  • Hyundai and kia abs module fires investigation # AQ23002

    The office of defects investigation (odi) has received eight hyundai motor america (hyundai) and eight kia america, inc. (kia) part 573 recall reports between 2016 and 2023. The combined 16 safety recalls all involve the antilock braking system (abs) modules or hydraulic electronic control units (hecu) manufactured by the equipment supplier mando. The part 573 recall reports contain varying defect descriptions and differing remedy descriptions for the same or similar equipment supplied by mando. Safety recalls included in this audit query (aq) review include: hyundai recalls 20v-520, 20v-543, 21v-161, 21v-160, 21v-303, 22v-056, 22v-810, and 23v-651. Kia recalls 16v-815, 20v-518, 20v-519, 21v-137, 21v-331, 22v-051, 22v-800, and 23v-652 the purpose of this aq is to evaluate the timeliness and scoping of hyundai and kia’s defect decision making and adherence with reporting requirements; and understand the varying defect descriptions and remedies between these recalls.

  • Pedestrian alert sounds investigation # DP22005

    Nhtsa received a petition on or about july 18, 2022, requesting that federal motor vehicle safety standard (fmvss) 141 be applied to all electric and hybrid vehicles operating in the united states. The petition can be reviewed at nhtsa.Gov under odi number 11486072.Fmvss 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 kg or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after march 1, 2021.The petitioner asserts that hybrid and electric vehicles to which the standard does not apply should be found to contain a safety defect. In support of the petition, the petitioner includes findings contained in a bill introduced in the house of representatives in 2009, the pedestrian safety enhancement act of 2009, h.R. 734, 111th cong. (2009). A related bill, the pedestrian safety enhancement act of 2010, became law in january 2011 (pub. L. 111-373).     The office of defects investigation (odi) has received a petition on july 13, 2022, requesting a defect investigation into an alleged defect of the "oil drain pan assemblies" on the following 83 models/model year vehicles stated by the petitioner: 2006-2019 hyundai sonata, 2013-2018 hyundai santa fe sport, 2010-2012 and 2019-2020 hyundai santa fe, 2010-2015 and 2018-2019 hyundai tucson, 2011-2021 kia sportage 2005-2020 kia optima, 2011-2020 kia sorento, 2010-2013 kia forte, 2010-2013,kia forte coup, and 2007-2010 kia rondo vehicles equipped with the 1.6l t-gdi gamma, 2.4l theta ii gdi, 2.4l in-line gdi theta ii, kappa, and 3.8l v-shaped gdi lambda-ii engines. The petition also cites additional hyundai/kia vehicles that use similar drain pan assemblies as the above-mentioned vehicles as having drain pan assembly failures. The petitioner provided subject and peer vehicles information related to alleged engine failures. The petition itself can be reviewed at nhtsa.Gov under odi number 11481745.According to the petition, the hyundai/kia vehicle owners have made allegations of oil leakage failures occurring at highway speeds, without malfunction, indicator lights, or the leakage associated with under-tightening an oil drain plug, or a plug or pan with damaged thread. The alleged defects are causing engines to seize and loss of motive power. A defect petition has been opened to evaluate the issue and determine whether to grant or deny the petition. The petitioner identified a number of complaints received by nhtsa alleging complete engine failure from a variety of models stated above along with other models.   • Engine fires investigation # EA21003

    In 2017, the office of defects investigation (odi) opened two recall queries (rq17-003 and 004) following multiple recall actions initiated by hyundai and kia for “theta ii” engine-equipped vehicles. The rqs were opened to investigate both the timeliness and scope of the recalls as well as the compliance of each company to meet their reporting requirements established under the motor vehicle safety act. Numerous violations of the requirements were found for both companies, and in on november 20, 2020, both companies entered into consent orders with nhtsa to resolve nhtsa’s assertions of violations of law. Both consent orders remain in effect.In 2018, the center for automotive safety (cas) petitioned the agency under defect petition (dp) 18-003 to investigate non-crash fires on various hyundai and kia models, citing 120 voqs alleging vehicle fires at varying points of origin and occurring under various non-crash related circumstances (including key-off fires). In opening dp18-003, odi noted that many of the 120 fire incidents appeared to originate in the engine compartment area and made reference to the two open rq investigations as likely pertinent. Odi issued information request (ir) letters to hyundai and kia and several peer oems in its efforts to properly evaluate and disposition cas’ petition.After reviewing responses to the dp requests, odi granted cas’ petition and in 2019 opened two preliminary evaluations (pe19-003 and 19-004, one for each company) to broadly investigate incidents of non-crash fires (regardless of origin or operational status of the vehicle) on various model year hyundai (sonata and santa fe) and kia (optima, sorento and soul) models. Odi issued additional, more broadly scoped ir letters to both hyundai and kia, which upon review, identified certain vehicle populations exhibiting higher rates of non-crash fires for various defects and points of origin, including some due to engine failure.To date, hyundai and kia have issued several recallsto address vehicle fires, including those due to engine failure involving various engine types (theta ii gdi, theta ii mpi, theta ii mpi hev, nu gdi, and gamma gdi). The remedies for the engine failure recalls typically involve engine inspection (to detect existing damage), and if necessary, engine replacement. Additionally, both manufacturers are installing an engine control software modification known as knock sensor detection software (ksds) which is intended to detect impending engine failure, alert the driver, and limit engine power (to lessen likelihood of engine failure). Ksds is being installed under recalls and non-safety field actions, and both companies have extended limited engine warranties (see recall files). They have even replaced engines if the ksds software detects an impending engine failure.The 78 voqs cited above, which all involve fires likely due to engine failure, consist of 36 remedy-related and 42 scope-related incidents. The injury allegations involve eye and burn injuries not requiring medical treatment. With the 83 other voqs, odi is aware of 161 fires potentially due to engine failures. First, the investigation will continue to evaluate the scope of recalls initially begun under prior odi actions (see below), and will also include other recalls for fires due to engine failures, which hyundai and kia have subsequently launched (many influenced by odi investigations). Second, the investigation will monitor and evaluate the efficacy of the recall remedies for engine fire related recalls, as well as the long-term viability of related programs and non-safety field actions being conducted by hyundai and kia.

