relay valves, in extremely cold conditions (at or below -18 degrees celsius), internal atr valve leakage can occur.   On certain trucks, the driveshaft may not have been welded correctly during the manufacturing process. This could allow the driveshaft to fracture along the length of the tube seam weld when torque is applied. The fractured tube can twist or sever, which could result in additional damage to the driveline, the vehicle to which it is attached, and any additional nearby components. Should the brake air lines become damaged, it could allow spring brake application, causing immediate brake lock-up. If this occurs while the vehicle is in motion, it could result in a crash. Also, failure of the driveshaft would result in lost vehicle propulsion which, in conjunction with traffic and road conditions, and the driver's reactions, could increase the risk of a crash. Either condition could result in property damage and/or personal injury. Correction: dealers will replace the driveshaft assembly.

  On certain vehicles, the slack adjuster housings may have been manufactured incorrectly, which could allow the housings to fracture and fail. As a result, the wheel brake to which the slack adjuster is attached would no longer function, which could increase vehicle stopping distances. Should a steer axle slack adjuster fail, it could also cause the vehicle to pull to one side during braking. Either problem could result in a crash causing property damage and/or personal injury. Correction: dealers will inspect and, if necessary, replace the slack adjusters.

  During a review of volvo trucks (volvo) 2016, 4th quarter ewr field report submissions, odi discovered, multiple communications that had not been submitted to the agency under cfr 579.5 ewr reporting requirements.Also, during a review of the nova lfs transit bus (a subsidiary of the volvo group) recall 17v-323 odi became concerned volvo may not have acted with appropriate speed in making a defect determination and filing the recall with the agency.Based on a review of these issues with volvo, their compliance office agreed to conduct an internal audit of all required reporting and found the following:1.Various forms of communications including, service bulletins, tech tips, case base reasoning reports and other data were not submitted.2.Reporting of some death and injury reports (d&i) had been overlooked and not submittedthis investigation will examine whether volvo acted in conformance with its responsibilities under the safety act.

• Stop lamp switch signal investigation # PE13002

  The office of defects investigation (odi) received early warning reporting (ewr) data from the manufacturer that indicated some number of subject vehicles equipped with the subject automated manual transmission had allegedly experienced a condition where the clutch failed to automatically disengage the transmission from the engine as intended. This may result in extended stopping distances and/or unintended forward motion of a vehicle previously at rest.To date, odi is aware of 134 subject vehicles with one or more complaints or field reports related to this condition.Also, odi is aware of 164 subject vehicles with one or more warranty claims related to this condition.By letter dated june 25, 2013, volvo trucks north america (volvo) notified odi that it has initiated a safety recall (nhtsa recall no. 13v268 and volvo recall no. Rvxx1303) to address a safety defect in approximately 23,003 model year (my) 2007 through 2014 volvo vah, vhd, vnl, vnm and vt model trucks. The affected vehicles were manufactured from december 20, 2006 through may 22, 2013 and were originally equipped with the subject automated manual transmissions.According to volvo, reports of the subject transmission clutch mechanism failing to automatically disengage as intended are associated with the removal of a brake pedal micro switch that provided a redundant signal to the transmission control module indicating the brakes had been applied.Previously, volvo had removed this brake pedal micro switch from new vehicle production and from certain vehicles already in-service via two field service campaigns (sp 432-024 and sp 432-026) because it decided the switch was unnecessary and/or a maintenance issue.In the absence of this redundant brake pedal micro switch, the clutch mechanism may fail to disengage the transmission from the engine at low vehicle speed should the stop lamp switch fail to send and/or the transmission control module fail to receive the primary signal.At the time of this closing, volvo is evaluating available remedies and will provide nhtsa with details when the recall remedy procedure has been finalized.Volvo will notify affected owners by mail by august 25, 2013.Please see nhtsa recall no. 13v268 for details.Accordingly, this investigation is closed.
