  • Recall 150631 [USA]: In the event of a crash, air bags and/or seat belt pretensioners that do not deploy as intended may increase the risk of injury.
    Toyota motor engineering & manufacturing (toyota) is recalling certain 2011-2019 corolla, 2011-2013 matrix, 2012-2018 avalon, and 2013-2018 avalon hybrid vehicles. During certain crashes, the air bag electronic control unit (ecu) may malfunction, possibly disabling the deployment of the air bags and/or seat belt pretensioners.
    toyota will notify owners, and dealers will inspect the ecu and install a noise filter between the air bag control module and its wire harness, as necessary, free of charge. Owners were notified of the safety risk beginning march 2, 2020. A second letter notifying owners of the remedy repair will be mailed between march 16, 2020 and june 27, 2020.   • Recall 109419 [USA]: A vehicle that unexpectedly brakes increases the risk of a crash.
    Toyota motor engineering & manufacturing (toyota) is recalling certain model year 2013-2015 avalon and avalon hybrid vehicles manufactured october 19, 2012 to october 27, 2015 and 2013-2015 lexus es350 and es300h vehicles manufactured june 18, 2012 to july 6, 2015. The affected vehicles are equipped with a pre-collision system (pcs) as optional equipment. In these vehicles, the pcs may unexpectedly activate and apply the brakes when the radar detects a steel joint or plate in the roadway as an object.
    toyota will notify owners, and dealers will replace certain pcs components, free of charge. The recall began on july 28, 2016. Owners may contact toyota at 1-888-270-9371. Toyota's numbers for this campaign are f1v and f2e.

  • Recall 2021723 [Canada]: Accessories
    Issue: this recall is for certain block heater kits installed on certain vehicles or sold as accessories at toyota or lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt. Corrective actions: toyota recommends that you should stop using the engine block heater immediately. Toyota will notify owners by mail and instruct you to take your vehicle to a dealer to disable the engine block heater. For some vehicles, the corrective actions for this recall are under development. For others, a new engine block heater cannot be installed, and the owner will receive a refund of the purchase price of the engine block heater.

  • Recall 2020013 [Canada]: Airbag
    Issue: on certain vehicles, the circuits within the airbag control module could be damaged in a crash. This could cause the airbags and seat belt pretensioners to not deploy in certain crashes. Safety risk: airbags or seat belts that do not work properly in a crash could create an increased risk of injury. Corrective actions: toyota will notify owners by mail and instruct you to take your vehicle to a dealer to install an electrical noise filter to prevent damage to the airbag control module.

  • Recall 2015521 [Canada]: Brakes
    On certain vehicles equipped with pre-collision system (pcs), the system could unexpectedly activate while operating the vehicle. Under certain situations, the system may interpret a steel joint or plate in the roadway as an obstacle, and the system may apply the service brake automatically. If the pcs applies unexpected braking force during normal vehicle operation when no impending collision object is present, it could increase the risk of a crash causing injury and/or damage to property. Correction: dealers will disable the pre-collision system (pcs) as an interim measure, and when parts become available, will replace the pcs components with improved parts.

    The office of defects investigation (odi) has received a petition dated september 19, 2023 from the owner of a 2012 toyota prius v requesting that odi reevaluate its decision to deny dp19-004. In the letter, the petitioner reported experiencing an unexpected and complete loss of brake power while driving, accompanied by multiple dashboard warning lights. This petition is based on the petitioners direct experience and assessment that odi closed dp19-004 because its prompting petition was withdrawn by that petitioner. Odi will review the work conducted during dp19-004 and a review of any additional related complaints and crashes via a new defect petition the petition and related materials can be reviewed at nhtsa.Gov under the following odi number:11553736.