    The office of defects investigation of the national highway traffic safety administration received a petition letter dated october 10, 2021 requesting an investigation of an alleged defect(s) in the electronic throttle control system in certain model year (my) 2005-2016 kia optima/k5, my 2006-2015 kia sorento, my 2007-2016 hyundai elantra, my 2007-2016 hyundai santa fe, my 2006-2015 hyundai sonata vehicles. The petition was filed as a vehicle owner questionnaire that can be reviewed at nhtsa.Gov under reference (odi) number 11439675. The petitioners allege the subject vehicles exhibit a range of dangerous vehicle speed control problems including sudden uncommanded acceleration, runaway throttle conditions, surging, stalling and loss of motive power, all due to various defects and vulnerabilities in the electronic throttle control system.This defect petition has been opened to evaluate the allegation and determine whether to grant or deny the petitioners request.

  • Non-crash vehicle fires investigation # PE19003

    In a letter dated jun 11, 2018, the center for auto safety (cas) petitioned the national highway traffic safety administration (nhtsa) to initiate a safety defect investigation into non-collision related fires on model year (my) 2011-2014 kia optima and sorento, and hyundai sonata and santa fe vehicles.In a letter dated july 24, 2018, cas requested that my 2010-2015 kia soul vehicles be added to the petition.Cas stated that there is a higher number of complaints to nhtsa regarding non-crash fires on these vehicles compared to other similar vehicles.The office of defects investigation (odi) had already opened a recall query (rq17-004) on all hyundai vehicles equipped with theta ii engines for engine failures that may cause stalling and fires.The query covers most of the petition subject sonata and santa fe vehicles.On august 21, 2018, odi opened a defect petition (dp18-003) to evaluate whether to grant or deny the cas petition.Odi sent an information request letter to hyundai (september 2018) requesting information on all engine and non-engine related fires in the petition hyundai vehicles.Odi performed extensive analysis on the information obtained from hyundai.During the dp18-003 analysis, hyundai initiated three additional safety recalls related to engine compartment fires on certain petitioned and non-petitioned vehicles, (nearly 240,000 total vehicles: recalls 18v-934, 19v-063, and 19v-204). In addition to engine compartment fires, odi has received allegations of vehicle fires affecting other components, including taillight housings, taillight wire harnesses, and/or light bulbs. Based on odis review of the applicable materials, including responses to information requests sent to multiple manufacturers, vehicle owner questionnaires (voq), and early warning data, nhtsa has decided to grant the petition.The petition is hereby granted. A preliminary evaluation has been opened to assess the scope, frequency, and potential safety-related consequences of alleged defects relating to non-collision vehicle fires in the 2011-2014 sonata and 2011-2014 santa fe.While prior recalls of vehicles covered by this pe have primarily related to engine fires, nhtsas preliminary evaluation is not limited to engine components and may cover additional vehicle systems or components as well.Attached is a list of the reference numbers for the 160 voqs cited above.

