  • Desiccated air bag inflator rupture investigation # EA21002

    From 2000 through 2017, takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("psan") propellant -- propellant 2004 and propellant 2004l. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 blomquist report at www.Nhtsa.Gov/sites/nhtsa.Gov/files/documents/expert_report-hrblomquist.Pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in us vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004l that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated psan inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All takata inflators produced with propellant 2004l contain desiccant, and none of these desiccated inflators using propellant 2004l are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated psan inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- takata (now known as tk global), the independent testing coalition, and exponent -- have been studying the long-term behavior of takata desiccated psan inflators using propellant 2004l (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004l is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004l.The office of defects investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated psan frontal inflators manufactured by takata. This investigation will require extensive information on takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated psan inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.Gov/recalls?Nhtsaid=ea21002 -- note this information is subject to change/revision as the investigation proceeds). The contact stated that while her son was driving and had approached a stop sign, the brake pedal failed upon depression and collided with another vehicle. The contact was uncertain if any warning lights illuminated prior to the failure. The airbags deployed upon impact. The contact nor any other individuals involved in the accident were injured. A police report was filed. Due to the failure, the vehicle was towed to an independent mechanic where it had been deemed a total loss. In 2019 i had the same issue as prior report. Coolant made it's way into the engine and my engine had to be replaced. Actually occurred within warranty so ford replaced it. On then new engine there is no warranty once i reach the original warranty in mileage / time. Now in 2021 i am replacing a transmission that is not covered under warranty. There is a recall for transmission on 2017 and 2018, mine is a 2017, and my vin does not indicate it is subject to that recall (recall no. 19v285000). Cost is $4,600.

The engine of my 2017 ford edge began to misfire intermittently in 2019. When it was finally able to be diagnosed in 2020, it was determined fuel had escaped around a spark plug. The spark plugs were replaced. Approximately one year later, the issue began again. Upon further inspection, it was found that a head gasket had gone bad. While there is not definite evidence of a coolant leak, it is suspected a leak may have initiated the issue. Research has also indicated the engine block is most likely cracked. While driving, the engine would often lull and stop accelerating. This happened several times as i attempted to drive down the interstate, being forced to exit the highway due to not being able to move with the speed of traffic. After the first repair of the spark plugs, the car seemed to drive fine, however, they shouldn't have gone bad within a year of install indicating a much larger issue. The car is unable to be driven. Even with replacing the head gasket, i do not feel the car would be safe with a cracked engine block. Nor should the motor have gone bad this soon.

I was backing up into a driveway (very slight incline) when my power steering went out. Lights came on stating, "steering assist fault, service required" and "service advanced trac." i could barely turn my wheel. There was no earlier warning of this issue and i had just driven 3 hours on the highway so was fortunate that it didn't fail while on the road. I tried restarting my car a few times and then i called a ford dealer. The car was towed in for a diagnostic. This revealed that the epas rack failed and needed to be replaced with an estimate of over $3000. The purpose of epas is to enhance the steering ability for the driver, but the car is not steerable without its proper functioning which is a huge safety hazard to the driver and anyone else near them on the road.

Engine defect - coolant leaking into cyclinder- requires whole new engine

Coolant leak into cylinder. Manufacture defect. Known issue by ford but will not cover any replacement as vehicle is out of warranty by 7000 miles. A 2017 with 67000 driven by a grandma 5 mph under the speed limit should not need a new engine. Engine light comes on. Misfires. Puffs of white smoke. Didn’t take to dealer as closest if 1/2 hour away. Took to local repair shop. They ran codes and said it is a known problem from ford. Nhtsa id number: 10162071 manufacturer communication number: tsb 19-2208 summary some 2015-2018 edge and 2017-2019 fusion/mkz/escape/mkc vehicles equipped with a 2.0l ecoboost engine may exhibit a low coolant level, white exhaust smoke and/or a runs rough condition with or without an illuminated malfunction indicator lamp (mil). Diagnostic trouble codes (dtcs) may include P0300, P0301-P0304, P0316, P0217, P1285 and/or P1299 stored in powertrain control module (pcm). This may be due to coolant intrusion into the cylinder. To correct the condition, follow the service procedure steps to replace the long block engine assembly. This needs to be a recall or at least ford needs to cover the replacement costs for those with issues.
