  The office of defects investigation (odi) has received a petition dated september 19, 2023 from the owner of a 2012 toyota prius v requesting that odi reevaluate its decision to deny dp19-004. In the letter, the petitioner reported experiencing an unexpected and complete loss of brake power while driving, accompanied by multiple dashboard warning lights. This petition is based on the petitioners direct experience and assessment that odi closed dp19-004 because its prompting petition was withdrawn by that petitioner.   Nhtsa received a petition on or about july 18, 2022, requesting that federal motor vehicle safety standard (fmvss) 141 be applied to all electric and hybrid vehicles operating in the united states. The petition can be reviewed at nhtsa.Gov under odi number 11486072. Fmvss 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 kg or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after march 1, 2021.On january 27, 2023, nhtsa opened defect petition (dp) 22-005 to evaluate the subject matter described in the petition. On june 24, 2023 and as supplemented on june 25, 2023, the petitioner notified nhtsa he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition.   On september 19, 2019, the office of defects investigation (odi) received a defect petition from mr. Roger hogan requesting that the agency investigate and recall certain toyota prius, prius phv, camry hybrid and avalon hybrid vehicles for a brake actuator valve wear condition covered under toyota customer support programs zjb and zkk (subject csp?S). In support of his request, the petitioner identified 117 nhtsa complaints allegedly related to the subject csp condition, including 60 reporting crashes. On september 30, 2019, odi opened defect petition dp19-004 to evaluate the petitioner?S request for an investigation. In august 2020, the petitioner notified the agency that he was withdrawing his petition. Because the petitioner has withdrawn the petition, we are denying the petition as moot and no further analysis of this petition is necessary.Nhtsa is authorized to issue an order requiring notification and remedy of a defect if the agency?S investigation shows a defect in the design, construction, or performance of a motor vehicle that presents an unreasonable risk to safety. 49 u.S.C. ?? 30102(a)(9), 30118. Because the petitioner has with withdrawn the petition, the petition is denied as moot. This action does not constitute a finding by nhtsa that a safety-related defect does not exist. Actuator and brake cylinder have gone. Abs, traction, and brake lights all have gone on. This is a very common issue with this car, and i am expected to pay $3,000 for this fix.

While driving on an uncrowded 2 lane rural highway at approx. 4pm on june 20th, 2021, i heard what sounded like a gunshot in my ear. I quickly looked around at my interior & saw no broken windows. I had no driving issues, but pulled over safely to inspect the exterior of my vehicle. As soon as i opened my driver's door & exited, i turned to close the door & saw that my sunroof had basically exploded. It was jagged & missing pieces around 3 of 4 edges. I was an hour from home & wanted to make sure i arrived safely. En route i heard chunks falling off & by the time i got home the middle portion had rippled & gaping holes were visible.

While driving in the highway had several times hard situations. Yesterday and truck became in front in my lane while, i had brake fast the car started loosing control and ended up on the side of the road. Is like the brakes makes the car loose all the control in a bad situation instead of help to prevent accidents. Each time i go to toyota i had mentioned this issue, but they say there's not recalls available. Sometimes i think i will die because of this.

My problem began when i pressed my brakes and instead it accelerated. I thought maybe my foot slipped and didn’t know what it meant. It happened again a few weeks later and then all of the dash lights went on that indicated that i had to have the big brake recall repair. Being that my car is past the 10 year warranty and past the extension for mileage, i had to pay out of pocket. This repair was done and upon receiving my car back, the gas mileage instantly began to drop. It went from averaging 42-48mpg in my town to 25-30mpg. I took it in the be looked at and they ensured me nothing was wrong with the car at all. The battery wasn’t holding charge on top of this and it wasn’t reading the same on the dash about the 0-100 current gas mileage you’re getting as you go (the lines scale thing) that goes up when you let off the gas, and i could feel a strange shift while i was driving so i took it in again. They told me this time it’d be $110 to diagnose even though it was the same issue that started after the brake system repair, so i left extremely upset. A few days later my cars steering went out while i was driving and the whole hybrid system died. The car now only works in power mode. The consumer stated the car was taken in for a brake recall and had to pay $2800 in repairs. The consumer stated what they thought was a battery failure turned out to be the beginning of an electrical motor failure related to overheating.

I have a 2010 toyota prius. I was driving home on i-80 and when i was getting down on the slope of the sacramento river bridge, three lights suddenly popped on the dashboard - abs light, brake light and traction control light. Following this, i lost efficient control on the brakes and i had to push really hard on the brake pad to even slow the car down. If i had to stop suddenly, it would have definitely resulted in a serious crash. Because the brakes stopped working as they are supposed to, i turned on the blinkers, took the exit and tried to stop slowly. I was able to stop on the side of the road just before the red signal. I was just couple of miles from my home so i kept the blinkers on, and slowly drove through internal roads to my home. Later when i found out the code, it came as C1391 which is usually for failure of the brake booster assembly or the brake booster pump assembly or both. When i contacted my local toyota dealership, they mentioned that the extended warranty for replacing these parts ended in november 2020 and because of that they asked me to take care of this >$3000 repair. I also contacted toyota corporate to file a complaint and was told the same thing. This issue is a serious safety concern and toyota should issue a recall and not an extended warranty. They should not be waiting for people to loose their lives before they issue a recall.

The contact owns a 2010 toyota prius. The contact stated that while driving approximately 30 mph, the automatic braking system failed and the brakes were making a strange sound and not responding as soon as the brake pedal was depressed. The abs, brake, and stability control warning lights were illuminated. The vehicle was taken to the dealer and diagnosed with hydraulic brake booster pump failure. The vehicle was not repaired and remained at the dealer. The manufacturer was made aware of the failure but informed the contact that the vehicle was over the extended warranty coverage. The failure mileage was 118,000.