  • Air bags may be disabled during crash investigation # PE18003

    The office of defects investigation (odi) opened pe18-003 on 3/16/18 to investigate incidents involving crashes, resulting in significant collision related damage, where the frontal airbags failed to deploy. Odi reviewed complaint data, available design, test, and field data on the model year (my) 2011 hyundai sonata, my 2012-2013 kia forte and forte koup vehicles. Odi also evaluated data from the automotive air bag system supplier, zf-trw (zf), to review vehicles that may contain the same air bag control unit (acu) used in the subject vehicles. Hyundai motor america (hma) issued recall no. 18v-137 for my 2011-2013 sonata and my 2012 sonata hybrid vehicles. Kia motors america (kma) issued recall no. 18v-363 for my 2010-2013 kia forte and forte koup, my 2011-2013 optima, my 2011-2012 optima hybrid, and my2011-2012 sedona vehicles. The two recalls address vehicles containing acus manufactured without adequate circuit protection by installing a wire harness filter kit to prevent internal damage to the acu. In the event of a crash, acu damage may result in the disablement of the vehicles airbags. Each respective manufacturer has mailed letters to affected vehicle owners informing them of the remedy and its availability. Odi plans to evaluate the susceptibility of the subject acu to electrical signals, as well as other vehicle factors that can either lead to, or reduce the likelihood of, an electrical overstress event in other vehicles manufactured with the subject zf acu.Accordingly, this preliminary evaluation has been upgraded to an engineering analysis (ea19-001), to expand the scope of the investigation to include the tier-one supplier and any manufacturers who installed the subject acu in production vehicles.   • Scope and timeliness investigation # RQ17004

    On september 10, 2015, hyundai motor america (hyundai) filed a defect information report (dir) recalling 470,000 my 2011 and 2012 sonata vehicles with “"theta ii" engines (recall no. 15v-568). Hyundais dir described the defect as an issue involving manufacturing debris. Hyundai limited the population of recalled vehicles to those produced on or prior to april 12, 2012, stating that a process change in april 2012 resolved the issue of manufacturing debris. On march 31, 2017, hyundai expanded its original recall to include 572,000 my 2013-2014 sonata and santa fe sport vehicles with "theta ii" engines (recall no. 17v-226). The dir continues to describe the defect as an issue involving manufacturing debris.This rq is being opened to investigate both the timeliness and scope of hyundais "theta ii" engine recalls, and hyundais compliance with reporting requirements.

  • Steering shaft separation investigation # PE10035

    The office of defects investigation (odi) is aware of 7 complaints and field reports and 10 warranty claims on the subject vehicles that allege the intermediate steering shaft became loose or in some cases separated from the upper steering column and/or the steering gearbox, causing (or potentially causing) the hand wheel to become mechanically de-coupled from the road wheels resulting in a complete loss of steering capability.The reported causes include excessive free play or separation of the intermediate steering shaft upper and/or lower universal joint cross shaft/yoke; missing, loose or damaged universal joint pinch bolts; improperly assembled intermediate steering shafts with misaligned splines or insufficient spline engagement preventing the universal joint pinch bolt from being secured in the retaining groove when fastened.On september 20, 2010, the manufacturer notified the agency that it would conduct a safety recall (nhtsa recall no. 10v-426) to address the alleged defect in approximately 139,500 subject vehicles produced from december 11, 2009 through september 10, 2010.In addition to inspecting the steering shaft, the manufacturer will also update the electric power-assist steering control software to address complaints that the steering wheel vibrates or shakes as a result of a a motor driven power steering malfunction.The manufacturer began notifying affected owners by written letter on october 14, 2010.To date, 119,383 subject vehicles have been serviced under the recall campaign and 58 subject vehicles have received replacement parts as a result.Accordingly, this investigation is closed.

Headlights have burnt out 4 times in 1 year w/install, break/tail lights have burnt out 6 times in 1 year w/ install. Cruise control isn't staying set and airbag light is on unknown reason after obd scan. Break lights have burnt thru the connection piece, causing damage, overpower from the wiring harness has caused over load of power which caused lights to burn out and even burst. Multiple drivers with similar problems, airbag may still deploy for no reason or not deploy at all. Cruise control malfunction possibly due to airbag issues, but may also be from faulty wiring throughout the vehicle and ultimately into the sterring console. Computer or motherboard may also now have damages cause by over amped volts of electricity without a serge protecter. Possibility of electrical damages, caused by overload from power supply and directed throught the vehicle may have cause exponential damages to electrical in the sonata. Causing a serious safety concern and hazard. Exterior lighting requires new wiring harnesses and connection pieces as well as new casing from over amp overload, resulting in overheating and bulb combustion damages. New airbag assembly is probable and reset of mother board with upgraded features and completely new wiring with serge protection is necessary.

Engine replaced under recall. “new” engine has low pressure after install. Car does not accelerate as needed. Check engine light and oil light come on. Took back to dealership several times, told an engine flush would fix problem, problem not fixed, told a tune up would fix problem, problem not fixed. Engine still has lower pressure. Dealership did testing requested by hyundai corporation. Engine light still illuminated, engine still has low pressure, car still does not accelerate as needed. Have talked to several other people with hyundai’s that have had engine replaced under recall, having same problems.
