  • Hyundai and kia abs module fires investigation # AQ23002

    The office of defects investigation (odi) has received eight hyundai motor america (hyundai) and eight kia america, inc. (kia) part 573 recall reports between 2016 and 2023. The combined 16 safety recalls all involve the antilock braking system (abs) modules or hydraulic electronic control units (hecu) manufactured by the equipment supplier mando. The part 573 recall reports contain varying defect descriptions and differing remedy descriptions for the same or similar equipment supplied by mando. Safety recalls included in this audit query (aq) review include: hyundai recalls 20v-520, 20v-543, 21v-161, 21v-160, 21v-303, 22v-056, 22v-810, and 23v-651. Kia recalls 16v-815, 20v-518, 20v-519, 21v-137, 21v-331, 22v-051, 22v-800, and 23v-652 the purpose of this aq is to evaluate the timeliness and scoping of hyundai and kia’s defect decision making and adherence with reporting requirements; and understand the varying defect descriptions and remedies between these recalls.

  • Pedestrian alert sounds investigation # DP22005

    Nhtsa received a petition on or about july 18, 2022, requesting that federal motor vehicle safety standard (fmvss) 141 be applied to all electric and hybrid vehicles operating in the united states. The petition can be reviewed at nhtsa.Gov under odi number 11486072.Fmvss 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 kg or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after march 1, 2021.The petitioner asserts that hybrid and electric vehicles to which the standard does not apply should be found to contain a safety defect. In support of the petition, the petitioner includes findings contained in a bill introduced in the house of representatives in 2009, the pedestrian safety enhancement act of 2009, h.R. 734, 111th cong. (2009). A related bill, the pedestrian safety enhancement act of 2010, became law in january 2011 (pub. L. 111-373). The petitioner requests that nhtsa require all electric and hybrid vehicles comply with fmvss 141, regardless of production date.The defect petition has been opened to evaluate the issue and determine whether to grant or deny the petition.The associated product list is not exhaustive and may contain vehicles that are equipped with pedestrian alert sounds.

  • Loss of motive power investigation # DP22003

    The office of defects investigation (odi) has received a petition on july 13, 2022, requesting a defect investigation into an alleged defect of the "oil drain pan assemblies" on the following 83 models/model year vehicles stated by the petitioner: 2006-2019 hyundai sonata, 2013-2018 hyundai santa fe sport, 2010-2012 and 2019-2020 hyundai santa fe, 2010-2015 and 2018-2019 hyundai tucson, 2011-2021 kia sportage 2005-2020 kia optima, 2011-2020 kia sorento, 2010-2013 kia forte, 2010-2013,kia forte coup, and 2007-2010 kia rondo vehicles equipped with the 1.6l t-gdi gamma, 2.4l theta ii gdi, 2.4l in-line gdi theta ii, kappa, and 3.8l v-shaped gdi lambda-ii engines. The petition also cites additional hyundai/kia vehicles that use similar drain pan assemblies as the above-mentioned vehicles as having drain pan assembly failures. The petitioner provided subject and peer vehicles information related to alleged engine failures. The petition itself can be reviewed at nhtsa.Gov under odi number 11481745.According to the petition, the hyundai/kia vehicle owners have made allegations of oil leakage failures occurring at highway speeds, without malfunction, indicator lights, or the leakage associated with under-tightening an oil drain plug, or a plug or pan with damaged thread. The alleged defects are causing engines to seize and loss of motive power. A defect petition has been opened to evaluate the issue and determine whether to grant or deny the petition. The petitioner identified a number of complaints received by nhtsa alleging complete engine failure from a variety of models stated above along with other models. The odi complaints cited in the petition can be reviewed at nhtsa.Gov under the following odi numbers: 11481745, 11433038, 11366918, 11431200, 11320689, 11245086, 11196311, 11114946, 11114946, 11097819.

