• Recall 243673 [USA]: A battery short circuit increases the risk of a fire.
  Toyota motor engineering & manufacturing (toyota) is recalling certain 2013-2018 rav4 vehicles. Certain replacement 12-volt batteries may not fit properly in the battery tray, allowing the battery to move and contact the hold-down bracket, possibly causing a short circuit.
  dealers will replace the battery clamp sub-assembly, battery tray, and positive terminal cover, free of charge. Interim owner notification letters, informing owners of the safety risk, were mailed december 8, 2023. Remedy parts are anticipated by june 2024. • Recall 117537 [USA]: Over time, the dripping water may cause corrosion and wear at the joint, potentially resulting in the separation of the wiper linkage. once this occurs, the wipers would not function, reducing driver visibility and increasing the risk of a crash.
  Toyota motor engineering & manufacturing (toyota) is recalling certain model year 2009-2012 rav4 vehicles manufactured october 27, 2008, to december 19, 2012 and 2012-2014 rav4 ev vehicles manufactured july 24, 2012, to august 29, 2014. In the affected vehicles, water may drip onto the joint that connects the wiper linkage to the wiper motor.
  toyota will notify owners, and dealers will replace the water diverter channel. In addition, a new wiper linkage with a wiper motor crank arm will be installed. The wiper linkage will have a protective cover over the wiper linkage joint to minimize water splash and a retainer ring to prevent the wiper arm from separating from the wiper motor.   Issue: this recall is for certain block heater kits installed on certain vehicles or sold as accessories at toyota or lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt.   Issue: on certain vehicles, the hook and loop material used to attach the carpet to its underpadding may not be flame resistant. As a result, when it is tested alone, the loop fasteners do not meet the requirements of canadian regulations. Safety risk: as the carpet is always assembled, and the loop fasteners are very small, this problem does not create a safety risk. Corrective action: this is not a recall. The company has made production changes to ensure that all new vehicles meet the requirements of canadian regulations.

  Issue: this recall is for aftermarket replacement rear window glass produced by vitro automotriz. Certain rear window glass may not have been properly tempered. If the window breaks, the glass may separate into large pieces instead of very small ones. Note: this is a voluntary recall that is not being conducted under the requirements of the motor vehicle safety act. Safety risk: glass that breaks in to large pieces could create a risk of injury. Corrective actions: the company will notify consumers by mail and instruct them to inspect the designation code marked on the rear window glass. • Recall 2015103 [Canada]: Steering
  On certain vehicles, the circuit board for the electric power steering electronic control unit may be damaged. A damaged circuit board could result in sudden loss of steering power assist and would trigger a warning lamp to illuminate. A vehicle experiencing a loss of steering power assist would revert to a manual steering mode which would require greater driver effort, especially at low vehicle speeds. A loss of steering power assist could ultimately result in a crash causing property damage and/or personal injury. Correction: dealers will replace inspect the electric power steering electronic control unit and, if affected, replace the electronic control unit with a new one.

• Pedestrian alert sounds investigation # DP22005

  Nhtsa received a petition on or about july 18, 2022, requesting that federal motor vehicle safety standard (fmvss) 141 be applied to all electric and hybrid vehicles operating in the united states. The petition can be reviewed at nhtsa.Gov under odi number 11486072.Fmvss 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 kg or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after march 1, 2021.The petitioner asserts that hybrid and electric vehicles to which the standard does not apply should be found to contain a safety defect. In support of the petition, the petitioner includes findings contained in a bill introduced in the house of representatives in 2009, the pedestrian safety enhancement act of 2009, h.R. 734, 111th cong. (2009). A related bill, the pedestrian safety enhancement act of 2010, became law in january 2011 (pub. L. 111-373). The petitioner requests that nhtsa require all electric and hybrid vehicles comply with fmvss 141, regardless of production date.The defect petition has been opened to evaluate the issue and determine whether to grant or deny the petition.The associated product list is not exhaustive and may contain vehicles that are equipped with pedestrian alert sounds.