  • Pedestrian alert sounds investigation as of 20230127

    Nhtsa received a petition on or about july 18, 2022, requesting that federal motor vehicle safety standard (fmvss) 141 be applied to all electric and hybrid vehicles operating in the united states. The petition can be reviewed at nhtsa.Gov under odi number 11486072.Fmvss 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 kg or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after march 1, 2021.The petitioner asserts that hybrid and electric vehicles to which the standard does not apply should be found to contain a safety defect. In support of the petition, the petitioner includes findings contained in a bill introduced in the house of representatives in 2009, the pedestrian safety enhancement act of 2009, h.R. 734, 111th cong. (2009). A related bill, the pedestrian safety enhancement act of 2010, became law in january 2011 (pub. L. 111-373).     On september 19, 2019, the office of defects investigation (odi) received a defect petition from mr. Roger hogan requesting that the agency investigate and recall certain toyota prius, prius phv, camry hybrid and avalon hybrid vehicles for a brake actuator valve wear condition covered under toyota customer support programs zjb and zkk (subject csp’s). In support of his request, the petitioner identified 117 nhtsa complaints allegedly related to the subject csp condition, including 60 reporting crashes. On september 30, 2019, odi opened defect petition dp19-004 to evaluate the petitioner’s request for an investigation. In august 2020, the petitioner notified the agency that he was withdrawing his petition. Because the petitioner has withdrawn the petition, we are denying the petition as moot and no further analysis of this petition is necessary.Nhtsa is authorized to issue an order requiring notification and remedy of a defect if the agency’s investigation shows a defect in the design, construction, or performance of a motor vehicle that presents an unreasonable risk to safety. 49 u.S.C. §§ 30102(a)(9), 30118. Because the petitioner has with withdrawn the petition, the petition is denied as moot. This action does not constitute a finding by nhtsa that a safety-related defect does not exist.   • Air bag acu electrical overstress investigation as of 20190419

    The office of defects investigation (odi) is expanding the investigation to include the equipment supplier and vehicle manufacturers (oems) using this unit.The investigation focuses on acus manufactured by trw, now zf-trw (zf) , the tier-one supplier to hyundai and kia and the other affected oems.The acu senses a vehicle crash to determine whether air bag deployment is required, and if so, deploys the appropriate air bags and other supplemental restraints.Zf supplied subject acus to six oems: fca, honda, hyundai, kia, mitsubishi and toyota.Internal to the acu is an electronic component (an application specific integrated circuit, or asic) that monitors signals from crash sensors.A failure of the asic may prevent deployment of the required air bags and devices, or may otherwise affect the proper operation of the acu.The acu is located in the passenger compartment, and electrical wiring connects the asic to sensors located at the front of the vehicle.Odi’s current understanding is that a crash event may, in and of itself, produce harmful signals on the sensor wiring capable of damaging the asic, although the probability of this occurring appears to be low.While the acu incorporates electrical circuitry intended to protect the asic from harmful signals, the level and effectiveness of the protective circuitry varies by oem customer.During pe18-003, hyundai and kia filed recalls (18v-137 and 18v-363 respectively) to address a defect that could result in acu disablement and non-deployments.According to the filings, the disablement occurs in certain types of frontal crash events.Both filings discussed a condition known as electrical overstress (eos) that affected the subject asic and was likely caused by electrical signals that entered the acu via sensor wiring.The recalled vehicles used acus that had the lowest levels of asic protection while non-recalled hyundai and kia products using subject acus had higher levels of protection.Odi has not identified any eos failures in the non-recalled kia and hyundai populations.In september 2016, fca filed recall 16v-668 for certain model year (my) 2010 to 2014 chrysler, dodge and jeep products also manufactured with the subject acu.In that filing, fca also discussed an eos condition that resulted in a failure of the subject asic, which caused air bag non-deployment. Fca noted that the defect condition had only been observed in vehicles equipped with sensor harnessing routed across the front of the vehicle.Other fca vehicles that also used the subject acu, but not the cross-car harnessing, had not experienced eos failures, despite similar time in service. The recalled fca vehicles used a mid-level form of asic protection.Other fca vehicles that did not use cross car wiring, or used higher levels of asic protection, have not been recalled.Odi has not identified any eos failures in the non-recalled fca population.Recently, odi has identified two substantial frontal crash events (one fatal) involving toyota products where eos is suspected as the likely cause of the non-deployments.The crashes involved a my 2018 and a my 2019 corolla equipped with the subject acu that incorporated higher levels of asic protection.Additionally, both acus were found to be non-communicative (meaning the acu could not be read with an event data recorder) after the crash, a condition found in other cases where eos occurred with other oems.No other eos events have been identified for other toyota products (including corolla models that used the subject acu since my 2011), or for the honda and mitsubishi vehicles that use the subject acu.Odi plans to evaluate the susceptibility of the subject acu designs to electrical signals, as well as other vehicle factors that can either lead to, or reduce the likelihood of, an eos event.Additionally, odi will evaluate whether an unreasonable risk exists that requires further field action.