  • Engine fires investigation # EA21003

    In 2017, the office of defects investigation (odi) opened two recall queries (rq17-003 and 004) following multiple recall actions initiated by hyundai and kia for “theta ii” engine-equipped vehicles. The rqs were opened to investigate both the timeliness and scope of the recalls as well as the compliance of each company to meet their reporting requirements established under the motor vehicle safety act. Numerous violations of the requirements were found for both companies, and in on november 20, 2020, both companies entered into consent orders with nhtsa to resolve nhtsa’s assertions of violations of law. Both consent orders remain in effect.In 2018, the center for automotive safety (cas) petitioned the agency under defect petition (dp) 18-003 to investigate non-crash fires on various hyundai and kia models, citing 120 voqs alleging vehicle fires at varying points of origin and occurring under various non-crash related circumstances (including key-off fires). In opening dp18-003, odi noted that many of the 120 fire incidents appeared to originate in the engine compartment area and made reference to the two open rq investigations as likely pertinent. Odi issued information request (ir) letters to hyundai and kia and several peer oems in its efforts to properly evaluate and disposition cas’ petition.After reviewing responses to the dp requests, odi granted cas’ petition and in 2019 opened two preliminary evaluations (pe19-003 and 19-004, one for each company) to broadly investigate incidents of non-crash fires (regardless of origin or operational status of the vehicle) on various model year hyundai (sonata and santa fe) and kia (optima, sorento and soul) models. Odi issued additional, more broadly scoped ir letters to both hyundai and kia, which upon review, identified certain vehicle populations exhibiting higher rates of non-crash fires for various defects and points of origin, including some due to engine failure.To date, hyundai and kia have issued several recallsto address vehicle fires, including those due to engine failure involving various engine types (theta ii gdi, theta ii mpi, theta ii mpi hev, nu gdi, and gamma gdi). The remedies for the engine failure recalls typically involve engine inspection (to detect existing damage), and if necessary, engine replacement. Additionally, both manufacturers are installing an engine control software modification known as knock sensor detection software (ksds) which is intended to detect impending engine failure, alert the driver, and limit engine power (to lessen likelihood of engine failure). Ksds is being installed under recalls and non-safety field actions, and both companies have extended limited engine warranties (see recall files). They have even replaced engines if the ksds software detects an impending engine failure.The 78 voqs cited above, which all involve fires likely due to engine failure, consist of 36 remedy-related and 42 scope-related incidents. The injury allegations involve eye and burn injuries not requiring medical treatment. With the 83 other voqs, odi is aware of 161 fires potentially due to engine failures. The downloadable version of the resume available at nhtsa.Gov contains additional information on the recall actions and id numbers for the voqs.The office of defects investigation (odi) is opening this engineering analysis for two primary purposes. First, the investigation will continue to evaluate the scope of recalls initially begun under prior odi actions (see below), and will also include other recalls for fires due to engine failures, which hyundai and kia have subsequently launched (many influenced by odi investigations). Second, the investigation will monitor and evaluate the efficacy of the recall remedies for engine fire related recalls, as well as the long-term viability of related programs and non-safety field actions being conducted by hyundai and kia.

  • Throttle control system vulnerabilities investigation # DP21003

    The office of defects investigation of the national highway traffic safety administration received a petition letter dated october 10, 2021 requesting an investigation of an alleged defect(s) in the electronic throttle control system in certain model year (my) 2005-2016 kia optima/k5, my 2006-2015 kia sorento, my 2007-2016 hyundai elantra, my 2007-2016 hyundai santa fe, my 2006-2015 hyundai sonata vehicles. The petition was filed as a vehicle owner questionnaire that can be reviewed at nhtsa.Gov under reference (odi) number 11439675. The petitioners allege the subject vehicles exhibit a range of dangerous vehicle speed control problems including sudden uncommanded acceleration, runaway throttle conditions, surging, stalling and loss of motive power, all due to various defects and vulnerabilities in the electronic throttle control system.This defect petition has been opened to evaluate the allegation and determine whether to grant or deny the petitioners request.