• Fire originating from 12v battery area investigation # PE21005

  Nhtsas office of defects investigation (odi) has received eleven complaints and additional early warning report (ewr) data alleging a non-crash thermal event originating in the left side of the engine compartment of the fourth generation (xa40 platform, model year 2013-2018) toyota rav4 vehicles. A majority of thermal events occurred during driving conditions, with four taking place with the ignition off. Drivers experienced stalling prior to the thermal event in half of the instances where the vehicle was in motion. The twelve-volt battery was identified as the area of origin in a majority of the incidents reviewed. Improper battery installation or prior front end collision repair was a factor in the ewr field reports. Despite these external influences on the battery retaining method, the overall number of vehicle fire allegations with the battery as the area of origin is larger than its peer population.This preliminary evaluation has been opened to better understand the contributing factors and frequency of vehicle fires originating from the battery region of the subject vehicles relative to peer vehicles. The contact owned a 2014 toyota rav4. The contact stated while making a right turn at an undisclosed speed, the steering wheel was difficult to turn. During the failure, the instrument panel flashed approximately 4-5 times before going black. The contact stepped out to look under the hood and noticed a small fire. The police department was contacted and arrived and put out the fire. A police report was filed. The vehicle was towed away and deemed a total loss. The contact was not injured. Medical attention was not needed. The dealer was not contacted. The manufacturer was made aware of the failure and opened a case. The approximate failure mileage was 102,000.

After a minor front end collision, the rav4 immediately began smoking and caught fire. With a minor accident where the other car only had their front bumper minorly damaged, the rav4 should not have caught fire. Their must be a defect in either the battery or oil line for the car to have caught fire. The car is a total loss and i am asking for a full investigation as my 2 children were in the car when the fire started.

Making a turn onto a local toll road and entire car went dead without warning of any kind. I had to struggle with loss of power steering to complete turn without going off road. Car came to a stop. I tried to call my wife to advise her i was having car problems which is surprising as i haven't had issues previously and had an earlier version of the rav4 that i drove for 210,000 miles. Anyway, another vehicle stopped alongside and inquired if i needed assistance. As i talked, i noticed a smokey smell from what i had initially assumed was steam. I stepped outside the vehicle and observed through the drivers side front wheel well to see fire at the bottom of the engine compartment. Since i was driving to trails to ride my mountain bike, i had a water bottle and squeezed it onto the flames which partially extinguished and then relit. I hit the hood release but decided it was better to leave the hood down. I went to the rear lift gate to open it to retrieve my expensive carbon fiber mountain bike but it was locked. I tried to open the back driver side door also without success and tried hitting the unlock button on the drivers door but there was no power. About this time the smoke coming from the hood increased and i decided to heed the advise being given to step away from the car. Had i remembered sooner, i probably could have reached around the driver's seat to recover my wallet from the back pocket of the seat. As the fire increased, i stepped increasingly further back from the vehicle. About 5 minutes into the fire a state trooper arrived and expended his only extinguisher in an unsuccessful attempt to put out the fire. It was probably another 12-15 minutes before the fire department arrived to extinguish the fire but by that time, the car and its contents were a total loss.

I was driving down the road. I slowed up for a light and it turned green and started to go and my car turned off. Electrical still worked but engine was off.

All but one lock has now failed. The truck is not able to be opened. There were no warnings/indications, and now the company is telling me that it is no longer under warranty and it cannot be repaired unless paid for. I do find similar stories online and that the locks fail a second time in some cases.

While i was driving in capitol reef national park on a well graded and fully maintained dirt road, all the lights in my dash started flickering, and then my cars engine shut down. It felt like i was driving / coasting in neutral. I pulled over and put the car in park, then smoke started coming from beneath the hood. I got out of the car. It was a little smoke, then more, and then subsided to being barley noticeable after a couple of minutes. After it subsided, i got back in the car to try to start it, hoping it was an overheated engine and i might get the car to turn over. It would not start. I called a tow truck. Shortly after that, i noticed a hissing noise coming from beneath the hood, saw a liquid dripping beneath the the front left (drivers side) part of the car. I also saw a small flame. I called 911 to report that my car was on fire. Within 10-15 minutes, the car was completely engulfed in flames. The park ranger, sheriff, and fire department showed up shortly after that. A charred car frame is essentially what's left.