On may 25th, 2020 i was pulling into a taco bell drive through. It was located up a little hill. As i drove up the hill i hit a little curbing. I backed up and adjusted the car to miss the curb. When i put the car into drive it lurched forward at a high speed. Seeing i was going to hit the car in front, i hit my brakes. The car did not stop until i had hit the car in front with a lot of force. That car was thrown into the truck in front of it. That was the beginning of a huge scare! I put the car in reverse so i could observe the damage to my car the car in front of me. When i did so the car accelerate at full speed in reverse going back down the hill and headed straight for a drugstore. The car was making a whiring sound, like when you rev up the engine. I pushed the brakes to the floor but the car would not stop but keep going faster! I pumped the brakes some more but it would not stop. When i saw i was headed into the store, i turned the wheels to avoid it and tried braking again without stopping. I finally hit the stop engine button but not before the car hit a parked truck in the store lot which stopped it. It was the most frightening experience i have ever had. She could hear the car engine racing and tried to put the car in park, but it went into reverse and rapidly moved backwards, even with her foot on the brake, hit the car behind her. She than once again tried to up the car in park, but put it back in drive and the our car accelerated forward again fill speed hitting the car ahead for a second time, all the time holding her foot down as hard as she could on the brake. After hitting the car ahead for the second time, she pushed the engine stop button and the engine stopped running at full rpm. She noted that there the front tires were smoking alot since the tires were rotaing even though the car was at a stop, due to the engine racing full rpm. Police were called and they dispatched a tow truck for our car. The police never arrived, the tow truck loaded our car and towed it to the aaa storage yard .