  • Air bag acu electrical overstress investigation # EA19001

    The office of defects investigation (odi) is expanding the investigation to include the equipment supplier and vehicle manufacturers (oems) using this unit.The investigation focuses on acus manufactured by trw, now zf-trw (zf) , the tier-one supplier to hyundai and kia and the other affected oems.The acu senses a vehicle crash to determine whether air bag deployment is required, and if so, deploys the appropriate air bags and other supplemental restraints.Zf supplied subject acus to six oems: fca, honda, hyundai, kia, mitsubishi and toyota.Internal to the acu is an electronic component (an application specific integrated circuit, or asic) that monitors signals from crash sensors.A failure of the asic may prevent deployment of the required air bags and devices, or may otherwise affect the proper operation of the acu.The acu is located in the passenger compartment, and electrical wiring connects the asic to sensors located at the front of the vehicle.Odi’s current understanding is that a crash event may, in and of itself, produce harmful signals on the sensor wiring capable of damaging the asic, although the probability of this occurring appears to be low.While the acu incorporates electrical circuitry intended to protect the asic from harmful signals, the level and effectiveness of the protective circuitry varies by oem customer.During pe18-003, hyundai and kia filed recalls (18v-137 and 18v-363 respectively) to address a defect that could result in acu disablement and non-deployments.According to the filings, the disablement occurs in certain types of frontal crash events.Both filings discussed a condition known as electrical overstress (eos) that affected the subject asic and was likely caused by electrical signals that entered the acu via sensor wiring.The recalled vehicles used acus that had the lowest levels of asic protection while non-recalled hyundai and kia products using subject acus had higher levels of protection.Odi has not identified any eos failures in the non-recalled kia and hyundai populations.In september 2016, fca filed recall 16v-668 for certain model year (my) 2010 to 2014 chrysler, dodge and jeep products also manufactured with the subject acu.In that filing, fca also discussed an eos condition that resulted in a failure of the subject asic, which caused air bag non-deployment. Fca noted that the defect condition had only been observed in vehicles equipped with sensor harnessing routed across the front of the vehicle.Other fca vehicles that also used the subject acu, but not the cross-car harnessing, had not experienced eos failures, despite similar time in service. The recalled fca vehicles used a mid-level form of asic protection.Other fca vehicles that did not use cross car wiring, or used higher levels of asic protection, have not been recalled.Odi has not identified any eos failures in the non-recalled fca population.Recently, odi has identified two substantial frontal crash events (one fatal) involving toyota products where eos is suspected as the likely cause of the non-deployments.The crashes involved a my 2018 and a my 2019 corolla equipped with the subject acu that incorporated higher levels of asic protection.Additionally, both acus were found to be non-communicative (meaning the acu could not be read with an event data recorder) after the crash, a condition found in other cases where eos occurred with other oems.No other eos events have been identified for other toyota products (including corolla models that used the subject acu since my 2011), or for the honda and mitsubishi vehicles that use the subject acu.Odi plans to evaluate the susceptibility of the subject acu designs to electrical signals, as well as other vehicle factors that can either lead to, or reduce the likelihood of, an eos event.Additionally, odi will evaluate whether an unreasonable risk exists that requires further field action.

  • Non-crash vehicle fires investigation # PE19004

    In a letter dated june 11, 2018, the center for auto safety (cas) petitioned the national highway traffic safety administration (nhtsa) to initiate a safety defect investigation into non-collision related fires on model year (my) 2011-2014 kia optima and sorento, and hyundai sonata and santa fe vehicles.In a letter dated july 24, 2018, cas requested that my 2010-2015 kia soul vehicles be added to the petition.Cas stated that there is a higher number of complaints to nhtsa regarding non-crash fires on these vehicles compared to other similar vehicles.The office of defects investigation (odi) had already opened a recall query (rq17-003) on all kia vehicles equipped with theta ii engines for engine failures that may cause stalling and fires.The query covers most of the petition subject optima and sorento vehicles but not any of the subject soul vehicles.On august 21, 2018, odi opened a defect petition (dp18-003) to evaluate whether to grant or deny the cas petition.Odi sent information request letters to kia (september and october 2018) requesting information on all engine and non-engine related fires in the petition kia vehicles.Odi performed extensive analysis on the information obtained from kia.During the dp18-003 analysis, kia initiated three additional safety recalls related to engine compartment fires on certain petitioned and non-petitioned vehicles (nearly 500,000 total vehicles: recalls 18v-907, 19v-101, and 19v-120).Based on odi’s review of the applicable materials, including responses to information requests sent to multiple manufacturers, vehicle owner questionnaires (voq), and early warning data, nhtsa has decided to grant the petition.The petition is hereby granted.A preliminary evaluation has been opened to assess the scope, frequency, and potential safety-related consequences of alleged defects relating to non-collision vehicle fires in the 2011-2014 kia optima, 2011-2014 kia sorento, and 2010-2015 kia soul. While prior recalls of vehicles covered by this pe have primarily related to engine fires, nhtsa’s preliminary evaluation is not limited to engine components and may cover additional vehicle systems or components as well.Attached is a list of the reference numbers for the 244 voqs cited above.

  • Scope and timeliness investigation # RQ17003

    On march 31, 2017, kia motors america (kia) filed a defect information report (dir) recalling 618,160 my 2011-2014 optima, my 2012-2014 sorento, and my 2011-2013 sportage vehicles with "theta ii" engines (recall no. 17v-224)."theta ii" engines are a family of engines used by both kia and hyundai motor america (hyundai). Kias dir described the defect as an issue involving manufacturing debris as well as a machining process causing an uneven surface roughness which could restrict oil flow within the engine. Previously, on september 10, 2015, hyundai filed a dir recalling certain vehicles containing engines from the shared "theta ii" engine family, describing the defect as an issue involving manufacturing debris. Kias march 31, 2017, dir stated that kia did not conduct a recall upon learning of hyundais september 2015 recall because kias "theta ii" engines were manufactured on a different production line and did not have the same manufacturing issue.This rq is being opened to investigate both the timeliness and scope of kias "theta ii" engine recall, and kias compliance with reporting requirements.

While in cruise control hitting the breaks resulted in the car not wanting to slow down or stop. Had to go off road inorder to not hit vehicles in front of me. I turned off abs and this issue isn't there. I have no warnings for my abs system and this doesn't happen often. However it's getting worse now this is the worse it has ever been.

Driving at 60 mph on the highway engine seized up with no warning what so ever no engine light or oil light. Once vehicle stopped checked oil and it was almost empty. No warning of oil pressure or oil low. Vehicle always inspected an oil change done on time. Can't find recall on my vin but other 2012 models had a recall for this exact reason. I was almost hit by a semi when the vehicle just died with no warning at all and everything locked up. Never had 1 issue with this vehicle until now. And i am worried kia will not fix it when clearly other optimas of the same year had the same issue. I can't understand why when vin search it doesn't show this recall. It's all over the internet.

I driving to park,i heard a loud popping noise and i them preceeded to pull out the driveway and al’s 5 minutes in the drive the kia hybrid warning system came on and then told me to park the ending at the nearest location to park. The car started smoking real lot bad and me and my kid exited the vehicle because of all the smoke. The oil was everywhere on the car, on the motor, leaking out of the car.I then got car towed back home

Oil changed every 3000 miles. Driving to work the other day check engine light came on. Car went into limp mode. Pulled code and it was for rod bearings. Supposedly rod bearings had been repaired by recall before. Car only has 91,000 miles on it. So shouldn't need another set of rod bearings if previously replaced.

I was driving normal and started to hear a hissing noise in the front of vehicle. A vehicle cut in front of me i hit the brakes and another vehicle didn't slow fast enough and i went to hit the accelerator and had no power as usual and swerved to avoid the collision. I called dealership they said possibly my wastegate is out of normal voltage or need new turbo. I had a second opinion and they noticed the bolts and nut to service wastegate are rounded showing an attempt to maybe service or repair but unsuccessful. The turbo shows full function just inadequate boost.. The manufacturer has a bulletin and testing procedure released on wastegate test and adjusting procedure if needed as possible remedy. But i saw no recalls for my vehicle so reporting what i almost experienced and glad i have good driving experience to avoid an accident or life threatening situation behind the wheel of this vehicle now i don't feel safe until its proven resolved not just temporary fix.

When getting on oil change at mycardoc they wrote the wrong mileage on my car so the carfax is off. My engine broke down randomly the other day on the highway and now i need a whole new engine..And to find out the mileage was off when getting maintenance done.