The avalon hybrids covered only by warranty enhancement zkk are suffering the same dangers and fvmss noncompliance due to brake booster pump assembly failures as those toyota covered by safety recalls (d0h & k0l). By excluding avalon hybrids with defective brakes from the d0h and k0l safety recalls, toyota is leaving the owners to helplessly experience life-threatening brake failure or malfunction before toyota will provide the replacement brake components needed to make the vehicle safe. The law requires toyota to prevent the life-threatening dangers of this brake defect before the failure occurs. Toyota's cost-cutting strategies are causing crashes and injuries. Injuries and crashes caused by the brake defect explained in this letter have occurred and been reported to the national highway traffic safety administration. Toyota's decision to provide a reactionary, post-failure repair - instead of a preventative safety recall remedy - explains many of the crashes and injuries on nhtsa's safercar.Gov website. Relevant complaints from safercar.Gov include 10515182 and 11093611. Diagnostic trouble codes C1256, C1253, C1252, and C1391 - indicating dangers and noncompliance - are being stored in vehicles excluded from the past safety recalls. About an year ago i got a recall for but when i brought my car to the toyota of noho they told me that they didn't find any problem, so nothing has been replaced and i still have both problems which hard to find out by driving couples of miles, how a technician explained to me. Pcs recall. The dealer disconnected this safety system the beginning of january and with numerous inquiries i have received no fix date. This is a new car and one of my reasons for buying was the pcs safety system and they can't fix it. I asked to be given a car with this system to drive until they come up with a fix but toyota refuses to help me in any way. I payed a lot of money for a car with this safety system but had it take away with no fix time. Two months wait is a reasonable wait time and something should be done. The 2016 models are being sold with this system working but my model doesn't. They will not take a system that is working on the 2015 and install on my car. Please help me. In december i received a recall for the pcs system that was dated 11/03/2015 telling the pcs had flaws and i had to turn off the system. I was not happy because that was the main reason i bought the car because it made it safe. On january 14,2016 i took the avalon hybrid to auto nation toyota in pinellas park florida for the 5000 mile checkup. I asked about the pcs recall and they had no information on the recall and no timeline for a fix. I was not happy with that response because one of the features that made me feel safe driving was eliminated with no fix in sight. I also receive an email from a toyota customer service representative stating that there was no timeline for a repair or parts replacement. A rental vehicle was also offered but it would also not be safe as a vehicle with a pcs system. I want toyota to fix my car or replace it with a 2016 toyota avalon hybrid with pcs. The 2016 toyota avalon hybrid does not have a problem with the pcs system. I am fully disabled and don't feel comfort driving my vehicle anymore. The problem is with the dynamic radar cruise control (drcc). It applies the brakes for no apparent reason. Below are descriptions of 5 of about 20 such incidents: -on i-40. I was behind a truck moving 62 in a 70 zone. There was a truck in the left lane about 200 yards ahead. I moved to the left lane. There was no one behind me in either lane. Drcc quickly slowed me to 40 mph. -again on i-40, in right lane behind a truck going about 62 in a 70 zone. The truck 20 yards behind me moved left and accelerated. Brakes automatically applied by drcc. -on i-8, right lane following a vehicle 10 mph below the speed limit. A car was closely following me. Upon reaching an exit ramp, he exited and quickly accelerated. The brakes were strongly applied. -entering freeway; light traffic, drcc set at 75. The entrance ramp came in from above the freeway and approached the freeway at a fairly sharp angle. There was a metal guard rail to the left of the left lane. Entering travel lane, still aimed at the guard rail, brakes sharply applied. -construction zone on i-80, . Speed limit 55 in the construction zone and 70 beyond it. Fairly heavy traffic was narrowed to one lane in the zone with orange barrels on the right and jersey barriers on the left. The nearest vehicle ahead of me was 100-200 yards ahead, but behind was a semi about 20 feet away. Changed set speed from 60 to 70 approaching end of construction.. I was probably 20-30 yards from the end of the construction when i adjusted the speed.. Just as i reached the end of the zone and was beginning to accelerate, the brakes were strongly applied. The semi veered to miss me. My guess is that if the drcc is confused by the situation it is programmed to apply the brakes. A far more appropriate, and safer, response would be to emit a warning beep(s) and disable the cruise control.

I have a safety concern. I bought a new car a couple of weeks before this incident; this is what i experienced with the pcs system. It went off with warning buzzer, retracting the seat belts. To my surprise, the frontal car was more than 120 to 150 feet away!; i believe i was driving at about 50-55 mph. Since this is my first experience with the pcs system, i am not sure if this was normal or whether i should bring the car in for inspection. I took the car to the dealer; they gave me a toyota case no. 152660366 ; mechanic said he performed health check on the vehicle and passed. Rear window visibility is seriously hampered by interior reflection on inside of rear window. This vehicle has a light grey interior color and the back package tray and speaker covers are also light grey. This light grey color is reflected on the inside of the rear window glass. When looking out the rear window using the inside rear view mirror, the rear window looks as if it is extremely dirty or fogged. The condition is worse when subjected to sunlight. At present, while awaiting if/when a repair/modification will to be made by the selling dealer, i have laid a black towel on the package shelf as a temporary repair. The black towel has virtually eliminated the reflection and provides clear rearward visibility. Toyota is aware of this vehicle's condition and the dealer has forwarded pictures to toyota showing the extent of the visibility impairment. Unfortunately, any other avalon with a light color interior will have the same defect.
